p53 protein expression and prognosis in gallbladder carcinoma and premalignant lesions.

BACKGROUND/AIMS: p53 protein expression in gallbladder carcinoma has recently been detected by immunohistochemical techniques, but the relationship between p53 expression and prognosis or clinico-pathological factors is still obscure. MATERIALS AND METHODS: We investigated 48 gallbladder carcinoma, 7 adenoma and 11 dysplasia cases for p53 expression by immunohistochemical techniques. RESULTS: p53 expression was positive in 39.6% of gallbladder cancer cases, but in no adenoma or dysplasia cases. No significant correlation was found between p53 overexpression and prognosis or recurrence in 20 patients with carcinoma involvement up to the subserosal layer. p53 overexpression was correlated with DNA aneuploidy pattern and the absence of stones, but was not correlated with clinical staging or lymph node metastasis. CONCLUSION: These results suggest that p53 gene mutation is related to the transition from premalignancy to malignancy in gallbladder carcinogenesis, as well as DNA ploidy alterations and carcinogenesis unassociated with gallstones, but has no bearing on the prognosis.

The effect of pulsing electromagnetic field on bone ingrowth into a porous coated implant.

Electromagnetic stimulation is known to promote bone formation in cases of fracture and pseudoarthrosis. The possibility of using electromagnetic field to promote bone ingrowth into a porous coated implant was investigated in this study using Japanese albino rabbits. A titanium implant coated with beads of 250-300 microns in diameter around a stem of 1.7 mm in diameter was inserted into the humerus cavity of a Japanese albino rabbit. The humerus was stimulated by pulsing electromagnetic fields at 2 Gauss, 10Hz, 25 microseconds for 14 days. The circumference of grown bone into the implant was measured, and calculated in area equivalent for determination of the area of new bone. Bone ingrowth was significantly promoted by consecutive 14 days. The result suggests that electromagnetic stimulation is useful for achieving further bone ingrowth into a porous coated implant.

IgE in respiratory tract allergies.

The IgE concentration was determined in the sera and nasal secretions of 166 patients with nasal allergy, 40 patients with bronchial asthma, and 50 control subjects, using Phadebas IgE kits. The IgE level was significantly elevated in both the sera and nasal secretions of allergic patients. The sera and nasal secretions of 142 patients, who were positive in HD or mites skin test, were subjected to a radioallergosorbent test (RAST) for estimating the specific IgE antibody activity to mites. The gradings of the specific antibody activity were significantly correlated with the strengths of the HD skin tests. The results of this study proved the usefulness of the determination of IgE and specific IgE antibodies in the diagnosis and study of the pathogenesis of respiratory tract allergies. The IgE in nasal secretions was much more closely related to pathogenesis of respiratory allergies than was IgE in sera. However, unless more concentrated nasal secretions are assayed, the diagnostic value is limited.

Multiple hepatic cysts along the intrahepatic bile duct--case report.

A case of multiple hepatic cysts of the periductal gland located along the left intrahepatic bile duct is described. Ultrasonography and computed tomography disclosed many cystic lesions along the left portal vein in the left lateral segment of the liver. Percutaneous transhepatic cholangiography showed many compressed lesions. The resected specimen revealed multiple cysts of 2-7mm in diameter along the intrahepatic bile duct. Microscopically, cysts within the large Glisson's capsule were intermixed with lobuli of the periductal glands, thus suggesting periductal gland origin. Histopathological features of these cysts were similar to those of "multiple hilar cysts" reported by Nakanuma, but lack of portal hypertension and underlying chronic liver disease is the significant characteristics in this case which is different from "multiple hilar cysts".

Laparoscopic partial hepatectomy.

This article describes the surgical techniques and indications of laparoscopic partial hepatectomy, which is not as widely available as laparoscopic cholecystectomy. Three patients with hepatocellular carcinoma and associated severe liver cirrhosis were candidates for this technique from July 1993 to August 1994. The tumor size was 4 cm or less and all the tumors were located in segment 5 or 8 which had grown nodularly and protruded from the liver surface. A microwave tissue coagulator was used for parenchymal dissection under ultrasonographic guidance in a gas-less method with or without low-pressure pneumoperitoteum of 4 mmHg. The principle of dissection consists of tissue coagulation and fragmentation with dissecting forceps. Three hepatectomies were performed uneventfully without blood transfusion and the patients rapidly returned to their preoperative conditions. The laparoscopic partial hepatectomy can be an option of treatment in selected cases where the tumor can be removed by minor, superficial resection.
