Three cases of idiopathic portal hypertension associated with Hashimoto's disease are described. All of the cases were middle-aged Japanese women showing splenomegaly, esophageal varices and pancytopenia in the absence of extrahepatic portal obstruction, and cirrhosis of the liver. Two patients were euthyroid with goiter, one of which revealed diffuse lymphocytic infiltration, obliteration of thyroid follicles, and fibrosis on histological examination of the thyroid; the third suffered from myxedema without goiter. Antithyroid microsomal antibody was positive in all patients and antithyroglobulin antibody was positive in none. These findings might imply an immunological role in the pathogenesis of idiopathic portal hypertension.

Previously, the author reported that long microvillous-like processes (long processes) containing core actin filaments seemed to be related to the formation of previllous ridges in the chick embryonic duodenum and that similar long processes were induced by cytochalasin B (CB) treatment. The appearance of long processes by CB (polymerization of core actin filaments) seemed to be related to the ability of cell proliferation. In this study, the effect of colchicine on the induction of long processes by CB was examined in 11-day-old chick embryonic duodena in the culture system. The results were as follows: The induction of long processes by CB was inversely influenced by the concentration of colchicine; the appearance of long processes by CB was differently influenced by local parts of the epithelial cells in the presence of colchicine; and alteration of the arrangement of intracellular actin elicited by CB and colchicine was confirmed by immunohistochemistory using actin antiserum. It was also suggested that sudden intracellular alterations elicited by higher concentrations of colchicine may act as a trigger of cell division differing from normal conditions.

Asterixis at the hip joints can be elicited by a simple diagnostic maneuver in which the hips are passively flexed and abducted about 60 to 90 degrees between the thighs. The test evoked asterixis in 8 of 10 patients who were stuporous or semicomatose because of hepatic encephalopathy. The asterixis seems to be provoked by the involuntary contraction of hip adductors against gravity.

To evaluate the origin of ectopic atrial rhythms, the beginning of atrioventricular inflow due to left and right atrial ejection was estimated using the pulsed Doppler combined with two-dimensional echocardiography. In ten normal controls, the beginning of transtricuspid flow due to atrial ejection preceded that of transmitral by 0 to 40 msec with an average of 22 msec. In contrast, the beginning of right atrial ejection flow lagged behind that of left atrial by 40 to 80 msec in case 1 and by 20 to 50 msec in case 2 of ectopic atrial rhythm. The significant delay of atrioventricular inflow due to right atrial ejection in these two patients strongly suggests that the ectopic atrial rhythm is of left atrial origin. The pulsed Doppler echocardiography is considered to be a useful clinical tool for noninvasive evaluation of the left atrial rhythm.

Intracranial anastomosis between the distal anterior cerebral arteries (ACA's) was performed on two patients. One patient had isolated occlusion of the proximal ACA on one side causing monoparesis in the leg, and the other patient suffered from occlusion of the internal carotid artery and a small anterior communicating artery. The weakness of the legs improved immediately after surgery and cerebral angiography revealed a good filling of the bilateral ACA's. Technical aspects and indications for the procedure are discussed.

Between 8 and 12 days of incubation the embryonic duodenum serially constructs with relative regularity the previllous ridges upon which the definitive villi later form. The effects of cytochalasin B (CB) on the formation of these previllous ridges of the duodena of developing chick embryos were studied, varying the concentrations and exposure time of CB in the organ culture system. The results were as follows: CB inhibited the formation of new previllous ridges from the epithelial cell sheets of 8- to 11-day-old embryonic duodena at cultured time of 24h. CB treatment blocked or delayed cytokinesis of the epithelial cells and the production of many long microvillous-like processes (long processes) from the surface of the epithelial cells. These long processes elicited by CB contained actin filaments and their appearance was influenced by the developmental stages of embryos and local parts of epithelial cells. With 11-day embryonic duodena, induction of long processes by CB was observed at various concentrations (1 microgram/ml-16 micrograms/ml) and even after short exposure of 15 min. Cytochalasin D (CD) and colchicine were used and long processes were induced by CD but not by colchicine itself. The appearance of long processes depended on the experimental concentration of CB, CD and colchicine. In normal developments, such long processes appeared and disappeared within a confined area during the formation of previllous ridges (Noda, 1981). This study seemed to provide experimental support for the previous reported suggestion that the long processes might be one of the important factors in the formation of the previllous ridges of chick embryonic duodena.
A phase II study of a new anthracycline, (2''R)-4'-0-tetrahydropyranyladriamycin (THP) was performed on 37 patients with gastrointestinal cancer in 6 co-operative study institutions. Twenty-five patients out of 37 were evaluable for response according to the Koyama-Saito's criteria. THP was administered weekly at doses of 10 to 30 mg/body or every 3 to 4 weeks at doses of 40 to 60 mg/body intravenously. Of the 14 patients with gastric cancer, we obtained one complete response and 3 partial responses (response rate 28.6%), and of the 6 patients with rectal cancer, we obtained one partial response (16.7%). Leukopenia of less than 3 X 10(3)/mm3 and erythrocytopenia of less than 300 X 10(4)/mm3 were seen in 48% and 26% of cases. Neither cardiotoxicity nor hair loss were seen. These results suggest that THP is useful in the treatment of patients with gastrointestinal cancer.
