We established a diagnosis of familial amyloidotic polyneuropathy (FAP) based on DNA and demonstrated a direct link between prealbumin gene mutation and FAP. The individuals with FAP, so far examined, were heterozygous for the prealbumin gene, carrying one normal and one mutant gene. To investigate the molecular pathogenesis of FAP, we isolated a normal prealbumin gene (7 kb in length) and also a mutant prealbumin gene associated with FAP. We compared their nucleotide sequences and found that they matched except for a single-base substitution present in the second exon. In an effort to construct mouse model systems for the FAP, we developed strains of transgenic mice carrying human mutant prealbumin genes.
The effect of various opiate agonists on the contraction of isolated rat atrium was investigated. Ethylketocyclazocine (EKC) (30-100 microM) and dynorphin-A(1-13) (10-30 microM), which are kappa-type agonists, caused positive inotropic effects on electrically stimulated left atrium in a dose-dependent manner. In addition, EKC decreased the frequency of spontaneous beating in the right atrium. Morphine (mu-type), [Met5]- and [Leu5]-enkephalin (delta-type) did not affect both the developed tension and frequency of contractions. These results indicate that the positive inotropic action is specific for kappa-type opiate agonists.
Plasma lipids and lipoproteins obtained from different places of the circuit of double filtration plasmapheresis (DFP) were measured and effect of DFP therapy on removal of them was examined. When 2A was used as a second filter, 69.8%, 52.4%, 63.0%, 58.0%, 60.8%, 59.2% and 63.9%, respectively of beta-lipoprotein, cholesterol, triglyceride, high density lipoproteins (HDL), phospholipids, free cholesterol and lipoprotein (a) (Lp(a)) were removed from the patient's plasma. When 4A was used as a second filter, 69.0%, 56.8%, 53.2%, 45.4%, 56.0%, 50.9% and 51.7%, respectively of beta-lipoprotein, cholesterol, triglyceride, HDL, phospholipids, free cholesterol and Lp(a) were removed from the patient's plasma. In contrast, concentrations of free fatty acids (FFA) after DFP therapy using filter 4A and 2A increased to 222.8% and 256.4%, respectively. Thus, it was shown that except for FFA, DFP therapy using either 2A or 4A as a second filter is effective in reducing concentrations of plasma lipid and lipoproteins.

We have reported a case of malignant lymphoma in associated with Hashimoto's disease arising in the right lobe of thyroid of a 57-year-old woman. The pre-operative aspirated biopsy was valuable for diagnosis Immuno-histochemical observations revealed the localization of anti B cell antibody in the cell surface of lymphoma cells and anti thyroglobulin antibody in intactly follicular epithelium within the lymphoma tissue. This lymphoma was diagnosed as diffuse, medium cell and B cell type. Radiation and chemotherapy were performed for multiple metastasis of liver and brain, but this case had died of DIC and diffuse hyaline membrane disease of lung.
A 38-year-old male was admitted to Ikeda Municipal Hospital because of thrombocytopenic purpura and abnormal liver function. Computed tomography and ultrasonography of the liver showed a 5 by 5 cm mass in the right lobe of the liver. Hepatic angiography revealed a hypervascular tumor in the right lobe of the liver and extrahepatic portal venous obstruction. A diagnosis of hepatocellular carcinoma with extrahepatic portal venous obstruction due to tumor invasion was made. Endoscopic examination showed marked esophageal varices with red color sign. Oral administration of UFT at a daily dose of 400 mg was started. Though ascites and frequent hemorrhage from esophageal varices were observed, these symptoms disappeared completely after 8 months administration of UFT. Endoscopic examination revealed disappearance of the esophageal varices, and recanalization of the extrahepatic portal vein was confirmed by the second hepatic angiography. Computed tomography showed no growth of the liver tumor. These data suggest that UFT was effective in this case.
The effects of 1 alpha, 25-dihydroxyvitamin D3 [1,25-(OH)2D3] on proliferation and de novo DNA synthesis were studied in the following established human leukemia cell lines: lymphoblastic T-cell lines HPB-ALL, CCRF-HSB-2, p12/lchikawa, and HPB-MLT; adult T-cell leukemia- (ATL) and human lymphotropic virus type I (HTLV-I)-infected T-cell lines HUT102, HUT-102B2, MT-1, MT-2, MJ, C2/MJ, KH-2, KH-2Lo, HPB-CTL-1, and ATN-C1; ATL-derived B-cell lines ATL-BK9 and ATL-BK10; lymphoblastic B-cell line Daudi; and myelocytic-monocytic lineage cell lines HL-60 and U937. 1,25(OH)2D3 inhibited proliferation and de novo DNA synthesis of phytohemagglutinin-P-activated T-cells and certain established HTLV-I-positive T-cells. However, it did not inhibit immature lymphoblastic T- and B-cells or ATL-derived B-cells. The degree of inhibition depended on the dose of 1,25(OH)2D3 and the heterogeneity of the established HTLV-I-positive T-cells. KH-2 and subclone KH-2Lo were markedly inhibited, and HPB-CTL-1 was moderately inhibited. Marked inhibition of DNA synthesis in KH-2Lo cells was observed in the proliferative phase of the cell cycle. No inhibition of KH-2Lo proliferation or expression of interleukin-2 and transferrin receptor was noted after removal of 1,25(OH)2D3 from the culture medium. 1,25(OH)2D3 inhibited 12-O-tetradecanoylphorbol-13-acetate (TPA)-induced multinucleated cell formation of various HTLV-I-positive T-cell lines and TPA-induced HTLV-I p19 expression in KH-2Lo cells.
Rearrangements of the T cell gamma (T gamma) gene were studied in primary neoplastic cells from 75 patients with leukemia or lymphoma. T gamma gene rearrangements were observed in 19 of 21 T cell neoplasms; 14 of 21 immature B cell leukemias, including 4 out of 5 patients with rearrangements of both immunoglobulin heavy-chain (JH) and T cell receptor beta chain (T beta) genes; none out of 16 nonlymphoid leukemias. Thus, T gamma gene rearrangement is frequently found in immature B cells and is not always found in T cells showing T beta gene rearrangement, but it is not detected in nonlymphoid cells. Furthermore, T gamma gene rearrangement in cells with the germline configuration of the JH and T beta genes was observed. These results indicate that the detection of T gamma gene rearrangement does not allow a clear assignment to a particular lineage. However, an analysis of T gamma gene rearrangement provides a further potential tool to establish the lymphoid cellular origin and clonality of hematologic neoplasms and identify the normal stages of lymphocyte differentiation.
We report a case of primary psoas abscess with paravesical extension. A 16-year-old man who had suffered from remittent fever for two months was admitted to our clinic. He had no urological symptoms or walking disturbances, but ultrasonography showed a well-defined echoic mass at the right paravesical space. Computed tomography confirmed this finding, and right psoas muscle appeared to be enlarged with irregular low density areas. Percutaneous drainage for right paravesical abscess was performed, and 15 ml of pus was obtained. The bacterial and fungal culture of the specimen resulted in no growth of any organisms. Six days after percutaneous drainage, he received en bloc resection of right paravesical abscess with segmental resection of bladder wall and peritoneum adherent to it. Furthermore, the affected region of the right psoas muscle was salvaged as highly upward as possible. Surgical specimen disclosed, pathologically, a non-specific granuloma with stellate necrosis. This finding suggested a feature of cat-scratch disease, tularemia, lymphogranuloma venereum and Yersinia infection. We reviewed the related literature on diagnostic images such as CT, ultrasonography and 67Ga scintigraphy. Moreover, bacterial causes and therapy of primary psoas abscess are discussed.
