Subjective symptoms in temporomandibular joint osteoarthrosis.

Questionnaires about subjective symptoms were sent by post to 120 patients referred to the clinic for mandibular dysfunction. A group of 20 patients with crepitation of the temporomandibular joint (TMJ) and a group of 19 patients with palpatory tenderness of the TMJ were compared with a reference group of 29 patients for any difference in local subjective symptoms, frequency of headache and symptoms in other joints. The patients with TMJ crepitation, which were considered to have TMJ osteoarthrosis, were older and reported a higher frequency of grating sound from the TMJ than the patients in the reference group. The patients with palpatory tenderness of the TMJ reported a higher frequency of toothgrinding, more severe symptoms of mandibular dysfunction and a higher frequency of symptoms in the hand joints. No correlation was found between subjective symptoms of mandibular dysfunction and those of other joints. The results of the study indicate that patients with TMJ osteoarthrosis do not differ from other patients with mandibular dysfunction with respect to subjective symptoms and that subjective symptoms of mandibular dysfunction in general develop independently of those in other joints.

Thickness of the soft tissue layers and the articular disk in the temporomandibular joint.

Out of 115 right temporomandibular joints from Swedish subjects aged 1 day to 93 years, 48 joints without any gross sign of arthrosis or deviation in form were examined histologically. The joint components were cut sagittaly, each into four parts. Histological sections were made of the condyle, the temporal component and of the articular disk. The total thickness of the soft tissue layers was measured in decalcified sections, cut from the medio-central and lateral parts of the condyle and the temporal component and from the medial, medio-central, latero-central and lateral regions of the disk. In the medio-central sections from the condyle and temporal component the thickness of the fibrous connective tissue layer i.e. the surface layer was also registered. The soft tissue layers were thickest in the condyle superiorly, about 0.4-0.5 mm, in the temporal component on the postero-inferior slope of the articular tubercle, about 0.5 mm, and in the disk posteriorly about 2.9 mm. In the roof of the fossa it was only 0.1 mm. The soft tissue layers on the condyle as well as the disk were thinner laterally while the corresponding tissue in the temporal component was thicker laterally. The thickness of the soft tissue layers seem to reflect the growth and functional load to which the joint is exposed.

[Prostate carcinoma: determination of progression following high-voltage therapy (author's transl)].

The fourth report of our long-term study concerns 70 cases of primary radiotherapy. The side effects of our radiation technique are minimal and temporary; in over 80% of the cases, potency was not affected. The morphologically demonstrable changes in the tumor tissue, in the original glandular parenchyma, and in the fibromuscular stroma leading to the vessels following high-voltage therapy are described. Three types of progressive patterns, based on the grade of histologic regression, can be determined. In more than half of the cases, with increasing distance from the source of radiation, no tumor or only minimal residual tumor was demonstrable. Sensibility and resistance of carcinoma of the prostate are not dependent upon the grade of differentiation of the tumor. A prognostic statement concerning the success of radiation therapy from initial histologic findings is not possible here. The occurrence of metastases in 18% of the cases following radiation probably resulted from the inexactness of the purely clinical determination of the stage of the tumor. The isolated, local treatment of prostate carcinoma through radiotherapy is justified only after exact determination of the stage of the tumor, i.e., certain exclusion of metastases via a diagnostic lymphadenectomy.

Reproducibility of response to a questionnaire on symptoms of masticatory dysfunction.

The aim of the present investigation was to check the reproducibility of answers to multiple-choice questions used in a questionnaire. The questionnaire was concerned with symptoms in patients with dysfunction of the masticatory system. Five main types of questions were tested by repetition within 1 week by 98 patients. Comparisons were also made between questionnaire data and clinical findings. The interval between the questionnaire and the clinical examination was 1 week. The best reproducibility was found for answers to a question about previous joint radiography, which was the simplest question with the fewest alternatives. The second best was found for a question about the duration of the symptoms referable to the masticatory system. The poorest reproducibility was noted for a question about masticatory muscle pain. The comparison between the questionnaire data and clinical findings showed the best agreement for joint sounds. The reported pain symptoms and palpatory tenderness seem to be different entities.

Topographical distribution of sulphated glycosaminoglycans in human temporomandibular joint disks. A histochemical study of an autopsy material.

The distribution of sulphated glycosaminoglycans (GAG's) in the human temporomandibular joint disk and its relationship to sex, age and osteoarthrosis was studied by histochemical methods in autopsy material from 18 individuals. The disks were embedded in paraffin and frontal sections, 5-7 mu thick, were cut at different levels. Two staining methods were used, toluidine blue at pH 0.5 and alcian blue with different concentrations of MgCl2. The two methods gave comparable results. The sulphated GAG's as represented by metachromatic staining with toluidine blue at pH 0.5 and staining with alcian blue in concentrations of MgCl2 above 0.55-M were found in the central load-bearing part of the disks evenly distributed in the medio-lateral direction. The findings from the alcian blue staining method indicated the presence of sulphated GAG's with characteristics similar to chondroitin/dermatan sulphate and keratan sulphate. The latter finding was most frequent in a surface zone )10-100 mu) located mainly in the central part of the disks. In macroscopically thin areas of the disks judged as early osteoarthrosis a significant reduction in the staining of sulphated GAG's could be observed. No age or sex differences were found in the distribution of GAG's, either in normal or in osteoarthrotic disks.

