105 women had been evaluated using a retrospective analysis for the period 1999-2000. Clinical and laboratory data indicated development of preeclampsia and the pregnancy progress, antenatal complications, mode of delivery and perinatal outlet has been observed. The group has been divided using the following criteria: preexisting disease, severity of preeclampsia, mode of delivery, gestational age and prematurity rate. The present trial is important for optimizing management of women with preexisting disease before, during pregnancy and after delivery.
A rare case of successful pregnancy and delivery of a patient with Hereditary angioedema is described. The complications of the disease during pregnancy are described as well as the possibilities for their prevention. The delivery was accomplished without complications and the preoperative preparations are listed. The treatment of HAE in general is divided into two groups: short-term (Fresh frozen plasma) and long-term prophylaxis (antifibrinolytic agents, androgen therapy) and acute treatment (epinephrine, steroids, antihistamines, eventually tracheotomy).
A clinical trial is conducted with 32 pregnant patients with a history of sterility and infertility, uterine malformations as well as progesterone deficiency. The mean week of gestation is 9-10. All patients were treated with Utrogestan in the usual doses 2 x 100 mg for vaginal application. No side effects were observed which is encouraging to its routine use.
In our investigation we wanted to evaluate the effect of HRT on the endometrial thickness and artery blood flow. For 10 years period from 1992 till 2002 we evaluated 275 patients in postmenopause. All were examined by vaginal ultrasonography. We divided the patients into 3 groups--the first group--85 patients receiving HRT (hormonal replacement therapy), the second group--146 asymptomatic women in postmenopause and the third group--40 women with pathological bleeding in postmenopause. There was no difference in the endometrial thickness between the patients receiving HRT and the other two groups of women. The endometrial pathology influenced the endometrial thickness as well as the uterine blood flow.
Candidiasis of the lower female genital tract during pregnancy is a leading cause for seeking medical help. During pregnancy candidiasis is encountered about 30% more often compared to non-pregnant (by the time of delivery about 18-22% of the pregnant women would have the infection). It is clinically manifested especially after 36 w.g. This might be the cause of a series of pregnancy complications. Most serious are considered prematurity, chorioamnionitis, descending infection from mother to the newborn during delivery. Significantly less attention has been paid to the birth canal injury and its higher rate if a Candida colpitis is present at the time of delivery.
An acidic extracellular pH is a fundamental property of the malignant phenotype. In von Hippel-Lindau (VHL)-defective tumors the cell surface transmembrane carbonic anhydrase (CA) CA9 and CA12 genes are overexpressed because of the absence of pVHL. We hypothesized that these enzymes might be involved in maintaining the extracellular acidic pH in tumors, thereby providing a conducive environment for tumor growth and spread. Using Northern blot analysis and immunostaining with specific antibodies we analyzed the expression of CA9 and CA12 genes and their products in a large sample of cancer cell lines, fresh and archival tumor specimens, and normal human tissues. Expression was also analyzed in cultured cells under hypoxic conditions. Expression of CA IX and CA XII in normal adult tissues was detected only in highly specialized cells and for most tissues their expression did not overlap. Analysis of RNA samples isolated from 87 cancer cell lines and 18 tumors revealed high-to-moderate levels of expression of CA9 and CA12 in multiple cancers. Immunohistochemistry revealed high-to-moderate expression of these enzymes in various normal tissues and multiple common epithelial tumor types. The immunostaining was seen predominantly on the cell surface membrane. The expression of both genes was markedly induced under hypoxic conditions in tumors and cultured tumor cells. We conclude that the cell surface trans-membrane carbonic anhydrases CA IX and CA XII are overexpressed in many tumors suggesting that this is a common feature of cancer cells that may be required for tumor progression. These enzymes may contribute to the tumor microenvironment by maintaining extracellular acidic pH and helping cancer cells grow and metastasize. Our studies show an important causal link between hypoxia, extracellular acidification, and induction or enhanced expression of these enzymes in human tumors.
UNLABELLED: The aim of the study is to determine the effectiveness of the single per oral application of 200 mg Fungolon (Fluconazolum) in adolescent girls with proven Candida spp. Vaginal infection. MATERIAL AND METHODS: A prospective, case control study in 50 girls, aged 9 to 18 years, was performed. Patients were recruited in the outpatient Clinic of Paediatric and Adolescent Gynecology at the University Hospital "Majchin dom", Sofia, Bulgaria. They suffered from vaginal discharge and/or pruritus of the external genitalia. Candida spp. Was proven in all of the microbiological investigation performed. The treatment with Fungolon was performed per os, in single dose of 200 mg. RESULTS: The complaints and Candida in the vaginal smear (controlled 1 week after the treatment) disappeared in 43 of 50 patients (86%) treated. No effect was observed in 7 girls (14%). A reinfection with Candida was observed in 6 of the 38 patients (16%) at the second week after the treatment. The typisation of the resistant to Fungolon Candida shown predominance of Candida glabrata. There were no side effects registered during the Fungolon treatment. CONCLUSION: The treatment of Candida spp. Vaginal infection in adolescent girls by a single per oral application of 200 mg Fungolon (Fluconazolum) is very convenient and efficient. It gives a prompt effect (the complaints disappear in one or two days), high percentage of healed patients (86%), with absence of side effects. The usage of Fungolon should be highly recommended for treatment of vaginal mycoses in puberty and adolescence.

