[Serum concentration of interleukin-6 in maternal serum and its expression in amniochorionic membranes of preterm premature rupture of membrane].

OBJECTIVE: To investigate the relationship between interleukin-6 (IL-6) and preterm premature rupture of membranes (PPROM). METHODS: Serum IL-6 in 20 cases of normal pregnancy (control group) and 30 cases with PPROM in which 14 had moderate and severe chorioamnionitis (group 1), 16 had no chorioamnionitis (group 2) were measured by enzyme-linked immunosorbent assay (ELISA), and IL-6 protein in fetal membranes were determined by immunohistochemistry technique. RESULTS: The level of serum IL-6 in group 1 (155.17 ng/L) was significantly higher than that in group 2 (79.11 ng/L) and control group (70.15 ng/L) (P < 0.01). Expression of the IL-6 had more positive signs in chorion with chorioamnionitis, especially in infiltrating inflammatory cells. CONCLUSION: IL-6 associated with the onset of preterm labor caused by infection.

Inhibitory effect of ligustrazine on proliferation of rabbit vascular smooth muscle cells after arterial injury.

AIM: To study the inhibitory effect of ligustrazine (Lig) on growth of cultured rabbit aortic vascular smooth muscle cells (VSMC) after balloon injury. METHODS: Twenty New Zealand white rabbits were subjected to arterial injury with a balloon catheter (810 kPa for three consecutive inflations, 1 min each time). The uptake of [3H]thymidine in primary cultural VSMC incubated with rabbit serum, which obtained from the animals treated without or with Lig (40 mg.kg-1.d-1, i.v.) for 21 d (7 d before and 14 d after the injury procedure) was determined. The determination was performed in direct addition of TMP to culture as well. And histological cross-sections of the blood wall were also analyzed. RESULTS: After balloon injury the intimal thickening (77 +/- 23) microns and lumen diameter narrowing (877 +/- 118) microns in dilated sites were increased significantly than the normal adjacent wall [(41 +/- 13) microns, P < 0.01; (1033 +/- 175) microns, P < 0.05, respectively]. Treatment with Lig decreased both intimal thickening (56 +/- 16) microns (P < 0.05) and lumen diameter narrowing (1023 +/- 157) microns (P < 0.05). Lig inhibited [3H]thymidine uptake in VSMC incubated with the serum obtained from these rabbits. Direct addition of Lig inhibited [3H]thymidine uptake in cultured VSMC in a dose-dependent manner (40-4000) micrograms/well. CONCLUSION: Lig shows a pronounced inhibitory effect on VSMC proliferation after balloon injury.

[Study on the relationship between first ovulation and infant feeding in lactating postpartum women].

OBJECTIVE: To determine the first ovulation after delivery and to provide scientific data for contraception in lactating postpartum women. METHODS: From January 1996 to December 1998, 101 lactating women were continuously monitored of their ovulation by ultrasound, basal body temperature (BBT) measurement and cervical mucus examination. RESULTS: The evidences of first ovulation (follicle > 1.8 cm in diameter) were found by ultrasound in 53 (52.5%) women during 154 days after delivery on the average and there were 11 (10.9%, 11/101) subjects who had first ovulation during 4 months after delivery and 42 (41.6%, 42/101) subjects had, after 4 months. Significant positive correlation was found between the time of supplementary feeding and the time of the first menses (n = 100, r = 0.4764, P < 0.01) and first ovulation (n = 53, r = 0.5554, P < 0.01). CONCLUSION: For postpartum mother it is suitable to begin with birth control method at the 4th month after delivery and to prolong the breast feeding is also a good method for birth control.

[Female malignant tumors of genital organs with pericardial effusion: report of 9 cases and review of literatures].

OBJECTIVE: To study the clinical characteristics and the management of malignant tumors of genital organs with pericardial effusion. METHODS: Nine cases of gynecologic malignant tumors with pericardial effusion were analyzed retrospectively. RESULTS: Nine cases of gynecologic malignant tumors were separated into 4 cases of epithelial ovarian carcinoma, 2 cases of diffuse ovarian granulosa cell tumor, 2 cases of cervix carcinoma and 1 case of endometrial carcinoma. The mean age was 41.6 years, the intervals from primary tumors to pericardial effusions were 2-49 months. The clinical symptoms were mainly severe shortness of breath. Nine cases were diagnosed by chest X-rays, 6 of them were confirmed by B ultrasonography simultaneously. Three cases were given pericardiocentesis and their cytologic examinations were positive. Two cases of ovarian malignant tumors have been surviving for 49 and 12 months respectively after pericardiocentesis with local chemotherapy and effective systemic chemotherapy, 1 case was survived for 5 months by hormone therapy, but 6 untreated cases were died of cardiac tamponade within one month. CONCLUSIONS: Malignant tumors of genital organs with pericardial effusion could be definitely diagnosed by X-ray, B ultrasonography and cytology. The surviving period may be prolonged by pericardio centesis combined with systemic effective chemotherapy.

