Comparison of iodine-123-vasoactive intestinal peptide receptor scintigraphy and indium-111-CYT-103 immunoscintigraphy.

UNLABELLED: Recently, we have shown that the expression of receptors for vasoactive intestinal peptide (VIP) on intestinal adenocarcinomas can be used for in vivo targeting of primary or metastatic tumor sites using 123I-labeled VIP. Several other receptors and antigens including the TAG-72 protein have also been implemented for in vivo localization purposes. In this study, we have compared the in vitro and in vivo binding of 123I-VIP and of the 111In-labeled monoclonal antibody (MAb) directed against TAG-72 (OncoScint; 111In-CR-103) in patients with intestinal adenocarcinomas in a single-blinded, prospectively randomized trial. METHODS: Twenty patients were administered either 123I-VIP (150-200 MBq; 1 microgram) or 111In-CYT-103 (150 MBq; 1 mg) for one imaging study. After interim analysis demonstrated superior imaging with 123I-VIP, the next 10 patients (accounting for a total of 50 patients) enrolled in this trial underwent both studies in random order to allow for a direct comparison. RESULTS: In total, 123I-VIP scans were true-positive in 28 of 30 patients (93%) versus 17 of 30 patients administered 111In-CYT-103 (56%). In the subgroup of 10 patients enrolled in the second part of the study, primary intestinal adenocarcinomas were imaged in five of five patients with 123I-VIP and in only two of these patients with 111In-CYT-103. Liver metastases were visualized in five of six patients by 123I-VIP receptor scanning and in four of these patients with 111In-CYT-103. The in vitro results indicated significant binding of 123I-VIP to primary colorectal tumors as well as to HT29 and COLO320 adenocarcinoma cells. In vitro, adenocarcinoma cells also expressed abundant numbers of the TAG-72 antigen. CONCLUSION: Intestinal adenocarcinomas co-express VIP receptors and the IAG-72 antigen. Despite significant in vitro binding of both agents, however, the VIP receptor scan is more sensitive in localizing intestinal adenocarcinomas and metastatic spread.

The C-terminal Src kinase (Csk) is widely expressed, active in HT-29 cells that contain activated Src, and its expression is downregulated in butyrate-treated SW620 cells.

Csk expression was examined in human glioma, breast and colon cell lines, along with murine tissues. Relative to colon and glioma cell lines, Csk expression was highest in the breast cell lines. Relative to brain tissue, expression was 100-fold higher in the heart, kidney, liver, lung and spleen. Subcellular fractionation and cellular immunostaining indicated that it was localized in the cytosolic fraction. The expression of Csk was downregulated upon chemical-induced differentiation of SW620 human colon cells by treatment with sodium butyrate. Its expression localization and activity relative to Src are discussed.

Catastrophic failure of a conforming type of total knee replacement: a case report.

A case study is presented to illustrate the concept of femoral component failure secondary to polyethylene wear in a 67 year old man, 13 years after he had conforming type total knee replacements. This case illustrates the theory that this observed problem may be a leading cause of failure in conforming and nonconforming total knee replacements in the future.

Effects of MTX and BN52021 on PAF-induced chemotaxis of PMNs and intraepidermal accumulation of inflammatory cells in guinea pigs.

OBJECTIVE: To investigate the effects of methotrexate (MTX) and platelet activating factor (PAF) antagonist ginkgolide B (BN52021) on PAF induced chemotaxis of neutrophils. MATERIALS AND METHODS: All guinea pigs were randomly divided into 12 groups. They were given different dosages of MTX and BN52021 by intra-abdominal injections. The random and chemotactic migration of polymorphonuclear leukocytes (PMNs) were measured by the agarose method. The backs of all guinea pigs were given intradermal injections of PAF and the numbers of the infiltration of inflammatory cells into the skin were determined. RESULTS: MTX inhibited random migration and chemotactic migration of PMNs to PAF, LTB4 and PAF-induced intraepidermal accumulation of inflammatory cells in dose- and time-dependent fashion. BN52021 specially inhibited PAF-induced chemotaxis of PMNs and intraepidermal accumulation of inflammatory cells, but did not inhibit PMNs random migration and LTB4-induced chemotaxis of PMNs. CONCLUSIONS: The inhibition of PMNs activities may be part of the mechanism of MTX therapy for psoriasis; BN52021 is a selective inhibitor of PAF-induced chemotaxis of PMNs, and therefore can be useful in the treatment of some inflammatory dermatoses such as psoriasis.

