[Study on the status of methylation of Rb gene promoter in human esophageal cancer and effect of NMBzA on Rb gene promoter in monkey esophageal epithelium].

OBJECTIVE: To elucidate the cause of abnormal Rb expression in esophageal cancer (EC). METHODS: Methylation of Rb gene promoter was examined with restriction endonuclease digestion and PCR amplification. RESULTS: Of the 30 specimens of EC, 10 (33.3%) had Msp I type hypermethylation of the Rb gene promoter while 12 (40.0%) had Hpa II type hypermethylation and one had both type of hypermethylation. In monkey fed with a single dose of NMBzA (30 mg/kg) on day 0, increasing hypermethylation of both types of the Rb gene promoter in esophageal epithelium was found on day 1, 2 and 3 but reduced on day 5. CONCLUSION: These data suggest that abnoromal expression of Rb gene may be related to hypermethylation of its promoter and carcinogen NMBzA can induce such hypermethylation in monkey esophageal epithelium.

[Cloning and identification of cDNA fragments related to human esophageal cancer].

OBJECTIVE: To search new genes related to human esophageal cancer (EC) for revealing carcinogenesis mechanism and genetic susceptibility of EC. METHODS: Three normal esophageal epithelia (including 1 tumor-adjacent tissue) and two primary squamous cell carcinomas collected from high incidence family in Lin-xian county were studied using technique of mRNA differential display. The differential fragments were sequenced and identified by RT-PCR assay. RESULTS: (1) Eighteen differential fragments were isolated and identified, 13 of which were expressed in normal esophageal epithelia but not in EC(assigned as normal esophageal gene, NEG), while 5 were expressed in EC but not in normal esophageal epithelia(assigned as mutated esophageal gene, MEG). (2) Four NEG fragments were not homologous to the known sequences in the public database of GenBank (of NLM in USA). These 4 fragments were assigned as esophageal cancer related gene (ECRG) 1 to 4. (3)The remaining 14 fragments were homologues to 12 known genes or gene fragment. Their role in EC remains unclear. (4) Using RT-PCR technique, the expression of ECRG between normal epithelia and EC was significantly different. (5) All 4 ECRG genes were expressed in cDNA libraries derived from normal fetal brain, adult brain, liver, kidney, testis, bone marrow and skeletal muscle. (6) In the 20 cancerous and tumor-adjacent tissues obtained from the liver, lung, breast, colo-rectum and endometria, ECRG1 and ECRG2 was not detected by RT-PCR, while ECRG3 was highly expressed. For the ECRG4 the expression was much stronger in tumor-adjacent tissues than in cancerous tissues. CONCLUSION: ECRG 1 and ECRG2 may contribute to the causation and progression of the EC in Lin-xian, and may be candidates of tumor suppressor genes.

[Deletion of MTS1/p16 gene in human esophageal carcinoma].

OBJECTIVE: To investigate the alteration of MTS1/p16 gene in human esophageal carcinoma. METHODS: A total of 60 human esophageal tissue specimens, comprising 30 squamous-cell carcinomas and 30 tumor-adjacent tissue specimens, were examined for homozygous deletion of p16 gene by using Southern blot hybridization and PCR method. RESULTS: The results showed that no deletions were detected in 30 tumor-adjacent tissue samples. However, of 30 esophageal carcinoma specimens, 7 were found negative for p16 gene in Southern blot assay, and the deletion of the p16 gene in 5 samples were confirmed by PCR with a 16.7% p16 gene deletion rate. CONCLUSION: These data suggest that MTS1/p16 gene alterations may play a role in the progression of human esophageal carcinoma.

[Detection of human papillomavirus in Chinese laryngeal carcinoma with digoxigenin labelled probe prepared by PCR].

The studies described here detect HPV DNA in 124 cases of fresh tissue samples with different lesions of larynx using a nonradioactive Digoxigenin Labelled Probe prepared by polymerase chain reaction (PCR). The labelled probe of HPV consensus primers was obtained, which can detect nine types of HPVs DNAs such as HPV 6, 11, 16, 18, 31, 33, 35, 42 and 58. The results showed that the rate of positive cases with HPV infection was 45.6% (31/68) in the group of laryngeal carcinoma, 20.0% (3/15) for the group of neck metastatic lymph node, 11.8% (2/17) for precarcinous lesions group and 6.3% (1/16) for the group of vocal cord polypus, respectively. Fifteen cases of normal laryngeal tissue adjacent to the carcinoma and 15 cases adjacent to the carcinoma were HPV DNA negative. The carcinogenic action of HPV in laryngeal carcinoma and the sensitivity and specificity of the probe prepared by PCR are discussed in this paper.

