Benzodiazepine intoxication treated with flumazenil (Anexate, RO 15-1788).

The efficacy and safety of flumazenil were assessed in comparison to placebo in a double-blind randomised study of 31 adults intoxicated with benzodiazepines. The criteria of efficacy were the degree of sedation, and orientation in time and space. Patients who received flumazenil awoke within minutes but central depression returned partly one hour later, which reflects the short elimination half-life of the drug. Side effects were few and the results indicate that flumazenil is effective in the primary management of benzodiazepine overdose and in states where benzodiazepines have been taken with other drugs.

Traumatic amputation of the upper limb: the use of body-powered prostheses and employment consequences.

Forty three patients with unilateral traumatic amputations were reviewed as to the use of prostheses and employment consequences of the amputation. Seventeen of 19 below-elbow amputees, and 12 of 24 above-elbow amputees used their prostheses. Non-users compared to users of prostheses were characterized by: 1) Higher level of amputation 2) Non-dominant arm amputation and 3) Younger age at the time of amputation. However non-users usually did well on the labour market for various reasons.

Flumazenil used in the antagonizing of diazepam and midazolam sedation in out-patients undergoing gastroscopy.

In two double-blind, randomized trials the efficacy and safety of flumazenil, the first benzodiazepine antagonist, were assessed in 100 adult patients undergoing gastroscopy under diazepam or midazolam sedation. The criteria of efficacy were the degree of sedation and anterograde amnesia. The median gastroscopy time was 20 min (range 5-40 min). The diazepam group received median 30 mg (range 15-60 mg) Diazemuls and the midazolam group median 15 mg (range 10-40 mg) Dormicum. Both groups were antagonized by median 0.42 mg flumazenil (range 0.4-0.6 mg). There was no inter-group difference with regard to blood pressure, heart rate and respiration rate. There was a significantly faster recovery of the patients after injection of flumazenil than after placebo. Patients were awake shortly after flumazenil, but remained drowsy or asleep after placebo administration. All patients, regardless of diazepam or midazolam sedation, antagonized with flumazenil were awake within 5 min and remained awake during the whole observation period of 3 h. The amnesia was totally eliminated by flumazenil. There were no significant differences in side-effects between the groups.

Cryopreservation of fetal rat brain tissue later used for intracerebral transplantation.

Intracerebral grafting of immature brain tissue is now widely used as a tool to study neuronal development and regeneration in the brain and spinal cord. This has stimulated the interest in methods for storage of such tissue before transplantation. In this study a method for cryopreservation of immature rat central nervous tissue is presented and discussed in relation to current cryobiological principles. The method was applied to brain tissue from 16- and 17-day-old fetal rats, including the neocortex, habenula, septum and basal forebrain, cerebellum, and retina. After storage in liquid nitrogen from 6 to 52 days the tissue was grafted into the brain of adult rats. The recipients survived for 23 to 673 days before their brains were processed by current neuroanatomical, histological methods. The presence of graft tissues was recorded and their cellular and connective organization was examined, including their exchange of nerve connections with the host brain. The results obtained were comparable with results from other studies where the same tissues were grafted immediately after removal from the donor, and a study of cryopreservation of developing hippocampal tissue. We conclude that cryopreservation is a reliable method for storage of immature neural tissue later to be used for intracerebral grafting.

Ibuprofen-induced meningitis in a male with systemic lupus erythematosus.

A 36-year-old male with systemic lupus erythematosus developed aseptic meningitis after ingestion of ibuprofen. He was unconscious, had a universal rash, high fever and a pathological CSF. Being severely ill he was treated with high doses of corticosteroids and plasmapheresis and recovered within a week.

Bilateral papilledema with normal CT-scan in neurosarcoidosis.

A 74-year-old woman was admitted with fever, muscle and joint pain and a mild hypercalcemia. Rather suddenly she turned unconscious and bilateral papilledema was found. A subsequent CT of the brain, however, did not disclose signs of space-occupying masses or increased intracranial pressure. She died two days later and at autopsy there were no signs of incarceration. Microscopic examination revealed non-caseating epithelioid granulomas throughout the meninges and brain consistent with sarcoidosis.

Randomized clinical investigation of Ro 15-1788, a benzodiazepine antagonist, in reversing the central effects of flunitrazepam.

In a double-blind, randomized study the efficacy of Ro 15-1788 as a benzodiazepine antagonist was evaluated in patients anaesthetized with flunitrazepam. The effects were compared with a comparable group of patients who did not receive the antagonist. Ro 15-1788 significantly reversed the sedative and hypnotic effects of flunitrazepam and reduced the degree of anterograde amnesia. No side-effects were attributable to the drug.

Intracephalic transplants of freeze-stored rat hippocampal tissue.

