Hypoxia induces endothelial cell synthesis of membrane-associated proteins.

Hypoxemia is associated with a prothrombotic tendency. In this study we report the purification and partial characterization of an activator of a central coagulation component, factor X, induced in endothelium by exposure to hypoxia (hypoxia-induced factor X activator or Xact). Expression of Xact occurred in a reversible manner when endothelial cell cultures were exposed to hypoxia or sodium azide but not in response to a variety of other alterations in the cellular milieu, such as heat shock or glucose deprivation. The activity of Xact, which was not detected in normoxic endothelial cells, was maximal under acidic conditions, pH 6.0-6.8, which often coexist with hypoxia in an ischemic milieu. By sequential isoelectric focusing and preparative SDS/PAGE of endothelial membrane-rich fractions, Xact was purified approximately 19,000-fold and found to be a single-chain, approximately 100-kDa polypeptide with pI approximately 5.0. Activation of factor X by purified Xact was not affected by blocking antibodies to other coagulation proteins or by phenylmethylsulfonyl fluoride or leupeptin but was prevented by mercury chloride or iodoacetamide. In addition to the induction of Xact, two-dimensional gel analysis of membrane fractions from metabolically labeled hypoxic endothelial cultures revealed two groups of approximately 10 additional spots: (i) a group for which expression was maximal after 24 hr and (ii) a group for which expression continued to increase up to 48 hr. The pattern of hypoxia-mediated modulation of protein expression was distinct from that seen with other cellular stimuli but could be duplicated, in part, by sodium azide. These results indicate that hypoxia elicits a specific biosynthetic response, including the expression of endothelial cell-surface molecules that can alter cellular function and may potentially serve as markers of hypoxemic vessel-wall injury.

Hepatitis C and hepatitis B in the etiology of hepatocellular carcinoma in the Japanese population.

We conducted case-control studies of hepatocellular carcinoma (HCC) and liver cirrhosis (LC) in relation to hepatitis C virus (HCV) and hepatitis B virus infection, involving 91 patients with HCC, 75 patients with LC who had no evidence of HCC, and 410 control subjects from the Japanese population. Serum antibody to HCV (anti-HCV) was detected by both enzyme-linked immunosorbent assay and recombinant immunoblot assay in 51, 51, and 3% of HCC, LC, and controls, respectively, whereas the corresponding prevalence of serum hepatitis B surface antigen (HBsAg) was 21, 11, and 2%, respectively. The relative risks (and 95% confidence intervals) for the presence of serum anti-HCV were estimated as 52.3 (23.9-114.3) for HCC and 64.4 (27.4-151.4) for LC. These values exceeded the relative risk of HCC (15.3) and that of LC (6.1) for positive serum HBsAg. Among male patients with HCC or LC, anti-HCV rates were very high in blood recipients (about 70%), heavy drinkers (46-62%), and those who had no identifiable risk factors (65-75%), indicating possible transmission of HCV via routes other than transfusion. No significant difference in anti-HCV status was observed between the HCC and LC groups. It was notable that anti-HCV was much less prevalent among HBsAg-positive patients with HCC or LC than among HBsAg-negative ones. There was a slight to moderate increase in HCC or LC risk among blood recipients and heavy drinkers after adjustment for anti-HCV status. These results indicate that, in Japan, the possible role of HCV infection in the etiology of HCC and LC is extremely large and seems to be more important than chronic hepatitis B virus infection.

Influence of local hyperthermia on the healing of small intestinal anastomoses in the rat.

