[Significance of perioperative nutritional support and activation of the reticuloendothelial system on the resection of the cirrhotic liver].

The cirrhotic patients were in poor nutritional condition and deteriorated reticuloendothelial function, which were further aggravated after hepatic resection. Preoperative nutritional repletion treatment as well as preoperative administration of OK-432 improved the nutritional condition and reticuloendothelial function of the patients, resulting in uneventful postoperative courses. In experimentally induced liver cirrhosis, however, an excessive amount of glucose administration in the early postoperative period induced the reduction of hepatic energy charge and ATP content. An adequate amount of glucose should be administered in a period associated with marked glucose intolerance. To determine daily glucose disposal rate rapidly, blood glucose curve obtained from intravenous glucose tolerance test (IVGTT) and insulin tolerance test (ITT) was analyzed in hepatectomized cirrhotic rats. It was possible to calculate prospected values of glucose disposal rate by the analytic index of IVGTT and ITT. An adequate perioperative nutritional support and the activation of the reticuloendothelial system are thought to have important therapeutic value to prevent complications of liver cirrhosis after resection.

[Allotransplantation of islet endocrine aggregates].

Dissociated pancreatic islets form aggregates from single cell suspension by rotation culture. Islet cell aggregates, or neoislets, newly formed from Lewis islet cells were transplanted into diabetic ACI rats intraportally and prolongation of neoislet grafts was observed (mean survival time greater than 45.8 +/- 6.4 days). Five out of seven recipients were still normoglycemic at 60 days after transplantation. Transplantation of neoislets combined with 3 day cyclosporine therapy for recipients resulted in 100% survival at 60 days after transplantation. These findings indicate that neoislets can provide endocrine constitution for streptozotocin diabetic rats and enjoy prolonged survival. As predicted by cell-cell recognition in rotation mediated aggregation, neoislets can exclude mesenchymal cells which bear Ia antigen. Therefore reduced immunogenicity is accomplished.

[Neural regulation of auto-grafted islets of Langerhans].

The possibility of vagal reinnervation to intrasplenic islets grafts was examined by measuring portal insulin response to electric stimulation of the dorsal vagus nerve in autografted dogs. Grafted islets responded appropriately to methacholine given intravenously. However, no insulin secretory response could be observed in grafted dogs after vagal stimulation, which markedly increased portal insulin levels in control dogs. Therefore, intrasplenic grafted islets are not under direct vagal control. At the basal state, normal oscillatory release of insulin was observed in grafted animals, suggesting that the mechanism of rhythmic basal insulin release is intrinsic to the islet of Langerhans with no regulatory input from the vagus or any element of normal pancreatic structure.

[Experimental study of the intra-operative radiation therapy in pancreatic cancer].

The radiosensitivity of pancreatic cancer, optimum dose of irradiation and the effect of 1-[(4'-Hydroxy-2'-Butenoxy) Methyl]-2-Nitrosoimidazole (RK-28) on irradiation were investigated using an experimental pancreatic cancer of hamster and the following results were obtained: i) The mean lethal dose (Do) and the 50% tumor control dose (TCD50) against the pancreatic cancer were 3.5 Gy and 73.7 +/- 6.9 Gy, respectively. These results indicate that the pancreatic cancer is resistant to irradiation, which could be explained by the existence of hypoxic cells consisting of 35% of the tumor. ii) The dose of intraoperative irradiation (10-40 Gy) seemed to be insufficient to bring long-term anti-tumor effect and long-term survival since that dose resulted in only temporary regression of the tumor. iii) The hypoxic cell sensitizer (RK28), which is known to specifically enhance the sensitivity of hypoxic cells to irradiation, lowered TCD50 of the pancreatic cancer to 53.8 +/- 1.57Gy. Therefore, RK-28 was effective in the treatment of the experimental pancreatic cancer (the enhancement ratio: 1.37). When combined with 30 or 40 Gy of irradiation, which is applicable to intraoperative irradiation, RK-28 induced a longer period of tumor suppression and a higher tumor regression ratio than irradiation alone. These results indicate that RK-28 significantly increases the effect of intraoperative irradiation and this combination therapy could possibly induce remarkable effect on tumor regression and long-term survival.

[Size and configuration of the functional aortic root after the arterial switch operation for complete transposition of the great arteries].

