[Perioperative adjuvant chemotherapy in the treatment of lymph node metastases of colorectal cancer].

This study was undertaken to evaluate the effectiveness of perioperative adjuvant chemotherapy for the treatment and prevention of lymph node metastasis of colorectal cancer. Namely, ten thousand unit (U) of NCS, the high molecular weight anti-tumor agent, was injected into the subserosa of the colon (n = 51), the tumor-feeding arteries (n = 6), or the peripheral veins (n = 6) immediately after laparotomy, and tissue concentration of NCS in the regional lymph nodes and intestinal wall was measured. After the injection of NCS into the subserosa, the regional lymph nodes showed higher concentration of 4.55-6.34U/g regardless of metastasis. Concentrations in the lymph nodes after NCS into the tumor-feeding arteries showed similar high level as those after the subserosal injection in Group 1 lymph nodes (paracolic and epicolic nodes), but not in Group 2 lymph nodes (intermediate nodes). The injection of NCS into the peripheral veins demonstrated a wide range of 0.01-5.0U/g, half of them being not effective concentrations. Perioperative injection of NCS into the subserosa in the colorectal cancer was technically simple and safe and a large amount of the agent was taken into the regional lymph nodes. Therefore, this strategy may be a rational treatment for slight or jumping metastases in the lymph nodes.

Aplastic anemia: lack of increase of in vitro colony formation after T cell depletion with monoclonal antibodies and complement.

To detect suppressor T cells to hematopoietic stem cells, growth of granulocyte-macrophage colony-forming cells (CFU-GM) and burst-forming unit (BFU-E) was compared before and after treatment of bone marrow cells with anti-T monoclonal antibodies and complement in 29 patients with aplastic anemia. The anti-T monoclonal antibodies used were 35.1 (CD2), Tp120 (CD6) and ATL27 (not clustered). Treatment of normal bone marrow with anti-T monoclonal antibodies and complement resulted in complete (greater than 99%) lysis of T cells with negligible effects on colony growth. Preincubation of marrow samples with monoclonal antibodies and complement did not enhance CFU-GM or BFU-E colony growth in patients with aplastic anemia. Using this assay, there was no evidence of T cell-mediated inhibition of colony proliferation in any of 29 patients.

Hemorrhagic cystitis associated with urinary excretion of adenovirus type 11 following allogeneic bone marrow transplantation.

We studied a total of 50 recipients who had received allogeneic bone marrow transplantation (BMT) and evaluated both the presence of hemorrhagic cystitis (HC) and the urinary excretion of adenovirus. Twelve recipients developed HC and eight of these 12 patients excreted adenovirus type 11 at the onset of cystitis. Urine for virus isolation was attempted 30, 60 and 100 days after BMT. Among 137 specimens examined, eight were positive for adenovirus type 11. Of these eight samples, six were collected during HC; while in the 129 samples which were negative for adenovirus, only three specimens was collected during HC. Female patients, seropositivity for the antibody to adenovirus prior to BMT and acute graft-versus-host disease (grade 2-4) showed a significant impact on the risk of adenovirus HC. It may be said that adenovirus type 11 is one of the causative agents of HC in BMT recipients.

[A case of persistent müllerian duct syndrome associated with seminoma].

A case of persistent Müllerian duct syndrome associated with seminoma is presented. A 28-year-old man was hospitalized with the complaint of a palpable mass in the left lower abdomen and lumbago. Physical examination revealed a normal right scrotal testicle and a small right inguinal hernia that did not contain a mass, but no testicle was found in the left scrotum. Laparotomy was performed for suspected tumor of an intraabdominal testis. At abdominal exploration the left testis was found to be cross-ectopic to just behind the pubic symphysis and to be replaced completely with a large tumor, which was supplied by the left spermatic vessels and had a vas deferens emptied to the right seminal vesicle. In addition another cord-like structure was traced into the right inguinal canal. The tumor was dissected with these structures. On histological examination the tumor was pure seminoma and the cord-like structure proved to be an immature uterus, Fallopian tubes and seminal vesicle.

[Histological malignancy grading of oral squamous cell carcinomas].

UNLABELLED: A modification of Jakobsson's criteria that determines the mode of cancer invasion (Yamamoto, 1982) has been reported to be useful in predicting the prognosis in patients with an oral squamous cell carcinoma. In this retrospective study, a five-factorial grading system of the histological malignancy (+ the differentiations, the nuclear polymorphism, the mitoses, and the cellular response) was evaluated in relation to clinical course of each patient. RESULT: 75 cases were classified into four grades from Grade 1, the lowest malignancy, to Grade 4, the highest malignancy. The percentage metastases was 10, 24, 54, and 75% for each grade, respectively. The percentage of survival was 75, 75, 35, and 25% for each grade, respectively. From these results, this grading system was seen to have a close-correlation with the clinical course of each patient.

