Ultrasonically guided percutaneous aspiration of human follicles under local anesthesia: a new method of collecting oocytes for in vitro fertilization.

Follicular aspiration was performed in 30 infertile patients by ultrasonically guided percutaneous puncture. In 18 cases the procedure was carried out under local anesthesia and in the remaining 12 under general anesthesia. The follicles were located with a dynamic sector scanner, and a needle was introduced through the abdominal wall and bladder into the follicles. Fifty follicles with a mean diameter of 19.6 mm were aspirated, and 26 oocytes were collected, giving an average recovery rate of 52%. Oocytes were obtained in 17 (57%) of the cases. Extensive pelvic adhesions did not impede the procedure, and the only complication was a transient hematuria in four patients. The technique was found to be atraumatic and inexpensive and therefore considered valuable for harvesting oocytes for in vitro fertilization.

Influence of lactation on oral glucose tolerance in the puerperium.

The influence of endogenous hyperprolactinaemia on oral glucose tolerance in normal women post-partum was studied. An oral glucose tolerance test (OGTT) was performed 8 weeks after delivery in 23 women, 13 of whom were breastfeeding. The lactating women had significantly higher plasma prolactin values before and during the OGTT than the non-lactating women (P less than 0.005). Fasting values of serum FSH, LH, progesterone and plasma glucagon were similar in the two groups. The non-lactating women had higher fasting concentrations of serum oestradiol (P less than 0.0005), plasma glucose (P less than 0.05) and insulin (P less than 0.05) than the lactating women. After oral glucose no differences were found between lactating and non-lactating women in changes in plasma glucose, insulin and glucagon concentrations. It is concluded that lactating women 8 weeks post-partum do not have an impaired OGTT compared to non-lactating women despite elevated plasma prolactin levels.

[First results of experimental animal studies on pervascular effects of indwelling intravasal catheters upon free tissue grafts. First communication (author's transl)].

This is the first in a series of two papers reporting first results of studies made to set up a basic model of intraarterial perfusions in free tissue grafts. Epidermal tissue grafts were obtained from thirty-two domestic pigs prior to slaughtering and pervascularly perfused for twelve to ninety-six hours. Histological examinations showed that it is possible for tissue necrosis to be arrested as a prerequisite of subsequent in vivo experiments performed with the object of pervascularly supporting the healing-in of tissue grafts.

[Results of experimental animal studies on pervascular influences of indwelling intravasal catheters upon free tissue grafts (author's transl)].

Free tissue grafting procedures were performed on sixteen experimental animals on the basis of previously described experiments (first communication). The objective of finding a suitable operating method for pervascular perfusion of epidermal grafts in animal experiments has been accomplished. At the same time, several possible ways of objectifying the results of blood gas analyses, temperature measurements, colorimetric determinations, and histological studies were subjected to tests.

Is the corpus luteum normal after ovulation induction?

Repeated ultrasound examinations and blood samplings for determination of estradiol (E), progesterone (P), and luteinizing hormone (LH) were carried out in 15 normal and 11 clomiphene citrate (CC)/human menopausal gonadotropin (hMG) stimulated cycles. Human chorionic gonadotropin (hCG) was administered in the stimulated cycles on the day of the expected LH-peak as determined in each woman's normal cycle. Ovulation and normalcy of the luteal phase were confirmed by the hormonal values. Ultrasound examinations showed development of a single follicle in the normal group and development of 4.1 follicles on average, of 15 mm or more in diameter in the stimulated group (total of 45 follicles). Midluteal phase images at the site of the former follicle showed echogenic structures of echo-free structures with scattered echoes in 25 cases in the stimulated group and in 11 cases in the normal group. There was no sign of the former follicle in 1 and 3 cases, respectively, in the two groups. A significantly higher number of "persistent follicles" was seen in the stimulated group: 42% vs. 7%. It is discussed whether a compromised corpus luteum function occurs after stimulation despite a normal luteal phase.

A possible prenatal evaluation of renal function by amino acid analysis on fetal urine.

