[A case of accessory cardiac bronchus with acute empyema treated by open drainage].

A 40-year-old man was admitted to the hospital because of acute empyema localized in the right lower posterior pleural space. The conservative treatment with antibiotics for one month failed to show any clinical improvement. Surgical intervention, therefore, was required. Broncho-fiberscopic and bronchographic findings prior to open drainage revealed a supernumerary bronchus with inflammatory sign on the right intermediate bronchus and a small sprig like bronchial fistula attached to the tip of the abnormal bronchus. The operation induced the swift alleviation of empyema and the disappearance of bronchial inflammation. From the location of empyema and the disappearance of bronchial inflammation associated with the improvement of clinical findings, the infection of the abnormal bronchus was thought as a main cause of empyema. Empyema caused by this kind of abnormality is extremely rare and no similar case has been reported in the literature to our knowledge. Some pathological problems of the abnormal bronchus were discussed and 54 cases of accessory cardiac bronchus reported in Japan were reviewed.

[Post pneumonectomy empyema with bronchopleural fistula--a successful management using pedicled intercostal muscle flap on an atypical mycobacteriosis following the irradiation].

A 48-year-old woman underwent a right pneumonectomy for advanced mycobacterial disease (M. avium Complex), which followed the postoperative radiotherapy against a malignant schwannoma of the right lower chest wall treated seven years ago. On the 13th postoperative day, re-suture of the bronchial stump was performed urgently because of early bronchopleural fistula development. On the heels of that, reclosure of the bronchial fistula with coverage of the stump by parietal pleural flap was performed on the forty-first post operative day. On the 110th day, however, open drainage with thoracoplasty was performed because development of insidious aspergillous empyema was detected. Since then, local instillation of amphotellisin B, with an oral administration of antifungus drug was started. After succeeding to control the mycotic infection, reclosure of the bronchofistula, covered with pedicled intercostal muscle flap were performed on the 280th postoperative day and extraperiostal air-plombage for reducing empyema cavity. Postoperative course was uneventful and the patient was discharged one year later. With respect to pathogenetic relationship between radiation pneumonitis and feasibility of infection to atypical mycobacteria, preoperative radiotherapy and concurrence of postoperative bronchofistula, and some problems on management of empyema bronchofistula were briefly discussed.

[Studies on HLA typing in a family line with spontaneous pneumothorax].

Our patient with spontaneous pneumothorax is the oldest brother of 3 siblings. Pneumothorax had occurred in his younger sister and in 3 of his mother's 4 siblings also. Namely, pneumothorax had occurred in 5 (including our patient) of 7 persons, family line on mother's side, and thoracotomy had been performed in 3 of them. Two cases in which operative findings were obtained were introduced. Next, HLA and alpha 1-antitrypsin (alpha 1-AT) were determined in this family line. As the result, the alpha 1-AT all remained in the normal range and HLA typing was not in a special relation to the occurrence of pneumothorax in this family line.

The tumour-localizing properties of porphyrin derivatives.

The tumour-localizing abilities of various kinds of porphyrin derivatives in tumour-bearing hamsters were assessed by nitrogen-pulsed laser spectrofluorometry (N2-PLS). On examination of porphine derivatives (from haemoglobin), it was found that the dimer and acetylated and amidated compounds had a high affinity for tumour tissue; the dimer and hydroxylated compound of phorbine derivatives (from chlorophyll) also showed a high affinity. Furthermore, of the metalloporphines (gallium, zinc and indium complexes), those which contained hydrophilic groups showed a high affinity for tumour tissue; of the metallophorbines (gallium, zinc and indium complexes), those which contained hydrophobic groups showed a high affinity. A correlation was found between the side-chain structure of the porphyrins and metalloporphyrins and their affinity for tumour tissue.

[A case of advanced thoracic esophageal cancer with marked response to bleomycin absorbed on activated carbon particles (BLM-CH44), methotrexate, and cisplatin].