Twelve patients with complete transposition of the great arteries who had arterial switch operation were investigated by postoperative cineangiogram to assess the size and configuration of the functional aortic root. Three patients underwent Damus-Kaye-Stansel operation and the remaining nine received Lecompte modification of Jatene operation. Aortogram showed trivial or mild aortic regurgitation in six patients who underwent Jatene operation but no regurgitation was detected in patients after Damus-Kaye-Stansel operation, arterial switch operation without coronary relocation. The patients were divided into two groups according to the aortographic findings: AR(-); patients with competent aortic valve, AR(+); patients with incompetent aortic valve. Comparison was made between these two groups, measuring new aortic root diameter at three levels. The systolic diameter of distal aortic root was significantly enlarged in AR(+) group as compared to AR(-) group. These results suggest that the relocation of the coronary arteries and the dilatation of aortic root may contribute to aortic regurgitation after Jatene operation.

[Nuclear DNA pattern and various prognostic factors of the gallbladder cancer].

Nuclear DNA patterns which was obtained by the Feulgen staining and microspectrophotometric measurement in the gallbladder cancer were analyzed. DNA score was determined by the peak ploidy unit and decided numbers of frequency in each ploidy unit and compared with the various factors influencing prognosis of the patients. DNA score was significantly lower in cancer with papillary form and limited involvement to the mucosa compared with that with other forms and involvement beyond the proper muscle layer. DNA score was also significantly lower in cancer without hepatic infiltration, vascular invasion and lymph node metastasis than that with those characteristics. In experimental studies VX2 cancer cells which were implanted in the gallbladder subserosa in rabbits, DAN score did not show any changes with the growth of carcinoma implanted. Biologic nature of the tumor which was evaluated with nuclear DNA pattern of cancer cells was fundamental to define the spread of carcinoma and the prognosis of the patients.

[Experimental study on the pathogenesis of pulmonary insufficiency in acute pancreatitis and changes in the pulmonary surfactant].

Serious pulmonary complications are often associated with acute pancreatitis. The destruction of pulmonary surfactant by the action of pancreatic phospholipase A2 (PLA2), together with pulmonary edema, is considered an important etiopathogenic factor of acute respiratory insufficiency. This experimental study was undertaken to elucidate the destruction of pulmonary surfactant in acute pancreatitis using the lung pressure volume curve (P-V curve). Acute hemorrhagic pancreatitis was induced in mongrel dogs by a retrograde injection of Na-taurocholate into the main pancreatic duct. Pulmonary surface tension was measured by P-V curve and the effect of PLA2 on pulmonary surfactant was assessed by the ratio of lysolecithin and lecithin, which are essential components of pulmonary surfactant (Ly/Le) in lung wash. Extravascular lung water volume (Ww/Dw) and blood gases were also measured. The value of Ly/Le and serum PLA2 rose significantly from the 3rd hour. On the contrary, no significant differences were seen on P-V curve until the 12th hour but after 20 hours surface tension increased significantly. Ww/Dw and A-aDO2 increased after 3 and 12 hours, respectively. These findings, the degradation of lecithin and the elevation of surface tension accompanied with an increase of serum PLA2, suggest that pulmonary surfactant is destroyed in severe acute pancreatitis, and that the increased capillary permeability of the lung precedes the deterioration of surface tension as the cause of pulmonary insufficiency.

[The pathogenesis of perineural tumor invasion--a computer reconstruction study on its relation with a lymphatic spread in human and experimental carcinomas of the bile duct].

The way in which tumors reach the perineural space of the nerves from adjacent tissues has been studied by a computer 3-D reconstruction of a bile duct wall invaded by a carcinoma, from the case histories of two patients with a bile duct carcinoma and from rabbits given a VX2 tumor that had been transplanted. It was found that while carcinomas readily grew along the perineural spaces in an axial direction these carcinomas also had, simultaneously, abundant connections with tumors growing outside, especially with lymphatic tumors. Thus it was likely that tumors reach the distant nerves mainly via this lymphatic connection, i.e., first forming satellite lymphogenous foci around the nerves and then, as a second step, breaking into the perineural space.

Pancreaticopleural fistulas.

Of ten patients with pancreaticopleural fistulas, who ranged in age from 28 to 62 years, eight had primarily thoracic symptoms and no abdominal symptoms. The other two had abdominal pain after acute pancreatic injury. Computed tomographic scans disclosed pseudocysts in nine patients. The effusions resolved in four of the ten patients with transthoracic drainage, although two of these patients later required surgery because of recurrent pseudocysts. One patient died 17 days after repair of an abdominal aortic aneurysm; this patient had been treated by tube thoracostomy. Five patients underwent surgery; in four of them pancreatography was used to delineate the site of disruption preoperatively. These patients all survived, and none required subsequent surgical treatment.

Malignant insulinoma with metastasis to gallbladder and bone, accompanied by past history of peptic ulcer and hyperthyroidism.

