[Evaluation of reliability of IPSS by 24-hour uroflowmetry].

BACKGROUND: Although the international prostate symptom score (IPSS) is now often used to assess the symptoms of BPH, whether or not patients answer the questions correctly has not been validated objectively. METHODS: Reliability of IPSS was evaluated by 24-hour uroflowmetry in 20 hospitalized male patients. Six of them had prostatic hypertrophy or cancer, and the evaluation was performed before and after TURP or hormonal therapy in these six patients. The objective scores for frequency and nocturia were obtained from the time recorded on IC card. The objective score for intermittency was calculated from the uroflow curves. RESULTS: The answer about frequency was not correct compared with the objective scores for frequency. The answers about nocturia and intermittency were almost the same as the corresponding objective scores. The answer for weak stream correlated with the average of peak flow rate. However, the threshold of peak flow rate for "weak stream" fluctuated markedly before and after the treatment in the same individuals. The answer for hesitancy had no correlation with the hesitation time. Patients seemed to understand the question translated in Japanese as "have you need force to urinate?" CONCLUSION: Before wider use of IPSS in Japan, the correct translation of the questions and verification of the usefulness of the questions in large number of Japanese patients seem necessary.

[Evaluation of advanced bladder tumor].

Ninety-two cases of advanced bladder tumor treated at the University of Tokyo and branch hospital from January 1977 to December 1992 were analyzed. The advanced bladder tumor was defined as that of higher than pT2 (according to the General Rule for Clinical and Pathological Studies on Bladder Cancer) or that with distant metastases. The following variants were evaluated, the therapeutic methods, the histological type, grade, stage, type of infiltration. The evidence of lymphatic infiltration, vessel infiltration, and lymph node metastases were also reviewed. The survival rate was calculated using Kaplan-Meier's method. In the cases with lymph node metastases, the survival rate was significantly lower than in the cases without metastases (p < 0.01), while no other factors affected the survival rate.

[Histochemical expression of proliferating cell nuclear antigen (PCNA) for pre and post chemotherapeutic bladder cancer].

We examined whether the staining of proliferating cell nuclear antigen (PCNA) could be useful as a marker of chemotherapeutic effectiveness in nine patients with invasive bladder cancer treated by bilateral internal iliac artery infusion of cisplatin (CDDP). Two types of monoclonal antibodies PC-10, 19A2 were used for immunohistochemical staining in formalin-fixed, paraffin-embedded tissue sections. There was no difference between positive rates of PC-10 and 19A2. The mean positive rates between pre-chemotherapeutic specimens (15.4 +/- 6.7%) and post-chemotherapeutic specimens (4.2 +/- 3.1%) showed statistically significant difference (p < 001). The immunohistochemical evaluation of PCNA has value for investigation of cell's turnover. Therefore, the changes of PCNA-positive rate could be a historical parameter for the evaluation of chemotherapeutic effects.

Detection of a glycosphingolipid antigen in bladder cancer cells with monoclonal antibody MRG-1.

The monoclonal antibody MRG-1 has been evaluated for the immunohistochemical detection of the type 3 chain of blood group A in human normal bladder epithelium and bladder tumours. Light microscope examination of paraffin sections demonstrated that this antigen was present in normal epithelium and superficial bladder tumour in patients with blood group A or AB, but was absent in the invasive type of bladder tumour. In normal epithelium, the plasma membrane was positive for this antigen, and the cytoplasm was diffusely stained. In superficial transitional cell carcinoma, the plasma membrane was negative, whereas the cytoplasm was intensely stained in the perinuclear region. This pattern was different from that observed for type 1 and 2 group A antigen, which was recognized mainly at the plasma membrane. However, in superficial transitional cell carcinoma, the staining was also seen on the plasma membrane. The pattern of the localization of this antigen in this carcinoma was influenced by the treatment of organic solvents. Electron microscopical observations confirmed that this antigen was localized on the plasma membrane and also in the Golgi apparatus of the superficial tumour. These results proved that the type 3 chain of blood group A is present in human bladder epithelium and low grade tumours in correspondence with the blood type, but disappears in tumours with high malignant potential. However, its expression is independent of the expressions of the other subtypes which have been studied. Furthermore, the changes in the staining pattern caused by pretreatment with organic solvents suggested possible differences in the microenvironment of the glycolipids containing this type of sugar chain.