[Re-assessment of the inframammary crease ligament].

OBJECTIVE: An anatomical study was carried out to examine the existence of the inframammary crease ligament. METHODS: 18 breasts of preserved female cadavers were dissected and dyed for the fascia. RESULTS: The so-called inframammary crease ligament described by other authors does not exist. CONCLUSION: The reinforced Cooper's ligament supports the inframammary fold in position.

[Clinical application of 96 hours' rapid skin tissue expanding technique].

OBJECTIVE: To reduce expansion course, diminish complications and improve expanding effect in skin tissue expander treatment. METHODS: We have chosen 38 cases of skin tissue defects of small and moderate areas, on which we have carried out 96 hours' rapid skin tissue expanding technique by altering the time and the amount of saline injected. RESULTS: All skin tissue defects have been successfully repaired. Half a year later, the incisions were inconspicuous. CONCLUSION: 96 hours' rapid skin tissue expanding technique has the following advantages, it is simple and safe, economic and convenient, rapid and reliable. This technique tends to be mature. What is more, it is easy to be popularized. We regard it as an ideal rapid expansion technique.

[A study of innervation of musculus temporalis and its clinical application].

OBJECTIVE: To study the innervation of the musculus temporalis and improve the successful rate of the operation for paralytic eyelid ectropion. METHODS: Anatomic dissection was performed in 12 fresh cadavers. RESULTS: The data show that the distance between the outer canthus and the crus helicis is 8.733 +/- 1.311 cm. The deep temporal nerve spread radially to the edge of musculus temporalis at 3.852 +/- 0.353 cm and 6.317 +/- 0.330 cm at a 60-degree angle to the horizontal line. The rami of the deep temporal nerve rise at a 60-degree angle to the horizontal line in 3 parts from the outer canthus to the crus helicis. CONCLUSION: In transferring the temporal muscle to treat paralytic ectropion, it is safe to make incisions at the middle one-third between the outer canthus and the crus helicis. The discovery has been used to treat 32 patients with eyelid ectropion of facial palsy, among whom 30(93.8%) got good results.

[Urinary diversion in the one-stage hypospadias repair].

OBJECTIVE: To investigate whether the urinary diversion is helpful for healing of the neourethra. METHODS: Twenty-four cases that had received one-stage urethroplasty were divided into two groups and analyzed retrospectively. The urinary diversion was used in one group of 10 cases. In another group of 14 cases no urinary diversion was used. RESULTS: One case developed a fistula and wound infection in the urinary diversion group. In the no urinary diversion group, 2 out of 5 cases treated with the penile ventrolateral island skin flap urethroplasty developed wound infection and fistulas; excellent results were achieved in all 4 cases treated with the scrotal septal vascular pedicle skin flap urethroplasty; 2 cases developed fistulas and one case had wound infection in the 5 cases treated with the transverse preputial island flap urethroplasty. CONCLUSION: The urinary diversion was not to be used in one-stage urethroplasty using scrotal septal vascular pedicle skin flap. When the neourethra was reconstructed by the penile ventrolateral island skin flap or the transverse preputial island flap, urinary diversion is helpful for spontaneous healing of small fistulas and useful in reducing the incidence of fistulas.

Resistant mechanisms of Candida albicans to fluconazole.

OBJECTIVE: To detect the resistant mechanisms of Candida albicans to fluconazole (FCZ) at molecular biology level, since the resistance mechanisms of azole antifungal agents have been the focus of attention these years. METHODS: Thirty-two FCZ-resistant C. albicans were selected as our test strains (MICs > or = 64 micrograms/ml). With 14-alpha-demethylase gene (ERG16 gene, target enzyme encoding gene of azoles) as our object, we chose six sets of primers from the ERG16 gene to amplify the interested fragments, and conducted Southern blot hybridization, restriction fragment length polymorphism (RFLP), and single strand conformation polymorphism (SSCP) analysis for the fragments which were amplified by the six sets of primers, and pre-resistant sensitive strains were used as controls. Three representative fragments, A66, D66 and E78, were selected to be cloned and sequenced. RESULTS: The polymerase chain reaction (PCR) amplification showed that several tested strains were negative for some primers. However, our Southern blot analysis reminded that their resistance did not result from the lack of target enzyme coding gene. SSCP analysis showed that differences were noted between the resistant and sensitive strains and inter-resistant strains. Statistical analysis showed that the most variable sequence lied in the amplifier of the sixth pair of primer, and all the tested 32 strains showed positive results. In the 11 mutation points we found, five resulted in amino acid alterations. It is likely that one or more mutational alterations (alone or in combination) might lead to the expression of an enzyme highly resistant to the inhibitory action of FCZ which in turn is responsible for the FCZ resistant trait in these strains. CONCLUSION: One or more mutational alterations might lead to the azole resistant trait in this strains.

