[Split skin-mesh transplants to cover burned hands].

Nonexpanded autologous mesh grafts of split thickness skin provide optimal wound drainage through their cuts. They also minimize disability from contraction and scarring when combined with Jobst compressive dressings. These properties assure good functional and cosmetic results in reconstructive surgery after burns. Since 1970 we have treated 18 hands of 15 patients with this method. The follow-up evaluation was performed at least 2 to 3 years after the operation and showed good functional and esthetic results.

[Transsexualism and its therapy].

After a review of the historical development of ideas about transsexualiam and the discussion of possible somatic and psychological causes of this condition, the results of explorations of 12 male and 6 female transsexuals who desired a hormonal and surgical approximation to the opposite sex are reported. Two of the nine patients whose chromosomes were examined were found to suffer from Klinefelter's Syndrome. From the personal history it was apparent that 8 of the 12 male transsexuals had difficulty in identifying with an absent, alcoholic or brutal father. The wish to be a member of the opposite sex could be traced in 10 of the 12 male and all 6 female transsexuals to the first five years of life. 7 of the 12 male transsexuals and 3 of the 6 female transsexuals were found to have attempted suicide or to have had suicidal tendencies in the past. In 9/12 male transsexuals and 5/6 female transsexuals the "change of sex" operation was considered to be indicated. The average age at operation was 28 for the male transsexuals and 26 for the female transsexuals. The follow-up studies were done 2 months to 3 years after operation for the male transsexuals (average 1, 7 years) and 2 to 3 3/4 years post operatively (average 2, 9 years) for the female transsexuals. 7/8 of the male transsexuals from whom we have katamneses and both female transsexuals reported that they felt subjectively better than before the operation. The present legal situation concerning the registration of change of name and sex in various countries is described.

[Indication for subcutaneous mastectomy as carcinoma prevention].

The indication for subcutaneous mastectomy is analyzed. Prospective investigations seem to confirm that different morphological forms of breast dysplasia (grade I, II, III, PRECHTEL [10]) do not implicate differences in risk for development of invasive breast cancer. Only if these forms of breast dysplasia are associated to high risk factors such as family history of breast cancer, should subcutaneous mastectomy be considered as a cancer prophylaxis.

[Transformation of monocytes into fibroblasts in wound healing (author's transl)].

Recent investigations of HELPAP and CREMER 1972 seem to confirm the supposition of COHNHEIM 1867 that monocytes of the peripheral blood are transformed into firbroblasts in healing wounds. The view of VIRCHOW 1871, MARCHAND 1901 and ASCHOFF 1924 that the fibroblasts in healing wounds are of local origin seems to be supported by the invesigation on parabiotic inbred rats of ROSS et al. 1970. We thought that irradiating the whole body of rabbits could be of value in resolving these apparent contradictions about the origin of fibroblasts in healing wounds.

Operative treatment of pressure sores in paraplegics by rotation flap.

Sixty rotation flaps (in 40 paraplegics) to cover pressure sores and unstable scars in the region of the buttocks are reported. There were 12 complications (20 per cent), none jeopardising the result of the operation. Advantages of the rotation flap are its good blood supply if generously outlines and the possibility of using the same flap a second or even third time if necessary.
