[Combination therapy of local administration of OK-432 and radiation on a murine fibrosarcoma (NFSa)--combination effect and histological examination].

The combined effect of radiation with local administration of OK-432 was studied using natural fibrosarcoma (NFSa) of C3H mice that are less immunogenic and resistant to radiation (TCD 50, 83, 15 Gy). When 2KE of OK-432 was administered together with 1 X 10(6) tumor cells, tumor survival was inhibited in 7/8 (87.5%) of mice. Single local administration of OK-432 (2KE, 4KE, 8KE) showed hardly any effect on tumor survival. Combined treatment of 2KE of OK-432 and 4-Gy radiation, however, not only controlled tumor growth but also eradicated 1/5 (20%) of them, compared with single application of 45-Gy radiation. OK-432 worked most effectively when administered just before or after the application of X-rays. The optimum dose of OK-432 was studied by administering 2KE, 4KE and 8KE of the agent together with the application of 55-Gy radiation. 8KE groups showed the best eradication rate but 4KE groups showed the best cure rate. Therefore, the optimum dose was considered to be between 4KE and 8KE. With regard to histological effects, both the single radiation treatment and its combined treatment with OK-432 were most effective on the 6th day. The degree of degeneration and necrosis of tumor cells together with lymphocyte exudation were much higher in the combined treatment than in single radiation application.

The relationship between lung colony and in situ assays.

The relationship between three different assays: tumor control, tumor growth delay and lung colony formation, was examined after fast neutron and gamma ray irradiations. Fibrosarcomas (NFSa) in syngeneic C3Hf mice were irradiated locally with 60Co gamma rays, fast neutrons or mixed beams (gamma rays and fast neutrons). A comparison between the lung colony assay and the TRT50 (50% tumor growth delay time) assay when cells were exposed to single doses of fast neutrons or gamma rays, resulted in identical growth delay times. The fraction of cells surviving a single dose of fast neutrons, was 10 times higher than the surviving fraction of cells after a single dose of gamma rays. Both doses resulted in the same tumor control probability (TCD50 assay). Neither repair of potentially lethal damage nor tumor bed effect was sufficient to explain the difference between cell survival and tumor control probability. The surviving fraction of cells following fractionated irradiations of gamma rays and fast neutrons were identical at 50% tumor control probabilities.

[Urinary excretion of cadmium, copper and zinc in workers exposed to cadmium].

Changes in urinary cadmium, copper and zinc excretion in workers who had considerable exposure to cadmium oxide fumes were investigated over a 5-year period following the cessation of exposure. The 22 male subjects aged 22 to 55 had been welders on automobile parts for periods ranging from 7 months to 23 years, using silver solder which contained cadmium. They were divided into three groups according to levels of urinary cadmium in the first medical examination in 1975: 5 in the high excretion group, 11 in the moderate group, and 6 in the low group. Renal tubular dysfunctions were indicated in most workers in the high excretion group but not in the other two groups. The high and moderate groups showed a rapid decrease in the levels of urinary-excreted cadmium, with biological half-times of 1.42 and 1.21 years, respectively. Excreted cadmium levelled off in the two groups about 1.7 and 1.5 years after the last exposure. Urinary copper excretion in the high cadmium excretion group was about twice that in the moderate and low groups, the difference being statistically significant in comparison with the controls. Urinary zinc excretion was high in all subjects immediately after cessation of exposure but decreased rapidly. After three years it was found that the reduction of urinary zinc was greatest in the high cadmium group. Urinary cadmium showed significant correlations with both copper and zinc. Urinary copper showed a negative correlation with zinc excretion in the high cadmium excretion group and a significant positive correlation in the other two groups.(ABSTRACT TRUNCATED AT 250 WORDS)

[Recent advances in the study of the mechanisms of silica-induced pulmonary fibrosis].

A review was made on the recent advances in the study on the pathogenesis of silica-induced pulmonary fibrosis. Alveolar macrophages which ingest silica particles liberate a fibrogenic factor, which stimulates the production of collagen of cultured fibroblasts. Silica deposited in the alveoli augments the demand of macrophages, the supply of which is maintained by monocytes recruited from the bone marrow. Attempts to demonstrate in vitro the presence of a fibrogenic factor in the supernatant of macrophages have been made in many laboratories, and an in vivo model utilizing diffusion chambers implanted in mice has been used by some investigators. A fibrogenic factor has been isolated and purified from the medium of silica-treated macrophages. Recent advances in immunological studies have demonstrated that silica stimulates macrophages to release monokines such as interleukin 1 (IL-1) and that IL-1 has chemical properties identical to the fibrogenic factor, which enhances the level of collagen production by modulating the proliferation of fibroblasts. Silica inhibits the suppressive effects of macrophages on fibroblasts. The increased protein synthesis in the fibroblasts is due partly to increase in mRNA. Collagen synthesis is stimulated not only by the fibrogenic factor released from silica-treated macrophages but also by the inhibition of macrophage ribonuclease activity. Information on the number of cells, collagen content and protease activity in the lung as well as in the bronchopulmonary lavage fluid has provided us a better understanding of the mechanisms involved in silica-induced pulmonary fibrosis.

