[Clinical significance of electrocardiographic P-terminal force in V1 in patients with hypertrophic cardiomyopathy].

The clinical usefulness of P-terminal force in lead V1 (P-TF) was evaluated in 49 patients with hypertrophic cardiomyopathy. These 49 patients were categorized as 17 with an abnormal P-wave (P-TF greater than or equal to 3.0 msec.mV: group A) and 32 with a normal P-wave (P-TF less than 3.0 msec.mV: group B). Systolic blood pressure response (delta SBP) during the treadmill exercise test was significantly less in the group A than in the group B (delta SBP: 36 +/- 22 mmHg, 58 +/- 26 mmHg, respectively). Left atrial dimension measured from the M-mode echocardiogram was 37.8 +/- 5.0 mm in the group A and 31.0 +/- 5.2 mm in the group B. These differences were statistically significant (p less than 0.05). Left ventricular end-diastolic pressure averaged 19.5 +/- 7.9 mmHg in the group A and 13.5 +/- 4.0 mmHg in the group B (p less than 0.05). When comparing the group A and B, negative dp/dt / positive dp/dt of the group A was significantly less (0.69 +/- 0.14, 0.90 +/- 0.18, respectively) and the time constant T of the group A was significantly prolonged (86 +/- 30 msec, 56 +/- 10 msec, respectively). According to classifications of hypertrophic cardiomyopathy, all patients with the obstructive type had an abnormal P-wave. [Intracranial neurinomas of the jugular foramen and hypoglossal canal].

Intracranial neurinomas of the jugular foramen and the hypoglossal canal are relatively rare. Most reports have been of either single cases or of only two or three patients, and few large series exist in the literature. Although some of these tumors present palsies of the ninth, tenth, and eleventh cranial nerves as a jugular foramen syndrome, this is not always present. Unexpectedly, symptoms involving the eighth nerve are more frequently than those of ninth, tenth and eleventh nerves. This variability of clinical symptoms frequently leads to an initial misdiagnosis of glomus jugulare, acoustic or hypoglossal neurinoma. We report 2 cases of jugular foramen neurinomas and 2 cases of hypoglossal neurinomas. Our review of literature has tabulated 92 jugular foramen neurinomas and 34 hypoglossal neurinomas. These neurinomas had become considerably large in size by the time they were detected. So, we should endeavor, by clinical or neuroradiological finding to detect them at the early stage when they exert less influence on the brain. In this series we discuss clinical pictures of these neurinomas.

Degenerative spondylolisthesis. Pathophysiology and results of anterior interbody fusion.

With special attention to the articular facets, an inspection of dry human spinal columns, microradiographic studies of cadaveric specimens, and animal experiments revealed that development of degenerative spondylolisthesis is attributable to rotational strain on the facetal joints at the level of involvement. Disc degeneration predisposes to intersegmental instability and rotational strain, which result in secondary osteoarthritic change of the articular processes and segmental canal stenosis. Thirty-six patients with degenerative spondylolisthesis were treated with anterior interbody fusion (AIF) for segmental canal stenosis at the authors' hospital during 1958-1985. The surgical results of these patients reveal that AIF corrects malalignment of the lumbar spine by complete discectomy, reduces the slip and restores the disc height, and resolves nerve compression, both from the front and from behind, by enlargement of the stenosing canal. In addition, AIF has consistent and satisfactory clinical results at long-term follow-up evaluation because it resolves intersegmental instability, an important problem of degenerative spondylolisthesis. AIF is a reasonable and reliable treatment for patients younger than 60 years of age with segmental stenosis.

Fracture of the posterior margin of a lumbar vertebral body.

Using computed tomography, we studied twenty-nine patients who had thirty-one fractures of the posterior margin of a vertebra in the lumbar spine. Based on our findings, these fractures were classified morphologically into three types: a simple separation of the entire margin; an avulsion fracture of some of the substance of the vertebral body, including the margin; and a more localized fracture. In this series, twenty-two of the thirty-one fractures were associated with a defect involving the affected end-plate. This fact suggests that the pathogenesis of these fractures may be the fragility of the end-plate.

Electrically stimulated periosteal grafting in the rat.

Osteogenesis of the periosteum was examined in rats. The periosteum electrically stimulated for 6 days was grafted into the axillar muscle, and new bone tissue was found in 40 out of 44 animals (90.9%). Periosteum without stimulation and that with only 2 days of electrical stimulation did not generate new bone tissue. The periosteum electrically stimulated for 4 days did not show substantial new bone formation in the muscle. The periosteal tissue which was initially electrically stimulated for 6 days was definitely transformed into new bone tissue in the muscle. This shows that boneless bone grafting can be accomplished successfully in rats by electrically stimulating the periosteum before grafting. Periosteal grafting is considered to be potentially valuable as a boneless bone grafting technique.

Hydroxyl radical production and human DNA damage induced by ferric nitrilotriacetate and hydrogen peroxide.

Reactivities of Fe3+ chelates of aminopolycarboxylic acids with DNA were investigated by the DNA-sequencing technique using 32P 5'-end-labeled DNA fragments obtained from the human c-Ha-ras-1 protooncogene, and the reaction mechanism was studied by electron spin resonance spectroscopy. Ferric nitrilotriacetate (Fe3+-NTA) plus hydrogen peroxide caused strong DNA cleavage in the presence of albumin. No or little DNA cleavage was observed with ferric chloride or Fe3+ chelates of other aminopolycarboxylic acids tested in the presence of hydrogen peroxide. The DNA cleavage by Fe3+-NTA plus hydrogen peroxide without piperidine treatment occurred at positions of every nucleotide although a specific cleavage was observed, whereas cleavages at the positions of guanine and thymine increased predominantly with piperidine treatment. Electron spin resonance studies using free radical traps demonstrated that of Fe3+ chelates of aminopolycarboxylic acids, Fe3+-NTA was the most effective catalyst in hydrogen peroxide-derived production of hydroxyl radicals under our conditions. The results suggest that Fe3+-NTA catalyzes the decomposition of hydrogen peroxide to produce hydroxyl radicals, which subsequently cause the strong base alterations of guanine and thymine, and deoxyribose-phosphate backbone breakages. The possibility that the Fe3+-NTA-induced DNA damage is the initiation and/or promotion of carcinogenesis by Fe3+-NTA is discussed.