[The use of bi-pedical rotatory door muscle-skin flap reconstruction in extended partial laryngectomy for late (T3 and T4) glottic cancer].

The purpose is to radical treatment for late glottic cancer by surgery, to restore the essential function of the larynx, 62 patients of late (T3 and T4) glottic cancer were treated by extended vertical partial and subtotal laryngectomy. At the same time, an appropriate method of reconstruction of laryngeal function by bi-pedical rotatory door muscle-skin flap was presented. The decannulation rate was 87.1% and 85.0% cases enjoyed satisfactory voice. All cases resumed normal mouth-food-taking. The conclusion is that selective treated with extended partial laryngectomy is effective for T3 and T4 glottic cancer.

[Culture of mycobacterium tuberculosis of gastric juice collected through different ways in diagnosis of pulmonary tuberculosis in children].

OBJECTIVE: To explore the value of culture of Mycobacterium tuberculosis through gastric juice specimens in diagnosis of patients with pulmonary tuberculosis in children. METHOD: Sputum, early morning vomited gastric juice (MVG), the other time vomited gastric juice (OVG) and early morning gastric lavage (MGL) were collected from 46 children with suspected pulmonary tuberculosis. All the specimens were processed to culture for Mycobacterium tuberculosis using pyruvate media, and the results were compared. RESULT: Of the 46 cases, Mycobacterium tuberculosis was found in 4 sputum samples (9%), 15 MGL samples (33%), 12 MVG samples (26%) and 2 OVG samples (4%). CONCLUSION: Collecting gastric juice through different methods for culture of Mycobacterium tuberculosis is specific for diagnosis of paediatric pulmonary tuberculosis.

[Percutaneous autogenous bone marrow grafting for repairing bone defect: an experimental study in rabbits].

OBJECTIVE: To observe osteogenesis of percutaneous autogenous bone marrow into cicatrix of bone defect. METHODS: 18 rabbits were used to observe the osteogenesis of percutaneous autogenous bone marrow into cicatrix of bone defect. A 1 cm bone defect model was made in each side of the radius. And 2 ml autogenous bone marrow was injected in the right radial bone defect after 6 weeks, and 2 ml peripheral blood in the left side. The animals were sacrificed in 1, 2, 3, 4, 12 weeks after injection. Bone formation was studied with histologic technique, X-ray examination and Ca-P quantitative estimates. RESULTS: After 1 week of injection, a big shadow of softtissue was found around the bone defect in the right side(bone marrow). Histologically a lot of new cartilage like tissues were found. In the left side(peripheral blood), no change was seen. After 4 weeks of injection, many of new bone-like shadows were observed around the defect by X-ray examination in the right side. The defect was smaller(average 1.5 mm) and bone formation was found. But in the left side, no signs for osteogenesis were noted. In 12 weeks defect was smaller(average 1 mm). Histologic sections proved well-developed bone tissue. In the left side, no bone formation found. CONCLUSION: Autogenous bone marrow can induce osteogenesis in cicatric bone defect, whereas no osteogenesis is found in the peripheral blood injection side.

[The reason and management of intraoperative femur fracture during hip arthroplasty].

OBJECTIVE: To study the pathogenesis, treatment and prevention of intraoperative femur fracture. METHODS: Intraoperative femur fracture occurred in 25(2.6%) of 945 hip arthroplasties performed from 1972 to 1996. 142(5.6%) occurred in revision arthroplasties, 12 (2.4%) 493 primary arthroplasties (2.4%), and 5(1.3%) in 310 semi-arthroplasties. RESULTS: The causes of these fractures included osteoporosis, iatrogenic, congenital alteration of bony anatomy, and inadequate soft tissue release prior to hip dislocation during revision of hip surgery. Femoral fractures can be prevented by preoperative templating of rontgenograms containing markers to measure magnification; routine overdrawing of the femoral canal when implanting long-stemmed prostheses; and a more extensive release of the external rotators and other scar tissue performed prior to dislocation. Any structures preventing free rotation of the femur should also be considered for the release. CONCLUSIONS: We recommend that intraoperative roentgenograms be made whenever preparation of the femoral canal or impacting of the prosthesis is more difficult or when resistance is suddenly lost during implication of implant. When diagnosed intraoperatively, it should be fixed with cerclage wiring. When an incomplete fracture near the hip of the stem was discovered postoperatively and the posterior part of the femoral cortex was intact, a spica cast was applied. For a complete fracture at the tip of stem, we recommend open reduction and internal fixation.

