[Multicenter study of the effects of sulbactam/cefoperazone on bacterial infections in the fields of internal medicine focused mainly on respiratory infections in Tochigi Prefecture].

Clinical effects of sulbactam/cefoperazone (SBT/CPZ) was studied on variety of bacterial infections in the fields of internal medicine focused mainly on respiratory infections. The total 135 infections were consisted of 103 respiratory infections, 15 urinary tract infections, 4 sepsis, 7 biliary tract infections, and 6 other infections, of which 86 patients had underlying diseases. The daily doses of SBT/CPZ were 2 to 6 g divided into 2 to 3 times i.v. or d.i.v., and the duration of administration was from 3 to 35 days. The clinical effects were judged by the attending doctors based on the changes in fever, cough, rales, chest rentogenograms, white blood cell counts, CRP values, ESR, etc. The total efficacy rate was 76.9%, and 69.0% of the isolated organism was eradicated by SBT/CPZ. The side effect was noted in 1 case, and the abnormal laboratory findings were noted in 1 case, however it was difficult to determine whether they were due to SBT/CPZ. These results suggest that the clinical usefulness of SBT/CPZ for the infections in the fields of internal medicine.

[A case of minimal size small cell carcinoma arising in abnormal branching bronchus with mycobacteriosis].

A 73-year-old male was referred to our hospital because of mycobacteriosis. Bronchoscopic examination revealed a depressed lesion with white pseudomembrane located between the orifice of B2 and B3 of the right upper lobe. From this pseudomembrane, Mycobacterium avium complex was cultured, and the patient was treated with antituberculous drugs. Four months later, a small white polypoid tumor was found at the same site, growing gradually. Biopsy specimens showed invasion of small cell carcinoma, so we started combination therapy with anti-cancer drugs and irradiation. With this therapy, the small polypoid tumor disappeared and we found the orifice of an abnormal branching bronchus between the orifice of B2 and B3. A case of minimal size small cell carcinoma arising at the orifice of an abnormal branching bronchus with mycobacteriosis was reported and case reports in Japan were reviewed.

[A case of double-valve replacement in a patient with idiopathic thrombocytopenic purpura].

The aortic and mitral valve replacements were successfully performed in a case with idiopathic thrombocytopenic purpura. High-dose-gamma-globulin therapy and splenectomy had been tried. Neither of them, however could increase thrombocyte. After administration of Danazol treatment, thrombocytes moderately increased. With no expectation for further increase in thrombocytes, it was decided to carry out the open heart surgery with the aid of transfusion of thrombocyte-rich fresh plasma. Haemostasis after cardiopulmonary bypass required much longer time as 3 hours than in usual cases, but being successfully controlled. It has been reported that the effectiveness rate of the high-dose-gamma-globulin therapy ranges from 80 to 90%. Therefore this strategy will be the first choice in a case with ITP requiring open heart surgery. In the event that the strategies, including splenectomy, failed, other means such as Danazol therapy and transfusion of thrombocyte-rich fresh plasma should be considered.

[Resection of posterior mediastinal cyst under thoracoscopy].

A 52-year-old woman with left posterior mediastinal cyst underwent surgical removal using thoracoscopy. Under general anesthesia by use of one lung ventilation, thoracoscopy was introduced through a trocar at the mid-axillary line of the 7th intercostal space. Grasping forceps were introduced through a incision in the anterior-axillary line of 4th and 5th intercostal space. The lung was held with grasping forceps. The lung was fixed to the region of anterior-mediastinum with the weight of forceps. Then posterior-mediastinum was able to be observed well by the operator. The thin cyst-wall was biopsied and excised as much as possible. Residual cyst-wall was burned by electric knife. A pathological diagnosis probed that the tumor was bronchogenic cyst without malignancy. Post operative pain markedly reduced. The patient rapidly recovered and was discharged on the 6th postoperative day. Thoracoscopic resection of mediastinum cyst may be a useful approach.

Significance of glutathione S-transferase-pi as a tumor marker in patients with oral cancer.

BACKGROUND: The authors analyzed the clinical usefulness of glutathione-S-transferase-pi (GST-pi) as a tumor marker in patients with oral cancer. METHODS: GST-pi levels in plasma of 61 patients with oral squamous cell carcinomas, 65 patients with benign oral diseases, and 78 healthy subjects were investigated with the sandwich enzyme-immunoassay (EIA) system. RESULTS: Elevated GST-pi levels in plasma were observed in patients with oral cancer, but patients with benign oral diseases had normal GST-pi levels. More than 70% of patients with Stage III or IV oral cancer and more than 50% of those with Stage I and II disease had elevated levels of GST-pi in plasma. Elevated levels of GST-pi in plasma were also discovered in most patients with tumor recurring after surgery before recurrence was detected clinically. GST-pi also was found to be a useful marker for evaluating the response to chemotherapy, for monitoring postoperative tumor resectability or tumor burden, and for predicting the recurrence of tumor in patients with oral cancer. CONCLUSIONS: GST-pi is considered to be a useful aid for early diagnosis, predicting tumor extent, and determining parameters of treatment efficacy and prognosis for oral cancer.

