[Early invasive squamous cell carcinoma of the tongue--report of a case].

A 65-year-old woman with early invasive squamous cell carcinoma of the tongue is reported. Atypical but not definitely malignant epithelium was detected by cytology, although the gross appearance of the glossal lesion looked like early invasive squamous cell carcinoma. On the surgical material, the basal and para-basal cell layers were occupied by cancer, but the upper portion of the epidermis from the prickle cell layer through the surface of the mucosa was histologically benign. In such cases, to obtain the correct diagnosis on cytology, repeated examinations as well as using a sharp instrument like a curatte are necessary.

[Effect of dopaminergic agents on the endocrine profile in labor and early puerperium].

The purpose of this study was to investigate the prolactin (PRL) releasing mechanism of the hypothalamo-pituitary axis during labor and early puerperium. Ten full-term gravidas during labor and 42 women in early puerperium were examined in this study. The plasma PRL levels rose significantly (p less than 0.001 approximately 0.05) after an intravenous bolus of 10mg metoclopramide (MCP). The peak values for PRL increase were 609.3 +/- 194.1ng/ml during labor and 447.0 +/- 62.3ng/ml in early puerperium. However, there were no significant differences in PRL response to MCP between these two groups. The plasma PRL levels dropped significantly (p less than 0.001 approximately 0.01) after an oral administration of 2.5mg bromocriptine (BRC), and the PRL release from the pituitary by MCP was suppressed significantly (p less than 0.001) by pretreatment with BRC in the puerperium. In addition, no significant changes in plasma 17 beta-estradiol (E2), progesterone (P) and cortisol levels could be observed after MCP. We concluded that the control mechanism of PRL secretion remained unchanged during labor and early puerperium.

Influence of long-term repetitive rotatostimulations on lateral semicircular canals.

Sixty-five guinea pigs were used to investigate the influence of rotatostimulations on the lateral crista ampullaris. After repeated turning stimulations with the cupulometric mode (the terminal turning velocity: 180 degrees/sec) for 24-72 hours, the morphological changes in the crista ampullaris on the ampullopetal flow side were compared with those on the ampullofugal flow side by using scanning and transmission electron microscopy. Postrotatory nystagmus recorded by ENG during this experiment revealed the response decline phenomenon in all animals, and the caloric test performed after the rotatostimulation showed 'canal paresis' in the ear on the ampullopetal flow side. In a group stimulated for 24 hours, no particular damage on the crista ampullaris was noticed. However, in a group stimulated for 72 hours, local damage was dominant in the central part of the crista, which showed a tendency to extend towards the planum semilunatum according to an increment of changes. These findings were more remarkable on the ampullopetal flow side than on the ampullofugal flow side, suggesting the predominant effect of the ampullopetal endolymph flow in the lateral semicircular canal.

[Total parenteral nutrition in inflammatory bowel disease. An evaluation of its clinical response].

We studied the effects of total parenteral nutrition (TPN) on the course of inflammatory bowel disease (IBD) which evaluation was still conflicting. Since 1972, in our department 32 courses of TPN were done for 27 patients with IBD. Definite improvement on nutritional status and clinical symptoms such as abdominal pain, diarrhea, vomiting was observed in most courses of TPN, especially for the patients with intestinal tuberculosis, non-specific multiple intestinal ulcers, and Crohn's disease. Clinical remission was obtained in 26 out of 32 courses of TPN (81.3%) on the short-term follow-up. But on the long-term follow-up ranging from 6 months to 11 years, 11 out of 20 patients (55%) were symptom-free without any medical treatment. Clinical relapse occurred in 6 patients and another TPN was required. These results suggest that TPN is useful as an adjunctive therapy for IBD patients requiring bowel rest and nutritional repletion. Two patients requiring prolonged TPN are returning to work with home parenteral nutrition (HPN) for 2.5 and 5 years respectively. HPN is safe and effective for improving life of quality in the patients receiving prolonged TPN.

Jejunal interposition hepaticoduodenostomy for congenital dilatation of the bile duct (choledochal cyst).

In eight cases of congenital dilatation of the bile duct encountered in our institution during the past 3 years, jejunal interposition hepaticoduodenostomy was performed following resection of the involved bile duct. This procedure has no serious technical difficulties but appears to have some advantages over conventional techniques. The postoperative course of the patients has been favorable for a mean period of 1 year and 5 months up to the present. A further follow-up study is necessary before definite conclusions can be drawn.

Changes of the auditory system after cerebellopontine angle manipulations.

The current tendency in acoustic neuroma surgery to attempt the preservation of hearing function and the problem of accidental hearing loss caused by microsurgical neurovascular decompression operations for hemifacial spasm or trigeminal neuralgia prompted us to study the exact surgical vulnerability of the auditory system. The surgical procedures for operation on the cerebellopontine angle of dogs were carried out according to the sequence of the posterior fossa transmeatal operation for acoustic neuroma. The operation was tentatively divided into three stages: (a) craniectomy and dural opening, (b) cerebellar retraction, and (c) identification of the cochlear nerve in the unroofed internal auditory canal (IAC). The postoperative behavior of the auditory system was evaluated electrocochleographically (EcochG) and histologically. Overzealous retraction of the cerebellar hemisphere caused transient disturbance of the EcochG pattern. Mechanical stretching of both the cochlear nerve and the internal auditory artery may cause a disturbance in the synchronized discharge of the cochlear neurons. Various manipulations at the porus acusticus internus or the IAC (such as pinching the nerve with forceps or electrocoagulation) produced thoroughly distorted EcochG patterns. From the histological findings, the main causative factor for these labyrinthine damages was considered to be vascular insufficiency. The current need for neurosurgical operations to preserve hearing is discussed in the light of these findings.

Common channel syndrome--diagnosis with endoscopic retrograde cholangiopancreatography and surgical management.

Six patients with intermittent bouts of vomiting, fever, abdominal pain, and jaundice beginning in infancy or early childhood were all demonstrated by endoscopic retrograde cholangiopancreatography to have an anomalous junction of the pancreaticobiliary ductal system with the formation of a characteristic long common channel. A varying degree of dilatation of the bile duct also was noted. Resection of choledochus followed by hepaticoduodenostomy was performed with satisfactory results invariably in all cases. The existence of a pathologic entity that might reasonably be designated "common channel syndrome" is discussed with some comments on its relationship with dilatation of the bile duct (choledochal cyst) as well as on the recommendable method of surgical treatment.

Simultaneous analyses of phenethylamine, phenylethanolamine, tyramine and octopamine in rat brain using fluorescamine.

A fluorometric method for the simultaneous analyses of phenethylamine, phenylethanolamine, tyramine and octopamine has been developed. The method involves ion-exchange chromatography, derivatization with fluorescamine, solvent extraction and then separation by thin-layer chromatography. The fluorescent spots are then quantitated by scanning. The detection limits of this method are about 10 pmoles for phenethylamine, phenylethanolamine and tyramine, and 20 pmoles for octopamine. The method was used for simultaneous analyses of putative neurotransmitter amines in whole rat brain.
