Preparation and characterization of NADP derivatives alkylated at 2'-phosphate and 6-amino groups.

Reaction of NADP with 3-propiolactone at pH 6 gave new NADP derivatives carboxyethylated at the 2'-phosphate or 6-amino group, or both: 2'-O-(2-carboxyethyl)phosphono-NAD (I), N6-(2-carboxyethyl)-NADP (II), and 2'-O-(2-carboxyethyl)phosphono-N6-(2-carboxyethyl)-NAD (III). Their structures were assigned on the basis of ultraviolet, 1H-NMR and 31P-NMR spectra, and also treatment with nucleotide pyrophosphatase or alkaline phosphatase. Carbodiimide-promoted reaction of derivative I with 1,2-diaminoethane gave 2'-O-[N-(2-aminoethyl)carbamoylethyl]phosphono-NAD (IV); derivative III gave 2'-O-[N-(2-aminoethyl)carbamoylethyl]phosphono-N6-[N-(2-aminoethyl ) carbamoylethyl]-NAD (IV). The same reaction of derivative II, on the other hand, gave a mixture of N6-[N-(2-aminoethyl)carbamoylethyl]-NADP (Va) and its 3'-phosphate isomer (Vb). The mixture was converted to Va via the 2',3'-cyclic derivative (Vc). Their structures were assigned on the basis of ultraviolet and 1H-NMR spectra, and also treatment with alkaline phosphatase or 3'-nucleotidase. All the NADP derivatives obtained in this work could be reduced with yeast glucose-6-phosphate dehydrogenase.

Urinary retention secondary to ovarian dysgerminoma in a girl.

A thirteen-year-old girl sought medical advice because of a markedly weakened urinary stream with severe hesitancy. She was referred to the pediatric clinic because of 3 episodes of acute urinary retention during a previous month. Pelvic examination showed a retrovesical tumor. An excretory urogram revealed bilateral ureterectasis and pyelocalicectasis. Two weeks after left nephrostomy, surgical exploration demonstrated a right ovarian mass causing compression of the left ureter and obstruction of the bladder neck. Excision of the tumor was performed without difficulty, following which the patient resumed normal voiding. The resected tumor weighed 815 Gm and pathologic examination showed dysgerminoma. Postoperatively the following two and one half years were uneventful.

A method for predicting postoperative lung function and its relation to postoperative complications in patients with lung cancer.

We predicted the postoperative forced expiratory volume in 1 second (FEV1) with a formula based on the premise that the total number of subsegments was 42: postop FEV1 = [1 - (b - n)/(42 - n)] (preop FEV1), where n and b are the number of obstructed subsegments and total subsegments, respectively, in the resected lobe. It was assumed that b was 6, 4, and 12 in the right upper, middle, and lower lobes, respectively, and 10 each in the left upper and the left lower lobes. The obstructed subsegments, n, were obtained from the findings on bronchography or bronchofiberscopy or both before operation. The linear regression line derived from the correlation between predicted (x) and measured (y) FEV1 was y = 0.850x + 0.286 +/- 0.296 (standard error) (N = 52; r = 0.821; p less than 0.001). We calculated the predicted postoperative FEV1 in 188 patients with primary lung cancer. The predicted values were corrected with the regression equation just mentioned and then normalized by the patient's height and sex (%FEV1(p,c). The correlation between %FEV1(p,c) and the surgical risk was studied. Postoperative respiratory complications were inversely related to %FEV1(p,c), and a significantly high incidence of complications (p less than 0.05) was observed in those whose %FEV1(p,c) was less than 60% of predicted normal. In aged patients (65 years old or more) without complications, %FEV1(p,c) was 67.3 +/- 18.0%; it was 52.2 +/- 12.8% in those with respiratory trouble and 53.3% +/- 9.6% in those with circulatory complications. Some properties of Vibrio vulnificus hemolysin.

