[Chlamydia trachomatis pneumonia in childhood. An underestimated problem?].

The study of infections due to Chlamydia trachomatis in children has advanced remarkably during the past ten years. It has been established that Chlamydia trachomatis is a major etiologic agent of conjunctivitis in newborns and genital infections among sexually active persons. Chlamydial pneumonia has come to be recognized as one of the most common forms of pneumonia during the first three months of life. The Authors reported a case of a child admitted to Bambino Gesù Hospital of Rome, affected by Chlamydia trachomatis afebrile pneumonia. The diagnosis was made on the basis of direct and indirect laboratory findings. The remarkable improvement in direct and indirect diagnostic techniques can help the clinicians in diagnostic and therapeutic evaluations.

[Neurological complications in drug dependence with special reference to the development of epileptic syndromes].

33 drug abusers were examined and all of them were addicts to several drugs and always to heroin. Neurological complications occurred rather frequently. One patient had subarachnoid hemorrhage and angiography showed no arteriovenous malformations nor other pathology. Polyneuropathy has been found in one case. 7 patients (21,2%) developed psychotic episodes. Epileptic attacks were found in 5 cases (15,1%). Latency between the onset of drug-abusing and occurrence of the attacks was found to be at least 1 year in two cases and 3 years in the other three. 4 cases suffered from partial complex seizures with secondary generalization and one patient had generalized tonic-clonic attacks. Fits occurred rarely in all cases; one patient had two episodes of partial status. Further research is required because there are few clinical reports in comparison with the experimental ones.

[Acute volvulus of the sigmoid colon].

Seventeen cases of acute volvulus of the sigmoid colon are presented. Comparison with other series showed that the mortality rate for fixation with or without a shunt are more or less the same. Excellent results were obtained with the Paul Mikulicz method. An intention to use this method in all cases in the future, irrespective of the patient's local and general condition, is expressed.

[Pseudorheumatoid nodules in childhood. A case report].

Subcutaneous nodules of the rheumatic type can occur in patients without other evidence of rheumatic disease. Nodules are characterized by subcutaneous location with predilection for pretibial regions and scalp, occasional large size, spontaneous regression and frequent recurrence. Sometimes lesion biopsy may be usefull for confirmation of the clinical diagnosis; etiology is unknown, therapy is unnecessary. One asymptomatic child with benign rheumatoid nodules is described by the Authors.

[Chronic progressive obliterating arteriopathy and muscle fatigue: the role of frequency analysis of the electromyographic signal].

Clinical evaluation of patients with peripheral arterial disease (PAD) is mainly based on Fontaine's classification, in which pain is the most important element. The use of objective techniques can allow a more precise evaluation, since pain is always subjective. In PAD patients, pain is frequently associated with early fatigue. Peripheral muscle fatigue can be easily assessed by a particular electromyographical method, based on the frequency analysis of the signal. In PAD patients, this method can provide objective information on muscular performance, with possible uses in medicine, surgery, rehabilitation and occupational medicine.

Effect of plasma exchange in a patient with prolonged deep coma from thrombotic thrombocytopenic purpura.

The clinical, laboratory and instrumental data of a patient with thrombotic thrombocytopenic purpura (TTP) are reported in detail, with reference to an articulated therapeutic regimen including plasma exchange, high-dose methylprednisolone and dipiridamole. Particular emphasis is placed on the dramatic improvement obtained with this treatment despite a very delayed diagnosis and prolonged, severe neurological involvement. A short discussion and review of the literature concerning the role of plasma-exchange in the treatment of TTP is reported.

[Patient-controlled postoperative analgesia in orthopedic surgery: epidural PCA versus intravenous PCA].