[Cleft palate repair with a combined method of mucosal flap pushback of the hard palate].

From January of 1992, we applied a combined method to repair cleft palate in 20 patients and received satisfactory results. The method is characterized by pushing back the mucosal flap of the hard palate, a Z-plasty on the nasal mucosa, repositioning the levator muscle to lengthen the palate, circumferential pharyng oplasty using denervated extensor hallucis brevis muscle, without making relaxing incisions and elevating the mucoperiosteal flap, avoiding interference to the greater and lesser palatine vessels and nerves, without relaxing palatal aponeurosis. The advantages of this method are preserving the normal anatomy and function of the palate and nasopharyngeal cavity, improving the function of velopharyngeal closure and minimizing secondary deformities.

[Study on the organ weight of sudden death cases].

In order to make clear any influence on the weight of various organs particularly in case of chronic diseases, Basing on the result of analysis carried out previously, data of 4402 autopsy cases of sudden death collected from 22 Medical Schools and Hospitals in China were studied. The organ weight analysed included those of the heart, lungs, spleen, liver, kidneys, brain and pancreas. The results showed that weights of the heart, lungs, spleen, liver and pancrease in males and weights of the the heart, spleen and pancreas in females were heavier than those of the non-sudden death cases in adults. The difference between male and female organ weights were that weight of the female brain aged above 3 months old and weight of all the 7 organs measured in adult female were lighter than the male.

[Experimental study and clinical application of primary choledochorraphy after choledochotomy].

An animal model was established in dogs to compare primary choledochorraphy with the traditional T-tube method. Authors observed cholangic histologic changes under microscope and electron-microscope between the two groups. The result showed that primary choledochorraphy was better than the traditional method. The advantage of primary choledochorraphy lies in its simplicity, safety and reliability, which have been shown in 30 patients of cholelithiasis in clinical application. It is suggested that for patients primary choledochorraphy may clesirably be used for patients who are indicated for this approach.

[The pulmonary function in scoliotic patient].

This paper investigated preoperative pulmonary functions in 134 scoliotic patients, of whom 32 cases was followed up for their postoperative pulmonary functions. The results show that the bigger the Cobb of the scoliosis, the greater the damages of the pulmonary functions. Congenital scoliosis is more severely affected by pulmonary functions than idiopathic scoliosis. There was no significant difference among the different sections of scoliosis with regard to their effect on the pulmonary functions. The authors concluded that the Harrington Instrumentation improved pulmonary functions in scoliosis.

[The PCR amplification, cloning, sequencing, expression in E. coli of gene encoding endoflagella subunit protein (fla B) from Leptospira interrogans serovar lai].

A pair of oligonucleotide primers were designed by ourselves to amplify the endoflagella gene of L. interrogans serovar lai. A fragment about 840 bp was generated with PCR and inserted into plasmid pUC8 after the fragment and pUC8 were digested respectively with Bam HI and Pst I. A recombinant plasmid (designated as pLF1) was obtained. SDS-PAGE analysis indicated that a 33 kd was expressed in E. coli JM103 harboring pLF1 and the expression level of the protein was 11% of total bacterial soluble proteins. Western blot analysis showed that the protein band could be recognized by the antiserum against the endoflagella (Axiall filament) of Leptospira interrogans serovar lai. Nucleotide seguence data showed an open reading frame encoding 282 aminoacids residues, corresponding to a protein of molecular weight 33.6 kd. The G + C content of endoflagella subunit protein gene was 48 mol%. Therefore, the G + C content of the leptospiral fla B Gene is significantly higher than the reported 39 mol% G + C content of leptospiral genome of L.interrogans serovar lai but similar to the G + C of the Treponema pallidum genome. Comparison of the deduced endoflagellar subunit protein (fla B) amino acid sequence with flagellins from other bacteria revealed a high level of identity with the Treponema pallidum fla B proteins. Immunization/protection experiment was performed on the model of BALB/c mice and showed that the survival rate in the group JM103-pLF1 was higher than that in the group JM103-pUC8, but statistically the difference between them was significant (P < 0.05) and pLF1 did not induce significant levels of agglutinating antibodies against L.interrogans serovar lai.

[16S rRNA gene PCR-SSCP analysis of the reference strains from 15 serovars (14 serogroups) of pathogenic leptospires in China].