Some properties of hemolysin produced by Vibrio vulnificus were investigated. The hemolysin was heat labile, and the hemolytic activity was inhibited by adding cholesterol or divalent cations. Cholesterol inhibited the temperature-independent hemolysin-binding step, suggesting that cholesterol made up the binding site of the cell membrane, whereas the divalent cations inhibited the temperature-dependent membrane-degradation step. However, the V. vulnificus hemolysin was stable to oxygen and sulfhydryl reagents and was not inactivated by antiserum against streptolysin O, suggesting that the V. vulnificus hemolysin differs from oxygen-labile hemolysins which bind to cholesterol. The V. vulnificus hemolysin seems to be one of the exceptional cholesterol-binding hemolysins.

Diagnosis of submucosal tumors by injecting a water soluble contrast medium: diagnosis of extra-gastric tumors and gastric varices.

Extra-gastric compression caused tumescent lesions that are difficult to differentiate from gastric submucosal tumors, and gastric varices similar in appearance to circumscribed tumors were sometimes experienced clinically. Up to now, the differential diagnosis of these lesions has been done by the palpation through x-ray examination or the tactile test under endoscopic examination. Even by the recent use of the CT scan, the differential diagnosis still remains unsatisfactory except in a few specific cases. Under these present circumstances, submucosography is recommended for routine screening test for outpatients. Our method is simple, safe and time-saving. Recently, it has become easy to diagnose hemangiomas as well as varices in the stomach by application of submucosography. Accordingly, in the cases of vascular tumors or tumescent lesions caused by extra-gastric compression, the risk of incidental major bleeding or perforation can be prevented by using the submucosography.

[Malignant lymphoma of the testis: report of two cases].

Two cases of malignant lymphoma arising from the testis are reported. One was 45 years of age, with the complaint of left scrotal swelling with dull pain and the other was 68 years of age, with the complaint of painless swelling of right scrotal contents. Microscopic findings were both malignant lymphoma of the testis, large cell and immunoblastic type in case 1 and diffuse mixed, small and large cell type in case 2 (Working Formulation, 1982). Biomedical and radiographic examinations revealed no apparent involvement otherwise. For the period of 6 months in case 1 and 2 months in case 2 following radical orchiectomy with postoperative irradiation (Lineac 4,000 rad), the patients have been doing quite well without any clinical evidence of recurrence. The literature is also reviewed briefly.

[Secondary penile tumors: report of two cases].

Two cases of secondary penile tumor are reported. The first was a 69-year-old man with the complaint of continuous painful penile erection. He had been diagnosed to have left lung cancer, squamous cell carcinoma, and was treated with chemotherapy as well as irradiation 10 months previously. He underwent amputation of penis and histopathologically diagnosed to have penile metastasis from the lung cancer. The second was a 60-year-old man who had been treated by Miles' operation due to rectal cancer, adenocarcinoma, 24 months previously. Autopsy demonstrated continuous invasion in a corpus cavernosum of rectal cancer which had locally recurred. We reviewed and discussed briefly 74 cases with secondary penile tumor collected from the Japanese literature.

[Distribution of cefotiam dihydrochloride to the bloodless lung].

The concentration of cefotiam (CTM) in the serum and the bloodless lung with time is discussed. Five patients who were undergone pneumonectomy or lobectomy, were given 1 g of CTM intravenously during the operation. Blood samples and lung samples were collected 15, 30, 60, 90, 120, and 300 minutes after the administration. The CTM levels in the blood and the lung samples were measured by the agar well bioassay. The CTM levels in the lung samples were modified as the CTM levels in the bloodless lung by calculating the contained blood volume, as lung samples were proven to have a mean value of 34.2% of blood in weight. The serum CTM level was 73.3 micrograms/ml at 15 minutes after injection and then decreased gradually. The CTM level in bloodless lung was elevated during the first 60 minutes, became equal to that of the blood during next 30 minutes and then exceeded the serum levels thereafter. These results indicate that the distribution of CTM to the bloodless lung was excellent and a satisfactory preventative effect against postsurgical infection could be expected.

[Effect of cefaclor on urinary tract infections (the third report)].