Quantitative autoradiography of alpha 1 adrenoceptors with [3H]tamsulosin in human hypertrophied prostate using computerized image analysis.

Fourteen specimens of human hypertrophied prostate were evaluated for the distribution of alpha 1 adrenoceptors using autoradiography with a computerized image analysis system. The hypertrophied prostatic specimens, obtained at open prostatectomy, were dissected vertically to the urethra, and sectioned at 10 microns. They were immersed in 1 nM of specific alpha 1 ligand, [3H]tamsulosin chloride ([3H]tamsulosin) and exposed to autoradiographic film. The images were analysed by a computerized image analysis system. The total binding of [3H]tamsulosin in the whole section (n = 14) was 0.82 +/- 0.21 (mean +/- SE) nCi mg-1. The autographic data were correlated with data obtained in a membrane-binding assay. The prostatic tissue studied was divided into urethral, glandular and stromal zones, the latter two zones being further divided into the inner and outer areas. The total binding of [3H]tamsulosin in the urethral zone (n = 7) was 0.65 +/- 0.32 nCi mg-1. The glandular zone contained significantly more abundant alpha 1 adrenoceptors than the stromal zone and their densities (glandular vs stromal) were 1.15 +/- 0.19 nCi mg-1 (n = 14) vs 0.72 +/- 0.15 nCi mg-1 (n = 14), respectively (p < 0.05). The data from the whole section were not affected by prostatic weight. This method described enabled the distribution of the receptors in different sites to be evaluated both morphologically and quantitatively.

Multidirectional contraction of human hypertrophied prostate.

1. The purpose of the present investigation is to evaluate muscle contraction in two directions (longitudinal and circumferential to urethra) physiologically and morphologically for alpha-adrenoceptor agonists. 2. Norepinephrine (10(-7)-10(-4) M), phenylephrine (10(-7)-10(-4) M) and clonidine (10(-7)-10(-4) M) induced contractions in a dose-dependent manner on human prostate from patients with benign prostatic hypertrophy (BPH). 3. No significant differences were observed between longitudinal and circumferential directions of human prostate in 50% of the maximal muscle contraction (EC50 values) and the maximal muscle contractions caused by any agents used. 4. Morphometric analysis for muscle in prostates was performed using formalin-fixed, paraffin-embedded sections stained by the Mallory-Azan method. 5. There was no significant difference in the density of the muscle area between longitudinal and circumferential directions of prostatic strips. 6. These results suggest that there are no significant differences in responsiveness of alpha-adrenoceptor agonists and the smooth muscle contents in longitudinal and circumferential directions to urethra, for human hypertrophied prostate.

[Two-color flow cytometric analysis of lymphocytic subsets in renal cell carcinoma].

The subsets of tumor infiltrating lymphocytes (TILs) in 17 patients with renal cell carcinoma were studied by two-color flow cytometry and immunohistochemistry. In grade 1 tumors, CD4-positive cells were significantly increased compared to grade 2 and 3 tumors (35 +/- 7% vs 25 +/- 10%) (p < 0.05). In 10 of 14 patients, CD8-positive cells were predominant over CD4-positive cells, resulting in less than one in CD4/CD8-ratio. Regarding CD4 cells in all 17 cases, the helper T-cells (CD4-positive and CD45RA-negative) were statistically predominant over the inducer T-cells (CD4-positive and CD45RA-positive) (p < 0.01). When CD8 cells were investigated in 16 out of the 17 cases, the cytotoxic T-cells (CD8-positive and CD11b-negative) were found also significantly predominant over the suppressor T-cells (CD8-positive and CD11b-positive) (p < 0.01). Immunohistochemically, the infiltrating pattern of TILs in the 17 patients was classified into cluster (N = 4), separate (N = 6), and mixed types (N = 7). The tumor in the cluster and mixed type had a tendency to be dominant in low grade tumors, and had the prominent CD8-positive cells (more than 90% were cytotoxic T-cells) over CD4-positive cells. However, no statistical significance was noted in this finding. The advanced tumor tended to show the single type of TIL infiltration. These results suggest that cytotoxic T-cells consisted of most of TILs in cluster and mixed types play an immunoreactive role against renal cell carcinoma.

