Role of dexamethasone in the outcome from acute stroke.

Forty patients of acute stroke of less than 48 hours duration above the age of 40 years were studied. All patients were evaluated as per the evaluation scoring system at the time of admission within 24 hours and 7 days after admission to hospital. 25 patients were given dexamethasone and 15 patients who were not given dexamethasone were matched for age, sex, underlying diseases and clinical condition as per the evaluation scoring. The mean evaluation score in study group showed significant improvement at 7 days (p less than 0.05) as compared to control group. There was no significant difference in mortality between the study and control group, being 36% and 33% respectively. Functional status also remained identical in the two groups at the time of discharge. There was also no significant difference in the outcome of patients of intracerebral haemorrhage vs. cerebral infarction whether treated with dexamethasone or not.

Outcome of low birth weight babies with special reference to some maternal factors.

Neonatal outcome of 178 low birth weight (LBW) babies in this study was associated with 26.4% neonatal mortality. A significantly higher mortality rate was noted in presence of adverse maternal factors, birth weight less than 1.5 kg, prematurity and respiratory distress at birth. Premature rupture of membranes and leaking (greater than 12 h) were recorded in 75 cases. Significant association was observed for septicemia. Maternal postpartum weight less than 40 kg was associated with higher incidence of neonatal infections than when mother's weight was greater than 45 kg.

Range of toxicity of topical cysteamine in rabbit eyes.

Cysteamine eye drops in concentrations from 0.1% to 10% were tested in albino rabbits in both short and long-term experiments. Toxicity, consisting of an inflammatory response, was proportional to the cysteamine concentration and absent at concentrations at or below 0.5%.

Left thalamic hypertensive haemorrhage presenting as transient global amnesia.

A case is described which presented as transient global amnesia (TGA) due to a critically placed intracerebral hypertensive haematoma in the left thalamic region, with epileptic discharges arising from the same area. It is proposed that intracerebral haematomas may occasionally present as a clinically well-defined neurological syndrome. The deficit in such cases is probably not due to the underlying direct tissue damage but secondary to indirect pressure or ischaemic insults.

The potentially Z-DNA-forming sequence d(GTGTACAC) crystallizes as A-DNA.

(GT)n/(CA)n sequences have stimulated much interest because of their frequent occurrence in eukaryotic DNA and their potential for forming the left-handed Z-DNA structure. We here report the X-ray crystal structure of a self-complementary octadeoxynucleotide, d(GTGTACAC), at 2.5 A resolution. The molecule adopts a right-handed double-helical conformation belonging to the A-DNA family. In this alternating purine-pyrimidine DNA minihelix the roll and twist angles show alternations qualitatively consistent with Calladine's rules. The average tilt angle of 9.3 degrees is between the values found in A-DNA (19 degrees) and B-DNA (-6 degrees) fibers. It is envisaged that such intermediate conformations may render diversity to genomic DNA. The base-pair tilt angles and the base-pair displacements from the helix axis are found to be correlated for the known A-DNA double-helical fragments.

Removal of corneal crystals by topical cysteamine in nephropathic cystinosis.

In patients with nephropathic cystinosis, corneal crystals develop by one year of age; they progressively accumulate and eventually cause recurrent corneal erosions and photophobia. After an in vitro study of cystinotic corneal stromal cells showed cystine depletion by cysteamine and after topical cysteamine was determined to be nontoxic in rabbits, we performed a controlled double-blind clinical trial of 10 mM cysteamine eyedrops in young patients with cystinosis, using one eye for treatment and the other as the control. Two children begun on the protocol before two years of age had a striking decrease in the number of corneal crystals in the cysteamine-treated eye within four to five months of entering the study. Cysteamine eyedrops appear to be safe and efficacious in the short-term treatment of patients with cystinosis who are under two years of age. The long-term value of such treatment and its effectiveness in older patients remain to be determined.

Encephalopathy due to inorganic lead exposure in an adult.

A 30 years old male presented with acute gastrointestinal symptoms and encephalopathy. History, investigations and the response to chelator therapy led to the diagnosis of lead poisoning. Encephalopathy in an adult resulting from industrial exposure to inorganic lead are the main features of the case.