The third part of our 3-years' continuous investigation on the clinical effect of cefaclor (CCL) against urinary tract infections was reported herein. CCL, a daily dose of 0.75 g t.i.d., has been applied for the treatment of (I) 69 cases with the uncomplicated acute cystitis in the women, and (II) 44 cases with the complicated. Rates of effectiveness obtained were 94.2% in (I), which was a little lower than the first or the second part of our report, and 70.5% in (II), which equaled the mean value of our first and second reports. Any definite difference was not obtained between the effectiveness rates in those who had ever been treated with CCL and those who not, in both (I) and (II) groups.

[Ectopic ureteral opening in four males: including a case of inverted Y ureteral duplication].

Four males with ectopic ureteral opening are reported herein. Case 1 was a 17 year old who complained of miction pain and macroscopic hematuria. Cystoscopy and radiological examinations showed left ectopic ureteral opening into the seminal vesicle associated with left renal agenesis. The left ureter and seminal vesicle were extirpated. Case 2 was a 21 year old who complained of lower abdominal pain. On physical examination, a child's head sized mass was palpable in the midline of the lower abdomen. Operation was performed under diagnosis of intrapelvic tumor, but the mass was cystic dilatation of left ureter which opened into the seminal vesicle. Case 3 was a 19 year old who complained of right CVA colic pain. On cystoscopy, the right ureteral orifice was absent. During the operation, right ureter was found to open into the posterior urethra. Case 4 was a 57 year old who complained of fever. Plain X-ray on the pelvic cavity showed a 82 X 10 mm calcified shadow. CT revealed a right ectopic ureteral opening into the posterior urethra with a ureteral stone in it. On cystoscopy, the right ureteral orifice was identified and pus discharge was observed to flow out of it. Operative exploration demonstrated that the right ureter was inverted Y duplication; one opened into the posterior urethra and the other into the trigone. Seventy nine males with ectopic ureteral opening and 3 with inverted Y ureteral duplication from the Japanese literature are reviewed briefly.

Effect of high doses of synthetic estrogen on lipid metabolism in castrated male rats.

The mechanism by which high doses of estrogen influences lipid metabolism was studied with a microtubular blocking agent. Castrated male rats received oral injection daily for 14 days of 3 mg hexestrol in olive oil, or oil alone as controls. About half of the animals in each group were injected intraperitoneally with 4 mg/100 g body weight colchicine 3 hr before they were killed. Hexestrol treatment caused an accumulation of esterified cholesterol in the liver while it decreased those in serum. Triglyceride concentrations slightly decreased in the liver but were unaffected in serum. On polyacrylamide-gel disc electrophoresis, the peaks of high density lipoproteins (HDL) and low density lipoproteins (LDL) were decreased remarkably. Electron microscopic examination of hepatocytes revealed electron-lucent lipid droplets in the cytoplasm. After a colchicine treatment of the control animals, concentrations of esterified cholesterol and triglycerides markedly increased in the liver, while those in serum decreased. Electron microscopic examination of hepatocytes revealed numerous secretory vesicles filled with nascent VLDL. In hexestrol-treated animals, the colchicine treatment was associated with marked decreases in serum-esterified cholesterol and triglyceride as seen in the controls. However, there were no further increases of esterified cholesterol in the liver, and the increase of triglycerides was slight. Electron microscopic examination showed less secretory droplets than in the controls. These data suggest that very low density lipoproteins (VLDL) synthesis in the liver of hexestrol treated rats was inhibited. An accumulation of esterified cholesterol with a marked decrease in serum could not be accounted for by the inhibition of lipoproteins secretion, but rather by their enhanced entry into the liver.

Abnormal persistence of the right vitelline vein.

The authors describe a case of intestinal obstruction due to a mesodiverticular band, in which both vitelline artery and vein remnants were affirmed to be present histologically. Reviewing case reports in literature, it was revealed that the vein found in the band was the right vitelline vein remnant and that this was an important fact which proved the hypothesis that the superior mesenteric vein derives from the right vitelline vein.