The DNAs of reference strains from 15 serovars (14 serogroups) of pathogenic leptospires in China were amplified with 16S rRNA gene primers, and then single-strand conformation polymorphism (SSCP) of the products were analyzed in 12.5% nondenaturing mini polyacrylamide gel (containing 5% glycerol) combining with silver-staining. All products showed two bands on electrophoresis at different parameters of voltage or current and concentration of gel. It proved that serovar lai, serovar canicola, serovar pyrogenes, serovar autumnalis, serovar australis, serovar pomona, serovar linhai, serovar hebdomadis, serovar haemolytica, serovar wolffi and serovar paidjan have the identical pattern (Leptospira interrogans), while serovar javanica, serovar ballum, serovar tarassovi and serovar manhao I belong to another pattern (L. borgpetersenii). The result was consistent with the classification of genetic species by Yasuda et al (1987) and Ramadass et al (1992).

A one-hand operation gun for transrectal prostate biopsy.

OBJECTIVE: To develop a new automatic biopsy device (ABD) that enables a radiologist to obtain multiple samples with one hand and free the other hand to handle the ultrasonic scanner for continuous monitoring. MATERIALS AND METHODS: The new ABD developed by us in April 1994 enabled a radiologist to complete all the biopsy procedures including loading and reloading the spring system, firing the device, emptying the specimen, selecting the depth of needle advancement, and multiple sampling, simply by pressing 3 buttons in turn with his/her right thumb. Using this new ABD, transrectal prostate biopsy under the guidance of transrectal ultrasonic imaging was performed in 60 consecutive cases of prostate diseases. Each biopsy procedure was completed by a single radiologist, who was able to obtain multiple specimens with one biopsy needle, using only one hand and freeing the other hand for the manipulation of the ultrasonic scanner for guidance. RESULTS: A total of 265 satisfactory core tissue specimens of 285 needle passes (93%) were obtained from the 60 patients. Definite pathologic diagnoses were made for all the 60 patients (100%). No serious biopsy-related complications were observed. The main complications were hematuria in 24 of 60 (40%) patients and fever in 5 (8.3%). CONCLUSION: On the basis of our experience with the 60 patients, this new ABD can be operated smoothly and effectively with the right hand, while the left hand is freed to handle the ultrasonic transducer for continuous monitoring.

Studies on prophylactic effect of artesunate on schistosomiasis japonica.

OBJECTIVE: To examine the prophylactic effect of artesunate on schistosomiasis japonica by killing schistosomula. METHODS: Mice, rabbits and dogs after infection with cercariae of schistosoma japonicum (S. japonicum) were treated with artesunate on the 7th day at a dose of 300 mg/kg, 20-40 mg/kg and 30 mg/kg respectively once a week for 4-6 weeks. A double-blind test was used. A total of 864 persons in highly endemic areas for schistosomiasis were administered either artesunate at a dose of 6 mg/kg once a week for 8 weeks or identical placebo with the same dose-schedule during transmission season for S. japonicum. Four weeks after the last dosing, fecal examinations for S. japonicum eggs and miracidia were carried out to evaluate the prophylactic effect. RESULTS: Worm reduction rates in mice, rabbits and dogs were 77.50-90.66%, 99.53% and 97.10% respectively. All of the 467 residents in 2 trials were free from eggs or miracidia upon stool examinations whereas in the control groups with placebo, 15 out of 218 (6.9%) and 26 out of 179 (14.5%) were stool eggs and/or miracidia positive in the first and second trial, respectively. Side effects were mild. No significant changes in routine blood and urine tests, electrocardiogram, hepatic and renal functions were observed after artesunate administration. CONCLUSIONS: Animal experiments and field trials have demonstrated good efficacy of artesunate on killing schistosomula with little side effects. Thus, the drug is suggested to be used on the population in endemic areas for prevention of schistosomiasis.

Effect of extended parietal cell vagotomy on long-term nutrition and digestive function.

