Asthma: comparative bronchodilator effects of ipratropium bromide and isoproterenol.

The effect of ipratropium bromide administered at two dosage levels, 40 and 80 mug, isoproterenol, 150 mug, and placebo using a metered dose inhaler was evaluated in ten adult patients with asthma in a double-blind, crossover study. The new atropine-like drug proved to be as effective a bronchodilator as isoproterenol in this study, although it had a later peak effect. Ipratropium bromide had a longer course of action than isoproterenol (4 hours compared to 1-2 hours) and was free of significant side effects. The larger dose of the new drug produced a slightly greater and longer-acting effect than the smaller dose. Ipratropium bromide seems to have had bronchodilator effects on both large and small airways.

Relative efficacy of methylphenidate and behavior modification in hyperkinetic children: an interim report.

Children reported to be hyperactive in school and with behavior difficulties at home were randomly assigned to methylphenidate, behavior therapy and placebo, or behavior therapy with methylphenidate for an 8-week period. Rating scales were obtained from teachers and parents. Independent blind observers rated childrens' classroom bheavior on a weekly basis. A behavior therapy program was implemented in the home and at school. Methylphenidate dosage was individualized. Ratings of behavior deviance were significantly reduced by all treatments. However, a significant advantage for the groups receiving methylphenidate was found over the group receiving behavior therapy and placebo. No significant differences between methylphenidate alone and methylphenidate combined with behavior therapy were obtained. Global ratings of improvement done by teachers favored the combined treatment of behavior therapy and methylphenidate over behavior therapy and placebo. No differences among treatments were found in the mothers' global ratings of improvement. The results indicate that though all three treatments were effective, methylphenidate was significantly superior to behavior therapy alone.

Left-handedness in twins: incidence and patterns of performance in an adolescent sample.

Data on handedness and cognitive performance in an adolescent sample of same-sex twins were collected, and questions about incidence of left-handedness in twins and the relation between handedness and cognitive performance were considered. Same-sex twins have been found to have a higher incidence of left-handedness than that usually reported in the general population. There is a high incidence of handedness discordance (one twin right-handed and his cotwin left-handed) in both monozygotic and dizygotic twin pairs. In this sample, males and Blacks had particularly high rates of left-handedness. Among the monozygotic and dizygotic discordant pairs, quite different patterns of cognitive performance were found. On the Raven Progressive Matrices, the Columbia Test of Mental Maturity, and the Peabody Picture Vocabulary Test, the left-handers did better than their right-handed cotwins in the monozygotic pairs and the right-handers did better than their left-handed cotwins in the dizygotic pairs. Within subjects, no signficant differences were found for superiority of spatial or verbal abilities for either right-handers or left-handers. No general statements can be made about the performance of left-handed twins, since performance differs according to zygosity and handedness of cotwin configurations.

Protein-metal ion interaction: volume effects produced by the interaction of proteins with metal ions.

The interaction of metal cations with single chain globular proteins produces volume increases, the magnitude of which is determined primarily by the ion and to a lesser extent by the protein. The cations are listed in ascending order of volume change: K(I) less than Mg(II) less than Sr(II) less than Ca(II) less than Co(II) less than Ni(II) less than Cd(II) less than Zn(II) less than Cu(II) less than Pb(II). This sequence held for all cation-protein systems investigated except for Cd(II) which produced a slightly larger volume effect than Zn(II) with lysozyme. The volume changes attributed to protein-cation interaction are positive and range from 8 ml/10(5) g of protein for the reaction on 0.05 M KNO3 with bovine plasma albumin to 2320 ml/10(5) g of protein produced by the 0.20 M Pb(NO3)2-myoglobin system. A similar classification scheme was not possible for the proteins. For example, volume increases of 45, 50, 80 and 95 ml/10(5) g of protein were produced when 0.05 M Mg(II) reacted with bovine serum albumin, ovalbumin, sperm whale myoglobin and lysozyme, respectively. However, when 0.2 M Pb(II) was the reactant the values were 1930, 846, 2320, and 1120 ml/10(5) g of protein. Volume effects produced by Cr(III), Al(III) and Fe(III) were determined, but the calculated results are considered dubious because the volume changes are a complicated function of protein-cation and protein-proton interaction.

12. Index of ADL.

The Index of Activities of Daily Living (ADL) encompasses six basic human functions: bathing, dressing, toileting, transfer, continence, and feeding. It provides an objective method of classifying heterogeneous groups of people with chronic illnesses, disabilities and impairments, and of describing their health needs and outcomes.

A measure of primary sociobiological functions.

