[Clinical and fundamental studies on ceftriaxone (CTRX) against urinary tract infections].

Clinical efficacy of ceftriaxone (CTRX) against complicated urinary tract infections in 20 patients was examined, and the serum CTRX level was also measured in the patients with chronic renal failure (CRF). CTRX was administered at a dose of 1.0 g once a day for 5 to 10 days. The overall clinical efficacy was rated excellent in 4 cases (20%), moderate in 13 cases (65%) and poor in 3 cases (15%) with a total efficacy of 85%. Seventeen out of 27 strains (62.8%) isolated were eradicated after CTRX administration. In the patients with CRF, serum levels were very high and showed a plateau 4 hours after the injection. Furthermore, CTRX may not be removed by usual hemodialysis treatment. There were no severe side effects due to CTRX in these patients.

[Evaluation of Candida HA titer in patients with urinary tract infections].

The serum candida antibody (HA titer) was measured by means of indirect hemagglutination test in 28 patients with complicated urinary tract infections (UTI) and 11 control subjects. The peripheral lymphocytes subsets (OKT-3,4,8 and 4/8 ratio) were also examined in same patients. The 17 UTI patients who had Candida species in their urine had a higher HA titer than the other UTI patients without Candida or control subjects, but elderly UTI patients (over 71 years old) did not show higher HA titer. A positive correlation was seen between HA titer and OKT-3,4 or 4/8 ratio. On the other hand, HA titer and OKT-8 showed a negative relationship. Therefore, the ability to produce the candida antibody might be closely related to the host cellular immune responsiveness.

[Clinical and fundamental study of guanabenz in hypertensive patients with renal failure].

Clinical and fundamental study of Guanabenz were performed in 20 hypertensive patients with renal failure, who were treated by maintenance hemodialysis treatment. The patients received 2 mg of Guanabenz orally, twice a day for 3 months or more. The clinical efficacy of good control was attained in 65%, and of fair control in 90%. On the other hand, the objective and subjective effects of Guanabenz were compared between short term (within one year long) hemodialysis patients and long term (over one year) patients. The overall clinical efficacy was higher in short term patients. The serum level of Guanabenz in hemodialysis patients showed a maximum value at 2-4 hours after administration. As Guanabenz might not be removed so much by hemodialysis, the administration schedule and daily dosage were discussed. There were no severe side effects arising from administration of Guanabenz.

[The effects of ninzin-to with daiokanso-to in patients with chronic renal failure].

The clinical effects of ninzin-to with daiokanso-to were studied on 15 patients with chronic renal failure. The level of urea nitrogen and creatinine in sera was slightly decreased in early days of treatment (about 3 months long), but thereafter these levels were gradually increased in many patients and 2 patients needed hemodialysis treatment. The effects of these drugs against subjective symptoms were found in bowel habits, general malasia and anorexia. The overall effective rate was 80% in early term and 60% in late term. There were no severe adverse reactions.

Generation of continuous large granular lymphocyte lines by interleukin 2 from the spleen cells of mice infected with Moloney leukemia virus. Involvement of interleukin 3.

Continuous cell lines could be reproducibly established by culturing spleen cells from adult mice injected with MLV-producer cells or directly infected with Mo-MLV with rIL-2, whereas the culture of normal splenic cells with rIL-2 induced only transient and limited proliferation resulting in no such lines. All of the lines showed morphological characteristics as LGL with Thy-1+,Lyt-1-,L3T4-,Lyt-2-,AsGM1+,FcR gamma+ phenotype without exception, and most of them exhibited typical NK-patterned cytotoxicity. Analysis of reverse transcriptase activity of the culture supernatants as well as Southern hybridization of the DNA from the lines using an Mo-MLV-specific cDNA probe indicated no evidence of retroviral replication or proviral integration, suggesting that the generation of cell lines reflected a reactive process and viral infection was not directly responsible. It was subsequently revealed that Thy-1+,Lyt-1+,Lyt-2- spleen cells from mice infected with Mo-MLV in vivo spontaneously produced surprising amounts of IL-3 in vitro, leading to the possibility that IL-3 was responsible for the generation of lines. The possibility was directly supported by the observation that continuous lines with identical characteristics could be generated completely in vitro by sequential stimulation with rIL-3 and rIL-2 from normal spleen cells without any involvement of Mo-MLV. The C beta gene of TCR was shown to be rearranged in all the lines examined, indicating the LGL lines were all genetically committed to T cell lineage. Unlike the situation in normal splenic populations expanded by rIL-2, where the expression of IL-2-R was progressively lost, constitutive expression of high-affinity-IL-2-R was observed in all the lines and thus, this was considered to explain the unlimited proliferation of them in response to rIL-2 alone. These results suggested the probable role of IL-3 in the regulation of growth and differentiation of a set of LGL committed to T cell lineage. The possible implications of the phenomenon in the regulation of hematopoiesis as well as in the control of Mo-MLV-induced leukemogenesis were discussed.

