Salt and geographical mortality of gastric cancer and stroke in Japan.

Age- and sex-adjusted mortality rates of gastric cancer and stroke in the years 1974-6 for 46 prefectures and 12 regions in Japan were related to regional differences of average per capita daily intake of salt during the period 1966-70 to test Joossen's hypothesis that salt is a common cause of both gastric cancer and stroke. While mortality rates of stroke were strongly correlated with salt intake in 12 regions (r = 0.85, p less than 0.001), mortality rates of gastric cancer showed hardly any correlation with salt intake (r = 0.02). There was no strong correlation between the two diseases in perfectural or regional mortality rates (r = 0.20). It was concluded that geographical mortality of gastric cancer and stroke in Japan did not support Joossens's hypothesis, and other evidence against the salt hypothesis has also been referred to.

Metabolic deterioration in shock state and its modulation.

The initial cellular reaction against the deleterious effects of shock-inducing stimuli apparently elevates the energy-producing capacity so that the cell is able to sustain its normal function. Several endocrine events are fundamentally important as triggering factors for this kind of reaction. As the shock state advances, cellular metabolism deteriorates progressively and cellular energy is exhausted. Depression of intracellular cAMP may induce cellular metabolic unresponsiveness to hormonal stimuli. Consistent degradation of high-energy substances, extreme deviation of the redox state in the NAD+-NADH system, and decrease of endogenous key substances such as L-carnitine ultimately may lead to a standstill of cellular enzymatic reactions (Fig. 13). Methods intended to sustain cellular membrane and enzymatic systems may offer the best contribution to the improvement of shock therapy.

[Clinical study on cefmenoxime (CMX) in urology].

Basic and clinical studies were made on Cefmenoxime (CMX), a new cephalosporin antibiotic, and the following results were obtained. The serum concentration of CMX was examined in four healthy adults after administration of 250 mg of CMX by intramuscular injection, intravenous injection and one-hour intravenous drip infusion (cross over). In the case of intramuscular injection, the peak value of 5.9 micrograms/ml was obtained 30 minutes after administration, and the half-life in serum was 1.41 hours. In the case of intravenous drip infusion, injection, a concentration value of 11.1 micrograms/ml on the average was obtained after 15 minutes of administration, and the half-life in serum was 1.26 hours. In the case of intravenous drip infusion, the concentration was 12.4 micrograms/ml upon completion of drip infusion, and CMX disappeared from serum at a half-life of 0.94 hour. The urinary recovery up to 6 hours was from 60 to 70% in each The efficacy rate of this preparation was 100% for 4 cases of acute simple cystitis. The efficacy rate of CMX was 70% for 10 cases of complicated urinary tract infection; the 3 cases in which CMX was not effective were patients with a residual catheter and Pseudomonas persisting or appearing as superinfection. It was noted that Serratia, which was resistant to the conventional cephalosporin antibiotics, became negative. No subjective side effects due to the administration of this preparation were observed. As for abnormal laboratory findings, a slight and transient rise in transaminases was observed in one case. On the basis of the above-mentioned results, it was concluded that CMX is an effective preparation for the treatment of urinary tract infections.

Cancer and other causes of death among female beauticians.

A retrospective cohort study was done of the mortality between 1953 and 1977 among 7,736 Japanese female beauticians who were registered from 1948 to 1960 in Fukuoka Prefecture, Japan. Mortalities from tuberculosis, heart disease, accidents, and all causes were significantly decreased as compared with the mortalities of general population in the prefecture. Observed deaths from all cancers combined were almost equal to those expected (148 observed vs. 139.26 expected). Among site-specific cancers studied, only slightly increased mortality from stomach cancer was statistically significant (61 observed vs. 45.59 expected). No measurable excess mortality was observed for the other sites of cancer.

A cohort study on the possible association between broiled fish intake and cancer.

To examine the possible carcinogenic effects of frequent intake of mutagenic pyrolysates of proteins and amino acids, 7,553 adult subjects whose personal characteristics had been examined were followed-up for about 11 years. The stepwise multiple regression analysis revealed that frequent consumption of broiled fish was significantly positively associated with mortality from cancer at all sites, as well as from cancer of the stomach. Among the 11 independent variables tested, consumption of broiled fish ranked high as a variable associated with cancer mortality (all sites, and stomach). Similar analyses done by sex and city (Hiroshima and Nagasaki) showed a consistently positive association of this variable with mortality from cancer at all sites, in both sexes and cities, but, with a few exceptions, the association was not statistically significant. Broiled fish consumption also showed a consistently positive but not statistically significant association with mortality from gastric cancer classified by sex and city. The relative risks of cancer mortality associated with frequent (twice or more weekly) consumption of broiled fish, as compared to less frequent consumption, were 1.3 (P less than 0.05) for cancer at all sites and 1.7 (P less than 0.05) for gastric cancer. The frequency of consumption of dried fish was hardly associated with deaths from cancer at all sites and from gastric cancer, but was significantly positively associated with deaths from cancer of the liver.