[Blind duplication of the ureter. Apropos of a case].

The authors report on a case of a 24 year-old man with a double-blind duplication of the ureter, which was diagnosed in the workup of a microscopic hematuria. The patient who had no symptoms and a normal renal function has been followed up at regular intervals. The diagnosis of this malformation is difficult as long as it is asymptomatic and does not affect the excretory function.

Clam nail deformity of the little finger.

A case of a 2-month-old infant who had an extremely rare congenital nail deformity of the right little finger was reported. The nail looked like a clam, wrapping three-quarters of the distal phalanx. A neurovascular pedicle island flap was transposed to reconstruct the pulp and lateral nail fold with good results. Early surgery is justifiable to avoid growth disturbance of the fingertip.

[Cystic lymphangioma of the scrotum. Apropos of a case].

The authors present the case of a cystic lymphangioma in the scrotum, a tumor which usually occurs in the neck, but is rarely observed in this region. Patient was a 22 year-old man, admitted for fever and a mass within the scrotum. The tumor was completely resected and no recurrence has been observed after a 6 month follow-up.

[Metastasis of hepatocellular carcinoma to the urinary bladder].

We report a case of metastasis of a hepatocellular carcinoma to the bladder. Clinically the tumor was suspected to be a primary bladder tumor with subsequent metastasis to the liver. However, the pathological diagnosis yielded different results. The tumor cells resembled liver cells and sometimes bile production was even observed. No therapy was available and the patient died of cachexia 6 months later.

Analysis of human tear proteins by two-dimensional electrophoresis.

Human tear proteins in the conjunctival sac were separated on the basis of the differences in their isoelectric points and molecular weights using micro two-dimensional electrophoresis combined with immunoblotting. The two-dimensional electrophoretic patterns of tear proteins from patients with conjunctivitis were compared with those from normal individuals. We also measured integrated intensities of seven protein spots, lactoferrin (LF), albumin and five specific tear proteins (STP), to examine differences in the amounts of these proteins in tears from normal individuals of different sexes. In the tears from patients with conjunctivitis, secretory immunoglobulin A (IgA), LF and STP spots were stained more weakly, whereas the albumin spot was stained more strongly as compared with those from normal individuals. Furthermore, haptoglobin and IgG spots appeared in the tears from patients with conjunctivitis. These were more prominent in the tears from patients with severe conjunctivitis. There were significant differences in the amounts of LF and two kinds of STPs in the different sexes. The amounts of these proteins were larger in females.

Postcataract against-the-rule astigmatism after phacoemulsification procedure. Characteristic changes over time.

To further determine whether postoperative astigmatism is related to the amount of preoperative astigmatism, we analyzed two types of against-the-rule (AR) astigmatism induced by phacoemulsification and aspiration (PEA) procedures. The operations all utilized 10-0 polyester (Mersilene) in double running fashion for the closure of the corneoscleral incisions. The induction and spontaneous regression of several types of astigmatism were analyzed over 6 months. Surgery was all done by one surgeon, with the same surgical procedure and the same number of sutures. In postoperative AR astigmatism less than 0.7 D at 6 months, although the amount of the astigmatism 1 week postoperatively was remarkably increased (2.66 +/- 0.17 D), postoperative astigmatism was decreased 6 months following surgery. On the other hand, in postoperative AR astigmatism greater than 1.5 D at 6 months, the power of the astigmatism 1 week postoperatively (2.12 +/- 0.13 D) was not much higher than the preoperative level (1.38 +/- 0.08 D), but tended to continue increasing even 6 months after surgery. There was no difference in the power of astigmatism between 1 and 3 months postoperatively. The eyes with preoperative astigmatism (e.g. 1.38 +/- 0.08 D) and especially preoperative AR astigmatism (e.g. 1.60 +/- 0.11 D), showed an increase of their astigmatism after the PEA procedure. We discussed some risk factors which may induce greater postoperative AR astigmatism.