A 67-year-old women, who suffered from advanced esophageal cancer with left main bronchus and azygous vein involvement, was treated by combined administration of bleomycin absorbed on activated carbon particles (BLM-CH44, 10 mg/body), methotrexate (30 mg/body i.m.), and cisplatin (75 mg/body i.v.). BLM-CH44 was injected around tumor and total dose of BLM was 120 mg, that of MTX was 180 mg, and that of CDDP was 225 mg. After therapy, the sign of extra-esophageal spread disappeared and a radical operation was performed. Microscopically, the lesion of the esophagus was almost disappeared, and CH44 staining lymph node involvement was completely disappeared.

[The effect of endogenous tumor necrosis factor (TNF)-induction therapy in the model of VX-2 ovarian carcinoma in the rabbit].

The rabbit ovarian carcinoma model was prepared by injecting VX-2 carcinoma into the ovary and the effects of endogenous Tumor Necrosis Factor (TNF)-induction therapy in combination with chemotherapy were studied. In this model, carcinoma metastasizes easily to the abdominal cavity, lung and liver, and resists treatment. Endogenous TNF was induced by a series of injections of OK-432 0.3KE and OK-432 3KE at 3 hour intervals (OK-OK group), or 1 x 10(10) cells of Bordetella pertussis vaccine (BPV group). Another group were sequential given a series of injections of CDDP (2mg/kg) intraperitoneally and OK-432 intravenously (an OK-OK and CDDP group). Higher activity of endogenous TNF was induced by BPV in comparison with OK-OK plus OK-OK and CDDP. Obvious hemorrhagic necrosis was observed in the implanted ovary in the BPV group. Compared with the control group, pulmonary metastasis was seen in the OK-OK and OK-OK plus CDDP groups (partial remission). These results show that the endogenous TNF induction therapy could be expected to be an effective antitumor therapy for advanced ovarian cancer.

Inhibition of in vitro vascular endothelial cell proliferation and in vivo neovascularization by low-dose methotrexate.

Neovascularization in the rheumatoid synovium plays an important role in the propagation of rheumatoid synovitis because the emigration of mononuclear cells and the growth of pannus are critically dependent on the development of small blood vessels. Inhibition of local vascular endothelial cell (EC) proliferation, which is essential for growth of these vessels, therefore, would have the potential to suppress rheumatoid inflammation. We investigated the effects of methotrexate (MTX), low doses of which are commonly administered to rheumatoid arthritis patients, on DNA synthesis by human umbilical vein EC in vitro and on rabbit corneal neovascularization in vivo. MTX inhibited both basal and EC growth factor-stimulated tritiated deoxyuridine (3H-UdR) incorporation into EC in a dose-dependent manner. Significant inhibition was observed at a concentration of 5 x 10(-9) M, which is that attained in the serum of treated patients. Neovascularization in vivo was also suppressed by low-dose intramuscular injections. These results suggest that MTX has an antiangiogenic effect, and may suppress rheumatoid inflammation through the reduction of synovial small blood vessels responsible for mononuclear cell infiltration and proliferation of synovial tissue.

Radioisotopic splenoportography in patients with portal hypertension.

Radio-isotopic splenoportography was performed by injecting 99mTcO4- into the spleens of 46 patients with portal hypertension and 14 patients with various disorders not having portal hypertension. No collateral circulation was demonstrated in the 14 patients without portal hypertension whereas some RI-images of portosystemic collaterals were found in 40 (87.0 per cent) of the 46 patients with portal hypertension. Collaterals were divided into an ascending group and a descending group, the appearance rate of ascending collaterals being 80.4 per cent and that of descending collaterals, 41.3 per cent. There were 3 image patterns in the ascending group, namely, an AZ-pattern in which the azygos vein was demonstrated; a SC-pattern in which the RI-bolus ascended along the esophagus to the neck and the subclavian vein; and an EG-pattern which showed stagnation of the RI-bolus in the esophagogastric region. There were 4 patterns in the descending group, namely; a pattern of gastro-renal caval shunt (GR-pattern); reverse flow patterns into the umbilical or paraumbilical veins (UV-pattern); into the superior mesenteric vein (SMV-pattern); and into the inferior mesenteric vein (IMV-pattern). The appearance of the EG-pattern was seen most frequently (74.4 per cent). The usefulness of this method for surveying the collateral circulation in portal hypertension, estimating the risk of esophageal variceal bleeding and evaluating its treatments, was suggested by the results of this study.