Object of the study are women with a history of unexplained recurrent embryo, fetal and early neonatal death, severe preeclampsia, fetal growth retardation, abruptio placentae, puerperal thromboses. Quite often placental insufficiency is linked to abnormal vascular system and hemostatic disturbancies. In about 65% of the women with a complicated and in 18% of the women with a normal pregnancy are observed different genetic anomalies that lead to a hypercoagulative state. A major place is taken by the Leiden mutation of hemostasis factor V, by protein C and protein S deficiency, etc. Another disease that leads to arterial and venous thromboses and is most often linked to recurrent miscarriage is the antiphospholipid syndrome. Many authors confirm the findings of large placental infarctions and thromboses in women who are positive for antophospholipid antibodies.
A case of a multiple pregnancy in each of the horns of a double uterus with a double cervix and a vaginal septum is discussed. The uterine malformation was known prior to pregnancy. In 34 g. w. a Ceasarean section was performed because of premature rupture of membranes. Two premature infants were born with no evidence of present pathology.
A case of pregnancy, malformation of the foetus--Spina bifida, and hydatidosis hepatis is discussed. The patient is 19-year old, primigravida, pregnant in the 31st gestational week who was admitted to the ward with an observation of Hydrocephalia and fetal compromise (low beat-to-beat variability and lack of accelerations of the fetal heart rate). Un ultrasound scan of the foetus proved hydrocephalia and Spina bifida in the lumbal segment of the fetal spine. Un extended ultrasound scan proved a hydatid cyst of the liver with a 10 mm diameter, corresponding with the data of the serological tests--RAHA 1:400.
Many oncogenes and tumour suppressor genes are connected with the steps of carcinogenesis. Fifteen patients with endometrial cancer were immunohistochemically examined for the presence of abnormal p53 protein. The immunopositivity was comparted in the cancer tissues and analyzed with the conventional clinicopathological prognostic factors as grade, stage, depth of myometrial invasion, lymph-vascular space invasion, metastases in the lymph nodes and metastases in the abdominal cavity. Abnormal expression of p53 was observed in 7 women with endometrial cancer and in 5 patients we observed moderate cytoplasmatic reaction for BCL-2. The correlation between the positive reaction reaction for p53 and BCL-2 means more aggressive tumor, respectively is a bad prognostic factor. The overexpression of p53 is a very important parameter for the progression of the endometrial cancer.
The quality of the preoperative diagnostic procedures D&C, hysteroscopy, transvaginal sonography, and computer tomography were evaluated. The results of these diagnostic methods were compared with the postoperative pathological results. The computer tomography was with limited abilities for determination of cervical canal involvement. The high negative predictive value of the rest of the methods gives us the ability to exclude cervical canal involvement with great accuracy and probability. But none of these methods could not on firm the presence of cervical canal infiltration, because of low percentage of positive predictive value, which was between 7.8% in hysteroscopy to 50% in transvaginal sonography and till 60% in MRI imaging. Between all these methods for diagnosis of cervical canal involvement--the transvaginal sonography was relatively efficient/78.2%/, specific in/79%/ and sensitive in /76%/ and relatively cheap. Because of this reason this method of transvaginal sonography is preferable in the evaluation of possible cervical infiltration.
We tried to evaluate the treatment of borderline ovarian tumours for the period of 1987 till 1999. We analyzed 62 cases as the age varied from 16 till 82 years of age, median 51.2. To most of them was performed surgical treatment, and to some of them immediately second look operation. In 38% of the women under 35 years of age there were, recurrences, as for the all group of border line tumours it was 19% recurrences. The recurrences appeared lately usually after the fifth year. We examined the time of operation, the stage, the histological type, the number of pregnancies, number of recurrences, and the disease free interval. The results showed that the conservative surgery in young women leads to high niveau of pregnancies, and also high level of recurrences. The young patients with unilateral disease stage IA were treated with conservative surgery, with intraoperative staging, without adjuvant chemotherapy. The menopausal women were treated as malignant tumours/Total hysterectomy bilateral salpingo-oophorectomy, omentectomy and appendectomy/ We show the need of active follow up of the treated women especially the young ones who are treated conservatively.
