[Smooth intramural and kink-resistant polyester vascular prosthesis].

OBJECTIVE: To study the healing effect, neointimal formation and patency rate of smooth intramural and kink-resistant polyester vascular prosthesis. METHODS: Experimental implantations of the smooth intramural and kink-resistant polyester vascular prosthesis of internal diameters of 0.6-1. Ocm were performed in segments of abdominal aorta and thoracic descending aorta. RESULTS: The patency rate was 100% in both abdominal aortic implantations and thoracic descending aortic implantations after postoperative observation for 425 days, while the total patency rate was only 70% in the crimped controls. The thickness of the neointimal linings of both implantation groups was determined, smooth intramural, kink-resistant polyester vascular prostheses were significantly thinner than that using crimped polyester vascular grafts (P < 0.001). CONCLUSION: Our study proves the feasibility of using smooth intramural and kink-resistant polyester vascular prosthesis to substitute small arteries with an internal diameter of 0.6-1. Ocm.

Effect of hypoxia on DNA synthesis and c-myc gene expression of pulmonary artery smooth muscle cells.

The neonate is particularly susceptible to the development of hypoxic pulmonary hypertension. The present study was undertaken to observe the effect of hypoxia on DNA synthesis and c-myc gene expression between newborn calf and adult bovine PASMC in vitro. DNA synthesis measured by 3H-TdR incorporation was increased after hypoxic challenge for 24h. Hypoxia enhanced the increment in 3H-TdR incorporation induced by EGF. Northern blot analysis revealed that PASMC cultured in both normoxia and hypoxia expressed c-myc gene transcript of 2.2Kb, but there is a higher 2. 2Kb mRNA expression in hypoxic PASMC than that in normoxia. We speculate that newborn calf PASMC exhibited potential response to hypoxia than adult, which was augmented by EGF. Enhanced c-myc gene expression may lead to a great understanding of the mechanism of PASMC growth in the development of pulmonary hypertension.

Surgical treatment and prognosis of bronchial carcinoid tumor.

In order to understand the effect of surgery in the treatment of the bronchial carcinoid tumor and the factors affecting prognosis, 18 cases of bronchial carcinoid tumor are presented, including 5 cases with Cushing's syndrome. There were lobectomy in 10, lung wedge resection in 3, excision of intraluminal tumor of bronchus in 3, exploratory thoracotomy in 2 cases. No operation death. Pathological examination revealed 14 cases were typical carcinoid tumor and 4 cases were atypical carcinoid tumor. By 2-13 years follow-up, 3, 5 and 10 years survival rate were 82%, 78% and 70% respectively. Bronchial carcinoid tumor is often confused with small cell carcinoma of lung, the correct diagnosis can be obtained by light microscopic, electron microscopic and immunohistochemistry studies. Those patients accompanied with ectopic-ACTH secretion always have Cushing's syndrome, resection of tumor can produce good result. Proper operation method depends on the location of the tumor and patient's extent of cardiac and pulmonary reserve. Atypical carcinoid tumor had high malignancy and poor prognosis. The size of tumor, lymph node involvement and adjuvant therapy seem no definite effect on the patients' survival rate.

The expression of c-erbB-1 and c-erbB-2 oncogenes in basal cell carcinoma and squamous cell carcinoma of skin.

The expression of c-erbB-1 and c-erbB-2 oncogenes were investigated by immunohistochemistry using monoclonal antibodies to c-erbB-1 and c-erbB-2 protein in 43 cases of basal cell carcinoma (BCC) and 26 cases of squamous cell carcinoma (SCC). We found that the expression of c-erbB-1 oncogene in all BCC increased by different degrees and the expression of c-erbB-2 oncogene in BCC was significantly reduced or lost when compared to that in normal epidermal cells. Furthermore, apparent negative and positive relationships were observed respectively between the tumor differentiation and the expression of c-erbB-1 and c-erbB-2 oncogenes in SCC. It is suggested that the abnormal expression of c-erbB-1 and c-erbB-2 oncogenes in BCC and SCC may play a role in the development of skin tumors. The pattern of the c-erbB-1 and c-erbB-2 oncogenes expression in SCC may assist in distinguishing the biological behavior and prognosis of SCC.

[Clinical application of repeated tissue expansion].

