[Case of double primary carcinoma of the lung and ureter].

A 73-year-old man visited our hospital complaining of chest distress in September 1979. A chest X-ray film showed a tumor shadow in the left lung and left inferior lobectomy was performed. The tumor measured 50 X 40 X 30 mm; it was diagnosed as large cell carcinoma. In November 1980 the patient suddenly noticed macroscopic hematuria although there had been no microscopic hematuria at his first visit. Ultrasound-guided percutaneous pyelography confirmed complete obstruction of the left ureter and he underwent left ureteronephrectomy. The papillary tumor, measuring 23 cm, was diagnosed as transitional cell carcinoma.

Immunohistologic detection of antigen related to primate type C retrovirus p30 in normal human placentas.

This study reports the immunohistologic detection of SSAV/GaLV type C retrovirus p30-related antigen in unfixed cryostat sections of normal human term placentas by the indirect immunofluorescence method. Goat anti-SSAV p28 serum reacted specifically with 10 of 10 anatomic specimens of human placenta. Goat anti-GaLV p29 serum reacted similarly with 8 of 10 specimens. Goat anti-BaEV p28, anti-RD-114 p28, anti-FeLV p27, anti-R-MuLV p30, and anti-MPMV p27 gave no specific reaction with placenta. The anti-SSAV p28 and anti-GaLV p29 reactive antigen was located in the placenta mainly at the basal aspect of syncytiotrophoblast near the underlying trophoblastic basement membrane where type C retroviruslike particles have been found electronmicroscopically. The specific antibody activity of anti-SSAV p28 serum against placenta was removed by critical absorption with disrupted SSAV or GaLV but not RD-114 or MuLV. These results suggest the presence in human placenta of a putative type C retroviral protein which cross-reacts with the p30 protein of the SSAV/GaLV type C retrovirus group.

Four cases of t(4;11) acute leukemia and its myelomonocytic nature in infants.

Four cases of acute infantile leukemia with translocation (4;11) (q21;q23) are reported. Although leukemia with this chromosomal abnormality has been classified as L2 acute lymphoblastic leukemia by the FAB classification, two of our cases appeared to be of myelomonocyte origin as demonstrated by cytochemical, immunologic, and electron microscopic studies and differentiation induction by 12-tetradecanoyl-phorbol-13-acetate and methylformamide. This chromosomal change is associated with poor prognosis.

Immunologic aspects of otitis media with effusion.

In order to understand the mechanism involved in otitis media with effusion (OME), we have studied middle ear effusions (MEE) immunochemically, and experimental otitis media in guinea pigs and chinchillas. The results indicated: (1) that serous MEE are a mixture of normal middle ear secretions and transudates from serum, while mucoid MEE are the results of enhanced epithelial secretory activity and the transudates; (2) that mucosal immunity acts in middle ears affected by OME; and (3) that the middle ear is an organ capable of responding immunologically to antigenic substances. However, the normal, and developing, middle ear has a poorly developed mucosal immune system. It can be said that the middle ear is a potential site of immunological activity during infection. The poorly developed immune system is thought to be one of the factors in the infection proneness of the middle ear that leads to OME. Results of IgE study of MEE failed to support the concept that allergy is a major causative factor of OME.

[A case of paraganglioma of the urinary bladder].

Herein we report a case of primary paraganglioma of the urinary bladder. A 21-year-old woman was admitted to an emergency hospital, because of gross hematuria, dysuria and complete urinary retention. Pelvic CT scan and ultrasonic examination revealed a bladder tumor and coagulated blood mass, then she was referred to our clinic for further treatment. On cystoscopy a dark brown lobular tumor was seen on the anterior wall. The bladder tumor was removed by partial cystectomy. The operative specimen measured 6 X 4 X 3 cm and weighed 50 g. Histological examination showed it to be a paraganglioma of the urinary bladder, which was chromaffin positive. Her serum and urinary catecholamine level were within the normal range after operation. She is now ten months postoperative, free from hematuria, and showing no other symptoms or signs of disease including metastasis.