Pituitary tumors composed of adenohypophysial adenoma and Rathke's cleft cyst elements: a clinicopathological study.

Nine cases of pituitary tumors composed of mixed tissue elements of adenohypophysial adenoma and Rathke's cleft cyst are reported. All cases were associated with hyperprolactinemia and were managed by transsphenoidal adenomectomy with good results. The clinical and pathological features are presented, and embryological pathogenesis for the occurrence of this unique association is discussed in connection with the high incidence of immunoreactive S-100 protein cells identified in tumor tissues. Patients with this kind of pituitary tumor can be managed in the same fashion as patients with other types of pituitary adenoma.

Antithrombotic effect of TRK-100, a novel, stable PGI2 analogue.

TRK-100, a stable PGI2 analogue structurally different from carbacyclines, was compared with other antiplatelet drugs for its effect on platelet functions using animal models. TRK-100 (10-300 nM) inhibited rat platelet aggregation induced by ADP (3 microM), collagen (12.5 micrograms/ml) and A23187 (10 microM), and its potency was about 1/3-1/7 that of PGI2. TRK-100 (0.3-3 mg/kg, p.o.) dose-dependently inhibited rabbit platelet adhesion (ED50: 2.2 mg/kg), and its effect lasted over at least 5 hr. In contrast, aspirin and ticlopidine (both at 300 mg/kg, p.o.) showed only a slight inhibition (4-7%). In the thrombocytopenia induced by collagen injection in rats, TRK-100 (3-300 micrograms/kg, i.v.; 0.1-3 mg/kg, p.o.) dose-dependently inhibited a decrease in platelet number, and its ED50 was 0.48-0.62 mg/kg orally and 13.7-16.4 micrograms/kg intravenously, while the inhibition by aspirin and ticlopidine (both at 1000 mg/kg, p.o.) was 40 and 37%, respectively. In the experimental thread thrombosis in rats. TRK-100 (0.03-3 mg/kg, p.o.) dose-dependently inhibited thrombus formation, and its ED50 was 0.46 mg/kg, being 21 and 87 times as potent as aspirin and ticlopidine, respectively. These results reveal that TRK-100 has a potent antiplatelet activity and is orally and intravenously effective for a variety of thrombosis models, suggesting that it may have a therapeutic value as an antithrombotic drug.

Hyperosmotic blood-brain barrier disruption in brains of rats with an intracerebrally transplanted RG-C6 tumor.

To test the results of blood-brain barrier (BBB) disruption in the treatment of brain tumor, RG-C6 glioma was transplanted into the brains of rats. Intracarotid infusions of normal saline and hyperosmotic mannitol were then made, followed by intravenous injection of Evans blue dye plus albumin (EB, MW 68,000), horseradish peroxidase (HRP, MW 40,000), and 5-fluorouracil (5-FU, MW 130). Uptake of the drug and the consistency of drug levels in the normal brain and tumor varied widely among these three agents. Both EB and HRP penetrated the brain tumors but did not stain the normal brain tissues. After BBB opening, penetration of EB and HRP into the normal brain was drastically increased; however, the uptake of EB and HRP in the tumor was not increased. The concentration of 5-FU in the tumor was higher than that in the serum and, although it increased 1.5-fold after BBB opening, the increase was not statistically significant. Conversely, there was a progressive increase in concentrations of 5-FU in the tumor-free brain regions (p less than 0.05). These observations suggest that an intracarotid infusion of hyperosmotic mannitol may increase neurotoxicity because it allows greater delivery of anticancer drugs into the normal brain tissue than into the tumor tissues.

[Clinical evaluation of ofloxacin against complicated urinary tract infections].

Clinical efficacy of ofloxacin (OFLX) against complicated urinary tract infections was examined in 20 patients. OFLX was administrated orally at a dose of 200 mg, three times (daily 600 mg) for 14 to 102 days. The overall clinical efficacy was rated excellent in 8 cases (40%), moderate in 7 cases (35%) and poor in 5 cases (25%) with a total efficacy of 75%. Bacteriological efficacy, 17 out of 24 strains (70.8%) isolated were eradicated after OFLX administration. The strains that appeared after OFLX treatment were P. cepacia and Candida sp. No severe side effects were found in these patients.

