Bilateral congenital grouped pigmentation of the retina in a patient with left superior rectus muscle palsy.

A 39-year-old man with left superior rectus muscle palsy had many sharply circumscribed, variable-sized pigmented spots in both fundi. His visual acuity, visual fields, and color vision appeared normal bilaterally. The electroretinographic responses were also normal. His family members were unaffected by pigmented spots in the retina. We believe that the bilateral congenital grouped pigmentation of the retina found in our patient may be rare.

Visual prognosis in patients with Behçet's disease receiving colchicine, systemic corticosteroid or cyclosporin.

We reviewed retrospectively the charts of 36 patients (72 eyes) with Behçet's disease. The visual acuities in 21 patients receiving colchicine (group A) gradually decreased with time. The visual acuities in 9 patients receiving colchicine and systemic corticosteroid (group B) gradually decreased after the onset of the disease and were significantly worse than those in group A 10 and 15 years after onset. The visual acuities in 6 patients receiving colchicine and cyclosporin (group C) gradually decreased, and their visual acuity scores were similar to those in group A.

Transconjunctival and transantral approaches are combined with antral wall bone graft to repair orbital floor blow-out fractures.

Orbital floor blow-out fractures in two patients were surgically repaired. The transconjunctival approach alone was unsuccessful in freeing the entrapped orbital contents, so the transantral approach with the use of antral wall bone graft was also used. The combined approaches produced good results. The transconjunctival approach supplemented with the transantral approach and bone graft may be a useful technique in the repair of orbital floor blow-out fractures.

Micrographia due to focal cerebral lesions as seen in the dysarthria-clumsy hand syndrome.

Five patients with dysarthria-clumsy hand syndrome developed micrographia. Four had lacunar infarctions involving the putamen or the genu of the internal capsule; the fifth patient had a small hemorrhage in the putamen. Micrographia may be a sign of focal cerebral disease, and basal ganglia dysfunction may be involved in the supratentorial dysarthria-clumsy hand syndrome.

Radiographic features of experimental Candida arthritis in rats.

Sprague-Dawley rats were inoculated intravenously (i.v.) with Candida albicans, and limb joints showing signs of Candida-induced arthritis were subjected to radiographic and histologic examination. New bone formation and bone resorption were morbidly enhanced in bones sampled from the arthritic joints. Sparsely distributed needle-shaped calcified deposits began to be formed on bony surfaces in parallel with the onset of joint swelling. The calcified deposits gradually became denser and then covered the bony surfaces almost entirely, giving rise to an exostosis-like profile. In addition to the new bone formation, bone resorption was also observed in regions adjacent to the sites of new bone formation, and punched-out bone lesions were produced. Eventually, severe deformation of joint bones due to new bone formation and bone resorption was evident. Reflecting these unusual radiographic changes, abundant osteoblasts and osteoclasts were demonstrated histologically in the bones. On the basis of these results, possible mechanisms for the induction of arthritis by Candida infection are discussed.

Detection of human T-cell lymphotropic virus type 1 infection by the polymerase chain reaction using dried blood specimens on filter papers.

A simple method for detection of proviral DNA sequences of human T-cell lymphotropic virus type 1 (HTLV-1) was developed using dried blood specimens on filter papers. The whole blood was blotted onto the Guthrie paper. After the blood has dried, the blotted paper was punched out into small discs. The discs were then boiled to prepare the template for PCR (filter paper-PCR method). The filter paper-PCR method detected even a single HTLV-1-infected cell in three discs. The sensitivity of the filter paper-PCR method was equivalent to that of the method in which DNA was extracted with phenol and used as the template for PCR (DNA extraction-PCR method). In addition, DNA in the blotted filter paper was still utilizable as the template after the storage at 25 degrees C for at least 7 wk. A total of 53 clinical specimens from 30 seropositive and 23 seronegative individuals who were screened by particle agglutination (PA) test were analysed for HTLV-1 DNA by both PCR methods. Of 30 PA-positive specimens, 28 were also positive for HTLV-1 antibody by Western blot (WB) analysis, but two were indeterminate. The twenty eight WB-positive and one of the two indeterminate specimens were positive for HTLV-1 proviral DNA by both PCR methods. Of 23 PA-negative specimens, 22 were negative for HTLV-1 proviral DNA by both PCR methods. However, one PA-negative specimen was positive by both PCR methods. This patient was a 16-mth-old infant who was born to an HTLV-1 carrier mother and fed thereafter without her breast milk. In comparison to DNA extraction-PCR method, the sensitivity and specificity of the filter paper-PCR method was 100%, respectively.