Vogt-Koyanagi-Harada disease in monozygotic twins.

We encountered two cases of Harada's disease in monozygotic twins, and this is the first such report as far as could be determined. Case 1 was a 32-year-old woman who presented a typical clinical picture of Harada's disease. Seven years later, case 2 (the younger sister), also presented with the disease at 39 years of age. Case 2 had a transitional diffuse type of Harada's disease, which was possibly due to differences in her treatment. Both patients had papilledema. Since these twins both developed Harada's disease, it is suggested that hereditary factors including HLA type are very important in its development. The HLA type of both these patients was A2, A26, B51, B7, CW7, DR1, and DR4.

Similarities and differences between the Kelman phacoemulsification and extracapsular cataract extraction procedures in the spontaneous regression of corneal astigmatism from against-the-rule astigmatism.

We perspectively examined the induction and spontaneous regression of corneal astigmatism from preoperative against-the-rule astigmatism [Kelman phacoemulsification (KPE), n = 618; extracapsular cataract extraction (ECCE), n = 192] among cataract extraction surgeries (KPE, n = 1,648; ECCE, n = 443). All data were set on a double-angle coordinate graph, from which the mean +/- SE of the axis and the amount of astigmatism were obtained simultaneously. In the KPE group, the surgically induced astigmatism was independent of preoperative state or age. In the ECCE group, surgically induced astigmatism shifted toward with-the-rule astigmatism, and postoperative recovery was delayed longer in patients over 80 years of age than in those under 70. Postoperative against-the-rule astigmatism was more frequent after KPE. Our first trial to calculate astigmatism from a double-angle coordinate graph was useful in showing astigmatism changes.

Ibudilast may improve retinal circulation in patients with diabetes mellitus.

Ibudilast selectively vasodilates cerebral vessels without reducing blood pressure. We investigated the effect of the drug on retinal circulation in 8 patients with diabetes mellitus, using the video-densitometric image analysis of fluorescein angiography. We compared the build up time, the time constant of washout rate and the mean circulation time (MCT) before and after oral therapy with ibudilast. After 2 weeks of daily administration (30 mg), MCT was shortened significantly (4.2 +/- 2.8 vs. 3.0 +/- 1.6 s, p = 0.0215). Since retinal circulation in patients with diabetes mellitus was improved by ibudilast, the drug may be useful to treat disorders such as diabetic retinopathy.

Effect of moxisylyte hydrochloride (alpha 1-blocker) on the retinal circulation of patients with diabetes mellitus.

Moxisylyte hydrochloride (alpha 1-blocker) selectively vasodilates cerebral vessels without reducing blood pressure. We investigated the effect of the drug on retinal circulation in 15 patients (17 eyes) with diabetes mellitus, using the video-densitometric image analysis of fluorescein angiography. We compared the build-up time (BT), the time constant of washout rate (TC) and the mean circulation time (MCT) before and after oral therapy with moxisylyte. In 16 eyes, BT was shortened significantly and MCT was only slightly shortened 1 h after oral therapy with moxisylyte. After 2 weeks of daily administration, MCT was shortened and TC of the artery was increased significantly in 6 eyes. Since retinal circulation in patients with diabetes mellitus was improved by moxisylyte, the drug may be useful to treat ocular disorders such as diabetic retinopathy.
