[Giant hydrocele: two case reports].

Two cases of giant hydrocele, having more than 1,000 ml of contents, are presented. The first patient was a 77-year-old man with the complaint of urination difficulty and a very swollen left scrotum of three years duration. Mild prostatic hypertrophy, mild renal dysfunction and hypertension were seen during hospitalization. A scrotal puncture gave 2,100 ml of serous fluid. Radical hydrocelectomy and plastic operation for surplus scrotal skin were done, and difficult urination improved markedly. The second patient was a 77-year-old man with the complaint of right scrotal swelling of fifteen years duration. He had developed pollakisuria and urination difficulty three years previously. When he was transferred our hospital on emergency due to engorgement, the diagnosis of giant hydrocele was made. Right orchidectomy was performed because of marked testicular atrophy and old hemorrhagic fluid of 1,200 ml. Urination difficulty and pollakisuria disappeared postoperatively.

Binding of low-density lipoproteins from chicken egg yolk to mouse anti-phospholipid B cells which include B cells against bromelain-treated mouse erythrocytes.

Low-density lipoproteins from chicken egg yolk (EyLDL), which are reactive with mouse antibodies against bromelain-treated mouse erythrocytes (BrMRBC), were conjugated with fluorescein isothiocyanate (FITC). FITC-EyLDL could bind specifically to mouse anti-phospholipid B cells, which comprised all the BrMRBC-rosette-forming cells and anti-BrMRBC lipopolysaccharide-reactive B cells, C3H/He mice at 12 weeks of age had, approximately, 7 x 10(5) EyLDL-binding cells in the peritoneal cavity, 3 x 10(5) EyLDL-binding cells in the pleural cavity, and 3 x 10(5) EyLDL-binding cells in the spleen. In ontogeny, the numbers of EyLDL-binding cells in the peritoneal cavity expanded greatly by 4 weeks. Other normal strains of mice and C3H/HeJ mice at 12 weeks of age had 4-7 x 10(5) EyLDL-binding cells in the peritoneal cavity; the numbers were large (19 x 10(5] in NZB mice, rather small (2 x 10(5] in MRL/lpr mice, and very small (0.1 x 10(5] in CBA/N mice. In some of various strains of mice at 12 months of age, more than 20% of peritoneal cells were EyLDL-binding cells; in particular, all of five older NZB mice examined had more than 10(7) EyLDL-binding cells in the peritoneal cavity.

Changes in the cerebello-cerebral response in the parietal cortex following ablation of the motor cortex in the cat: early occurrence and persistence.

To elucidate the compensatory mechanism which begins to work soon after damage to the brain, changes in the cerebellar-induced cerebral cortical response in the parietal association cortex after ablation of the frontal motor cortex were studied in the cat. Stimulation of the interpositus or the lateral nucleus of the cerebellum before decortication, as reported in intact animals, induced two distinct types of response in the frontal motor and parietal association cortices respectively. The response in the frontal cortex was a sequential occurrence of a surface positive-depth negative (sP-dN) wave and a surface negative-depth positive (sN-dP) wave, and the response in the parietal cortex was mainly an sN-dP wave. In a small proportion of animals, the latter wave was preceded by a small sP-dN wave or a small dN wave without an sP wave. Ablation of the frontal motor cortex induced in the majority of animals a marked change in the cerebello-parietal cortical response, i.e. the occurrence of a new sP-dN wave preceding the sN-dP wave or enhancement of the pre-existing small dN wave, resulting in the parietal cortical response similar to the frontal cortical response of intact animals. The earliest post-lesion time observed for the occurrence of change was less than 1 hour, whereas its persistence was confirmed up to 213 days post-lesion.

Changes in negative symptoms of schizophrenic patients two years later.

This study investigated the course of negative symptoms by examining the psychiatric symptoms of 59 new schizophrenic patients at the first consultation and again two years later using a structured interview--the Present State Examination. An examination of the total score of the nine negative symptoms, included in the "Chronic Schizophrenic Syndrome" by Wing, showed that 27 out of the 59 patients (45.8%) had lower symptom scores two years later than they did at the first consultation ("negative symptom improvement" group), while 20 patients (33.9%) had higher scores two years after the first consultation ("negative symptom deterioration" group). There were 12% more patients in the improvement group than in the deterioration group. No significant differences were observed between the "improvement group" and "deterioration group" in relation to the following factors--subclassification of schizophrenia, mode of onset, age at the first consultation, marital status at the time of onset, academic history, employment status prior to the onset, the history of hospitalization after the onset and maintenance therapy of neuroleptics. In relation to psychiatric symptoms at the first consultation, the patients in the "improvement group" had a variety of symptoms compared to the patients in the "deterioration group," and the presence of "incoherence of thought," "delusion of reference" and "special features of depression" was significantly higher in the improvement group than in the deterioration group.

[A case of Akiyami C disease, infected in the area of Yoshino].