Peritoneal hyperthermia may have a role in limiting serosal metastatic disease. When applied to the peritoneal cavity immediately after surgery, it is important to know the optimum temperature, and to investigate the subsequent healing of intestinal anastomoses. To study the first problem, local hyperthermia was applied to the intestinal loop of rats for 30 min. Treatment at 46.0 degrees C or 45.0 degrees C resulted in 100 per cent and 90 per cent mortality respectively, but 100 per cent survival was obtained at 44.0 degrees C. To study the second problem rats with intestinal anastomoses were studied in three groups: group A, local hyperthermia (44.0 degrees C x 30 min) applied to the intestinal loop containing the anastomosis; group B, local hyperthermia (44.0 degrees C x 30 min) applied using saline supplemented with mytomycin C (10 mg/l); group C (controls) no thermal treatment was applied. Anastomotic healing was assessed by breaking strength and histological examination. On the third day after operation, the breaking strength of anastomoses decreased to the lowest values in each group, but no statistically significant differences were noted. On the seventh and 14th day, increased resistance to breaking developed in all three groups and was greatest in the thermally treated groups. Histological findings supported these results. Local hyperthermia up to 44.0 degrees C x 30 min had no adverse effects on the healing of intestinal anastomoses.

Multiloculated renal cell carcinoma.

Multiloculated renal cell carcinoma and multilocular renal cyst with renal cell carcinoma are uncommon diseases. We report on 2 cases of multiloculated renal cell carcinoma. To our knowledge 16 cases of multiloculated renal cell carcinoma and multilocular renal cyst with renal cell carcinoma have been previously described in detail. The authors review the literature and discuss the aetiology and diagnosis of the disease.

Quantitative measurement of regional cerebral blood flow with I-123 IMP SPECT: a correction of the microsphere model by global extraction between artery and internal jugular vein.

Quantitative measurements of regional cerebral blood flow with N-isopropyl-(Iodine 123)p-iodoamphetamine (I-123 IMP) as a microsphere model were performed in forty cases. The regional cerebral blood flow values obtained with I-123 IMP were slightly underestimated compared with those of Xe-133 inhalation methods (y = 0.90x-2.1, r = 0.85, p less than 0.01). After correction by global extraction (87%) between the artery and internal jugular vein, which was measured in four patients by means of a catheter technique, the underestimation of the values obtained with I-123 IMP was improved (y = 1.0x-2.4, r = 0.85, p less than 0.01). Several problems in the accurate quantitative measurement of regional cerebral blood flow with I-123 IMP are discussed.

Highly sensitive chemiluminescence method for determining myeloperoxidase in human polymorphonuclear leukocytes.

Myeloperoxidase activity was assayed by a chemiluminescence method, using a cypridina luciferin analog as a chemiluminescence probe, after extraction from peripheral human polymorphonuclear leukocytes. The chemiluminescence method was based on the detection of 1O2 generated by myeloperoxidase-catalyzed HOBr formation followed by the interaction of HOBr with H2O2 at pH 4.5. With this method, myeloperoxidase in less than 100 polymorphonuclear leukocytes could be detected and myeloperoxidase in 10(6) polymorphonuclear leukocytes would be calculated to be 14.4 pmol. Eosinophil extract, which contains eosinophil peroxidase, catalyzed 1O2 generation to a great extent, compared with the polymorphonuclear leukocyte extract at pH 4.5. Myeloperoxidase activity in extract of neutrophil fraction could be greatly influenced by eosinophil contamination.

Mechanism for the generation of superoxide anion and singlet oxygen during heme compound-catalyzed linoleic acid hydroperoxide decomposition.

Heme compound, hematin or cytochrome c, catalyzes the decomposition of 13-hydroperoxy linoleic acid yielding both O2- and 1O2 under aerobic conditions. No 1O2 is produced when hydrogen peroxide and cumene hydroperoxide are used as substrates. In these experiments, both O2- and 1O2 could be precisely detected by a chemiluminescence method using a cypridina luciferin analog, 2-methyl-6-(p-methoxyphenyl)-3,7-dihydroimidazo[1,2-a]pyrazin++ +-3-one, as a chemiluminescence probe, in the absence and presence of Cu-Zn superoxide dismutase in catalytic amounts. The reduction and oxidation cycle of ferric heme compound and the bimolecular reaction of peroxyl radicals are plausible reaction mechanisms for O2- and 1O2 production, respectively, in the systems studied.