[Muscle fiber type abnormalities in bulbospinal muscular atrophy. Comparison with amyotrophic lateral sclerosis].

Bulbospinal muscular atrophy (BSA) is a distinct clinical type of chronic spinal muscular atrophy of adult onset, which was first reported by Kennedy et al. Histochemical findings of involved muscles are poorly described. In the present study, the muscle fiber type was studied on biopsied specimens by the histochemical method. The results were compared with those of amyotrophic lateral sclerosis (ALS). Muscle specimens were obtained mainly from biceps brachii muscles of 7 cases of BSA and 10 cases of ALS. Both group of patients were matched in respect to age, sex and stage of muscle weakness. The following results were obtained. Histopathological findings of BSA are classed as stage 4 abnormalities of Jennekins. Both fiber type grouping and type 2B deficiency are more frequent in BSA than in ALS. Type 2C fiber seems to be more in ALS than in BSA. Atrophy factor of Dubowitz is the same in BSA and in ALS. Hypertrophy factor is more prominent in BSA than in ALS. Significant correlation between fiber type grouping and type 2B deficiency was confirmed in muscles from other neurogenic atrophy. Since there is a significant association of the type 2B deficiency with fiber type grouping, type 2B fiber might be transformed to type 2A during the reinnervation process. The adaptation to the increased activities of the survived muscles and motoneurons might be contributory to the transformation, which is already known to occur in normal subjects during the endurance training.

[The process of developing and improving the analytical method of benzene in organic solvents for household use: interlaboratory collaborative study].

A method for the determination of benzene in spot remover using gas chromatography (GC) and high-performance liquid chromatography (HPLC) was developed. The method was evaluated by collaborative study. There are various types of household products containing organic solvents. The collaborative study clarified that a clean-up procedure is necessary for the analysis of products other than spot remover. Thus, our improved method adopted the head space method in GC and microdiffusion method in HPLC as the clean-up procedure.

[Alpha-bromocinnamaldehyde, its mutagenicity and contents in commercial products].

The amount of alpha-bromocinnamaldehyde (BCA), an anti-mildew agent, in some commercial products, was examined by high performance liquid chromatography (HPLC) using the following conditions: column, Nucleosil 50-5 (Nagel, 250 mm x 4.6 mm i.d.); mobile phase, hexane-chloroform (12:5); flow rate, 1.0 ml/min; detector, ultraviolet detector; detection wavelength, 290 nm. Large amounts of BCA were detected in anti-mildew mat (10,700-43,500 micrograms/g), anti-mildew cloth (1,100 micrograms/g) and BCA gel (10,050 micrograms/g). BCA was also tested for mutagenicity against Salmonella typhimurium TA100, TA98 and TA2637, with and without metabolic activation (S9 mix). The result showed that BCA was a very potent mutagen and induced 1.0 x 10(6) of revertants per mg against TA100 without S9 mix. The mutagenicity decreased to 1.3 x 10(4) revertants per mg in the presence of S9 mix.

[Transparency test for plastic containers].

A new sensory test method for judging the transparency of plastics containers for aqueous infusion was developed. The test procedure is as follows: Prepare four different grades of reference suspensions, grade 1 to 4, according to the procedure in ISO/DIS 3826. 4. Take ten empty containers. Fill two containers with the grade 1 suspension, two with the grade 2, and so on, filling 8 containers in all. Fill the remain 2 containers with water. Show each one of the ten containers to a volunteer in random order and ask him each time how does he judge it, turbid or not turbid. Repeat these tests among five different volunteers. If all volunteers judged the containers filled with the most turbid reference suspension, grade 4, to be turbid and the similar containers filled with water not turbid, such containers were judged to be transparent. On the other hand, the containers were not considered to be transparent if a significant number of volunteers judged even the containers filled with grade 4 suspension to not be turbid and vice versa. The test accurately determines the difference in the transparency of containers.

[Prolonged thrombocytopenia after autologous bone marrow transplantation].