A case of a malignant insulinoma in a 53-year-old female is presented. In 1973, the patient underwent caudal pancreatectomy for a malignant insulinoma. Ten years later, it was discovered that the insulinoma had spread to the bones. On admission for cholecystectomy because of a gallbladder polyp and gallstones, she often experienced hypoglycemic attacks, and both calcium and glucagon provocation tests elicited marked release of insulin. Selective angiography of the common hepatic artery showed a tumor blush near the hilum of the liver. Immunohistochemical staining of the gallbladder polyp and the bone tumors proved positive for insulin. Plasma levels of insulin and prolactin were abnormally high. The patient had also been treated for a perforated duodenal ulcer and hyperthyroidism. It is concluded that this may have been a case of a multiple endocrine neoplasia.

Cold-adaptation of human rotavirus.

A human rotavirus strain was cold-adapted for possible future use as a live vaccine. The original strain was isolated in 1980 in primary cynomolgus monkey kidney cells and has a serotype I and subgroup II antigenicity. The virus was serially passaged in African green monkey kidney cells; it was cultivated at 37 degrees C at the first stage of passages, and the cultivation temperature was then shifted down stepwise by 3 degrees C per each 10 passages. Finally the virus was passaged 10 times at 25 degrees C (total passage number of 55). The virus formed small-size plaques with irregular shaped borders at 31 degrees C. Growth at 25 degrees C of the cold-adapted virus was higher than that of the original virus. There was no difference between the migration patterns of 11 dsRNA segments in polyacrylamide gel electrophoresis of the original and the cold-adapted viruses.

The daily profile of ventricular premature beats in the patients with ischemic heart disease.

To clarify factors affecting the prevalence of ventricular premature contraction (VPC) in patients with stable ischemic heart disease, daily profiles of VPC prevalence were studied in 92 patients using continuous 24-hour electrocardiographic recordings. VPCs in patient groups with effort angina and inferior infarction showed similar daily profiles. However, a significant relationship between VPC prevalence and heart rate was observed in inferior infarction group, while patients with resting angina exhibited another relationship between the two parameters. Higher prevalence rate of VPCs in the early morning in patients with resting angina suggested the contribution of cardiac autonomic nerve activity to the increased frequency of VPCs, while constant frequency of VPCs during the day in anterior infarction group provided no influences on heart rate, effort, and autonomic nerve activity on the VPC prevalence. Age, sex, left ventricular ejection fraction, and nitrates and diltiazem used for the treatment of angina pectoris did not affect the daily profile of VPC prevalence in any patient groups. These results indicated that the clinical and pathophysiological differences of ischemic heart disease, even in stable patients, showed the different distribution of VPC prevalence within a day which is due, in part, to the varied activities of cardiac autonomic nerves.

[Evaluation of pN factors in patients with primary lung cancer by using perfusion, inhalation and ventilation studies].

The interpretation of scintigraphic patterns and the role of pathophysiological mechanisms in patients with primary lung cancer were investigated. To determine the relative roles of perfusion, inhalation and ventilation scintigraphy, the relationship between the count ratio of the affected side to the healthy side and the post-surgical histological lymph nodes factors were observed in this study. These scintigraphic count ratio's in patients with primary lung cancer did not reflect the pN factors except in the perfusion study in patients with hilar primary lung cancer.

Prognostic factors in the surgical treatment of gallbladder carcinoma.

A clinical and pathologic review was undertaken of 36 patients with primary carcinoma of the gallbladder who could tolerate curative resection. The 5-year survival rate of the patients with tumor exposed to the serosa (n = 17) was significantly lower than that of the patients with tumor limited to the mucosa (n = 7), muscularis (n = 7), or subserosa (n = 5). Macroscopically, all tumors limited to the mucosa and 6 of 7 limited to the muscularis were of the papillary form, which was associated with a better patient survival (p less than 0.05) than the nodular or infiltrative form present in 11 of 17 tumors that penetrated to the serosa. The papillary or well-differentiated carcinoma--present in all patients with tumors superficial to the gallbladder subserosa--had a higher survival (p less than 0.001 and p less than 0.01) than the moderately or poorly differentiated carcinoma present in 7 of 17 tumors with spread to the serosa. Incidences of venous, lymphatic, or perineural invasion were higher in tumors that spread beyond the serosa than those limited to the mucosa (p less than 0.05), and survival differed between patients with and without these characteristics (p less than 0.001). Analysis for nuclear DNA pattern of those tumors limited to the mucosa revealed its lower cellular malignancy. Patients with tumors superficial to the subserosa survived longer after undergoing extended cholecystectomy than those who underwent simple cholecystectomy (p less than 0.05). The prognosis of gallbladder carcinoma was highly influenced not only by the biologic nature of tumor but by the surgical procedures used.