The Index of Independence in Activities of Daily Living (Index of ADL) is a scale whose grades reflect profiles of behavioral levels of six sociobiological functions, namely, bathing, dressing, toileting, transfer, continence, and feeding. Its hierarchical nature makes it possible to rank the overall functional status of people in an ordered manner, to make comparisons among them as individuals or groups, and to detect changes over time. Developmental studies have encompassed children as well as adults, the mentally retarded as well as physically disabled, and noninstitutionalized as well as institutionalized people. In view of relationships to behavior of primitive societies and parallelisms between the order of index functions and patterns of child growth and development, the Index of ADL appears to be based on functions of sociobiological primacy. This concept is supported by twenty years of methodological and applied observations. The Index has been used to produce predictive information about chronic conditions and to evaluate the benefits of long-term services. It has been used in profiled measures of severity of illness. As a screening measure and survey measure, it has contributed information about health needs and outcomes which is useful for management, planning, policy making, research, and techning.

A comparison of 6,7-dihydroxytetrahydroisoquinoline, salsolinol and tetrahydropapaveroline as inhibitors of monoamine oxidase within the adrenergic nerve plexus of the isolated mouse atrium.

Three dopamine-derived tetrahydroisoquinolines (TIQs) were tested for their ability to inhibit monoamine oxidase (MAO) within the adrenergic plexus of atria from reserpinized mice. Inhibition of MAO was shown by improved accumulation of 3H-norepinephrine with a concomitant decrease in 3H-deaminated catechols. Salsolinol was less potent than 6,7-dihydroxy-TIQ as a MAO inhibitor. Tetrahydropapaveroline did not appear to inhibit MAO in this system.

Spectrum of pleural involvement in sarcoidosis.

Pleural involvement has generally been considered an unusual aspect of sarcoidosis. In this paper, five previously unreported cases of sarcoidosis involving the pleura were discussed. Pleural sarcoidosis may be manifested by the presence of a pleural effusion, and the diagnosis is based on the exclusion of all other possible causes and the demonstration of noncaseating granulomas in the pleura. When the pleura is involved without effusion, it may only be recognized if noncaseating granulomas involving the pleura are looked for specifically. Including the five cases form this study, there have been 73 cases of pleural sarcoidosis reported, but only 37 have been verified by histologic evidence. When strict diagnostic criteria are used, clinical involvement of the pleura in sarsoidosis remains an unusual entity, but it may be that histologic involvement of the pleura is more common than generally appreciated.

Ca-2+-stimulated membrane phosphorylation and ATPase activity of the human erythrocyte.

1. Human erythrocyte membranes were preincubated with ethyleneglycolbis-(beta-aminoethyl)-N,N' tetraacetate (EGTA) and subsequently labelled for short periods with micromolar concentrations of [8-3-H, gamma-32-P]ATP. Under these conditions, and at temperatures smaller than or equal to 22 degrees C, both ATP hydrolysis and membrane phosphorylation were stimulated by Ca-2+. 2. The properties of the Ca-2+-stimulated ATP hydrolysis and associated phosphorylation of a 150 000 molecular weight protein component, previously described (Knauf, P. A., Proverbio, F. and Hoffman, J. F. (1974) J. Gen. Physiol. 63, 324-336), have been studied. The behavior of the phosphorylated component, ECaP, has properties consistent with its role as a phosphorylated intermediate of Ca-2+-ATPase activity, including: (1) similar dependence of the steady-state level of ECaP and Ca-2+-ATPase on ATP concentration; (2) rapid turnover apparent upon the addition of excess non-radioactive ATP; and (3) good correlation between the steady-state levels of Ca-2+-dependent phosphorylation and Ca-2+-ATPase activity in separate preparations possessing variable specific activity. Addition of excess EGTA to ECaP caused only partial dephosphorylation. Sensitivity of Ca-2+-stimulated ATP hydrolysis and associated phosphorylation to micromolar concentrations of Ca-2+ implicates this activity in the "high-affinity" Ca-2+-pump system of the human erythrocyte (Schatzmann, H. J. (1973) J. Physiol. London 235, 551-569).

Comparative serum and urine analyses by dual-detector anion-exchange chromatography.

A high-pressure anion-exchange chromatographic system has been modified to provide measurement of large numbers of molecular constituents in serum and for direct comparison to similar measurement in urine. Operating parameters have been adopted which greatly extend the range of elution for strongly retained anionic constituents and limit resolution of early-eluting basic and neutral compounds which were of less interest in this study. Dual monitoring by UV absorption and fluorescence produced by cerate oxidation provides both sensitive and wide-ranging detection capability. Comparative serum and urine chromatograms for a clinically normal subject, a subject after ingesting the drug 4-hydroxyacetanilide, and an infant suffering from extreme acidosis, illustrate the potential usefulness of this analysis in studying the origin, transport, in vivo reactions, and disposition of metabolites.