OBJECTIVE: To study the effect of extended parietal cell vagotomy (EPCV) on long-term nutritional status and digestive-absorption function. PATIENTS AND METHODS: Thirty-six patients undergoing EPCV were compared with 30 patients with subtotal gastrectomy (SG) 7-14 years after operation. The patients were evaluated in terms of nutritional status, gastric secretion, gastrointestinal motility, and intestinal digestive-absorption function. RESULTS: The nutritional status was generally less satisfactory in 33.3% patients with anemia after SG and body weight decreased in 54%. The patients after EPCV were all in good nutritional status, and had no symptoms of anemia; 56% of them had body weight increased by more than 3 kg. The mean basal acid output (BAO), maximal acid output (MAO), and peak acid output (PAO) of the EPCV group were significantly reduced after operation (P < 0.01), and moderately reduced as compared with those of the control group (P > 0.05). But the BAO, MAO, and PAO of the SG group were more markedly reduced than those of the control group (P < 0.001) before operation. Vitamin B12, folacin, intrinsic factor and iron protein level of serum in the EPCV group were slightly decreased as compared with those of the control group (P > 0.05), but more significantly decreased in the SG group than in the control group (P < 0.001). Iron protein and intrinsic factor level was lower than the normal level. The gastric emptying time and small intestinal passthrough time of EPCV were more close to the normal physiological status, but in the SG group, they were significantly fast (P < 0.001). The results of intestinal D-xylose absorption test in EPCV and SG groups were similar (P > 0.05). CONCLUSIONS: EPCV is capable of preserving gastric acid of physiologic level and normal digestion-absorption function. The effect of EPCV on long-term nutrition and digestion absorption function was extremely milder than that of SG. Therefore, a theoretical basis is provided for the rationality of EPCV.

[Study on sister chromatid exchange in lymphocytes of women with fetal intrauterine growth retardation].

OBJECTIVE: To study sister chromatid exchange (SCE) frequency in lymphocytes of pregnant women with intrauterine growth retardation (IUGR) infants. METHODS: The frequency of SCE in peripheral blood lymphocytes of 14 pregnant women with IUGR babies and 90 normal pregnant women as well as the umbilical blood of their infants were investigated. RESULTS: The mean frequency of SCE in maternal and umbilical blood with normal pregnancy were 7.58 +/- 0.32 and 5.05 +/- 0.29 respectively, while they were 10.53 +/- 2.69 and 7.25 +/- 1.34 in IUGR group respectively. The difference of SCE levels between the 2 groups of mother was statistically significant (P < 0.01), so was that of the umbilical blood group. CONCLUSION: One of the causes of IUGR may be related to heredity injury. Thus, SCE will be a new diagnostic index to predict IUGR.

[The changes in plasma nitric oxide level in normal pregnant women and pregnancy induced hypertension].

OBJECTIVE: To investigate the relationship between nitric oxide (NO) level and pregnancy induced hypertension (PIH). METHODS: The plasma level of NO in 36 cases of normal pregnant women (normal pregnancy group), 23 cases of PIH (PIH group), and 16 cases of non-pregnant women (control group) were measured and compared using fluorophotometry. RESULTS: The NO level of normal pregnancy group was significantly higher than that of control group (P < 0.01), particularly in the second trimester of pregnancy. The NO level of PIH was significantly lower than that of third trimester of normal pregnancy (P < 0.01), and it decreased as the disease became severe. CONCLUSIONS: Decreased synthesizing and releasing of NO might play an important role in pathogenesis of PIH.

[Expression of cytokines in labial gland of Sjögren's syndrome].

OBJECTIVE: To explore the role of cytokines in the process of inflammation and destruction of minor salivary gland in Sjögren's Syndrome. (SS) METHODS: Using in situ non-isotope double hybridization method, we detected the mRNA of the following cytokines: TNF alpha, IL-1 beta, IL-6, PDGF alpha and PDGF beta in 24 labial gland biopsied sections. RESULTS: No cytokine could be detected in the normal labial glands sections. The positivity of all cytokine expression was higher in the pSS and sSS patients than that in the control. The intensity of TNF alpha expression was 3-4 fold increase in pSS compared with sSS. The different cytokines were expressed with the different grade of lymphocytic focus. The site of different cytokines was also expressed differently. The coexpression of mRNA of TNF alpha and IL-1 beta or TNF alpha and IL-6 was only seen in the inflammatory mononuclear cells but not others. The expression of TNF alpha in aciniar cells could occur earlier than the infiltration of lymphocytes in the interstitial region. It was found that the IL-6 positive group had higher positivity of serum ANA compared with the IL-6 negative group according to Ridit statistical analysis (P < 0.05). The serum anti-SSA and anti-SSB antibody had an increase tendency in the IL-6 positive group. CONCLUSION: We hypothesize that the development of SS may be secondary to some trigger agents, which activated the aciniar cells in exocrine gland of predisposed individuals. The activated acini cells secreted TNF alpha, then TNF alpha induced other cytokine expression, followed by the lymphocytes infiltration, which led to autoimmne reaction and the destruction of the exocrine gland.
