Muscle mutability. Part 2. Adaptation to drugs, metabolic factors, and aging.

Muscle fibers undergo changes in response to a variety of stimuli. This review examines metabolic, endocrine, and pharmacological factors that lead to alterations in muscle structure and function. These various stimuli may be considered nonphysical. This review also examines the interaction between physical stimuli (eg, exercise) and hormonal influences (eg, diabetes) on muscle. It is suggested that physical stimuli may often have a more potent effect on muscle than do nonphysical stimuli.

Review of length-associated changes in muscle. Experimental evidence and clinical implications.

Movement dysfunction that may be caused by length-associated changes in muscle is a problem of people treated by physical therapists. The purpose of this article is to review the literature related to length-associated changes in muscle. An analysis of length-associated changes in animal and human studies is presented. The methods used to produce the changes in animals are discussed, and the clinical implications of the length-associated changes in muscle are suggested.

Neonatal meningitis due to Citrobacter koseri.

A five day old neonate was diagnosed as having bacterial meningitis and commenced on Ampicillin and Flucloxacillin. The organism was then found to be Citrobacter koseri and the antibiotics changed to Chloramphenicol systemically for two weeks. The child made an uneventful recovery. At four weeks of age her head circumference had increased unacceptably and a computerised axial tomography scan revealed a large frontal lobe abscess. Aspiration revealed a large pus filled cavity and Citrobacter koseri grown from the pus. The abscess was treated with repeated aspirations, often of 30-40 mls., and installation of Chloramphenicol combined with systemic Chloramphenicol for three weeks. At six months of age her head circumference was within normal limits and the was developmentally normal with no detectable neurological sequelae.

Variations in perceptions of child neglect.

This study explored the perceptions of the seriousness of specific components of neglect held by mothers from three cultural groups and held by public child welfare workers in two different roles in the Chicago area. It then compared the perceptions of the mothers to those of the workers. The findings suggest that members of minority groups perceive some types of child neglect as more serious than do their Caucasian counterparts, that investigative workers see most types of child neglect as more serious than do service workers, and that workers see all types of child neglect as less serious than do the mothers. Comparisons revealed that various groups perceive the dimensions of neglect differently in terms of their potential harm to the child.
