A 60-year-old woman, who had been receiving chronic hemodialysis for chronic renal failure due to polycystic disease for 7 years, was admitted to our hospital with the chief complaints of right flank pain and high-grade fever on February 3, 1986. She had a family history of polycystic kidney disease and her elder sister has been undergoing chronic hemodialysis. The patient has had repeated episodes of urinary tract infection. The right markedly distended kidney was palpated with severe tenderness, and the performance status was poor. Computerized tomography showed the right kidney enlarging beyond the median line, but localization of the infected cysts was not clear. The 7-day intensive antibiotic therapy had no beneficial effect. On the 8th hospital day, percutaneous drainage, which was carried out for a large cyst with low-level echoes using ultrasonic guidance, yielded 600 ml of hemopurulent fluid. Bacterial culture of the aspirated fluid was positive for Escherichia coli. As the clinical symptoms did not improve after the 8th day, 10 Fr. catheter placement, percutaneous drainage and catheter placement were added for other cysts containing hemopurulent fluid. Although hematopyuria persisted, the patient became afebrile and the right kidney became smaller and softer. Both catheters were removed on the 30th hospital day. Repeated computerized tomography or ultrasonography revealed a few hyperdense or hyperechoic cysts in the right kidney. However, no recurrence of high-grade fever or right flank pain has been seen for 3 months. Ultrasonic-guided percutaneous drainage has a great potential for patient care of infected cysts in polycystic kidney disease.

Complement-dependent anti-glial fibrillary acidic protein (GFAP) antibody-mediated cytotoxicity was utilized in order to prepare astrocyte-deficient cultures from mechanically dissociated cells from 18-day-old embryonic rat cerebral hemispheres. Neurons and a lesser number of oligodendroglia were the major cell components in such cultures.
The morphology of mitotic cells identified as neuronal precursors by neurofilament immunoreactivity was studied by scanning electron microscopy in primary dissociated cultures of fetal rat cerebral hemispheres. It is shown that neurofilament-positive neuroblasts maintain their neuritic arborization during cell division suggesting a cytoskeletal function for the neurofilament.

The adjuvant effect of fly ash injected via various routes on IgE antibody production was examined in mice. Primary anti-ovalbumin (OA) IgE antibody production was induced by intratracheal and intraperitoneal injection with fly ash and OA, but not by the intragastric route. Both the intratracheal and the intraperitoneal routes showed similar efficiency in inducing IgE antibody production. High secondary anti-OA IgE antibody response by exposure to aerosolized OA was also observed in mice primed with intratracheal instillation of fly ash plus OA. The activity to induce IgE antibody production by fly ash instillation was almost the same as that by aluminum silicate, studied previously. Primary and secondary anti-OA IgG antibody production was also stimulated by intratracheal instillation of fly ash. These results suggest that fly ash has an adjuvant activity in the induction of IgE and IgG antibody production in mice.
Localization of ganglioside GM1 in cholinergic neurons from the septal area of a primary culture newborn rat brain was studied with a double avidin-biotin complex system. Cholinergic neurons were identified by double immunolabeling techniques that use choline acetyltransferase (ChAT) and neurofilament (NF) protein-antibodies. ChAT-positive neurons also were stained for ganglioside GM1 by using an avidin-biotin complex technique.

In order to research the influences of the somites or their derivatives on the migration and differentiation of neural crest cells, unilateral extirpation of somites was performed in the larvae of the salamander (Hynobius lichenatus Boulenger). Two types of neural crest cells appeared in the somite-free environment. They could already be distinguished from each other shortly after beginning of migration. One type of the cells was distributed in the expanded space between the epidermis and the neural tube, with melanocytes differentiated from them. The other type of the cells migrated closely to the lateral wall of the neural tube. They aggregated as a cord on the ventrolateral side of the neural tube and gave rise either to neurons or to satellite cells of spinal ganglia. The sequence of cytological events along the development of the cells and the time required for each event were essentially identical with those of the melanocytes and spinal ganglion cells developing on the control side. The present results suggest not only that migration and differentiation of neural crest cells are independent of the possible regulation of the somites or their derivatives but also that the fate of neural crest cells is determined before the onset of migration.
The histogenesis of the mouse jejunal mucosa and distribution of cells capable of proliferation were studied by scanning and transmission electron microscopy and light microscopic autoradiography using 3H-thymidine. Villous primordia were found to commence their development as mucosal ridges on day 15 of gestation. Individual villi are identifiable on day 16 of gestation. Some villi reach their adult height in the neonate, while the rest gradually become longer during postnatal development. Epithelial cells lining the entire mucosal surface are undifferentiated and proliferative by day 15 of gestation. On day 16 of gestation, cells lose their proliferative capability at the upper part of the high villi, while cells lining the lower part of the villi and intervillous region remain undifferentiated as well as proliferative. As the crypt develops during postnatal development, undifferentiated proliferative cells are restricted in it, coming to reside in the lower two thirds of the crypt by day 28. The fine structural differentiation of absorptive cells starts at the upper part of villi on day 16 of gestation. The apical tubular system develops during the perinatal period. Large vacuoles, many lysosomes and lipid globules appear in suckling mice. The cells show the typical morphology of the absorptive cells by postnatal day 21. Undifferentiated proliferative cells show only little change during development: cytoplasmic organelles increase slightly and small apical granules appear during postnatal development.

We present three cases of benign prostatic hypertrophy associated with chronic renal failure for three years from 1982 to 1984. Endogenous 24-hour creatinine clearance (Ccr) on admission ranged from 8.7 to 29.4 ml/min. Temporary hemodialysis treatment was required in one patient at the beginning of hospitalization. Indwelling intraurethral catheterization for 3 months or more improved the renal function in one patient, but brought troublesome complications of gross hematuria, intractable urethral pain or recurrent pyelonephritis in the other patients. These complications might arise from strong uninhibited detrusor contractions triggered or accelerated by stimuli and/or urinary tract infection induced by urethra-indwelt catheters. Intermittent self catheterization reduced these complications in one patient. In two patients, Ccr increased beyond 30 ml/min as a desirable standard level for safe operations. Suprapubic prostatectomy was successfully performed in all the patients. However, severe gastric ulcer or fatal duodenal ulcer occurred in two patients. Hypoproteinemia and/or urinary tract infection was thought to be highly related to ulceration. In conclusion, we would like to emphasize that a Ccr of more than 30 ml/min is needed for safe operations concerning renal function in patients with benign prostatic hypertrophy associated with chronic renal failure.
