[Primary intracranial malignant melanoma associated with nevus of Ota: a case report].

A case of intracranial malignant melanoma associated with nevus of Ota is presented. A 77-year-old man was admitted to the department of neurosurgery, Fukui Medical School on June, 1985 because of developing disturbed consciousness. The physical examination on admission revealed pigmented lesion diagnostic of the nevus of Ota, anisocoria (R greater than L) and left hemiparesis. Computed tomographic (CT) scans demonstrated a high density mass in the left temporal lobe and the mass was homogenously enhanced after the injection of contrast agent. Left common carotid angiogram showed elevation of the middle cerebral artery and a slight vascular blush. Subtotal removal of the dark-colored tumor within the left temporal lobe and biopsy of the nevus were performed. Histopathological examination of the tumor revealed malignant melanoma and that of the skin biopsy was consistent with a nevus of Ota. Postoperatively DTIC, ACNU and vincristine were administered intravenously but the patient died three weeks after operation. At autopsy, widespread black pigmentation of the leptomeninges, communicating with the pigmented tumor in the left temporal lobe, covered the base of the brain and spinal cord. No melanoma was found outside the central nervous system. Primary intracranial melanoma associated with nevus of Ota is rare and only 6 cases have been reported in the literature. In this paper, primary intracranial melanoma associated with nevus of Ota and some other subjects are discussed.

Prognosis of intraventricular hemorrhage due to hypertensive hemorrhagic cerebrovascular disease.

The prognosis of intraventricular hemorrhage (IVH) due to hemorrhagic cerebrovascular disease has been regarded as unfavorable. The authors studied factors affecting the prognosis in 55 patients with IVH due to hypertensive thalamic and putaminal hemorrhages. The mortality rate in this study was 38%, a poorer rate than that in patients without IVH (18%). As for morbidity, 50% of the survivors (31% of all patients) with IVH had nor or moderate disability, but the other 50% had severe disability or were in a persistent vegetative state. On the other hand, there was no or moderate disability in 55%, and severe disability or a persistent vegetative state in 27% of the patients without IVH. Patients with intracerebral or intraventricular hematoma volumes greater than 25 ml, Glasgow Coma Scales of less than nine, intracranial pressures above 30 mm Hg, and ventriculocranial ratios over 0.22 had poor prognoses. These results suggest that greater volumes of intracerebral or intraventricular hematoma and the presence of acute hydrocephalus are of great significance when considering the outcomes of IVH cases.

[Extension and reduction of surgery of cancer occupying mainly the upper third part of the stomach--indications and limitations].

To improve the therapeutic result of patients with advanced carcinoma of the upper third of the stomach, it is necessary to extend further the range of resection and dissection especially in case of invading the lower esophagus with serosal involvement. The extended surgery consists of the following: abdominophrenicotomy or abdominothoractomy, lower esophagectomy and total gastrectomy, dissecting completely the second group of nodes combined with distal pancreatectomy and splenectomy, a block resection of adjacent organs in case of direct invasion, and even dissection of nodes in the lower mediastinum, within the hepatoduodenal ligament, behind the head of the pancreas and along the superior mesenteric vein. Operative procedures are, however, to be palliative in case of moderate or severe peritoneal or hepatic involvements or distant metastasis to the fourth group of nodes. Reduced procedures in case of no serosal invasion nor lymph node metastasis consist of 1/2 proximal gastrectomy and of dissection of the first group of nodes especially in superficial carcinoma. However, in cases free from serosal invasion but with metastasis to the first group of nodes or in cases with a slight serosal invasion, total gastrectomy with dissection up to the second group of nodes is recommended.

[Ventricular septal dissection: a case report].

A rare case of dissection in the posteroinferior interventricular septum complicating acute myocardial infarction was described. A 79-year-old man was admitted to our hospital for treatment of a bleeding gastric ulcer and an acute inferior myocardial infarction. His hospital course was uneventful. Two-dimensional echocardiography performed before his discharge revealed an echo-free space representing an aneurysmal bulge into the posteroinferior interventricular septum, probably due to its dissection. Dynamic computed tomography of the chest demonstrated a cyst-like filling of the contrast medium within the posteroinferior interventricular septum. It communicated with the left ventricular cavity, but not with the right ventricular cavity. Left ventriculography demonstrated an aneurysmal bulge at the posteroinferior portion of the interventricular septum. Through a narrow orifice, there was a jet-like expulsion of radiopaque material from the left ventricular cavity, but there was no communication with the right ventricular cavity. This is the first reported case of ventricular septal dissection uncomplicated by ventricular septal perforation.