Intrauterine treatment of 4 fetuses with urethral obstruction was attempted in the third trimester of pregnancy. The fetuses displayed varying sonographic findings including pyelectasis, caliectasis, hydroureter, bladder dilatation, ascites, hydrops, missing kidneys and oligohydramnios. Ultrasonically guided aspiration from the dilated structures was carried out to relieve pressure on the kidney parenchyme and to collect fluid samples for diagnostic purposes. Amino acid concentrations in the fetal urine showed a pattern similar to plasma in 2 fetuses, a pattern almost like urine in 1 fetus and an intermediate pattern in the 4th fetus. Only the fetus with normal amino acid concentrations in the urine survived: the other 3 died in uremia shortly after birth. In 3 cases cells from the aspirated urine were cultured and used for chromosome analysis. The cell cultures grew fast and karyotyping was possible within 1 week. In 2 fetuses an intrauterine catheter was inserted to drain the kidney permanently into the amniotic cavity. In the first case the catheter was displaced to the fetal abdomen after some days of successful drainage. In the second case the catheter tore the placenta, and the child had to be delivered immediately.

Ultrasonically guided aspiration of human oocytes.

Ultrasonically guided puncture of ovarian follicles was attempted in 125 menstrual cycles to collect oocytes for in vitro fertilization and embryo transfer. The punctures were performed through a full urinary bladder under local anesthesia on out-patients. A sector scanner with a puncture transducer guided a stainless steel needle and the follicular fluids were aspirated either in a chamber connected by tubings to a controlled vacuum and the needle or in a syringe attached directly to the needle. Follicles were punctured in all but 2 cycles and oocytes were collected in 80% of the cycles. A total of 287 follicles with diameters ranging from 14 to 30 mm were punctured. The oocyte recovery rate per follicle was 64%. The only complication was transient hematuria in 7 patients.

Coccygeal muscle injury in English Pointers (limber tail).

A condition colloquially referred to as "limber tail" and "cold tail" is familiar to people working with hunting dogs, primarily Pointers and Labrador Retrievers. The typical case consists of an adult dog that suddenly develops a flaccid tail. The tail either hangs down from the tail base or is held out horizontally for several inches from the tail base and then hangs straight down or at some degree below horizontal. Initially, the hair on the dorsal aspect of the proximal tail may be raised and dogs may resent palpation of the area 3-4 inches (8-10 cm) from the tail base. Most dogs recover spontaneously within a few days to weeks. Anecdotal reports suggest that anti-inflammatory drugs administered within 24 hours after onset hasten recovery. Less than one half of affected dogs experience a recurrence. Affected Pointers almost always have a history of prolonged cage transport, a hard workout the previous day, or exposure to cold or wet weather Most owners and trainers familiar with the condition do not seek veterinary assistance. In cases where people are not familiar with this disease, other conditions such as a fracture, spinal cord disease, impacted anal glands, or prostatic disease have been incorrectly diagnosed. We examined 4 affected Pointers and found evidence of coccygeal muscle damage, which included mild elevation of creatine kinase early after onset of clinical signs, needle electromyographic examination showing abnormal spontaneous discharges restricted to the coccygeal muscles several days after onset, and histopathologic evidence of muscle fiber damage. Specific muscle groups, namely the laterally positioned intertransversarius ventralis caudalis muscles, were affected most severely. Abnormal findings on thermography and scintigraphy further supported the diagnosis.

Significance of the ultrasonic morphology of preovulatory ovarian follicles prior to in vitro fertilization.

Forty-eight oocytes were collected from 47 follicles in 36 cycles from 24 patients with tubal infertility by ultrasonically guided percutaneous puncture under local anesthesia. The ultrasonic represented morphology of the aspirated follicles was compared to the presence of an egg-cumulus-mass in the follicular aspirate and the result of oocyte fertilization. There was a higher frequency of a cumulus-mass in the aspirate from follicles containing echoes (90 per cent) compared to ultrasonically empty follicles (37 per cent). Thirty-four oocytes were fertilized corresponding to a fertilization rate of 71 per cent. The mean diameter of follicles in the fertilized group was bigger (20.9 mm) than the mean diameter of follicles in the non-fertilized group (18.5 mm). The fertilization rate was higher in oocytes collected from follicles with internal echoes (87 per cent) compared to echo-free follicles (50 per cent). It seems that the ultrasonic demonstration of a fine cloud projecting into the follicle or a clouding of the cavity increases the possibility of collecting an oocyte suitable for in vitro fertilization.