Clinical observation on vertical transmission of human papillomavirus.

OBJECTIVE: To observe the possibility of maternal-fetal vertical transmission of human papillomavirus (HPV) via amniotic fluid. SUBJECTS AND METHODS: Specimens of cervical secretions from 30 pregnant women were obtained during the third trimester before rupture of membrane, and specimens of pharyngeal secretions of their neonates were obtained 12-48 h after birth. Amniotic fluids were collected in 13 pregnant women during cesarean section. The presence of HPV types 6, 11, 16, 18, 31, 33, 35, 38 deoxyribonucleic acid were detected by consensus polymerase chain reaction. RESULTS: HPV deoxyribonucleic acid was found in 16 cervical secretions, 14 pharyngeal secretions and in 3 amniotic fluids, the positive rate was 53.3%, 46.7%, 23.1% respectively. The pharyngeal secretion was also HPV positive in one of the three neonates from the amniotic fluid positive mothers. CONCLUSION: The results indicate that HPV can be transmitted in utero through amniotic fluid and cesarean section can not protect the neonates against vertical transmission completely.

[Haversian remodeling in guided bone regeneration].

OBJECTIVE: To study the relationship between Haversian remodeling and bone regeneration in guided bone regeneration (GBR). METHOD: 10 mm long segmental defects were created in the diaphyses of one radius in 24 rabbits. The defect was covered with silicone membrane shaped as a tube. Rabbits were killed at the 3rd day, 1st, 2nd, 3rd, 4th, 6th, 10th, 12th weeks. Undecalcified section was prepared for histologic examination. RESULT: Bone resorption was seen in Haversian canal, connection between osteocytes and osteoblasts in Haversian, and connection of osteoblasts in Haversian and in callus. The process of Haversian remodeling was involved in the process of bone regeneration, just like primary bone healing. The process of Haversian remodeling and regeneration was coupled in topography, time and size. CONCLUSION: Bone repair is related to Haversian remodeling.

[Influence of stripping periosteum on bone formation in guided tissue regeneration].

OBJECTIVE: To analyze the role of periosteum in guided bone regeneration by means of undecalcified sections and X-ray films. METHOD: 10 mm long segmental defects were created in the diaphyses of both radii in 24 New Zealand rabbits. The defect on the control side was covered with a silicone membrane shaped as a tube. On the experimental side, 10 mm periosteum of both sides of the defect was stripped and the defect was covered with a silicone tube. The animals were killed at the 3rd day and the 1st, 2nd, 3rd, 4th, 6th, 10th, 12th weeks. Samples were treated for radiologic and histologic examinations. RESULT: (1) periosteal stripping exerts no effect on callus formation in or out of the tube. (2) osteoblasts in the cambium layer of the periosteum come from bone surface, endosteum or the Haversian envelopes. (3) the tissue in the two layers of the periosteum has different origins, the fibrous layer comes from soft tissue and osteoblasts in the cambium from bone surface or Haversian envelopes. CONCLUSION: Stripping periosteum has no influence on bone formation in guided tissue regeneration.

[Repair of articular cartilage defect with a large area: an experimental study].

OBJECTIVE: To compare and evaluate potentialities and biological characteristics of grafts of chondrocytes cultured on fascia, periosteum and articular cartilage in repairing large defects of articular cartilage. METHOD: The large defects of articular cartilage were repaired with grafts of freeze-stored and fresh chondrocytes cultured on fascia, periosteum and articular cartilage, which were evaluated by a number of observation methods. RESULT: The fresh and freeze-stored fascial chondrocytes were superior to free grafts of chondrocytes in potentiality of formation of new cartilaginous tissue, structure and metabolism of newly formed tissue. Relatively good results were also obtained similarly by using fresh autografts of periosteum and cartilage. CONCLUSION: Chondrocytes can be cultured normally on fascia, which can be regarded as an ideal carrier of chondrocytes.

[Memorial metal ball for the treatment of avascular necrosis of femoral head].

