Total and differential serum proteins and globulins in leprosy.

Total and differential serum proteins were estimated in 177 adult male patients and globulins in 135 patients. Total serum proteins were slightly lowered in BB, BL and LL patients. Total albumin was low in all types of leprosy, especially in the BL group. Alpha-1 and alpha-2 fractions were unchanged all through the leprosy spectrum. Beta globulins were significantly lowered in LL patients as compared to the controls. Gamma globulins were elevated in all types of leprosy more so in BB, BL and LL patients. Details are presented.

Endemicity of leprosy in the Union Territory of Chandigarh and surrounding states.

Data on 603 leprosy patients registered with the Leprosy clinic at the Nehru Hospital attached to Postgraduate Institute of Medical Education and Research, Chandigarh was analysed. The sample was collected over a period of 6 years in this area of very low endemicity. The overall average age at onset of the disease was 35.07 years. Majority of the patients had contacted the disease by the age of 39 years. Only 2% patients presented with the disease in the first decade. Males were more often seen with the disease than the females (M:F, 3:6:1). The highest represented type of leprosy was borderline (47.7%) and least represented was lepromatous (24.4%) with tuberculoid group inbetween (27.9%). The most common sites of first lesion were the exposed areas i.e. hand, trunk, feet and forearm. Most of the patients came from high prevalence states of the country. There were, however, 118 Punjabis and 41 Haryanvis who had never been outside their home states to any endemic area. The findings are presented.

Concomitant occurrence of leprosy and tuberculosis--a clinical, bacteriological and radiological evaluation.

One hundred and seventeen consecutive patients of leprosy were bacteriologically and radiologically investigated for evidence of concomitant tuberculosis anywhere in the body. Out of the total, only 9 patients (7.7%) showed evidence of active tuberculosis bacteriologically and/or radiologically. Three patients had sputum positive for AFB, 2 out of these were radiologically negative while one had evidence of pulmonary tuberculosis. From 7 patients with radiological evidence of tuberculosis, tubercle bacilli could be grown in only one. Tuberculosis was found to occur throughout leprosy spectrum. It is important to recognise the presence of tuberculosis in leprosy patients so that proper therapeutic measures may be taken to avoid monotherapy of tuberculosis.

Factitious urticaria (dermographism): treatment by cimetidine and chlorpheniramine in a randomized double-blind study.

The HI-antihistamine chlorpheniramine and the H2-antihistamine cimetidine, given alone and in combination, have been compared with placebo in twenty patients with factitious urticaria (dermographism), in a double-blind, randomized cross-over trial. Of these regimes, the combination was the only treatment which significantly reduced weal size, flare size and duration of weal, compared with placebo, although other treatments approached statistical significance. Continuation of the most effective of the four treatments in nineteen of the patients for a further 3 months without breaking the randomization code provided further evidence of the great effectivness of combined cimetidine and chlorpheniramine. No significant side effects were note.

Sotalol in hyperthyroidism.

Ten hyperthyroid patients were studied before and after 2 weeks' beta-adrenoceptor blockade with sotalol. The following variables were measured: resting pulse rate, blood pressure, weight, thyroid hormone levels, plasma lipids, alkaline phosphatase, plasma glucose and insulin responses to oral glucose, bromsulphthalein retention and the 24-h urinary excretion of calcium, hydroxyproline, creatine and creatinine. Sotalol produced a significant fall in pulse and blood pressure. Weight loss continued during treatment. No metabolic changes of any consequence were found. It is concluded that sotalol should not be used as the sole treatment of a patient with hyperthyroidism.

Effect of low doses of alpha chlorohydrin on the enzymes of glycolytic and phosphogluconate pathways in the rat testis and epididymis.

A biochemical study has been made of the effects of low doses of alpha chlorohydrin on all the glycolytic enzymes and two key enzymes of phosphogluconate pathway i.e. glucose-6-phosphate dehydrogenase (G-6-PDH) and 6-phosphogluconate dehydrogenase (6-PGDH) of rat testis and epididymis. All the glycolytic enzymes of testis and epididymis are decreased after treatment with alpha chlorohydrin. G-6-PDH and 6-PGDH are decreased only in epididymis and not in the testis. LDH, ADH and glucose-6-phosphatase were also studied histochemically to show that the drug affects the glycolytic enzymes of epididymal cells and various testicular cell types of testis. Possible significance of these results is discussed.

Biochemical observations on the protein and nucleic acid metabolism of the rat testis and epididymis after treatment with low doses of alpha-chlorohydrin.

A biochemical study was made on the effects of low doses of alpha-chlorohydrin on the protein and nucleic acid metabolism of the rat testis and epididymis. RNA and protein levels were decreased both in the testis and epididymis. The DNA content of the testis and epididymis did not change after exposure of the animals to the drug. The reduced concentrations of RNA and protein were closely paralleled by the increased activity of proteinase (protein hydrolyzing enzyme) and ribonuclease (RNA degrading enzyme) in the testis and epididymis. The gamma-glutamyl transpeptidase activity of both the testis and epididymis was also reduced indicating the slow transfer of amino acids across the cell membranes of testis and epididymis and thus low protein synthesis. The DNAase levels of rat testis and epididymis did not show any appreciable change in response to the alpha-chlorohydrin treatment. These studies indicate that although there may not be any histological damage in the tissue the metabolic pathways may become defective much earlier before any visible morphological change is discernible.

Pathologic changes in striated muscles in leprosy.

Eighteen muscle biopsies from male adult patients of lepromatous and borderline lepromatous leprosy were studied. BI in the biopsies ranged from 1 + to 3+. There was conspicuous absence of granuloma except for a collection of foamy histiocytes in intermysial region and on two occasions in the intermysial nerve endings in LL patients. At places solid staining bacilli were seen in normal looking muscles. None of the biopsies showed loss of muscle striations, fatty change, sarcolemmal changes, fibrosis and necrosis. Hyaline change was seen in two specimens only. Details of these are discussed in this paper.

BCG induced immunity to Mycobacterium leprae in mice.

One to three BCG vaccinations were given to various groups of mice--25 to 2 days preceding challenge with Myco. leprae. Harvesting of Myco. leprae was done every month upto a period of 7 months. Lymph node enlargement was taken as an indicator of successful vaccination. An increasing degree of protection was noted in BCG immunised mice as observed by a reduced number of bacilli harvested. Protection afforded was proportional to the number of doses of BCG given.