Effect of insulin, glucagon or dexamethasone on the production of apolipoprotein A-IV in cultured rat hepatocytes.

We studied the effects of insulin, glucagon or dexamethasone on the production of apolipoprotein A-IV (apo A-IV) by cultured rat hepatocytes, using specific radioimmunoassay for rat apo A-IV. We also compared the effect of these hormones on the production of apo A-IV with those of albumin and apo A-I, reported previously. In the absence of hormones, apo A-IV and albumin in culture medium increased almost linearly for periods up to 24 h. The rates of accumulation of apo A-IV and albumin in the medium were 15.4 ng/mg cell protein per h and 1.2 micrograms/mg cell protein per h, respectively. The concentration of intracellular apo A-IV remained constant during the incubation. Insulin stimulated the production of albumin, but inhibited the production of apo A-IV dose-dependently. Glucagon inhibited the production of both albumin, and apo A-IV dose-dependently. Dexamethasone showed no significant effects on albumin production, but stimulated apo A-IV production. Thus, apo A-IV production in hepatocytes is regulated by several hormones with different effects on albumin production. The regulatory effects of these hormones on apo A-IV production were almost identical with the effects observed in a course of apo A-I synthesis, suggesting that the production of the two apoproteins are regulated by similar mechanisms.

Gastric leiomyoblastoma in childhood.

A case of gastric leiomyoblastoma in a 7-year-old girl, who underwent partial gastrectomy combined with complete removal of perigastric lymph nodes, is reported. Characteristics of the behavior and prognosis of gastric leiomyoblastomas in childhood are discussed in a review of the world literature.

Staghorn calculi--long-term results of management.

We have treated 167 patients with staghorn calculi. Conservative therapy was used in 61 patients who have been followed up for 1 to 18 years (average 7.8). Chronic renal failure occurred in 22 of these patients and 7 died from uraemia. The causes of chronic renal failure were bilateral staghorn calculi, staghorn calculi and contralateral urinary calculi, and chronic pyelonephritis of the contralateral kidney. The morbidity and mortality rates following conservative treatment were higher than those following surgical management. The pathological findings in 47 kidneys after nephrectomy showed severe hydronephrosis, renal abscess and xanthogranulomatous pyelonephritis. These results indicated that staghorn calculi destroyed the kidney and early complete removal of these stones is advisable.

Decidual activation in parturition: examination of amniotic fluid for mediators of the inflammatory response.

The accumulation of bioactive agents (characteristic of an inflammatory-type response) in amniotic fluid is common during term and preterm labor, viz., interleukin-1 beta (IL-1 beta), interleukin-6 (IL-6), and tumor necrosis factor-alpha (TNF-alpha). In addition, prostaglandins, including PGE2, PGF2 alpha, and PGFM, also accumulate in amniotic fluid in some cases of term and preterm labor. From these observations, a number of critical questions arise. Namely, 1) what is the tissue source of origin of these agents?; 2) what are the stimuli that evoke this inflammatory response?; and, 3) are these bioactive agents of inflammation involved in the commencement of labor or else a natural accompaniment of the parturition process? It is reasonable to suspect that the decidua is activated during parturition as the membranes-decidua are exposed after cervical dilation to the vaginal/cervical secretions. Amnion and chorion laeve, in the human, are avascular tissues that produce PGE2 but not PGF2 alpha. Therefore, the accumulation of PGF2 alpha and PGFM in amniotic fluid during labor cannot be attributed to a fetal membrane origin. Moreover, the fetal membranes and decidua do not convert PGE2 to PGF2 alpha. In addition, the fetal membranes do not produce mature, i.e., secreted 17kD IL-1 beta. On the other hand, the decidua does produce PGF2 alpha and PGFM and is stimulated to do so by agents in the vaginal secretions, namely, bacterial endotoxin and IL-1 beta. After the fetal membranes and contiguous decidua are exposed during the time of cervical dilatation, these tissues are acted upon to cause 1) an influx of mononuclear phagocytes into the forebag compartment of the amniotic fluid; 2) to produce PGF2 alpha and PGFM; and 3) to produce cytokines, including IL-1 beta, IL-6, and TNF-alpha. Exposure of the fetal membranes-decidua to bioactive agents in vaginal/cervical secretions will effect an inflammatory response both in vivo and in vitro. We conclude that the accumulation of bioactive agents characteristic of the inflammatory response in amniotic fluid during term and preterm labor is usually an accompaniment of parturition and not its cause.