Surgical treatment of trigeminal neurinomas.

OBJECTIVE: To investigate the best surgical approach for the removal of trigeminal neurinomas (TNs). METHODS: A retrospective analysis of 75' patients with TNs in Huashan Hospital was carried out. RESULTS: In the early group (1978-1984), a series of conventional intradural approaches were used; in the late group (1985-1995), an epidural approach via the skull-base craniotomy was used. Total tumor removal was achieved in 58% (20/35) of patients in the early group and 80% (32/40) in the late group (P < 0.025). Temporary and permanent cranial nerve morbidity were 62.7% and 37% in the early group and 28.1% and 10% in the late group (P < 0.001). CONCLUSION: The best microsurgical approach for the removal of trigeminal neurinomas except those confined to the posterior fossa is epidural approach or epidurotransduro-transtentorial approach via the skull-base craniotomy.

A rapid one-step method of EIA for detection of circulating antigen of Schistosoma japonicum.

OBJECTIVE: To establish a highly sensitive serologic method for detecting the circulating antigen of Schistosoma japonicum for the diagnosis of this disease and the evaluation of drug therapy effect. METHODS: A set of complex support made of polyvinyl-chloride (PVC) film slide, adsorbed with specific antibody, was used to test serum samples collected from cases with schistosomiasis japonica, by adding specific enzyme conjugate and substrate tetra-methyl benzidine (TMB) with 3% H2O2 in 10:0.1, developed or prepared in our laboratory. Tests were carried out with the set on cases with malaria, paragonimiasis, clonorchiasis, visceral leishmaniasis and cysticercosis as well as normal individuals as control. Serum sample of 50 microliters was used in each test and reacted with reagents at room temperature for 30 minutes. The color of positive reaction was blue while negative reaction was colorless. RESULTS: Positive rate among cases with schistosomiasis japonica was 100% (45/45) in acute stage, 94.8% (530/559) in early chronic stage and 52.4% (66/126) in late stage. False positive reaction was found neither among all normal individuals (0/513), nor among cases of 155 with malaria, 120 with clonorchiasis, 24 with visceral leishmaniasis and 110 with cysticercosis, except one among 29 cases with paragonimiasis (1/29). CONCLUSIONS: The rapid one-step enzyme immunoassay (EIA) to detect circulating schistosome antigen (CSA) has been well established in our laboratory with high sensitivity and good specificity as well as reproducibility. The reaction result can be read easily even in field conditions without power supply. Moreover, the assay is time-saving, simple to handle and suitable for the diagnosis of schistosomiasis japonica and evaluation of drug therapeutic effect in practice in the control program of the disease.

Interstitial brachytherapy using iridium-192 for malignant brain tumors: clinical results.

OBJECTIVE: To determine the effects and toxicity of interstitial brachytherapy using iridium-192 on brain malignant gliomas. METHODS: Between January 1992 and January 1995, 56 patients with anaplastic astrocytoma and glioblastoma multiforme were treated with stereotactic brachytherapy using temporary high-activity iridium-192 sources. RESULTS: The median survival for patients receiving brachytherapy was 28 months. The survival rates at 1, 2, 3 years were 92.8%, 83.9% and 71.4% respectively. CONCLUSIONS: Brachytherapy may improve the control of local tumor and prolong the survival, when used in deep malignant brain gliomas, by temporary implanted high doses of iridium-192 sources.

[The scapular free flap crossing the dorsal midline].

OBJECTIVE: A large defect needs a large flap to repair. This clinical study was to explore the possibility and safeness of transferring the extended scapular free flap. METHOD: We have used the transverse scapular flap to reconstruct large defects in the mandibular or cervical region in 8 patients. The flap was designed to cross the midline of the back as far as 15 cm. The largest flap measured 31 x 34 cm. RESULTS: Of the 8 flaps, six survived completely; local epidermal necrosis appeared in two. CONCLUSION: The free scapular flap supplied by a single circumflex scapular vascular pedicle can be safely extended beyond the midline of the back. The flap axis is the horizontal branch of the circumflex scapular vessel.

[The periareolar incision technique for reduction mammoplasty].

OBJECTIVE: A technique is introduced to reduce the scar of classic reduction mammoplasty. METHODS: Two concentric circles were designed with the nipple in the centre. The skin between the two circles was deepithelized. Then the glandular tissues situated mainly in the lateral superior quadrant are resected. Finally, round block suturing is made. Eight patients were operated on with this method. RESULTS: The operative results were satisfactory with inconspicuous periareolar scar. CONCLUSION: Periareolar round block reduction mammoplasty is a simple, practical and easy method.