Human herpesvirus-6 infection (exanthem subitum) without rash.

Two patients with a three-day febrile episode in whom exanthem subitum was expected showed no appearance of skin eruption after subsidence of the fever. Virus was isolated from peripheral blood mononuclear cells in the acute stage, which was identified by the cytopathic effects of infected cells and the specific immunofluorescent staining of the cells with convalescent sera from exanthem subitum, but the virus could not be isolated from those in the convalescent stage. The morphology of the virus was similar to herpes group virus. There were seroconversions against representative strain of the causative agent of exanthem subitum in the two patients. The results indicated the presence of atypical clinical course of exanthem subitum.

Immunohistochemical evidence for the production of tumor necrosis factor by murine MH134 tumor cells as well as monocytes in tumor lesions after systemic administration of a polyalcoholized mannoglucan from Microellobosporia grisea.

The endogenous production of tumor necrosis factor (TNF) by tumor tissues was examined. MH134 tumor cells as well as monocytes in MH134 tumor tissues produced TNF after systemic administration of a polyalcoholized mannoglucan (MGA) from Microellobrosporia grisea as shown by the indirect immunofluorescence technique. MH134 tumor cells also produced TNF when stimulated with lipopolysaccharide in vitro. These results suggest that regression of MH134 hepatoma can be ascribed to TNF produced not only by monocytes but also by tumor cells themselves in the tumor tissue.

[Tracheobronchial tuberculosis observed from the chest X-ray findings and its pathogenesis].

Twelve cases of tuberculous tracheobronchitis were identified by bronchofiberscopy out of 185 tuberculous patients, and they are classified into four groups based on the chest X-ray findings and its pathogenesis, Group 1 (two cases); extensive endobronchial tuberculosis without radiographically demonstrable lesion. Group 2 (three cases); endobronchial tuberculosis occurring in airway that drains a pulmonary cavity or active lesion. Group 3 (four cases); endobronchial tuberculosis occurring in minimal cases of tuberculosis with radiographically fibrocaseous or fibroproductive type lesions. Group 4 (three cases); invasion of bronchus by perforation of hilar tuberculous adenitis. This classification would undoubtedly be better to understand tuberculous involvement of the bronchial tree than those previously reported classifications based on the bronchoscopic findings and clinical symptoms. Furthermore, it was found that the extent of the lung lesions showed no relation with the incidence of tuberculous tracheobronchitis. There is some differences among these four groups in terms of chest roentgenographic features, clinical symptoms and bronchoscopic findings. However, the presence of the third group has not been clearly recognized, as such type of the disease believed to exist in cases with extensive pulmonary involvement. Characteristics of the third group are mild clinical symptoms, inactive radiographic appearances and insidious clinical course, and most such cases are detected by the mass survey, and the diagnosis can be made only by endoscopic examinations. The incidence of tracheobronchial tuberculosis in our clinic was 6.5%, and that of the fourth group was 1.6%. Brief discussion was made on the endoscopic classification, pathogeneses, endoscopic changes during the course of chemotherapy and management of bronchial stenosis.

[Studies on antigen associated with human squamous cell carcinoma (SCC antigen) in patients with esophageal carcinoma].

