BACKGROUND: Currently, pancreatic exocrine carcinoma presents high mortality, poor survival after curative surgery and poor resectability rates at the time of diagnosis. The factors which mostly influence prognosis and therapeutic management (curative surgery or palliative treatments) of the patient affected by pancreatic carcinoma, particularly the peroperative stage, are analyzed and discussed in this article. METHODS: From 1969 to 1997, 142 patients with pancreatic ductal carcinoma were admitted to our Department: at the time of diagnosis only 32 patients (22.5%) were considered resectable, and 30 pancreaticoduodenectomies (PD), 1 distal pancreatectomy (DP) and 1 total pancreatectomy (TP) were performed. RESULTS: Postoperative morbidity and mortality was 53.1% and 12.5% respectively; the survival at 1, 3 and 5 years was 45.5%, 36.4% and 17.6% respectively. The worst prognosis was seen in N+ and T4 patients, with a mean survival of 9 and 10 months. CONCLUSIONS: On the basis of these results and of the literature, the indications for curative surgery, the operative strategy and the lymphoadenectomy extension are discussed: these problems are still open and not resolved definitively. The authors conclude by indicating the need for curative surgery for T1/2 N0 M0 tumors: for T3/4 and/or N+ tumors a careful evaluation of single case is necessary, because of high risk/benefit rate. Pancreatic resections (PD, DP) and standard lymphoadenectomy (D1) are performed by the authors, rather than total pancreatectomy and radical lymphadenectomy (D2-3).

Emphysematous patients that were once treated with double lung transplantation can now also be treated with single lung transplantation. However, single lung transplantation in emphysematous patients presents problems in lung size matching, choice of side to transplant and post-operative assistance. Analysing their own experience of three single lung transplants performed on emphysematous patients, the Authors evaluate the effectiveness of the operation, the results and the difficulties encountered. Single lung transplantation is a good therapeutic option for end-stage emphysematous patients. In these patients right lung transplantation is preferable and the donor organ should be oversized. Positive and expiratory pressure in the native emphysematous lung should be applied with extreme caution.
Local recurrence (LR) is a major problem following curative resection of rectal cancer. Intraoperative radiation therapy (IORT) is considered an ideal boost technique for increasing the dose of radiation therapy within a restricted area without introducing a significant toxicity. The aim of this study is to present the results of a multimodality treatment containing external beam irradiation, chemotherapy, surgical resection, and IORT delivered by a movable linear accelerator (NOVAC7, Hitesys SpA, Italia), employed in a "traditional" operating room.

The authors report on a case of disseminated Langerhans' cell histiocytosis with a clinical presentation and a bone marrow simulating, at onset, an acute leukemia non lymphoid. A hepatic needle biopsy performed for the progressive enlargement of the liver oriented the diagnosis towards a Langerhans' cell histiocytosis. The morphological, immunohistochemical and ultrastructural study of these cells showed them to be undetermined, i.e. Langerhans' cell precursors.
External plasty of the pre-ostial valve is a surgical technique which has the aim of correcting reflux resulting from incompetence of the sapheno-femoral valve and at the time retaining the anatomical structure of the region. The artificial stenosis produced by external valvuloplasty of the pre-terminal valve complex has been studied in physical experiments, the first results of which already appear to be significant. The authors have operated upon 18 patients, with follow-up too short for the presentation of final results, but already sufficient to permit discussion of findings after a short period of time.
Using an electrode selective for the ion fluorine, it has been determined the amount of fluorine in samples of human milk. In a sample of 35 nursing mothers, only 10 have shown the required parameters. The amount of fluorine in the water all 35 mothers imbibed in their daily diet has been measured. The results showed a small amount of fluorine in human milk and a possible connection between the amount of fluorine in the mothers' daily diet and the amount of fluorine in human milk. From the results of the research it has been determined the necessity to give supplements of fluoride to nursing babies.
Changing attitudes to animals in research and practical considerations prompted the authors to evaluate whether the pig might be a suitable substitute for dog and baboons for single left lung transplants. Twenty-nine paired pigs were used. The first transplants on 13 pairs (group 1) were done to adapt the lung transplant technique to pigs; later transplants on 16 pig pairs (group 2) were done to evaluate operative survival, and function and histological modifications of the transplanted lung in the absence of immunosuppressive treatment. Surgical and anesthetic techniques for both donor and recipient are described in detail. The survival rate in group 2 was 68%. Hemodynamic and blood gas changes were assessed during operation. PaO2 did not drop significantly after occluding the right pulmonary artery by an inflatable cuff placed around it; this suggests that the function of the transplanted lung was preserved. The pigs were put down on the third postoperative day. Vascular and bronchial anastomoses were patent and intact, but the transplanted lung was macroscopically and microscopically altered. Lung transplants can be performed in pigs and the transplanted lung seems to be capable of functioning immediately after the operation. Alteration in the lung after 3 days is probably due to rejection.
The authors describe an experimental study in man, using electron microscopy to study the effect of sclerosing injections on the great saphenous vein, in particular concerning the initial phases of the process. Results may be summarised as follows: The endothelial lesion develops immediately and is visible only by electron microscopy during the first 15 minutes. It involves necrotic changes with subendothelial edema and, finally, detachment of the endothelial cells. This process is complete on average 2 hours and 30 minutes after the injection. The first deposits of fibrin, which are not abundant, englobe blood cells and loosen already within the first 15 minutes. The first lamellar platelet microclot forms on average 2 hours and a half after the injection but this involves a microclot and not a thrombosis which will become organised. The first massive thrombosis seen occurred between the second and third day after injection and involved a recent thrombus.

The authors report their experience related to a series of 96 consecutive central venous catheters location through subclavian, right internal jugular, basilic and cephalic veins for TPN administration. Because of the specific complications reported, they are in favour of the trans-basilic peripheral approach for a short term TPN; the internal jugular or the subclavian way are indicated for long term ones. In addition, they stress the importance to limit the use of multilumen catheters just when absolutely necessary, due to the increased infection percentage.
