New nonsurgical technique for multiple pericardial biopsies.

We report a new, nonsurgical technique for obtaining multiple pericardial biopsies in patients with pericardial effusion. A short catheter is introduced by the Seldinger technique under fluoroscopy through the subxiphoid approach. The pericardial fluid is aspirated and the catheter replaced by an 8F teflon sheath with a curved tip. A bioptome is inserted through the sheath, and air is allowed to enter the pericardium. This method outlines the parietal percardium. The curved sheath directs the bioptome to the left or right wall. Eighteen consecutive patients had an average of eight pieces of pericardium removed. On histological examination, three patients had malignancy. Six had tuberculous granuloma, and mycobacterium tuberculosis was cultured from all six tissue specimens but only once from the fluid. Tissue smears showed acid fast bacilli in four out of six, whereas the fluid was negative in all. The biopsy yielded diagnostic information in nine out of ten patients with a thickened pericardium. There were no complications.

Phosphorylation of synaptic proteins in chick forebrain: changes with development and passive avoidance training.

We have used synaptic plasma membranes (SPMs) and postsynaptic densities (PSDs) to study protein phosphorylation at the synapse in the developing chick forebrain and in 1-day-old chick forebrain following training on a passive avoidance task. Endogenous phosphorylation patterns in SPMs and PSDs prepared by extraction with n-octylglucoside isolated from chick forebrain were investigated by labelling with [32P]ATP. The phosphoprotein components of the SPM and PSD fractions were separated using sodium dodecyl sulphate gradient polyacrylamide gel electrophoresis. Autoradiography and densitometry of the Coomassie Blue protein staining pattern revealed phosphate incorporation into several SPM components including those of molecular mass 52, 37, and 29 kilodaltons (kDa). Bands of similar molecular mass were not phosphorylated in PSD fractions. This difference in phosphorylation between SPMs and PSDs was not due to the detergent n-octylglucoside. In a developmental study in which SPM and PSD fractions were prepared from 1-day-old, 14-day-old, and 21-day-old chickens, the phosphorylation patterns of SPMs were similar throughout, but striking differences occurred in PSDs, both in the level of phosphorylation and in the components phosphorylated. A time-course study was carried out in which phosphorylation of SPMs and PSDs from 1-day-old chicks trained on a passive avoidance task was compared with patterns from control chicks trained on a water-coated bead and untrained chicks. In SPMs prepared from forebrains removed 10 mins following training, a consistent but nonsignificant decrease (-21%) in phosphorylation of a 52 kDa band occurred in chicks with passive avoidance training compared with water-trained and untrained chicks.(ABSTRACT TRUNCATED AT 250 WORDS)

Towards a digital model for an electron-microscope image.

An image model is defined based on the boundaries between image regions with different textures and series of descriptions of those textures. Six models of texture are studied under the categories of pixel-based and region-based models. Several techniques for the determination of the unit-cell of textures are presented. The model is applied to the consideration of (a) image correction, (b) the classification of image texture, (c) image enhancement including averaging of detail in periodic specimens, and (d) image data compression. A floating point format, which provides a significant simplification for the Huffman code, is also introduced.

Soft tissue sarcoma in Kuwait: a review of 114 patients.

One hundred and fourteen patients with soft tissue sarcoma were seen in the Kuwait Cancer Control Centre over a period of 11 years. Fifty per cent were liposarcomas, fibrosarcomas and rhabdomyosarcomas. The lower extremity was the commonest site involved, followed by the trunk. Treatment was in most cases surgery and adjuvant radiotherapy. The overall rate of local recurrence was 36%. Local failure was highest in abdominal tumours (66%). In extremity lesions, local recurrence was higher in proximal sites than in distal sites. Twenty-eight per cent of the patients had metastases, the lung being the commonest site (68%). The histological type had no bearing on local failure or dissemination. Histopathological grade was found to be an important prognostic indicator, both for local failure and dissemination. The rates of local recurrence and metastases were lower in low grade than in high grade tumours.

Immunocytochemical localization of relaxin in corpora lutea of sows throughout the estrous cycle.