Perirenal hematomas caused by SWL with EDAP LT-01 lithotripter.

A total of 419 calculi in the upper urinary tract of 402 patients were treated by SWL with the EDAP LT-01 lithotripter from July 1988 to September 1989. Subcapsular hematomas resulted in 17 kidneys of 16 patients, an incidence of 4.1%. On CT scan, fractures with subcapsular hematomas were observed in nine kidneys, and a retroperitoneal hematoma was observed in one patient. There was no significant difference in stone location and size, grade of hydronephrosis, number of shock waves, applied energy, or post-treatment fever between the groups with and without hematomas. However, the hematoma group revealed significantly (P < 0.01) higher incidences of pretreatment hypertension (> 160/95 mm Hg) and use of antiplatelet agents. It is difficult to detect some disorders of blood coagulation in ordinary laboratory studies in patients receiving low dosages of antiplatelet agents; and many drugs including analgesic and anti-inflammatory agents, calcium antagonists, coronary vasodilators, antiplatelet agents, beta-blockers, and lipid-regulating agents have antiplatelet actions. Therefore, particular care should be taken in SWL when a patient is using those drugs.

Increased D-arabinitol/creatinine ratio in sera of patients with Behçet's disease during an active phase.

The ratios of D-arabinitol (a major metabolite of Candida species) to creatinine were examined in 46 sera of 40 patients with ocular inflammatory disease and in 50 age-matched normal controls (25 men and 25 women). The mean ratio in patients with endogenous Candida endophthalmitis was higher than in the normal controls. Seven samples taken from patients with Behçet's disease during an active phase showed higher values than in the controls. Fourteen patients with Behçet's disease in remission showed D-arabinitol/creatinine values within normal range. In four patients with sarcoidosis, in two patients with Vogt-Koyanagi-Harada disease, and in 16 patients with bilateral uveitis of an unknown cause, the ratios were within normal range. It is possible that D-arabinitol, or a metabolite of Candida species, may be related in part to the active phase of Behçet's disease.

Thalamic experiential hallucinosis.

Two patients with an infarct limited to the thalamus developed auditory and visual experiential hallucinations. Neuropathological studies in one patient showed a small cavity in the right intralaminar nuclei surrounded by focal spongiform change, partly involving the right dorsomedial nucleus. Neuroradiological data in another patient indicated that the same nuclei in the left thalamus were also affected. It was concluded that a unilateral thalamic lesion could cause experiential hallucinations and the intralaminar and dorsomedial nuclei might be important structures to explain the phenomenon.

Psychiatric complications after ocular surgery.

Delirium was demonstrated after cataract surgery in a 64-year-old man (case 1). The patient struck his head against the floor; thereafter an extradural hematoma was found and extracted. Postoperative delirium was also noted in a 58-year-old woman (case 2) and in a 26-year-old woman (case 3) with rhegmatogenous retinal detachment. All 3 patients recovered from their psychiatric complications within 1 month.

Corneoscleral ulceration and corneal perforation after pterygium excision and topical mitomycin C therapy.

A 78-year-old woman complained of ocular pain in the right eye. Thirty-three months before, the patient had a pterygium excised and mitomycin C, 0.04%, was instilled 3 times a day postoperatively for 10 days in the right eye. The same surgery was performed and the same drug was used for a course of 5 days in the left eye. Corneoscleral ulceration and corneal performation occurred in the right eye. The lesions were successfully treated with a lamellar patch graft with reserved corneosclera. We believe that it may not be safe to instill 0.04% mitomycin C 3 times a day postoperatively for 10 days.

Distribution of vinculin in the duodenum of chick embryos: immunohistochemical studies at the light and electron microscopic level.

Studies on the localization of vinculin and actin in the embryonic chick duodenum between the 9th and 13th days of incubation were performed by light and electron microscopy. In the light microscopic studies, vinculin and actin were localized by immuno-fluorescent double staining with anti-vinculin and rhodamine phalloidin for actin. In the electron microscopic studies, the immunocytochemical distribution of vinculin was examined on Lowicryl K4M thin sections with anti-vinculin. Vinculin was prominent near or adjacent to the apical inner membrane of epithelial cells of the previllous ridges in all stages, often located near cell junctions including tight and adherent junctions and the top of microvilli, and weakly in the cytoplasm. Our findings suggested the possible interaction between actin and vinculin in differentiation on the apical side of epithelial cells including the formation of microvilli or terminal webs.