Thirty two patients with hematologic malignancies and solid tumors were treated with intensive therapy and autologous bone marrow transplantation. In nine out of 32 patients, it took more than 50 days to achieve a sustained platelet count of 50,000/microliter or greater. Significant associations with poor platelet recovery were found for patient age, diseases, period of cryopreservation, the kinds of eradicative therapy and in vitro purging. But most of these factors overlapped each other in the same patients. No correlation was found between platelet recovery and number of cells or CFU-GM infused.

[Release of peripheral blood stem cells following chemotherapy in childhood malignancies].

Peripheral blood stem cells released following chemotherapy were examined in 21 children with malignancies in early remission. In order to obtain more than 1 x 10(5) CFU-GM per liter which is the minimal concentration to achieve autologous blood stem cell transplantation by cytapheresis, myelosuppressive chemotherapy which reduced leukocyte count below 1,000/microliters or neutrophil count below 200/microliters was necessary. Because the repetition of chemotherapy reduced the release of CFU-GM in peripheral blood, blood stem cells should be collected early after the beginning of chemotherapy. By long term culture method, peripheral blood stem cells seemed to contain pluripotent stem cells. Using our therapeutic protocol, more than 1 x 10(5) CFU-GM per liter could be obtained in malignant lymphoma and acute non-lymphoblastic leukemia, thus autologous blood stem cell transplantation seemed to be possible in these diseases.

[Severe aplastic anemia remarkably improved by a treatment with antilymphocyte globulin, high-dose methylprednisolone and danazol].

Sixteen-years-old female with severe aplastic anemia received a therapy combined with antilymphocyte globulin (ALG), high-dose methylprednisolone (m-PSL) and danazol. At the hospitalization, hematological examination demonstrated as follows; reticulocyte 21,000/microliters, granulocyte 350/microliters, platelet 10,000/microliters and hypocellular bone marrow. Treatment schedule were 1) m-PSL 1,000 mg (day 1-4), 500 mg (5-8)--then tapered. 2) ALG lg/day (day 4-8) 3) danazol 600 mg/day. During ALG administration, leukocytopenia and thrombocytopenia appeared but thereafter hematological recovery was obtained and the patient was free from supportive care. She developed mild diabetes mellitus and moderate liver dysfunction, nevertheless, both of which were controlled. At 3 months after the beginning of the treatment, hematological examination demonstrated as follows; reticulocyte 236,000/microliters, granulocyte 1,900/microliters, platelet 56,000/microliters and normocellular bone marrow. Although this immunosuppressive therapy was remarkably effective to this patient, immunological relation to the onset of aplastic anemia was not demonstrated in in vitro examination. This combined therapy seems to be effective one for patients with severe aplastic anemia.

[Estimation of the intrarenal hemodynamics in patients with primary aldosteronism].

Intrarenal hemodynamics were estimated in patients with primary aldosteronism (PA) using the Gomez's equations and by analyzing their renal function curve. The study was performed in 6 patients with PA for 2 weeks; they were given a regular sodium diet (12-15 g/day) in the 1st week and a sodium restricted diet (1-3 g/day) in the 2nd week. Blood pressure and urinary sodium excretion (UNaV) were measured on the last three days of each stage. Glomerular filtration rate (GFR) and renal plasma flow rate (RPF) were also measured on the regular sodium diet. Afferent (RA) and efferent (RE) arteriole resistances, and glomerular hydrostatic pressure (PG) were calculated using Gomez's equations. UNaV was plotted on the ordinate as a function of mean blood pressure (MAP) on the x-axis. Assuming that the difference between MAP and the x-intercept of this renal function curve represents the effective filtration pressure, RA, PG and gross filtration coefficient of capillaries (KFG) were also calculated. GFR and RPF were 102 +/- 6, 469 +/- 27 ml/min, respectively. Estimated RA (6600 +/- 700 dyns.sec.cm-5) was markedly elevated, while RE (2500 +/- 100 dyns.sec.cm-5), PG (57 +/- 2 mmHg) and KFG (0.195 +/- 0.041 [ml/sec]/mmHg) remained normal. The intrarenal hemodynamic parameters obtained by analyzing the renal function curve were consistent with those by Gomez's equations. Recently, in DOCA-salt hypertension, an animal model analogous to human PA, PG has been reported to be increased, leading to renal damage. However, the increase in RA was as marked as seen in essential hypertension, and further the increase in PG was not observed in the patients of this study. Thus, it is not clear whether there is a meaningful relationship between glomerular hypertension and renal damage in PA as reported in DOCA-salt hypertension.

[HTLV-I associated myelopathy (HAM) complicated with chronic polymyositis].