Since 1992, the technique of repeated tissue expansion has been used to repair defects from resection of burn scars. Ten patients were treated with this technique and obtained satisfactory results. The clinical application demonstrated that a new expander could be placed again at the time of the expanded flap advancement for repeated expansion. The operative procedure, advantages, indications and key points were discussed. We concluded that repeated tissue expansion is an effective technique for the repair of large skin defects.

[The anatomical study of the ascending cutaneous branch of the circumflex scapular artery].

The circumflex scapular artery was investigated in 20 cadavers (40 sides) with radiography. We found that the presence rates of the ascending, transverse and descending cutaneous branches of the circumflex scapular artery were all 100%. The ascending cutaneous branch could be divided into 3 types: the first type came from the circumflex scapular artery in 4 cadavers (4 sides) with 10% rate of presence; the second type came from the cutaneous branch of the circumflex scapular artery in 20 cadavers (36 sides) with 90% rate of presence; the third type consisted of two branches in 10 cadavers (12 sides) with 30% rate of presence. It was also found that the ascending branch ran in an upward direction to the acromial area or over the scapular spine where it communicated with the suprascapular artery, the transverse artery of neck and the thoracicoacromial artery. Based on the results, a scapular flap can be designed over the scapular artery.

[Response and mechanism of vasovagal syncope induced by tilt table test].

Tilt table test (TTT) is done in 110 cases of unexplained syncope and 37 healthy controls. As a result, there is no significant difference in basal diastolic blood pressure (DBP), basal heart rate (HR) x DBP among TTT positive, TTT negative and healthy controls; but DBP and HR x DBP of TTT positive patients at the instant of syncope is not significantly different from their basal values, the HR at the instant of syncopy statistically in not different from the basal HR. These data suggest that not the same mechanism is responsible for the changes of BP and HR. In seventy percent of the TTT positive patients vasovagal syncope (VS) may be provoked by isoproterenol. So the occurrence of VS may be related to hypersensitivity of beta-receptor. TTT may be an approach to unravel the pathogenesis of VS.

[Different techniques of internal fixation applied in lumbosacral fusion: 83 cases analysis].

Posterior lumbosacral fusion using bone graft without internal fixation requires long immobilization and has high rate of pseudoarthrosis. To overcome such limitation technique of spinal internal fixation has been developed and applied in past twenty years. Since 1982, the authors have done lumbosacral fusion with internal fixation and bone graft by using Harrington alar hook (32 cases), luque ring (26 cases), Galveston technique (7 cases), Steffee plate (10 cases), Dick pedicle screw (1 cases), RF instrumentation (3 cases) and CD technique (4 cases). Totally 83 cases have been treated, which include paralytic pelvic tilt (9 cases), spinal fracture (4 cases), lumbosacral tuberculosis (5 cases), degenerative stenosis (21 cases); L5-S1 spondylolisthesis (38 cases), L5-S1 disc protrusion (4 cases); L5 semivertebra (1 cases) and neurofibromatosis (1 cases). Patients have been follow-up for 63.7 months on average. Good fusion has been seen in 82 cases (98.8%). The procedure does not need long immobilization and has less complication.

[The role of substance P in the spinal dorsal horn in the pathogenesis of autoimmune diseases].

The aim of the present study is to explore the role of immunosuppression mediated by substance P (SP) in spinal dorsal horn (SDH) in the pathogenesis of the autoimmune diseases. The experimental allergic neuritis (EAN), experimental allergic encephalomyelitis (EAE) and adjuvant arthritis (AA) animal models were established in the guinea pigs and Wistar rats, respectively. The effects of alteration of SP activity in SDH on immune responses and the pathogenesis of the autoimmune diseases were observed. The results showed that decreasing activity of SP in SDH by pretreatment of capsaicin or intrathecal SP antagonist could enhance cellular and humoral immune responses and aggravate the autoimmune diseases, while intrathecal SP agonist could suppress the immunity and alleviate clinical signs. The contents of SP in SDH was elevated dramatically at the peak of immune responses. These results suggest that SDH SP might participate in the pathogenesis of the autoimmune diseases. The increase of SP contents in SDH may inhibit the immune system via unknown pathway and ease clinical severity of the autoimmune disease, where SP might act as neurotransmitter in the immunoregulation of the negative feedback. To elevate SP content in SDH might be beneficial to the autoimmune diseases.

[Research on the recombinant plasmid pDJH2 of L. interrogans serovar lai: sequencing and alignment with other known bacterial Omp sequence].