Development of bacillus Calmette-Guérin-induced antitumor activity in peripheral blood mononuclear cells.

Peripheral blood mononuclear cells (PBMNCs) after bladder instillation of bacillus Calmette-Guérin (BCG) to patients with superficial bladder carcinoma showed lytic activity against natural killer (NK)-sensitive and NK-resistant target tumor cells. PBMNCs in normal volunteers were isolated and cultured with BCG (5 micrograms/ml) in vitro. Maximum cytotoxicity was obtained within 24 h. Cytosine arabinoside as a DNA synthesis inhibitor did not inhibit this killer cell activation. Monoclonal antibody treatment revealed that both precursor and effector cells were NK cell type. However, effector cells had a wider spectrum of target cells than usual NK cells. The recognition specificity from cold target competition experiments was selective.

[Study on the limitation for detecting anaerobic threshold by respiratory frequency].

It has been reported that respiratory frequency (F) serves to determine anaerobic threshold (AT). The purpose of this study was to investigate whether the method detecting AT by using F is influenced by the subject's condition such as the existence of sport experiences. Ten healthy adults volunteered to perform progressive cycle ergometer exercise with workloads increased by 30-W (female:20-W) every 2 min at 60 rpm. VO2 at AT were determined by four different methods, which detect the point of 1)nonlinear increase in VE, VCO2, and increase in VE/VO2 without increasing in VE/VCO2 (AT-v), 2) nonlinear increase in F (visual estimation: AT-VF), 3) inflection in F by multisegment linear regression (AT-CF), 4) inflection with omitting above RC point as with 3) (AT-CF2). The mean VO2 at AT-VF (40.8 +/- 9.2 ml/kg/min) and AT-CF (42.7 +/- 9.9 ml/kg/min) was significantly higher compared with AT-V (28.2 +/- 10.4 ml/kg/min) and not RC (42.3 +/- 10.0 ml/kg/min). It would be possible that AT-VF and AT-CF indicated RC, but not AT. There were no significant differences between AT-CF2 (28.2 +/- 10.9 ml/kg/min) and AT-V, and a highly positive correlation (r = 0.79, p less than 0.05) was observed between them. It was recognized that F reached a plateau at AT in four of the subjects. The error between AT-V and AT-CF2 was observed individual variations and the error between them within 5% was observed in only one subject. These results suggest that F is inadequate as an indicator of the AT, because F may be influenced by entrainment of breathing and pedalling frequency.

[Awareness and electromyograph biofeedback in the acquisition of control of a novel muscular activity].

Two experiments were conducted to examine what kind of awareness to the internal control process should be encouraged in order to effectively acquire the control of a novel muscular activity with electromyograph (EMG) biofeedback. The m. auricularis posterior (the muscle to draw an ear backward) was selected as the target muscle of this study. Experiment I investigated the relation between control ability and awareness of the target muscle activity. Results showed that subjects who were able to move their ears could be aware of the target muscle activity more precisely than those who were unable to do it. In experiment II, 32 undergraduate and graduate students who could not move their ears were required to activate their left m. auricularis posterior. Results provided evidences supporting the hypothesis that, in the initial stage of the acquisition of control, subjects who were encouraged to be aware of ways and feelings of striving (efferent process) and were given EMG feedback signals from the target muscle could acquire the control of the target muscle activity more effectively than those who were encouraged to be aware of a bodily feeling brought about by the striving (afferent process) and who were given no EMG feedback signals.