OBJECTIVE: To evaluate both effectiveness and incidence of side effects of two techniques of postoperative pain treatment: intravenous and epidural PCA. DESIGN: Prospective analysis of data from two groups of randomized patients. SETTINGS: Orthopedic and trauma center. PATIENTS: Figty ASA class II-III patients undergoing total hip replacement under combined Spinal-Epidural Anesthesia. METHODS: Patients were divided into 2 groups who received different postoperative pain treatment. One group (group PCA) received a patient-controlled intravenous analgesia with morphine 30 mg and ketorolac 90 mg in 100 ml of saline (back-ground infusion 2-4 ml, according to body weith, bolus 1 ml, lockout 5 min, 4 h dose limit 40 ml). PCEA group received a patient-controlled epidural analgesia with morphine 4 mg and bupivacaine 0.125% 100 ml, (background infusion 3-4 ml, according to patient' height, bolus 1 ml, lockout 10 min, 4 h dose limit 25 ml). Postoperative pain intensity was evaluated, through 24 postoperative hours, by a verbal analogue scale (VPS = 0 to 3) and a total pain score (TOTPAR) was calculated for each patient at 6 and 24 postoperative hours. Side effects were recorded and their incidence was obtained for each group. Statistical data analysis was performed by one-way ANOVA and non-parametric tests for ordinal data. Nominal data were analyzed by chi 2 test. p < 0.05 was considered significant. RESULTS: Patient receiving PCEA showed a significant (p < 0.005) decrease of incident pain, while VPS at rest was similar in the two groups. TOTPAR VPS was lower (p < 0.05) in PCEA group both at 6 and 24 postoperative hours. Somnolence was observed more often in PCA patients (8% vs 2%; p 0.05), while no significant differences were noted among other side effects incidence. CONCLUSIONS: Our data show a better control of postoperative pain arising from total hip replacement during PCEA when compared to PCA. It should be emphasized that incident pain is far more decreased by PCEA, so that this technique is particularly indicated when an early postoperative mobilization is required.

[Reoperations in surgical oncology: recurrence of colon carcinoma after surgery].

Colon cancer is the second leading cause of death for cancer disease, after lung cancer, with nearly 18,000 deaths per year in Italy. In spite of the progress that have taken place over the past 30 years, little improvement has been gained in this dismal outcome, and the 5-year survival remains around 50%. Over one half of the patients will suffer from recurrence after a potentially curative resection. A major challenge lies in better detection of recurrences in order to diagnose those patients still amenable to curative resection. Locoregional recurrence is of particular interest and its frequency, diagnostic limitations and surgical treatment are herein discussed.

Direct demonstration of cytokeratin filaments by electron microscopy in K562 cell lines in liquid culture.

Our previous study suggested the presence of cytokeratins in the supernatant of human K562 erythroleukemic cell line. In this study we confirm, by using an electron microscopy technique, that K562 cells contain typical intermediate tonofilaments with the characteristics of cytokeratins. After variable intervals of liquid culture, K562 cells have been examined for their clonogenic ability by immunostaining and ultrastructural study. K562 cells showed variable amounts of medium-sized filaments (intermediate filaments) of 10 nm mean size, having the submicroscopic pattern consistent with tonofilaments, arranged as electron-dense curve-shaped bundles, in perinuclear position independently from the different growth phases. A pool of monoclonal antibodies against cytokeratins confirmed the presence of cytokeratins in immunostaining, while the cytofluorimetric analysis showed an unexpected positivity of the CD34 antigen, associated with the typical adhesion molecule VLA5. In conclusion, the immunostaining with monoclonal antibodies and the ultrastructural findings suggest the expression of epithelial features in human K562 leukemic cells.

[Surgical treatment of tumors of the ampulla of Vater by local resection].

The cases of six patients suffering from Vater's ampulla neoplasia were studied 1981 and 1995. All cases are described both clinically and in terms of diagnostic techniques. All patients underwent a transduodenal excision of the neoplasia. A number of factors provided an indication for ampullectomy: the extremely high surgical risk, the suspected benign nature of the lesion or the very small size of the neoplasm associated with no pre-surgery finding of lymphadenomegaly in the area. The following issues were analysed: the accuracy of results obtained by a correct pre-surgery diagnosis, short and long term complications and mortality related to surgery. All patients received a five years post-surgery follow-up. Results show that the ampullectomy can be considered, in selected cases, as a valuable alternative to pancreatoduodenectomy even if further and wider studies are necessary in order to obtain an accurate definition of the correct pre-surgical diagnostic approach and surgical indications.