The Leptospira whole cell vaccine (LWCV) currently used in China is safe and effective, out the immunity following vaccination with two doses of the fluid medium vaccine is of low order. The duration of immunity conferred by this vaccine is rather short, six months or at most one year. Therefore, it is necessary to develop new generation vaccines against Leptospirosis for the developing world. In this paper we report the sequencing of the insert fragment of pDJH2 from genomic DNA of L. interrogans sevovar lai strain 017 and its alignment with other bacterial omp sequences. A genomic library of Leptospira interrogaans serovar lai strain 017 was constructed with the plasmid vector pUC18. A recombinant plasmid designated pJDH2 was screened from the genomic library. Inserted fragment of pDH2 is 1.9 kb by gel electrophoresis. Immunization/protection was studied in BALB/c mice model. The results showed highly significant difference between pDJH2 and pUC18 (control). Inserted fragment of pDJH2 DNA sequencing was performed by Dr Yan Zhengxin (Max-Planck-Institut for Biology. Tubingen, Germany). Insert fragment was cloned into pBluescript II KS-(stratagene) and sequenced by using AB1 (Applied Bio Systems, Model 373A). Two open reading frames of 565 and 662 nucleotides were identified. There were identifiable initiation codons, terminators, Shine-Dalgano ribosome combining site, Pribnow boxes and Sextama boxes within the 2 sequenced regions. Nucleotide sequences were analysed using Gene Work, a suit of computer program developed by Department of Biochemistry St. Jude Children's Research Hospital Memphis. U.S.A. The results of formatted alignment showed the predicted nucleotide sequence of ORF1 of the serovar lai had significant similarity with ORF2 (49.36%). L. kirschneri ompL1 (49.26%), Borrelia burgdoferi omp (48.97%), Treponema phagedenis omp (47.3%); Salmonella typhimurium ompC(46.87%), Yersinia enterocolitica ompH (46.7%), Leptospira borgpeterseni pfap (46.3%), and Serratia marcescens omp (43.3%). The close relationship of the pDJH2 ORF1 and ORF2 nucleotide sequences from Leptospira kirschneri ompL 1 is apparent. Whether the recombinant pDJH2 will prove useful for vaccine development remains to be tested.

[Subclones of fragment DNA recombinant rpDJH2 of L. interrogans serovar lai strain 017 and it's expression of high level in E. coli].

Fragment of 1.9 kb recombinant DNA of pDJH2 was linked with vectors pT7-7 and pRSETs. Then they were transformed into E. coli JM109 (DE3) respectively. Expression of subclones was achieved in E. coli JM109 (DE3) with IPTG inducement. SDS-PAGE showed that the molecular weights of products were 68kd and 23 kd respectively. The amount of production seemed to be higher than that of the outer membrane proteins of L. interrogans serovar strain 017 in nature. Immunoblotting of pDJt and pDJrB2 (both are subclones) with the specific antiserum of anti-OMP of L. interrogans serovar lai strain 017 and the experiment of initiative immuno-protection in guinea pigs showed both protein-68 kd and 23 kd might be the antigens of immuno-protection on the outer membrane of L. interrogans serovar lai strain 017.

[Pericardial devascularization in the treatment of portal hypertension with Larjet's innervation].

From 1982, 48 cases of portal hypertension were treated by pericardral devascularization with Larjet's innervation. Results were compared with that without preserving the Larjet's nerve in 57 cases. The incidence of rebleeding, mortality, abdominal distension, diarrhea, and gastric retension was 2%, 6.3%, 6.3%, 4.2%, and 0%, respectively in the former; and 12.5%, 12.3%, 24.6%, 15.6%, and 14%, respectively in the later. The authors believe that gastric function remained normal postoperatively when the Larjet's innervation was preserved.

[The postoperative effects of tapered terminal ileal continent urinary reservoir].