A 47-year-old male was admitted because of persistent jaundice. Akiyami C disease was diagnosed by serological tests using the microscopic agglutination tests. The source was considered to be a water contaminated with leptospiras which he had drunk while working at a civil engineering job. The reported cases of Akiyami C disease have recently decreased and this is the 6th case reported in the last 15 years.

[A case of Mycoplasma pneumoniae infection followed by Staphylococcus aureus bacteremia].

A 19-year-old young man was admitted to our hospital complaining of fever and general fatigue. There were infiltrative shadows and pleural effusions in the both lung fields. Mycoplasma pneumoniae infection was diagnosed because of the elevation of mycoplasma antibody titers in the serum and pleural fluid. There was no recovery in clinical symptoms in spite of the administration of the EM (1200 mg) and CLDM (1200 mg) combination chemotherapy. Three week after admission, a cavity-like shadow appeared in the lt. middle lung field on the chest X-ray film, suprative arthritis and penicillin resistant S. aureus by blood culture test were found. Mycoplasma pneumoniae infection followed by S. aureus bacteremia was diagnosed. After the administration of antibiotics (CTT, FMOX) the clinical symptoms and laboratory findings improved 2 month after admission. Clinical and basic studies about the dual infection between M. pneumoniae and several bacteria were discussed.

[Effect of continuous arterial infusion of protease inhibitor on experimental acute pancreatitis induced by closed duodenal loop obstruction].

The effectiveness of continuous arterial infusion of protease inhibitor on acute experimental pancreatitis was investigated. Acute hemorrhagic pancreatitis was induced by closed duodenal loop obstruction using mongrel dogs. The obstruction was released at 16 hr, and dogs were divided into three groups; Group I: non-treated control, Group II: nafamostat mesilate (FUT-175) was admitted intravenously (5 micrograms/kg/min), Group III: FUT-175 was admitted via celiac artery. At 24 hr, the concentration of FUT-175 in the pancreatic tissues in group II and III were 905 and 4453 ng/g, respectively. The trypsin like activities in the pancreatic tissues in group I, II and III were 2.1, 1.4 and 0.7 nmol/min/mg protein, and the extent of necrosis of pancreatic parenchyma in each group were 49.5, 25.6 and 12.4%, respectively. Serum calcium, amylase and lipase levels were significantly improved in group III. These results suggest that continuous arterial infusion of protease inhibitor markedly decreases the extent of pancreatic necrosis in severe acute pancreatitis.

Lipopolysaccharide-reactive B cells against bromelain-treated mouse erythrocytes: preferential migration to and survival in the peritoneal cavity.

The ratio of lipopolysaccharide (LPS)-reactive B cells specific for bromelain-treated mouse erythrocytes (BrMRBC) to general LPS-reactive B cells is far higher in the peritoneal cavity of normal mice than in their spleen. To investigate the high concentration of anti-BrMRBC LPS-reactive B cells in the peritoneal cavity, spleen and peritoneal cells from (CBA/N X C3H/He)F1 female normal mice were injected intravenously and intraperitoneally into F1 male X-linked immunodeficient mice. Both groups of B cells in the intravenously transferred cell population were able equally to home in the spleen, but only anti-BrMRBC LPS-reactive B cells could be detected migrating into the peritoneal cavity. About half the anti-BrMRBC LPS-reactive B cells in the intraperitoneally transferred cell population could be recovered from the peritoneal cavity, but general LPS-reactive B cells were eliminated rather rapidly from the peritoneal cavity. Neither group of B cells could be detected migrating from the peritoneal cavity into the spleen. These findings suggest that the high concentration of anti-BrMRBC LPS-reactive B cells in the peritoneal cavity may be caused by their preferential ability to penetrate into the peritoneal cavity through circulation and survive there.

Immunohistochemical localization of atrial natriuretic peptide receptor in bovine kidney and lung.

Receptors for atrial natriuretic peptide (ANP) were localized in the alveoli and bronchiolar smooth muscle cells of bovine lung and in podocytes of the kidney by immunofluorescence and immunoperoxidase methods. Two specific antisera were raised against the ANP receptor purified from bovine lung plasma membranes: anti-Rc 140 and anti-Rc 70. Anti-Rc 140 was raised against the 140 KD native receptor having a homodimeric structure, and anti-Rc 70 was elicited by immunizing a rabbit with the 70 KD reduced subunits. Essentially identical staining patterns were obtained with both antisera. Identification of ANP receptor sites would provide useful information in understanding the pulmonary and renal actions of ANP.

Reactivity of mouse antibodies against bromelain-treated mouse erythrocytes with thrombin-treated mouse platelets.