Distribution of protein kinase C in the villi of duodenum of developing chick embryos and chicks: immunohistochemical studies at the light and electron microscopic levels.

We examined the immunohistochemical localization of protein kinase C (PKC) in chick embryonic and chick duodenal villi contained in cultured duodena using monoclonal antibodies that recognize types 1, 2 and 3 at the light and electron microscopic levels. In the light microscopic studies, staining with the type 2 and 3 PKC antibodies revealed immunoreactivities in the absorptive epithelial cells of the villi that approximated the time course after hatching, but was weak and partial just before the time of hatching. The epithelial cells of crypts seemed to lose their stain at all times. In the transmission electron microscopic studies, positive immunoreaction was found predominantly in the endoplasmic reticulum and nucleus, and also in the terminal web in the absorptive epithelial cells of villi. In the cultured duodenal fragments, a change in the distribution of PKC was observed on the sheet-like duodenal fragments and its immunoreaction was mainly seen on the basal side of absorptive cells. In contrast, no change in the staining of PKC was revealed on the ring-like duodenal fragments. These results suggested that PKC in the chick duodenal villi may be functionally related with the differentiation of absorptive epithelial cells and sensitive to changes in the cellular interactions of absorptive epithelial cells.

Immunoelectron microscopic localization of sucrase-isomaltase in rat small intestine.

The localization of rat small intestinal microvillous sucrase-isomaltase was studied by immunoelectron microscopy to investigate its intracellular transport to the microvillous membrane. At the same time, the usefulness of a rapid embedding method of tissues in Lowicryl K4M for immunocytochemistry of sucrase-isomaltase was examined. Sucrase-isomaltase was present not only in the microvillous membrane, but also in the apical vesicles and the apical plasma membrane invaginations. Negligible labeling was observed in the other portions of the absorptive cells. These findings suggest that the final step of intracellular transport of sucrase-isomaltase to the microvillous membrane is via smooth apical vesicles. The rapid immunoelectron microscopic method adopted in this study seemed to be a useful technique for the study of the intracellular localization of sucrase-isomaltase.

Immunoelectron microscopic localization of lactase-phlorizin hydrolase in rat small intestine.

To provide insight into the intracellular translocation of lactase-phlorizin hydrolase, an immunoelectron microscopy was performed on rapidly embedded Lowicryl K4M sections of rat jejunum. Lactase-phlorizin hydrolase immunoreactivity was detected not only in the microvillous membranes and in the smooth apical vesicles, but also in the lateral membranes, suggesting an alternative route for intracellular transport of lactase-phlorizin hydrolase via the lateral membranes to the microvilli.

Patients with asteroid hyalosis and visible floaters.

Four patients with asteroid hyalosis who complained of seeing multiple spherical floaters were examined. Except for multiple brilliant particles in the gelatinous vitreous, no abnormalities were found by ophthalmoscopic examination. Asteroid bodies usually cause no visual symptoms in most patients. Therefore it is unusual that our patients with asteroid hyalosis perceived floaters. We concluded that it was because there had been no vitreous detachment so that the asteroid particles were located close enough to the retina to be perceived as floaters.

Initial rapid decrease in visual acuity in siblings with Stargardt's disease.

Two siblings with Stargardt's disease who had an initial rapid decrease in visual acuity were reported. The 8-year-old boy and his 5-year-old sister experienced a bilateral visual decrease within a period of 3 months. Their parents had a consanguineous relationship. Macular changes and blocked choroidal fluorescence were noted bilaterally in both children. Normal color vision, normal or subnormal electroretinographic findings, and subnormal electro-oculographic responses were found. Stargardt's disease should be included in the list of conditions showing rapid decrease in visual acuity.

A Japanese family with Grayson-Wilbrandt variant of Reis-Bücklers' corneal dystrophy.

A 50-year-old man (the proband) experienced recurring attacks of ocular irritation and had had bilateral ring-shaped anterior corneal opacity since adolescence. Corneal sensation was normal. The proband's 15-year-old daughter also had recurring episodes of ocular irritation and bilateral fleck-like anterior corneal opacities. The proband's deceased father and brother reportedly had similar corneal lesions from adolescence. We believe that the corneal findings in this family may be similar to those found in patients with the Grayson-Wilbrandt variant of Reis-Bücklers' corneal dystrophy.