Serial measurement of urinary VMA and HVA levels from one infant: a study for neuroblastoma mass screening.

Urinary vanillylmandelic acid (VMA), homovanillic acid (HVA), and creatinine in 188 samples from one infant during the ages of 1 to 12 months were measured serially by high-performance liquid chromatography (HPLC). The mean VMA and HVA levels of the total 188 samples in this study were 11.9 +/- 1.39 and 22.9 +/- 2.92 micrograms/mg creatinine with coefficients of variation (CVs) 11.7% and 12.8%, respectively; and of 76 samples from 5 to 12 months of age, 12.3 +/- 0.84 and 23.1 +/- 2.03 micrograms/mg creatinine with CVs 6.8% and 8.8%. The CVs of VMA and HVA levels expressed in microgram per milligram of creatinine had decreased after 6 months of age (VMA, 5.0% to 8.3%, HVA, 5.3% to 8.4%). Chromatograms demonstrated the similar pattern from 1 to 12 months of age, although the infant had been fed foods typical in Japan, which included breast milk, seasonal fruits, vegetables, fish, chicken, and meat. Diurnal fluctuations of VMA and HVA levels in urine were not significant. When HPLC is used for measurement of VMA, HVA, and creatinine, it is not necessary to restrict foods that cause false-positive results in the qualitative VMA test from the infant's diet. Furthermore, random urine samples obtained throughout the 12 months could be effectively used for the measurement of these substances.

Distribution of cells containing progesterone receptor mRNA in the female rat di- and telencephalon: an in situ hybridization study.

In an attempt to examine regional synthesis of the progesterone receptor (PR) in the brain, the distribution of mRNA encoding PR was investigated in the female adult rat di- and telencephalon by in situ hybridization using T7 RNA polymerase transcripts of a 320 base pair rat PR cDNA clone. The rat PR cDNA had been partially cloned and sequenced by using the reverse transcription-polymerase chain reaction (RT-PCR) method. The primer set corresponds to a part of the progesterone binding domain of human PR cDNA. Large numbers of strong labeling were observed in the arcuate nucleus, medial preoptic nucleus, and ventrolateral part of the ventromedial nucleus which are relative to sexual behavior. Moderate labeling was found in layers II and IV of the isocortex, in the pyramidal layer of the CA1 and CA3 fields of the hippocampal formation, in the cortical nucleus of the amygdala, in the nucleus of the diagonal band, and in the anterior periventricular nucleus. Weak labeling was found in many other regions. These results were largely in agreement with the distribution of PR previously reported by ligand binding assay and autoradiographic studies. This present in situ hybridization study may provide a useful tool for the analysis of the regional regulation of PR synthesis in the rat brain.

Expression of estrogen receptor in the rat brain: detection of its mRNA using reverse transcription-polymerase chain reaction.

Reverse transcription-polymerase chain reaction (RT-PCR)-Southern blotting was employed for biochemical detection and measurement of the estrogen receptor messenger ribonucleic acid (ERmRNA) in various parts of the Wistar strain female rat brain. Total RNA extracted from the tissues was subjected to RT-PCR using the primer set which flanked the part(287bp) of the estrogen binding domain-corresponding region of the rat ERcDNA. It was confirmed that the RT-PCR product corresponded to the part of the ERcDNA by direct nucleotide sequencing of the product. Moreover, the level of ERmRNA could be determined by Southern blotting which was highly sensitive and produced quantitative results. The RT-PCR product of about 290 bp, corresponding in length to the distance between two primers, was generated from RNA of all the tissues examined. The levels of the product were as follows; anterior hypophysis greater than hypothalamus and preoptic area, amygdala much greater than cerebral cortex, cerebellum. These results indicate that ERmRNA is widely distributed in the whole brain, implying some physiological action of estrogen on target and 'non-target' brain regions.

Expression of progesterone receptor in the neonatal rat brain cortex: detection of its mRNA using reverse transcription-polymerase chain reaction.

Our previous reports have revealed the postnatal developmental pattern of progesterone receptor (PR) in the rat cerebral cortex, but little is known about PRmRNA changes in the tissue so far. In the present study, we have attempted to detect and quantify PRmRNA in the tissue by the use of the reverse transcription-polymerase chain reaction (RT-PCR)-dot blot analysis. Cerebral cortical tissues were dissected from female Wistar rats at 2 and 8 days of age. Total RNA extracted from the tissues was reverse transcribed, followed by PCR. The PCR primer set, whose sequence was derived from the human clone, flanked the part (320 bp) of the human PRcDNA which corresponded to the progesterone binding domain. The 320 bp of the RT-PCR product was generated from rat uterine RNA. The amino acid sequence deduced from the nucleotide sequence of the product had a 95.5% identity with the corresponding region in human PR, confirming that the product had originated from the rat PRmRNA. Moreover, a highly sensitive and quantitative assay for rat PRmRNA had been developed by the use of RT-PCR-dot blotting. The PRmRNA was detectable in the cerebral cortex of both 2- and 8-day-old rats by the assay. Furthermore, the level of cortical PRmRNA of the 8-day-old rats were greater than the 2-day-old rats. These results indicate that the RT-PCR assay is useful for detection and quantification of PRmRNA in the neonatal rat cerebral cortex, and that increment of the cortical PRmRNA in the 8-day-old rat is associated with a drastic change of the level of the cortical PR around day 10.