[Surgical treatment of frontal lobe epilepsy--40 cases].

OBJECTIVE: To compare the methods and outcome of the treatment of frontal lobe epilepsy (FLE). METHODS: We combined such techniques, as intraoperative electrocorticography (ECoG), corpus callostomy(CCS), and multiple subpial transection(MST) to improve the surgical results. When preoperative semiology suggested the involvement of the bilateral frontal lobe or ECoG detected bilaterally synchronized epileptic discharged, anterior corpus callosotomy was performed before the main surgical procedure. MST was applied to the unresectable motor or involved speech zone. This technique was also useful for the treatment of widely disseminated epileptic zone. Satisfactory results were obtained in surgical treatment of FLE. RESULTS: One month after operation, most patients were free from seizure. No severe complications and operative death occurred. Follow up lasted for 0.5 to 3 years(mean 2.5 years). Seizure free was noted in 19 patients(47.5%), seizure attack less than 3 times per year or no real attack in 11(27.5%), significant reduction of seizure attack in 6(15%); and no change in 4 (10%). The total effective rate was 90%. CONCLUSION: Indicated by ECoG, combined resection with corpus callosotomy or multiple subpial transection is an effective method for FLE.

[Correlation between apoptosis and proliferation in gastric pre-carcinoam].

OBJECTIVE: To clarify the correlation between apoptosis/proliferation and the expressions of oncogene bcl-2 and P53 in the changes from superficial gastritis, intestinal metaplasia, dysplasia, gastric cancer in early stage to advanced gastric cancer. METHODS: TUNEL method was used to detected apoptosis. Immunohistochemical analysis was used to detect the expressions of proliferating cellular nuclear antigen(PCNA), bcl-2 protein and P53 protein. Results In the patients of superficial gastritis, atrophic gastritis, intestinal metaplasia, dysplasia, early gastric cancer and advanced gastric cancer and advanced gastric cancer, apoptotic indexes were: (6.4 +/- 1.9)%, (14.1 +/- 4.1)%, (23.3 +/- 6.1)%, (17.4 +/- 2.6)%, (11.3 +/- 3.7)%, (6.3 +/- 2.0)% respectively. PCNA indexes were: (11.3 +/- 2.2)%, (18.9 +/- 6.5)%, (20.3 +/- 7.3)%, (40.0 +/- 10.6)%, (53.1 +/- 10.9)% and (72.4 +/- 18.4)% respectively; the expression rates of bcl-2 were 10.0%, 23.3%, 40.0%, 56.7%, 85.7% and 46.7%. The expression rates of P53 were: 0%, 0%, 0%, 4.3%, 14.3% and 53.3%. The difference in the majority of observed indexes were significant (P < 0.05-0.01). CONCLUSION: During the changes of gastric cancer, the apoptotic and proliferating indexes incerased simultaneously in superficial gastritis, atrophic gastritis and intestinal metaplasia. However, PCNA indexes further increased but apoptotic indexes decreased in dysplasia, early gastric cancer and advanced gastric cancer and advanced gastric cancer. The former may be the phenomenon in early gastric cancer and advanced gastric cancer and advanced gastic cancer. The former may be the phenomenon in ear early gastric cancer the latter may be the phenomenon in advanced gastric cancer.

[Expression of tumor metastasis suppressor gene KAI1/CD82 in human cancer cell lines with different metastasis potential].

OBJECTIVE: To investigate the correlation of a newly-defined tumor metastasis suppressor gene KAI1/CD82 and metastatic potential of human cancer cell lines. METHODS: Expressions of KAI1/CD82 in eight human cancer cell lines of prostate, lung and melanoma with different metastasis potential were studied by Northern blot hybridization and the mutation were detected by PCR-SSCP. RESULTS: Expressions of KAI1 mRNA decreased in human prostate cancer cell lines PC3 and PC3M, and lung cancer cell line PG which were metastatic cancer cell lines (integral light density values: 0.0319, 0.0266 and 0.0549, respectively). Expression of KAI1 mRNA showed high level in PAa, a human lung cancer cell line with no metastasis (integral light density value: 0.7313). In human melanoma cell lines with different metastasis potential (WM35, WM1341b, WM983a and WM451), expression of KAI1 showed moderate expression (integral light density values are 0.1798, 0.1582, 0.1501, 0.1800 respectively). PCR-SSCP analysis indicated the existence of band shift in exon 7 of KAI1 gene in PC3, PG and PC3M. CONCLUSION: Decreased KAI1/CD82 expression may be associated with enhanced metastatic potential in prostate cancer cell lines and lung cancer cell line other than melanoma cell lines. The decreased expression may be partially nelaled with mutation of KAI1 gene.