The patient was a 49-year-old woman. Since her age of 42, she noticed a proximal weakness of both legs. She also experienced pedal paresthesia and urinary frequency. Physical examination disclosed a diffuse goiter and bilateral Babinski sign. Results of EMG and muscle pathology were compatible with the diagnosis of polymyositis. Treatment with prednisolone improved muscle weakness, urinary difficulties and struma. 7 years later, HTLV-I antibody happened to be strongly positive both in serum and CSF. Then most of her neurological problems were attributed to HAM. However, recent studies of re-biopsy muscle specimens disclosed scattered necrotic fibers, phagocytosis and endomysial or perivascular infiltration of inflammatory cells. These infiltrating cells were classed mostly as helper/inducer T cells. These observations suggested the coexistence of polymyositis in the present case as well. So far, the combination of HAM and polymyositis has not been reported. It seems important to decide if HTLV-I could induce chronic polymyositis as in the case of HIV infections.

[Magnetic resonance imaging in multiple sclerosis: investigation of brain and spinal cord lesions].

Magnetic resonance imaging (MRI) of the brain was performed on forty-five patients with multiple sclerosis (MS), using T1-weighted inversion recovery and T2-weighted spin echo images, and the results were compared with X-ray computed tomography (CT). Some of the 45 MS patients were also examined by neurophysiological studies (visual evoked potentials and auditory brainstem responses) to compare with the brain MRI findings. MRI showed demyelinating plaques of the brain in 20 (74%) of 27 patients with brain symptoms, 11 (61%) of 18 patients without symptoms and 31 (69%) of all 45 patients. In 27 patients with brain symptoms, MRI was able to detect brain lesions in 6 (86%) of 7 acute stage patients and 14 (70%) of 20 non-acute stage patients. Furthermore, MRI was able to detect brain lesions in 21 (70%) of 30 clinically definite MS patients and 10 (67%) of 15 clinically probable MS patients. X-ray CT was performed on all 45 patients and was able to detect brain lesions in 9 (33%) of 27 patients with brain symptoms and 1 (6%) of 18 patients without symptoms. Visual evoked potentials were evaluated in 31 patients, and showed abnormalities in 1 (11%) of 9 patients without symptoms of optic neuritis and 100% of 22 patients with symptoms. Auditory brainstem responses were evaluated in 19 patients, and showed abnormalities in 1 (11%) of 9 patients without brainstem symptoms and 3 (30%) of 10 patients with symptoms. MRI of the brain was markedly superior to X-ray CT, visual evoked potentials and auditory brainstem responses in detecting clinically unsuspected lesions.(ABSTRACT TRUNCATED AT 250 WORDS)

[Clinical implications of enhanced caffeine contracture in malignant hyperthermia (MH) and Duchenne muscular dystrophy (DMD)].

The caffeine contracture response was studied on the skinned muscle fiber in patients with MH episodes or with DMD. An abnormally enhanced caffeine contracture was observed in 12 out of 14 subjects with MH episodes and 11 out of 13 patients with DMD. The extent of abnormality was almost the same in both group of patients. Ca-induced Ca release (CICR) of the sarcoplasmic reticulum was also analyzed on the same specimens. CICR was increased in 3 out of 4 cases with MH episodes, but it was decreased or unchanged in 4 patients with DMD. It was suggested that the mechanism of abnormal caffeine contracture might not be the same between MH and DMD. Thus, adverse anesthetic reactions, which are occasionally reported in DMD, could be different from genuine MH in its pathophysiology. However, before concluding this issue, further studies seem necessary concerning the molecular mechanism of abnormal Ca release. In the meantime, studies of caffeine contracture on the skinned fiber are helpful as one of predictive tests of MH and related disorders.

Preparation of monoclonal antibody against Angiostrongylus cantonensis antigen.

Monoclonal antibodies (MAb) against A cantonensis were produced through fusion of immunised spleen cells from BALB/c mice with NS-1 myeloma cells at a ratio of 10:1. The successful fusion rate on the 3rd day of fusion was 90.1%. Ten MAb were characterised, six of which were IgG1 and the remaining four were IgG2a, IgG2b, IgM and IgA respectively. Among 6 IgG1 MAb, four were A. cantonensis-specific, of which three reacted to adult worm antigen only and one reacted to both adult worm and juvenile worm antigens. Two other IgG1 MAb showed cross-reaction with other helminthic antigens of Toxocara canis. Ascaris suum. Paragonimus westermani, Dirofilaria immitis, Anisakis Spp, Gnatostoma Spinigerum and Clonorchis sinensis.
