An outbreak of Norwalk-related gastroenteritis at a boys' camp.

An acute gastrointestinal tract illness affected 213 (52%) of 407 campers and 64 (53%) of 121 staff members attending a boys' camp in the Catoctin Mountains of Maryland during the summer of 1981. Nausea was the predominant symptom for ill campers and staff members (73%), but more staff members experienced diarrhea (49%) than did campers (9%). Twenty-three individuals had more than one episode of illness compatible with the case definition. Eight of nine paired blood specimens from ill staff members showed a fourfold increase in antibody titer to Norwalk virus by radioimmunoassay. Environmental inspections and laboratory tests failed to implicate a common source of exposure.

Postconceptional age and gentamicin elimination half-life.

We examined the correlation between the elimination half-life of gentamicin and postnatal or postconceptional age in 104 infants greater than 24 weeks to less than or equal to 48 weeks postconceptional age. A weak correlation was observed between half-life and postnatal age (R = -0.353). A better correlation existed between postconceptional age and half-life (R = -0.572). A plot of the log of the mean half-life against postconceptional age was virtually linear. We conclude that postconceptional age is a major determinant of an infant's ability to excrete gentamicin, and speculate that the relationship between half-life and decreasing postconceptional age is exponential.

Class and subclass anti-pneumococcal antibody responses in splenectomized patients.

Antibody titres against pneumococcal capsular and cell wall antigens and the immune response to polyvalent pneumococcal vaccine were measured in 21 splenectomized patients and 12 healthy controls. Most individuals possessed anti-pneumococcal capsular polysaccharide antibodies of IgG, IgA and IgM classes. The anti-capsular IgG was predominantly of the IgG1 and IgG2 subclasses; only occasional individuals had any detectable titre in IgG3 or IgG4 subclass. Most individuals responded to immunization with Pneumovax. There was no clear difference between groups of control and splenectomized subjects, although three of the splenectomized patients had undetectable pre-immunization anti-capsular titres in one or more subclass which failed to rise following immunization. All subjects tested had anti-phosphocholine antibodies in IgG, IgA and IgM classes with the exception of a single splenectomized patient who lacked detectable anti-phosphocholine IgM. Pre-immunization titres where similar in healthy controls and splenectomized patients. There was no demonstrable rise in anti-phosphocholine titre following immunization with Pneumovax.

Continuities of common behaviour problems in preschool children.

This paper describes the prevalence of behaviour problems in 2 populations of preschool children in London, and the continuities over time of common problems. Night waking was the most common problem under the age of 2, being reported in 21% of 1-year-olds. Just under half of these night wakers were still waking at 18 months, and from 18 months to 2 years just over half the children continued waking The continuities of feeding problems were stronger after the age of 2 years, with poor appetite being reported to persist more than food fads. Over 2 years the most common problem reported was temper tantrums, and 45% of the children having frequent tantrums at 2 years were still having frequent tantrums at 3 years. The implications for services and the need for intervention studies are discussed.

The behaviour, development, and health of the young child: implications for care.

Children with recurrent physical illnesses such as infections of the ears and chest are more likely than average to have developmental and behavioural disorders as well. Specialist skills are needed for the assessment and management of these disorders; the belief that "the child will grow out of it" is rarely correct. Any reorganisation of the child health services should take account of the need for children with developmental and behavioural disorders to be treated within the community by doctors and other health workers with the appropriate skills.

A study of pre- and postsurgical transsexuals: MMPI characteristics.

This study examines assessment issues concerning transsexualism through the use of the Minnesota Multiphasic Personality Inventory (MMPI). The MMPI was administered to 20 transsexuals matched within sex on age and education: five presurgical male-to-females, five postsurgical male-to-females, five presurgical female-to-males, and five postsurgical female-to-males. Mean T scores for each of these four subsamples were examined in comparison to normative groups. Comparisons among the four subsamples showed significant differences in mean raw scores attributable to both sex and surgical status. The most striking of these comparisons indicated that postsurgical subjects had a higher level of psychological adjustment.

Use of the child health clinic.

Attendance at child health clinics in two areas of London, north Westminster and south Camden, is described. Routine examinations were attended by 97% of children although such visits accounted for only one-third of attendances. There were more non-routine visits than have been reported in most other studies--about 14 visits in Westminster and 12 in Camden during the first year. Over half the mothers sought advice on a wide variety of medical, developmental, and behavioural problems.

Amblyopia cannot be explained by considering only detection thresholds.

Threshold function and suprathreshold performance for nonacuity stimuli are compared for an amblyope who exhibits a low as well as a high spatial frequency threshold abnormality. The results show that (i) a suprathreshold abnormality involving discrimination and localization of easily visible stimuli occurs in amblyopia and (ii) suprathreshold abnormalities occur in amblyopia independent of any threshold dysfunction.

Assessment of speech and language development in the young child.

A clinical method of assessing speech and language development in preschool children is described. Sixty-two 3-year-old children were assessed by a pediatrician, a speech therapist, and a psychologist. In 55 children there was agreement between all three examiners. Three children with articulation problems were rated as having normal comprehensive and expressive language by the psychologist. Three children were rated as having normal speech and language development by the pediatrician and speech therapist but delayed by the psychologist; all three had behavior problems. One further child rated as having a speech and language problem by the pediatrician was rated as normal by the speech therapist and psychologist. It is concluded that pediatricians can make reliable assessments of speech and language development.