The authors studied on SCC antigen in patients with esophageal carcinoma. Serum SCC antigen was found in 9 (40.9%) of 22 patients with esophageal squamous cell carcinoma and 5 (55.5%) of 9 patients with lung squamous cell carcinoma, but was not found in other malignant diseases, such as gastric cancer, hepatoma, colon cancer, pancreas cancer and biliary try tract cancer. SCC antigen positive cases increased in association with progression of histological invasion, grade of nodal metastasis and clinical stage. However, in early esophageal carcinoma, SCC antigen was rarely positive. There was no positive case in patients with poorly differentiated squamous cell carcinoma regardless of clinical stage. Positive rate of SCC antigen increased in association with progression of clinical stage in patients with moderately and well differentiated squamous cell carcinoma. Immunoreactivity of SCC, which was investigated immunohistologically with TA-4 rabbit serum, was not found in cases with poorly differentiated squamous cell carcinoma, but was found in keratinized portion and cytoplasm of moderately and well differentiated carcinoma. From the above, SCC antigen is intimately related with keratinization of squamous cell carcinoma, and it was thought that it could be useful as a good marker for diagnosis of moderately and well differentiated squamous carcinoma of the esophagus.

[Traumatic diaphragmatic hernia].

Five cases of traumatic diaphragmatic hernia were reported and 495 cases reported in Japan were reviewed. Analysis of cases of hernia following blunt trauma in the present study provided a simple classification of fissure of the diaphragm, i.e., "tension type (indirect type)" and "impact type (direct type)". In the former type, the fissure occurs in the central tendon or its vicinity in the diaphragm as a result of distortion of the bony thorax without direct force on the diaphragm, while in the latter type, the fissure occurs in the diaphragm close to the struck bony thorax as a result of blunt force on the bony thorax to which the diaphragm is attached.

[Angiographic findings in a patient with spontaneous hemopneumothorax].

We experienced one patient with spontaneous hemopneumothorax in whom angiography was performed preoperatively. The patient was a 23-year-old female and her plain chest X-ray revealed an index-finger-tip-sized bulla at the left apex and a narrow restiform shadow connecting the pleural cupola with the bulla. Left subclavian arteriography revealed preoperatively that the restiform shadow consisted of aberrant vessels branched from the left costocervical trunk and distributed in and around the bulla at the apex. In the present study very rare angiographic findings and excised specimens obtained from the patient are shown, with reference to surgical indication of spontaneous pneumothorax.

[Chondrosarcoma of the ribs: report of two cases].

Chondrosarcoma of rib origin is relatively rare in Japan. Two patients with chondrosarcoma of rib origin were treated surgically. Case 1: A 79-year-old male, 4 years ago, he noticed a tumor in the left anteroinferior part of the chest. The histological examination of the specimen obtained by incisional biopsy showed osteochondroma in another hospital and he had been under observation. Since the tumor has recently increased in size, he was referred to our department. Radical resection of the chest wall was performed. As expected, the tumor was found to be a secondary chondrosarcoma by postoperative histological examination. Case 2: A 49-year-old female. In the follow up studies after radical nephrectomy, Tc-MDP scintigram revealed an accumulation in the left second rib. Under the diagnosis of rib metastasis, radical resection was performed. The histological examination showed chondrosarcoma of rib origin postoperatively. This lesion was a primary central type, and is considered to have been resected in the earliest stage of all cases we have found in the Japanese literature.

Whole-body autoradiography of tumor-bearing hamsters with a new tumor imaging agent, indium-111-labeled porphyrin.

We have synthesized a new tumor imaging agent, 111In-labeled porphyrin (111In-ATN-2). In order to image transplantable pancreatic carcinoma in Syrian golden hamsters, we investigated the biodistribution of 111In-ATN-2 72 hr after injecting the agent by means of whole-body autoradiography. The efficacy of the agent was compared with that of 67Ga citrate. The images with 111In-ATN-2 were found to be clearer than those with 67Ga citrate. Tumor-to-tissue radiodistribution ratios of the former were higher than those of the latter. Thus, 111In-ATN-2 seems to be more useful for tumor diagnosis.
