Parents' attitudes to developmental screening in the first year of life.

It has been suggested that screening for impairment in early childhood may cause anxiety to parents. Using self-administered questionnaires, we studied the attitudes and concerns of parents of infants aged 6 months and 8-9 months. Parents were randomly allocated to one of two groups. One group completed the second questionnaire after the 8-month assessment routinely performed by health visitors, and the other group completed the second questionnaire before the assessment. We showed changes in attitude and concerns over the 2-3 month period, but these were not related to the assessment. It appeared that particular concerns and anxieties were not resolved by a recent contact with a health visitor, although a high proportion of mothers stated that they found the assessment reassuring. There was some evidence of a lack of appreciation, both of the purpose of the tests and of the implication of test failure. Screening tests performed by health visitors at an age of 8 months do not appear to generate undue anxiety in parents. However, as many of the tests used are of doubtful validity, a review of the purpose and content of this early health visitor assessment is needed.

Development of the cardiac surgeon assistant.

In 1989, a pilot scheme to evaluate the role of a non-medically qualified assistant working in cardiac surgery was commenced. The appointee was a registered nurse and, 3 years on, the scheme is now in its final stages of evaluation. Approval has been given for other hospitals to adopt such schemes. This article examines the implications that this decision might have for the nursing profession in the future.

Nutrition and the surgical patient.

The importance of surgical patients' nutritional status has been clearly established, yet inadequate dietary provision continues to be common in surgical units. Susan Holmes describes the metabolic response to surgery and offers guidance on how the nutritional needs of these vulnerable patients can be met.

Barriers to effective nutritional care for older adults.

This article reviews the relationship between ageing and nutrition. The vulnerability of older adults to nutritional decline is explored and the importance of nutritional screening in this population is discussed. The author considers some of the barriers to achieving nutritional adequacy once patients have been admitted to hospital and suggests how these can be overcome.

Nutritional screening and older adults.

Nutrition is rarely included in the screening programmes of older people. However, nutritional screening of this population is important to distinguish between deterioration in their health status due to the physical decline of ageing and that due to nutritional deficiency.

Effectiveness of a specially trained pharmacist in a rural community mental health center.

Pharmacists having training in psychopharmacology and psychiatry are being specially trained to function in community mental health centers as a resource in psychopharmacology and to provide direct care to patients. Management information data over a 3-year period from eight rural mental health clinics indicate that these pharmacists can successfully maintain large numbers of stabilized psychiatric patients within their communities. The cost of a pharmacist's services is, conservatively, one-half that of a psychiatrist's services. A followup study of the stabilized and active aftercare outpatients in the 10-county rural area in which the 8 clinics are located revealed that those patients who received all their direct care from the pharmacist were functioning at a slightly healthier level than the other aftercare patients. The pharmacist's patients indicated that they were at least as satisfied with their care as were the aftercare patients who received care from other mental health professionals. If the results of this study can be generalized to other community mental health centers in rural areas, a pharmacist can provide services effectively when psychiatrists are inaccessible or unavailable or when funds for mental health professionals are limited.

Color drawings: a means of identifying problems in children.

Between the ages of seven and nine years, the child develops a schema or way to draw his version of the world. Drawings are revealing in that they can help assess a child's behavior. Figure drawings are useful in determining general cognitive development, self-image, and level of maturity (Harris, 1963; DiLeo, 1973; DiLeo, 1983; and Koppitz, 1968). As the child progresses through developmental stages in perceiving and using color, she similarly progresses in her drawing abilities. Distinction, identification and aesthetic reaction to color are thus functions of the cortex; they are developmental and educational results rather than instinctual and reactive responses (Scott, 1969).