Clinical effects of long-term use of neutralized dialysate for continuous ambulatory peritoneal dialysis.

The long-term effects of neutralized dialysate used in continuous ambulatory peritoneal dialysis (CAPD) were evaluated in 8 well-controlled patients. Twelve milliliters of 8.4% sodium bicarbonate was added to Dianeal PD-1 immediately before every administration. The final pH was 6.8 and the concentration of sodium bicarbonate was 6 mmol/l. The final sodium level was 138 mEq/l. This dialysate was used for 5 months. For 2 months before and 3 months after this period, Dianeal PD-2 was used as the dialysate for comparison. Blood bicarbonate levels significantly improved during the use of the neutralized dialysate. Blood sodium, chloride and magnesium levels and the effluent volume significantly increased. Sodium balance improved during the period when neutralized dialysate was used. Total leukocyte counts in the effluent decreased, and leukocyte viability increased. Abdominal distention, abdominal pain during instillation, nausea and headache improved. No side effects, including peritonitis, occurred during the trial of neutralized dialysate. The results suggest that this dialysate was less irritating to the peritoneal membrane than the control dialysate and that the therapeutic effects were satisfactory.

[Clinical and histological effects of preoperative chemotherapy for squamous cell carcinoma of the tongue].

According to the effect of preoperative chemotherapy for tongue cancer, if the operation method is modified, the modality of treatment for tongue cancer will contribute to preserve the tongue in terms of function and aesthetics as well as improve the quality of life. A clinicopathologic study on 81 patients with tongue cancer was conducted to elucidate the relationship between clinical and histological effects of preoperative chemotherapy with a single agent and multiple-drug regimens, in order to determine the safe surgical margins in the resection of squamous cell carcinoma of the tongue. Fifty patients were treated with bleomycin alone, sixteen with bleomycin and methotrexate and another fifteen with bleomycin, OK-432 and cisplatin. Multiple-drug regimens resulted in greater response rates than the single agent, but there was not always agreement in the relationship between the tumor regression rate and the histological effects of chemotherapy. The histological effects, including the residual aspects of the tumor cell population, were associated with the grade of histological malignancy according to the mode of tumor cell invasion, mitotic index of tumor cell, the tumor differentiation, the degree of stromal lymphocyte infiltration and cellular atypism. These data indicate that special consideration should be also given to the degree of histological malignancy for biopsied specimen, in addition to the tumor regression rate, in determining safe surgical margins and in the modality of treatment of tongue cancer.

[A case of pulmonary blastoma].

A 45-year-old male with pulmonary blastoma was described. The patient suffered from headache and gait disturbance, and was diagnosed metastatic brain tumor. The histopathology of the removed tumor indicated metastasis from pulmonary blastoma. The pulmonary tumor, which had been detected on the chest x-ray film but misdiagnosed tuberculous lesion, was subsequently diagnosed pulmonary blastoma by TBLB. The patient underwent left upper lobectomy with mediastinal lymph node dissection, but died 12 months after the operation. To our knowledge, only 54 cases with pulmonary blastoma have hitherto been reported in the Japanese literature.

[So-called "open healing" of chronic calcified pleural empyema--a study on the etiological and clinical problems].

Surgery performed on a 70 year-old male revealed a case of chronic calcified pleural empyema open healing that was induced through an effective bronchiolar drainage mechanism. The patient had previously received anti-tuberculosis medication, including INH and RFP, 7 years prior for positive sputum cultures and symptoms of fistulation observed in an x-ray finding of right lung pleural calcification. The four cardinal signs of chronic calcified pleural empyema open healing are as follows: 1. Failure to isolate tubercle bacilli, 2. Inability to locate necrotic coagulation or specific inflammatory granulation tissue on the cavitary wall, 3. Calcified pleural plaque detached from the cavitary wall, and 4. Microscopic recognition of many drainage bronchiolar openings to the empyema and the inner surface of the empyema covered with stratified squamous epithelium, which possibly penetrated via the entering bronchioles.

[Nd-YAG laser treatment of tracheo-bronchial stenotic lesions].

Fourteen patients were treated with a Nd-YAG laser. Treatment to relieve airway stenosis was administered on fifteen occasions, and on one occasion to treat an obstruction. In each case the flexible fiberoptic bronchoscope was used. In 4 emergency cases, three involving tracheal lesions and one involving a bronchial lesion, the airway was widened and the patients' condition was improved after laser ablation. In the palliative treatment of 12 lesions, the stenoses were severed and treated at an interval of time varying between one and three weeks. Complications occurred in 3 cases. There were 2 instances of massive haemorrhage and one of pneumothorax which occurred while treating the 5 stenoses situated at broncho-plastic anastomoses.