Polyamine lowered the hepatic lipid peroxide level in rats.

This is the first report for the hepatic lipid peroxide lowering effect of spermine in vivo. The influence of administration of polyamines on hepatic lipid peroxide level has been investigated by using normal or carbon tetrachloride (CCl4)-treated rats. Spermine was found to lower the hepatic lipid peroxide level most efficiently among polyamines used in CCl4-treated rats. In addition, the extent of liver enlargement caused by CCl4 treatment was reduced by spermine administration. Lipid peroxide lowering effect of spermine was also observed in normal rats. Hepatic spermine content was significantly increased in both normal and CCl4-treated rats after administration of spermine. Clear inverse relationship between the content of lipid peroxide and the concentration of spermine was observed. In reconstituted system containing NADPH-cytochrome P-450 reductase and extracted hepatic microsomal lipid, spermine inhibited the NADPH-dependent lipid peroxidation effectively at the concentration of 0.1 mM. From these results, we concluded that spermine exerted an inhibitory effect of lipid peroxidation in vivo as well as in vitro.

Studies on murine IgE with monoclonal antibodies. I. Characterization of rat monoclonal anti-IgE antibodies and the use of these antibodies for determinations of serum IgE levels and for anaphylactic reactions.

Rat monoclonal antibodies were constructed by fusion of immunized rat spleen cells with a nonsecreting mouse myeloma cell. Two monoclonal antibodies (6HD5 and HMK-12) were selected for further study. Both reacted with various IgE molecules of different specificities and different allotypes, but did not react with immunoglobulins of other isotypes and with light chains. These antibodies were therefore anti-isotypic (IgE) and not anti-allotypic or anti-idiotypic. It was shown by competition studies that these antibodies recognize different epitopes on the FcR epsilon fragment. A sensitive ELISA for the quantitation of murine IgE was developed with these monoclonal antibodies; the sensitivity was between 2 and 250 ng/ml for detection of serum IgE levels. Good correlation was obtained with protein amounts as determined by enzyme-linked immunosorbent assay (ELISA) and passive cutaneous anaphylaxis (PCA) activities. Both monoclonal antibodies were used to study anaphylactic reactions elicited by IgE antibodies. Both could inhibit PCA reactions and both could elicit reverse PCA reactions.

Association of HLA-DR2/DR4 heterozygosity with systemic lupus erythematosus in Japanese patients.

The frequency of HLA-DR2/DR4 heterozygotes was significantly higher in 85 Japanese patients with systemic lupus erythematosus (SLE), compared with healthy controls. There was no difference in frequency of the heterozygotes of HLA-DR2/non-DR4. The association with HLA-DR2/DR4 heterozygosity was highly significant in patients with SLE in whom the onset occurred under age 30 and there was no significant association in patients with SLE in whom the onset occurred at age over 30 years. The possible role of HLA-DR2/DR4 heterozygosity in the development of SLE is discussed.

Tumoricidal activity of lymphotoxin (tumor necrosis factor beta) in vivo: its effects on macrophages.

We investigated the cytotoxic activity of recombinant human lymphotoxin (rHuLT) against Meth A sarcoma in vitro and in vivo. In direct cytotoxic assay, Meth A cells showed very low, if any, sensitivity to rHuLT. However, rHuLT was found to be strongly cytotoxic for subcutaneously implanted Meth A cells. In order to explain this discrepancy, we examined the indirect effects of rHuLT on Meth A cells particularly through macrophages. rHuLT showed macrophage chemotactic activity in vitro and also in vivo. Thus, injection of rHuLT into the mouse peritoneal cavity caused migration of macrophages to the peritoneal cavity. In addition, histologic analysis revealed that 6 h after intratumor injection of rHuLT, nonspecific esterase-positive cells started to migrate into Meth A tumor tissues, and after a further 24 h, a larger number of nonspecific esterase-positive cells had migrated into the Meth A tumor tissues. Moreover, peritoneal exudate cells induced by rHuLT showed considerably higher cytotoxic activity against Meth A cells than proteose peptone- or phosphate-buffered saline-induced peritoneal exudate cells. Furthermore, the cytotoxic activity of peritoneal exudate cells was found to reside in macrophages. Investigation of the mechanism of macrophage activation by rHuLT revealed that rHuLT can activate macrophages for cytotoxicity synergistically with mouse interferon-gamma. These results strongly suggest that rHuLT, when injected into the tumor, can induce the migration of macrophages into the tumor and activate macrophages in situ to kill tumor cells.