Degenerative disease in the temporomandibular, metatarsophalangeal and sternoclavicular joints. An autopsy study.

The right temporomandibular, sternoclavicular and first metatarsophalangeal joints were removed post mortem from 39 subjects. The surfaces of the various parts of the joints were examined and evaluated according to a grading system. Repeated evaluation of the joints showed a good reproducibility of the grading system. Of the 33 temporomandibular joints, 12 showed deviations in shape. Degenerative disease was uncommon and seen in only 3 joints. The total "score for degeneration" showed only a weak correlation with age (r = 0.28, rs = 0.08). Degenerative disease was common in the 35 sternoclavicular joints. All joints except 7 showed degenerative changes. A moderate correlation was found between age and the score for degeneration (r = 0.58). Of the 39 metatarsophalangeal joints only 5 were allotted no points of degeneration. A moderate correlation was found between age and score for degeneration (r = 0.61). No significant differences of degeneration were found between sexes. The coefficient of correlation between the score for degenerative disease of the temporomandibular joint and the degeneration score for the sternoclavicular joint and the toe-joint were r = 0.22 (rs = 0.16) and r = 0.46 (rs = 0.44), respectively. The significant association of degenerative disease found between the toe-joint and the sternoclavicular joint (r = 0.54, rs = 0.54) was reduced (to r = 0.29) when age-dependence was excluded. The degenerative changes described in this material are probably mostly due to local factors.

Interleukin-1beta influences the effect of infliximab on temporomandibular joint pain in rheumatoid arthritis.

OBJECTIVES: The aim of this study was to investigate the influence of plasma and synovial fluid tumour necrosis factor alpha (TNFalpha), interleukin-1beta (IL-1beta), IL-6, soluble TNF receptor II (TNF-sRII), IL-1 receptor antagonist (IL-1ra), soluble IL-1 receptor II (IL-1sRII) and IL-10 on the effect of the TNFalpha antibody infliximab on temporomandibular joint (TMJ) pain in patients with active rheumatoid arthritis (RA). METHODS: Fifteen patients with TMJ pain taking methotrexate were included in the study. The effect of intravenous infusions of infliximab was assessed after 14 or 22 weeks. TMJ resting and movement pain was assessed by a visual analogue scale (VAS) (0-100 mm) and samples of venous blood and TMJ synovial fluid were collected before and after treatment. RESULTS: The effect of infliximab on TMJ pain was influenced by pretreatment plasma levels of IL-1beta, IL-1ra, and IL-10 as well as pretreatment levels of TMJ synovial fluid IL-1sRII. High pretreatment levels of these cytokines and receptors as well as the presence of rheumatoid factor (RF) were associated with no or minor reduction in TMJ pain after treatment. CONCLUSIONS: Systemic treatment of RA with a combination of infliximab and methotrexate seems to be insufficient to alleviate TMJ pain in patients with RF or high pretreatment plasma levels of IL-1beta.

[Effect of inoculation with Azospirillum spp on forage grasses in highland pastures of the Sierras de Córdoba, Argentian].

A field assay was performed in an impoverished hill grassland of the Sierras of Córdoba, with the objective of evaluating the effects of Azospirillum spp on gramineous plants. Native grasses were inoculated with different strains of Azospirillum lipoferum and Azospirillum brasilense, during two consecutive years. The inoculation effect on yield, nitrogen content and number of Azospirillum spp was evaluated in three annual harvests. The first and the second inoculation increased significantly the diazotroph number of the grasses with respect to the controls. This positive effect on the rhizophere population did not correlate with a significant increase in the forage quantity and quality as was expected.

Regional versus systemic chemotherapy for advanced pancreatic cancer: a randomized study.

BACKGROUND/AIMS: In an attempt to improve treatment protocols for advanced pancreatic cancer, the value of regional chemotherapy compared with systemic chemotherapy was investigated in this randomized study. METHODOLOGY: Fourteen patients with advanced non-resectable pancreatic adenocarcinoma were randomized receiving either systemic chemotherapy with mitomycin, mitoxanthrone and cisplatin (5pts.) or celiac axis infusion regional chemotherapy with SpherexR microembolization. In the systemic group one patient was stage III, four patients were stage IV, in the intraarterial group two patients were specified stage III and seven were stage IV. RESULTS: In the systemic group one stable disease and four progressive diseases were noted, in the regional group two stable diseases and seven partial responses were noted. Median survival was 11 weeks in the systemically treated patients versus 33 weeks in the patients treated with intraarterial infusion (p=0.001). One patient became resectable (R0). CONCLUSIONS: Performance status improved during regional chemotherapy whilst it steadily decreased in the patients treated systemically. The study was terminated at that point.