Simultaneous superficial squamous cell carcinoma of the esophagus and early gastric adenocarcinoma.

Although cases of simultaneous invasive cancer of the esophagus and primary gastric cancer have been reported sporadically, the incidence of the association of superficial esophageal cancer and early gastric cancer is extremely low. In this paper we report on two cases of the rare combination of superficial squamous cell cancer of the esophagus and simultaneous early adenocarcinoma of the stomach. A total of 18 such cases in the literature are reviewed and discussed with respect to surgical procedure and the choice of alimentary tract for the reconstruction of the esophagus.

[A case report of idiopathic myxedema with secondary amenorrhoea and hyperprolactinemia: effect of thyroid hormone replacement on reduction of pituitary enlargement and restoration of fertility].

A 37-year old housewife was admitted to our department because of long-standing amenorrhoea and galactorrhoea. After several hormonal examinations, she was proved to be suffered from primary hypothyroidism with hyperprolactinemia. In addition, brain computed tomography (CT) showed the finding of enhanced pituitary enlargement, suggesting pituitary hypertrophy or pituitary adenoma. Based on some therapeutic experiences in similar cases in several reports, we have performed only thyroid hormone replacement and followed up the patient. Plasma thyroid stimulating hormone (TSH) and prolactin concentrations returned to normal range in a few months after starting thyroid hormone replacement. Furthermore, the finding of pituitary enlargement has completely disappeared on brain CT and come to pregnancy during the course. Thus, it seems that the finding of pituitary enlargement might be due to pituitary hypertrophy. Therefore, we think that thyroid hormone replacement should be a first choice therapy preceding the pituitary surgery or bromocriptine therapy in such a case.

[Cancer chemotherapy of high-age patients with gastrointestinal malignancies].

Recently, high aged patients with malignancies have increased in number. When cancer chemotherapy is applied for the high aged patients, kinds or doses of anti-cancer drugs must be more carefully selected or decided than for younger patients, because it is said that the side effects of anti-cancer drugs would easily induce irreversible organ disorders and death in high aged patients. The effects of cancer chemotherapy were compared between high aged patients (over 75 years) and younger patients (5 decade years) with special reference to side effects. The patients underwent cancer chemotherapy were 79.7% of high aged patients and 93.6% of younger. The reasons why cancer chemotherapy was not carried out were high age (5.6% of high aged patients), poor general conditions (7.0% in high aged, 2.3% in younger) and post operative complications (7.0% in high aged, 3.2% in younger). The proportion of patients suffered side effects was almost same in both groups. Dead cases caused by side effects of anti-cancer drugs were 5 in high aged patients (4.4%) and 7 in younger (3.4%). The reason why the proportion of side effects in both groups was not different was that the doses of anti-cancer drugs given for high aged patients were reduced to 80-90% of those for younger patients.

Decreased proapolipoprotein A-I processing in liver disease: evidence for hepatic participation in proapolipoprotein A-I conversion.

We evaluated the proapolipoprotein A-I (proapo A-I)-converting activity to clarify the pathogenesis of the high proapo A-I/apo A-I ratio in the high-density lipoprotein (HDL) from patients with acute hepatitis and liver cirrhosis. The serum proapo A-I-converting activities were measured using 3H-labeled proapo A-I. 3H-labeled proapo A-I was purified from the media of cultured Hep G2 cells by immunoaffinity chromatography. The serum proapo A-I-converting activities were found to be significantly reduced in the patient with liver cirrhosis (140 +/- 53 dpm/ml per h) or acute hepatitis (140 +/- 48 dpm/ml per h) compared to normal subjects (315 +/- 32 dpm/ml per h). Serum proapo A-I-converting activity has a positive correlation with liver function tests such as serum albumin, choline esterase activity, ICG clearance and inverse correlation with proapo A-I/apo A-I ratio in HDL. These results suggest that the high proapo A-I/apo A-I ratio is due to the decreased proapo A-I-converting activity, and that the liver plays a significant role in the conversion of proapo A-I to apo A-I.