The reactivity of mouse antibodies against bromelain-treated mouse erythrocytes (BrMRBC) with mouse platelets before and after thrombin treatment was assessed by flow cytometry. Anti-BrMRBC antibodies could bind to thrombin-treated platelets, although normal platelets were also weakly reactive with the antibodies. The binding of anti-BrMRBC antibodies to platelets was confirmed by complement-dependent lysis. It is suggested that thrombin-activated platelets may be a real target for anti-BrMRBC antibodies.

[A case of hypertrophic obstructive cardiomyopathy with left atrial giant thrombus during anti-platelet therapy].

A case of a 77-year-old female with hypertrophic obstructive cardiomyopathy having the left atrial "giant" thrombus was reported; the diagnosis of the localization and size of the thrombus was made by employing echocardiography and contrast enhancement computed tomography as well. The clinical record indicated that the thrombus formation, which happened in the course of the antiplatelet therapy, was to be considered due to the complicated condition of the patient, i.e., the combination of atrial fibrillation, low cardiac output, enlargement of left atrium and left ventricular diastolic disfunction, which assumed to predispose the giant thrombus. Avoiding the surgical removal of the thrombus because of the poor conditions of the patient, urokinase treatment was attempted, but not successful, which might be derived from the nature of the thrombus already organized. In general, left atrial giant thrombus is very uncommon status; if any, it is occasionally complicated with mitral stenosis. The case reported deems to be very rare, enough worthy to be reported.

Atrial fibrillation following methylprednisolone pulse therapy.

Two children, one with renal manifestations of systemic lupus erythematosus and the other with idiopathic nephrotic syndrome, were treated with methylprednisolone pulses. Neither had previous evidence of underlying cardiac disease. Within 24 h of pulse therapy, they complained of palpitations and developed atrial fibrillation which reversed spontaneously or after anti-arrhythmic therapy. Subsequent serial electro- and echocardiograms were normal. We propose that the arrhythmias were a complication of steroid pulse therapy.

Induction of antibody response to bromelain-treated mouse erythrocytes by low-density lipoproteins from chicken egg yolk.

The administration of low-density lipoproteins from chicken egg yolk (EyLDL) to mice induced T-dependent antibodies against bromelain-treated mouse erythrocytes (BrMRBC). The anti-BrMRBC antibodies produced antigen specifically by EyLDL appear to be identical to the antibodies produced polyclonally by lipopolysaccharide (LPS); both antibodies reacted with the liposomes of phosphatidylcholine and showed identical idiotypic patterns. It is suggested that the anti-BrMRBC antibodies may be produced antigen specifically by exogenous antigens to be useful to the mouse. The activation of anti-BrMRBC B cells specifically by EyLDL or polyclonally by LPS resulted in neither expansion nor regression of anti-BrMRBC LPS-reactive B cells. This finding indicates that the population size of anti-BrMRBC LPS-reactive B cells is controlled independently by exogenous specific or polyclonal stimuli.

Suppression of glomerular IgA deposition in ddY mice through periodic injections of lipopolysaccharides.

We investigated the role of lipopolysaccharides (LPS) in glomerular IgA deposition of ddY mice. The incidence of IgA deposition was lower in the LPS-injected mice at 10 and 13 months of age as compared with that of their age-matched controls (10 months, LPS 20% vs control 60%; 13 months, LPS 14% vs control 100%). Serum levels of IgA were lower in the LPS-injected mice than in the control mice. There were no appreciable differences in the percentage value of Thy-1+ cells, the ratio of Lyt-1+/Lyt-2+ cells, mitogenic responses, or IL-1 activities between the LPS-injected and the control mice. On the other hand, the IgA responses of spleen and Peyer's patch cells in the LPS-injected mice were lower than those observed in the control mice. These results suggest that the persistent use of LPS directly suppresses gut-associated lymphoreticular tissue (GALT), and that the lower IgA response in GALT induced by LPS causes suppression of glomerular IgA deposition in ddY mice.

Intermittent versus long-term tapering prednisolone for initial therapy in children with idiopathic nephrotic syndrome.

Forty-six children with steroid-responsive nephrotic syndrome were randomly allocated to receive two different prednisolone regimens for initial therapy. Twenty-nine children (group 1) received an intermittent regimen (60 mg/m2/day for 4 weeks, followed by 40/mg/m2/day on 3 days a week for 4 weeks); 17 children (group 2) had a long-term regimen (60 mg/m2/day for 4 weeks, followed by the same dose on alternate days for 4 weeks and the doses tapered by 10 mg/m2, given on alternate days every 4 weeks for 5 months). There was no difference between the two groups in the regimen used to treat relapses, steroid responsiveness, number of patients with relapses, and frequency of toxic reactions to steroids. However, the number of patients with a relapse within 6 months after initial therapy and the number of those with frequent relapses or steroid dependence were significantly higher in group 1 than in group 2 (P less than 0.05 for both). The data indicate that the long-term tapering regimen appears to be both safe and preferable to the intermittent regimen for initial therapy in children with idiopathic nephrotic syndrome.