Reduction in responsiveness of thymocytes to phytohemagglutinin during bladder tumor induction and restorative effects of thymosin fraction 5 in rats.

Urinary bladder tumors were induced in female Wistar rats by oral administration of N-butyl-N-(4-hydroxybutyl) nitrosamine [(BBN) CAS: 3817-11-6]. Hypertrophy or vascular formation was observed in the bladder membrane 8 weeks after BBN administration, and tumors became visible at the 15th week. At the 20th week, approximately 95% of these rats had bladder tumors. The response of thymus lymphocytes to phytohemagglutinin (PHA) in these BBN-treated rats began to decrease from the 10th week and decreased significantly from the 18th week as compared with that in untreated rats. This reduced response to PHA was slower in comparison with the reduced response to concanavalin A. When thymus lymphocytes were preincubated with thymosin fraction 5 in vitro, the response to PHA improved between the 8th week and the 20th week. Furthermore, when the thymus small lymphocyte population was separated into 3 subpopulations by discontinuous density gradient centrifugation and adhesion column, the restorative effect of the thymus hormonal product was greater in the intermediate and heavier subpopulations.

[Study on effectiveness of carmofur (Mifurol) in urogenital carcinoma, especially bladder cancer, as a post-operative adjuvant chemotherapeutic agent].

This study was carried out to evaluate the effects of carmofur (Mifurol), a masked compound of 5-fluorouracil (5-FU), in preventing postoperative recurrence of urogenital carcinoma (especially in bladder cancer). Carmofur was given orally at the dose of 300 approximately 600 mg/day to 134 patients (90 males and 44 females) after the resection. Out of 134 patients, 121 patients were evaluable and the rate of their recurrence was 10.7%. The rate of cumulative nonrecurrence for 65 patients it was 96.0, 73.0 and 60.8% in 1, 2 and 3 years, respectively. The group of curative resection demonstrated a higher cumulative nonrecurrence rate than that of noncurative or palliative resection group. In considering the resection methods, transurethral resection of bladder tumor group demonstrated a higher rate than partial or total cystectomy. The average concentration of 5-FU in serum was 0.65 microgram/ml at 120 minutes after oral administration of 200 mg of carmofur. Moreover, the concentration of 5-FU (15 approximately 35 micrograms/ml) in urine was higher than that of carmofur. The rate of side effects that appeared was 19.6%. The side effects were hot feeling (12.2%), pollakisuria (4.3%), anorexia (4.3%), an antabuse-like reaction by drinking (3.8%) and so on. These results were similar to those already reported.

Crystal-matrix interrelations in brushite and uric acid calculi.

Brushite and uric acid calculi were studied by means of scanning electron microscopy with the partial dissolution method and transmission electron microscopy. Brushite calculi consist of radially oriented columnar crystals which have sheet-like substructure. The organic matrix is identified chiefly at the outside of the crystals but partly included between the substructure. The concentric matrix bands are often dislocated between the neighbouring crystals. Uric acid calculi also consist of radially oriented columnar crystals, and a fine meshwork of the organic matrix is incorporated within the crystals. The concentric matrix layers of different density are angled according to the crystal lattice. These findings indicate that the organic matrix arose from a mucinous surface coat, at least in the radially striated calculi. The crystals continued to grow in this gel-state milieu, either thrusting the matrix aside or incorporating it within the crystals.

Marked reduction in percentage of rosette-forming thymocytes of rats during induction of bladder tumors and restorative effect of thymosin in vitro.

Urinary bladder carcinoma was induced in W rats by oral administration of N-butyl-N-(4-hydroxybutyl) nitrosamine [(BBN) CAS: 3817-11-6], and rosette formation of the thymus lymphocytes with Hartley strain guinea pig erythrocytes was studied until the 20th week after BBN administration. Percentage of rosette-forming cells (RFC's) began to decrease about the 8th week (stage of hypertrophy or vascular formation in the bladder wall) and remarkably declined around the 12th week (before the incidence of carcinoma) after the administration. However, no significant reduction of RFC percentage was noted in the control (without BBN) group during the observation period. Pretreatment of thymic lymphocytes from BBN-treated rats with thymosin fraction 5 (TF5), an extract from calf thymus, significantly enhanced the percentage of RFC near to that of the control level. Two (#9 and 13) subfractions separated from TF5 by high-performance liquid chromatography were found to be more effective in recovering the percentage of RFC. These results show that a maturational impairment of thymus lymphocytes may be caused from precancerous stage in BBN-administered rats and that this impairment would be restored by the thymus products in vitro.