Porcine relaxin has been sought by localization in the corpus luteum of sows on Days 3, 7, 9, 11, 12, 15, 18, 19, and 21 of the estrous cycle, using the avidin-biotin immunoperoxidase method and an antiserum to purified porcine relaxin. Simultaneous localization of relaxin in corpora lutea from sows on Days 108 and 113 of pregnancy was used to compare the intensity of immunostaining with that of corpora lutea of cyclic animals. However, the antiserum dilution necessary for optimal localization differed considerably in these two states (1:10,000 in pregnancy and 1:750 in the cycle), suggesting that lower levels of antigen are present in the luteal cells of the cycle. Relaxin immunostaining was undetectable on Day 3 of the cycle but became evident by Days 7 and 9. At Day 11 staining intensity increased and persisted through Day 15. On Day 18 some stain was still evident, but by Days 19, 20, and 21 there was complete absence of immunostain. Relaxin immunostaining appeared to be located throughout the cytoplasm of the luteal cell, as clear areas in the nuclear region were often observed. The results suggest that relaxin is produced in low amounts by the luteal cells of the cyclic sow and that the levels fluctuate with stage of the cycle. Lack of evidence from radioimmunoassay for a surge of relaxin secretion into the systemic circulation prior to luteolysis in the pig estrous cycle suggests that the relaxin localized in the luteal cells of the cycle may have an intraovarian function.

Effect of metal ions on the antimicrobial activity of 5-sulphosalicylic acid.

Bis-(5-sulphosalicylato)-diaquo chelates of VO(II), Cu(II), Ni(II), Co(II), Fe(II) and Mn(II) exerted antifungal activities. The chelates have been characterised for elemental, thermal and infra-red spectral studies and were found to be more fungicidal than the chelating agent. The antifungal activity was found to be in the order: Cu(II) congruent to Ni(II) greater than Co(II) greater than Fe(II) greater than Mn(II) greater than VO(II).

Primary gastrointestinal lymphoma in Kuwait. An 11-yr retrospective analysis of 108 cases.

One hundred and eight cases of primary gastrointestinal lymphoma from the files of Kuwait Cancer Control Center over a period of 11 yr were analysed retrospectively. The occurrence was 47 in the proximal small intestine, 38 in the stomach, 18 in the distal ileum and five in the colon and rectum. The majority of the patients were in stage III. Using a modified Rappaport's classification, lymphocytic lymphoma was the commonest histologic type (60%) as compared to histiocytic lymphoma (19%). Four patients had early IPSID (immunoproliferative small intestinal disease). The 'Western' type of lymphoma occurred in the fourth decade while the 'Mediterranean' type occurred in the third decade of life. The latter occurred more commonly among people of low socioeconomic background. Chemotherapy was the single most effective mode of treatment. Addition of surgery, radiotherapy or both did not improve the 2-yr survival but did improve the 5-yr survival.

Patterns of chronic liver disease in Kuwait with special reference to localisation of hepatitis B surface antigen.

Two hundred and fifty six consecutive liver biopsy specimens (without secondary malignancy) collected over five years were reviewed to characterise the pattern of liver diseases encountered in the Kuwait region. A relatively high proportion of chronic active hepatitis (19%) and cirrhosis (40%) was found. Localisation of HBsAg was carried out by the histochemical orcein method and the immunohistological peroxidase-antiperoxidase (PAP) procedure. The PAP technique was superior to orcein both in quality and quantity in addition to its specificity. Three immunohistological staining patterns were observed: diffuse pancytoplasmic , partial perinuclear, and peripheral cytoplasmic. The positivity rate of HBsAg in chronic hepatitis was 29% and 27%, in all cases of cirrhosis. The results of immunohistology and serology of HBsAg were compared in 52 patients in whom both tests were carried out; almost one third of chronic active liver diseases were positive by both methods. Our data clearly show the sensitivity of immunohistology and its value in detecting HBsAg, especially in retrospective studies where serology is not always available. Additionally, the data show that hepatitis B infection is often associated with the development of chronic liver disease in Kuwait.