[A case of adenoid cystic carcinoma in the paranasal sinuses with 8-year survival after pulmonary metastasis-- clinicopathological reference to 6 non-squamous cell carcinoma cases in the paranasal sinuses].

Most cases of paranasal sinuses cancer are histologically squamous cell carcinoma. However, adenocarcinoma is rarely found even among cases of submucosal mucous gland origin. As adenocarcinoma has tendencies to local recurrence and distant metastasis, the five-year survival rate of this type of tumor has been reported to be worse than that of squamous cell carcinoma. On the other hand, some investigators insist that the survival rate at 10 or 20 years should be discussed because of the slow-growing nature of this tumor. We report herein clinicopathological studies on six cases of adenocarcinoma of the paranasal sinuses, including one long survivor, a 63-year-old man who lived 11 years and 3 months after the first visit and 8 years after pulmonary metastasis.

Increased radiosensitivity of a recurrent murine fibrosarcoma following radiotherapy.

The radiosensitivity of a recurrent tumor following local radiotherapy was investigated in order to determine whether or not the cell(s) surviving radiotherapy are radioresistant. A tumor which recurred in a C3Hf/HeMsNrs male mouse 200 days after local irradiation of a transplanted syngeneic fibrosarcoma was examined. The recurrent tumor showed the same biological properties, such as karyotype, histological features, and ability to produce colony stimulation factor, as the original tumor. On the other hand, the tumor control dose and the D0 value of the recurrent tumor were much smaller than those of the original tumor. Moreover, the recurrent tumor grew more slowly than the original tumor. It was concluded that initial radiotherapy might have changed the intrinsic radiosensitivity of the original tumor cells.

[Three cases of colonic-type adenocarcinoma in the head and neck region with positive CEA].

Adenocarcinoma arising in the head and neck region is found very rarely. Recently, we have experienced 3 cases of papillary adenocarcinoma (2 of paranasal sinus and 1 of epipharyngeal origin) with a histological similarity to colon cancer. Histochemical study of CEA by the PAP method was positive in all 3 cases, and this resembled colon cancer with respect to the tumor marker. Adenocarcinoma of the colonic type in the head and neck region should be further investigated for tumor characterization including the significance of positive CEA, as this tumor might be regarded as usual adenocarcinoma so far.

[A case of Burkitt's lymphoma with total ophthalmoplegia].

Burkitt's lymphoma was first reported by Burkitt in 1958 as a sarcoma involving the jaw in African children with characteristic symptoms. Forty three Japanese cases have been reported since the first description by Oboshi et al. in 1969. We report a case of Burkitt's lymphoma with left total ophthalmoplegia. A 73-year-old Japanese female was admitted in Sadamoto Hospital on July 11, 1983 with a two-week history of headache, ptosis and double vision. The patient was exposed to the atomic bomb in Hiroshima and had ten-year history of hypertension. On admission, physical examination showed hypertension and neurological examination revealed only left total ophthalmoplegia (such as left ptosis, external ophthalmoplegia, mydriasis and deficit of light reflex). Plain X-ray film and enhanced CT scan showed no remarkable abnormalities. Laboratory examinations revealed high serum levels of GOT(51 K.U.) and LDH (1300 U.). Left carotid and right retrograde branchial angiograms showed no remarkable abnormal findings. While the patient was treated only conservatively, left abducent and trochleal nerve palsy appeared on August 5, 1983. On plain and enhanced CT scans at the time, abnormal density mass with bone destruction of the left sphenoidal sinus was demonstrated. Biopsy specimen from the left sphenoidal sinus showed lymphosarcomatous cells. Peripheral blood and bone marrow smears showed lymphoma cells which are compatible with L3-Burkitt's type according to FAB leukemia classification. The patient was diagnosed as leukemic transformation of Burkitt's lymphoma and treated with CHOP; Cyclophosphamide (C), Hydroxydaunorubicin(H), Vincristine (O), and Prednisolone (P).(ABSTRACT TRUNCATED AT 250 WORDS)