We report postoperative effects of 23 cases having tapered terminal ileal continent urinary reservoir is reported. The operative technique was characterized by tapering the terminal ileum with stapler to form a continent efferent tract, segments of intestine were cut open and reformed a pouch, and the ureter implantation was performed by means of inserting the end of ureters into the pouch. Ileocolonic pouch wes constructed for 14 patients and colonic pouch for 9 patients. 22 patients were followed up for 1 to 6 years except one died of cerebral hemorrhage 2 weeks after operation. Urine continence were achieved in all patients and self catheterization could be easily undertaken with 20F-22F catheter in 21. Difficulty in catheterization was happened in one early case. One year after operation, the maximum volume of the ileocolonic pouch was 900 ml-1000 ml. The basal and intermittent pressure of the filling pouch was 0.49 kPa-1.96 kPa respectively. The maximum volume of the colonic pouch was 400 ml-500 ml. The basal and intermittent pressure of the filling pouch of the colonic pouch was 0.98 kPa-1.9 6 kPa and 1.96 kPa-3.93 kPa respectively. The complication of pouch overdistention was found in 6 cases, pouch stone formation in 2 and pouch infection in 12 episodes. There was no evidence of ureteral obstruction or reflux and significant hyperchloric acidosis. It is recognized that tapered terminal ileum is a good way to construct a continent efferent tract, both ileocolonic pouch and colonic pouch can meet the need of low inner pouch pressure so long as the intestines were cut open and reformed, and efforts must be made to prevent the complications of pouch overdistension, stone formation and pouch infection.

[Inhibition of rabbit lens epithelial cell proliferation].

OBJECTIVE: To study the ability of homoharringtonine, 5-fluorouracil and adriamycin on inhibiting the proliferation of rabbit lens epithelial cells (RLEC) and the prevention of after cataract by using homoharringtonine. METHODS: RLEC were isolated and cultured. (1) The passage RLEC were placed in 24-well tissue culture plates and incubated for 48 hours, then exposed to different concentrations of homoharringtonine, 5-fluorouracil and adriamycin for 24 and 72 hours; (2) The passage RLEC and homoharringtonine, 5-fluorouracil, adriamycin were placed and cultured for 24 hours to investigate the rate of attached cells; (3) The morphological changes of RLEC were studied under light microscope. RESULTS: The ID50 of homoharringtonine, 5-fluorouracil and adriamycin exposed to RLEC for 24 hours were 0.84 micrograms, 0.58 micrograms and 4.50 ng/ml and those for 72 hours were 0.49 micrograms, 0.33 micrograms and 3.85 ng/ml respectively. In the homoharringtonine group, the rate of attached cells was less than that of 5-fluorouracil and adriamycin groups. The study of the morphological changes showed that the different concentrations of antiproliferative drugs affected on RLEC at different regions. CONCLUSION: The authors consider that homoharringtonine may be more effective for the prevention of after cataract than 5-fluorouracil and adriamycin.

The healing of excimer laser ablated cornea and the effects of corticosteroid.

OBJECTIVE: To investigate the wound healing of the cornea following 193 mm excimer laser photorefractive keratectomy (PRK) and the effects of corticosteroid on it. METHODS: PRK was performed on both eyes of 6 monkeys. Corticosteroid was given to the right eyes, but not to the left ones. At 4 days, 1 week, 2 weeks, 1 month, 3 months and 6 months, corneal pachymetry was examined and the eyes of one monkey were enucleated and the corneas were stained immunohistochemically for collagen type III, type IV and type VII, fibronectin and laminin. RESULTS: The expression of collagen type Vii recovered to the normal linear patter at 1 month. With corticosteroid, the deposition of collagen type III and fibronectin was markedly reduced and the pachymetry of central cornea was closer to the expected value. CONCLUSION: This study suggests that corticosteroid help to minimize the postoperative myopic regression and corneal haze after PRK.

[Secondary intraocular lens implantation in children].

OBJECTIVE: The study was designed to determine the visual outcome and indication of secondary posterior chamber intraocular lens (IOL) implantation. METHODS: The operation was performed on 34 eyes with aphakia in children after separation of posterior iris synechia from posterior capsule and formation of a posterior chamber space large enough to insert an intraocular lens. Based on whether the integrity of the posterior capsule was complete or not, various techniques of secondary IOL implantation were adopted. RESULTS: The follow-up periods ranged from 6 to 24 months. The results showed that 29 of the eyes (85.29%) achieved a post-operative corrected visual acuity equal to or better than that of the best corrected visual acuity before the surgery; the postoperative uncorrected visual acuities in 16 eyes (47.06%) and corrected visual acuities in 28 eyes (82.35%) reached 0.5 or better. CONCLUSION: Secondary posterior chamber IOL implantation is a safe and effective method in children who have a complete or partial posterior capsule.