OBJECTIVE: To introduce a new method for the treatment of avascular necrosis of the femoral head. METHOD: A technique has been developed what the memorial metal ball, and where the graft of the wing of the ilium is harvested with the deep circumflex iliac vascular pedicle and implanted into the avascular area of the femoral head. This procedure has been performed in 28 hips in 22 patients. RESULT: Preoperatively, 8 hips were of Ficat Arlet Stage II and twenty stage III. Postoperatively, eighteen patients were pain free, 3 had mild pain, and 1 had moderate pain. The radiographic appearance improved in 2 cases. No major complications occurred. CONCLUSION: It is one of the better alternatives that can prevent the necrotic femoral head from progressing to collapse and promote revascularization and new bone formation by direct mechanical support. The technique is suitable for the treatment of avascular necrosis of femoral head of stage I to III.

[Classification and related operation procedures of pelvic tumor treated with partial pelvic resection].

OBJECTIVE: To establish a classification system of pelvic tumor in terms of the location of lesion in order to project surgical treatment with less extent of damage to the affected limb function. METHOD: Thirty-six cases were treated by partial pelvic resection with limb salvage, and 20 were followed up. The lesions were divided into four groups on the basis of sacral-iliac and/or hip joint involved. In group I, lesion was noted in the ischium or pubis only;in group II the lesion was located at sacral-iliac joint or hip joint. The patients with acetabulum involved were classified into group III both sacral-iliac joint and hip joint were destroyed by tumor in group IV. Surgical treatment was designed as resection, resection with sacral-iliac joint reconstruction, resection without hip joint reconstruction according to the classification of the lesion. The patients in group IV were treated with traditional hemi-pelvic amputation. RESULT: Patients were followed up from 4 months to 14 years (average 51.7 months). The limb function was preserved to some extent, and pelvic stability was satisfactory. Walking gait was not chlanged significant in group I and II. But resection without hip joint reconstruction in group III resulted in limb discrepancy (4.0 - 8.0 cm) and walking lamely. The patients needed custom-made shoes with heightened heel, but some of them walked with crutch when the discrepancy exceeded 8 cm. There were no nerve and vessel injury in the long run. The strength of muscles around hip joint was III - IV grade after postoperative exercise, and the capability of walking and viability recovered 3 months after the operation. CONCLUSION: Different surgical treatments should be selected according to the site of pelvic tumor. The classification system depending on the lesion's location in pelvic tumor is useful in selecting operative procedure to retain limb function to great extent.

[Tissue engineering of hyaline cartilage].

OBJECTIVE: To study the new way to repair the defect of the articular cartilage. METHOD: The articular-epiphyseal cartilage complexes from 8 newborn rabbits were enzymatically dissolved, and the chondrocytes were seeded onto the fibrous chitin mesh in a 96-well plate in DMEM medium. RESULT: Cultured for 21 days, the "membrane-like cartilage" was strong enough to be transferred. At the 110th day, the semitransparent cartilage discs were 4.4 mm in diameter and 14.8 mg in weightness. Staining of SatranO showed that engineered "cartilage" was bound of GAGs and the chondrocytes expressed the mRNA of type II collagen by in situ hybridization, which is resemble to that of the normal hyaline cartilage beside the biodegraded fibrous chitin embedded in. The "membrane-like cartilage" cultured for 21 days was grafted into the articular cartilage defects of knee of adult rabbits. Seven of 9 defects healed with new cartilage and the structure was well organized in 16 weeks. CONCLUSION: The biodegradable fibrous chitin is a good "matrix" for cartilage tissue engineering. The engineered "cartilage" is true hyaline cartilage, which is a new "alive cartilage tissue" for the repairment of the defect of articular cartilage.

[The changes of pancreatic function after cardiopulmonary bypass].

OBJECTIVE: To evaluate the changes of endocrine and exocrine pancreatic function in patients who underwent heart valve replacement under cardiopulmonary bypass (CPB) in order to optimally conside clinical management perioperatively. METHOD: Sixteen patients with heart valve disease were selected. Serum glucose, lactate, insulin, C peptide, amylase and lipase were measured. RESULT: Hyperglycemia and hyperlactacemia were noted after CPB and peaked at 19:00 on the operative day. Their changes were correctly correlated. Serum glucose and lactate returned to the baseline level on the third postoperative day. The average level of serum insulin were normal, but higher on the third postoperative day. However, the level of serum C peptide was higher than the baseline. It doubled the baseline level at 19:00 on the operative day. The average level of serum amylase and lipase was normal, except the higher level of serum lipase on the fourth postoperative day. In our group, 12 patients had normal serum amylase level perioperatively, and 4 increased serum amylase level (25%), among them 2 patients accompanied with hyperlipasimia (16%). There were 8 patients with hyperlipasimia (50%). CONCLUSION: The endocrine and exocrine pancreatic functions were influenced by CPB and did produce pathophysiologic changes.