[A case of spontaneous pyeloduodenal fistula].

A 71-year-old man was admitted for right loin pain and fever. DIP showed a non-visualizing right kidney. Retro- and antigrade pyelography demonstrated right hydronephrosis secondary to uretero-pelvic junction obstruction with leakage of the dye from the right kidney into the duodenum. Diagnosis of spontaneous pyeloduodenal fistula was confirmed at the operation. Right pyeloplasty was performed after closure of the duodenum. We believe that this is the 9th case reported in the Japanese literature.

Radioimmunoassay and metabolism of the catechol estrogen 2-hydroxyestradiol.

Plasma levels of 2-hydroxyestradiol (2-OHE2) were measured using a new RIA procedure. Values were below the detection limit of the assay (less than 10 pg/ml), except in the third trimester of pregnancy, when they rose to approximately 15 pg/ml. The infusion of 130 microgram/h purified 2-OHE2 elevated its plasma concentration to 155 pg/ml, consistent with a plasma MCR (MCRp) of approximately 20,000 liters/day. The infusion of [3H] 2-OHE2 to equilibrium and chromatographic separation of the extracted plasma metabolites yielded an MCRp of about 13,000 liters/day; the major plasma metabolite comigrated with 2-methoxyestradiol, and [3H] xi-methoxyestrone was also formed. The MCRp, of 2-OHE2 is approximately half that of 2-hydroxyestrone (2-OHE1), but much higher than those of other steroids. As is true for 2-OHE1, the clearance of 2-OHE2 must occur primarily in the blood compartment. Together, the measured MCRp values and estrogen receptor affinities of 2-OHE2 and 2-OHE1 predict a relative potency for effects upon gonadotropin secretion which is close to that observed in vivo.

[Specific radioimmunoassay and physiological significance of unconjugated 2-hydroxyestrone and 2-hydroxyestradiol-17 beta in human blood (author's transl)].

Recently it has been attached importance to the physiological function of catecholestrogens. To elucidate it a specific radioimmunoassay of conjugated 2-hydroxyestrone (2-OHE1) and 2-hydroxyestradiol-17 beta (2-OHE2) in human plasma was attempted. After extracting with ethyl acetate, samples were purified by a short Sephadex LH-20 columnchromatography and determined using the antiserum to 2-OHE1-17(O-carboxymethyl) oxime-bovine serum albumin conjugate. The following results were obtained: 1) A long Sephadex LH-20 columnchromatography which was used for the purification of catecholestrogens produced chemically, had a superior faculty in separation and high capacity 2) The antiserum cross-reacted 26.4% with 1-OHE2, but less than 1% with other steroid hormones. 3) The sensitivity of the method was around 10pg in both 2-OHE1 and 2-OHE2 assays. The method blank determined using 2ml plasma of bilaterally adreno-oophrectomized women was below 15 pg/ml in both 2-OHE1 and 2-OHE2 assays (n = 24). 4) The coefficient of variation in both accuracy and between-assay precision of the method was less than 17%. 5) The plasma 2-OHE1 concentration was below 15 pg/ml in normal men (n = 8) and non-pregnant women (n = 13). The concentration in pregnant women was 20 +/- 8 pg/ml (SD, n = 7) in 1st trimester of pregnancy, 58 +/- 13 (n = 3) in 2nd trimester and 177 +/- 66 (n = 12) in 3rd trimester. The E1/2-OHE1 and E2/2-OHE1 ratios in 3rd trimester of 9 pregnant women were 19.7 +/- 11.9(SD) and 78.0 +/- 27.8, respectively. 6) The plasma 2-OHE2 concentration was below 15 pg/ml in normal men (n = 8), non-pregnant women (n = 8), and 1st to 2nd trimester (n = 10) and 15 +/- 9 pg/ml (SD, n = 13) in 3rd trimester of pregnant women.