The survival and cellular and connective organization of intracephalic transplants of developing, freeze-stored rat hippocampal tissue were examined. Blocks of tissue containing the hippocampus and fascia dentata were obtained from late embryonic (E16-E22) and early postnatal rats (P0-P4) and immersed in a tissue culture medium with 10% of the cryoprotective agent DMSO, frozen at a cooling rate of approximately 1 degree C/minute, and stored for 1-226 days in liquid nitrogen. After quick thawing and washing out of the DMSO the tissue blocks were transplanted to the brain of adult rats. From 2 weeks to 3 months later the recipient brains were processed histologically. The cellular and connective organization of the transplants and their interaction with the host brains were analyzed after thionin cell staining, Timm's staining for hippocampal and dentate afferents, immunohistochemical staining for enkephalin-, CCK-, and somatostatin-reactive neurons and afferents, AChE staining for cholinergic afferents, and silver stains for fiber architectonics and tracing of connections by anterograde axonal degeneration. Freeze-storage narrowed the range of donor ages with good transplant survival. The best surviving hippocampal and dentate transplants thus came from 17-21-day-old embryos. There was no correlation between the length of storage and survival. Structurally the transplants of stored tissue were more frequently fragmented than the transplants of fresh tissue when located outside the brain parenchyma in the brain ventricles. This was in accordance with the results of a previous study of grafts of freeze-stored and fresh hippocampal tissue placed in the anterior eye chamber. Despite the decrease in survival and the tendency for fragmentation many well-structured and organotypically organized hippocampal and dentate transplants were recovered corresponding to the donor ages E19-E21. In addition to the main cell types (granule cells and pyramidal cells) the freeze-stored transplants also contained peptidergic nerve cells reacting for CCK, somatostatin, and enkephalin. The organization of the intrinsic nerve connections and the exchange of connections with the host brain were similar for transplants of stored and fresh tissue. Besides the consistent innervation of the hippocampal and dentate transplants by host cholinergic afferents monitored by AChE staining, several appropriately located dentate transplants thus sent mossy fibers to the host CA3. Others received host perforant path projections. A CA3-associated transplant projection to the denervated perforant path zones in the host fascia dentata was also observed.(ABSTRACT TRUNCATED AT 400 WORDS)

Monitoring of environmental cancer initiators through hemoglobin adducts by a modified Edman degradation method.

Tissue doses of cancer initiators/mutagens are suitably monitored through hemoglobin adducts formed in vivo, but the use of this method has been hampered by a lack of sufficiently simple and fast procedures. It was previously observed that when the N-terminal amino acid in hemoglobin, valine, is alkylated it is cleaved off by the Edman sequencing reagent, phenyl isothiocyanate, in the neutral-alkaline coupling medium, as opposed to the acidic medium required by normal amino acids. Based on this principle, conditions for a functioning procedure for gas chromatography/mass spectrometry (GC/MS) determination of N-terminal alkylvalines in hemoglobin were worked out. Derivatizing the protein in formamide solution with pentafluorophenyl isothiocyanate, using a 2H-alkylated protein as internal standard, and applying on-column injection during analysis, permit reproducible determination of hydroxyethylvaline and other adducts down into the dose range where cancer risks may be considered acceptably low.

Separation of lipophilic substances in environmental samples with special reference to toxaphene.

A microcolumn of alumina, activated at 250 degrees C over night and deactivated with 2% water in an oven at 150 degrees C, has shown better separation characteristics than similar ones, deactivated at room temperature, for a great number of organochlorine compounds, particularly with respect to multicomponent toxaphene and PCB. The microcolumn is a simple disposable Pasteur pipette. Three fractions are collected: one with PCB and DDE, one with toxaphene components having similar lipophilicity to DDT and the third, toxaphene components similar to DDD in lipophilicity. Two such toxaphene fractions give a better GLC-pattern than only one fraction. DDD and DDT, which interfere with the analysis of toxaphene can be eliminated, after their GLC analysis, from the "Toxaphene fractions" by nitration followed by reduction of the nitro compounds formed to their corresponding amines. Performance of the column using environmental samples showed that it is a useful tool in routine pesticide residue analysis, especially when toxaphene is present.

Tissue doses of ethylene oxide in cigarette smokers determined from adduct levels in hemoglobin.

Determination of adducts to hemoglobin (Hb) is a useful approach for monitoring tissue doses of ultimate carcinogens. This approach provides a basis for both risk estimation and for the identification of a priori unknown environmental carcinogens. This paper describes the application of a new method for the analyses of Hb adducts to cigarette smokers and non-smokers. The results demonstrate a raised level of hydroxyethylation of N-terminal valine of Hb of smokers that is quantitatively compatible with ethene in the smoke being the source. The magnitude of the tissue doses of ethylene oxide originating from inhaled ethene suggests that this factor is a major contributor to smoking-associated cancer risk.

Benzodiazepine antagonist Ro 15-1788. Antagonism of diazepam sedation in outpatients undergoing gastroscopy.

In a double-blind, randomised trial the efficacy and safety of Ro 15-1788, a new benzodiazepine antagonist, was assessed in forty adults undergoing gastroscopy under diazepam sedation. Criteria of efficacy were the degree of sedation and anterograde amnesia. There was a significantly faster recovery of the patients after the injection of 0.6-1.0 mg of Ro 15-1788 than after placebo. Patients were awake shortly after Ro 15-1788, but remained drowsy or asleep after placebo administration. There were no side effects of note.
