Postoperative changes of the soft tissue profile after mandibular vestibulolingual sulcoplasty with free skin grafting.

Soft tissue changes of the upper and lower lips and the chin after mandibular vestibulolingual sulcoplasty with free skin grafting have been evaluated between one and five years postoperatively. Increases in chin prominence averaging 2.5 mm appeared within the first year after the operation. Prominence of the chin then remained unchanged at least through five years. A mean decrease in the height of the lower lip of 2.6 mm was found. These changes are possible complications of mandibular vestibulolingual sulcoplasty with free skin grafting, and must be considered in the final choice of preprosthetic surgery to be performed on a given patient.

Cephalometry↗

The pattern of postoperative bone resorption following mandibular vestibulolingual sulcoplasty with free skin graft.

The results of a cephalometric analysis of 70 patients in whom a mandibular vestibulolingual sulcoplasty with free skin graft was done are presented. The subjects were 56 women and 14 men who had worn dentures for an average of 17 years and had been followed up for as long as five years after the operation. The analysis showed an average of 1.7 mm loss of anterior height in the symphysis region in the first year after operation. After that time, no further increase in bone loss was observed. There was a tendency for greater resorption in patients with a small gonial angle. No correlation was found between the degree of resorption and sex, age, beta angle, and symphysis angle. There was a strong correlation between the loss in anterior height of the symphysis region and the loss of area in square millimeters.

Preprosthetic mandibular vestibuloplasty with buccal mucosal graft. A 2-year follow-up study.

21 patients with denture retention problems due to lack of mandibular vestibular denture space and alveolar ridge atrophy had a vestibuloplasty with free grafting of check mucosa performed under local anaesthesia on an out-patient basis. Follow-up examinations based on metric records showed that 18% of the sulcus extension is lost during the first post-operative month, but regained by the time of the 6-month control, leaving 99% of the surgically created sulcus extension as a permanent gain. The grafted check mucosa served well as a denture-bearing tissue, and almost all patient had a significant improvement of denture retention and stability. Complications were few. The biology of healing is discussed in the light of recent concepts of wound healing.

Healing reactions of relapse in secondary epithelization vestibuloplasty on dog mandibles.

The healing of the secondary epithelization vestibuloplasty wound was studied in five dogs. Special attention was paid to the nature of the relapse. Clinical records by means of tattoo marks and tracings at the time of operation and follow-up examination show that the loss and regain of sulcus extension follow the pattern of healing of an excision wound. The attachment of new mucosa (gained from secondary epithelization of the periosteum) to the bone eventually becomes loose in the muscle bearing region of the jaw. The detached muscles reinsert into scar tissue near their preoperative attachment. Thus, the relapse seems to be the result of an interaction of wound contraction, loosening of new mucosa and muscle reattachment. The findings are discussed in the light of recent concepts of wound healing. Techniques implying a grafting of skin or mucosa to the periosteal wound are advocated as a logical approach to the prevention of a relapse.

Preprosthetic vestibular sulcus extension by the operation of Edlan and Mejchar. A 2-year follow-up study-I.

Nineteen patients suffering from denture retention problems and/or pain on function due to atrophy of the mandibular alveolar ridge were operated on by the method of EDLAN & MEJCHAR. All patients felt an improvement of denture retention. Three out of seven still had pain on function after the operation. An average of 80% of the surgically created sulcus extension remained after 2 years.

Profile changes of bone and soft tissues following vestibular sulcus extension by the operation of Edlan and Mejchar. A 2-year follow-up study-II.

The progression of alveolar resorption and the alteration of the soft tissue profile of the chin were studied in 15 patients operated on by the vestibular sulcus extension technique of EDLAN & MEJCHAR. Lateral cephalograms and tracings exhibited an apparently accelerated bone resorption of the alveolar crest. Furthermore, a significant reduction of lower lip height and increase of mental soft tissue protrusion have been recorded.

Aneurysmal bone cyst--simple bone cyst, two aspects of the same pathologic entity?

An unusual case of aneurysmal bone cyst recurring after operation as a simple bone cyst is reported. Different theories on etiology and pathogenesis of aneurysmal bone cyst, simple bone cyst, and central giant cell granuloma of the jaws are resumed and similarities demonstrated. It is suggested that these three lesions have a common dysvascular etiology and that local environmental factors within the bone may differentiate the pathogenesis.

Diagnosis of sarcoidosis from oral manifestation.

The diagnosis of sarcoidosis may be a matter of complexity. Two cases of sarcoidosis in which oral manifestations were helpful in the diagnosis are presented. Similar cases from the literature are reviewed and discussed, and it seems that sarcoid involvement of clinically normal-appearing oral mucosa occurs with some frequency.

[Diagnostic and therapeutic aspects of myeloma from the point of view of the dentist and the oral surgeon].

Data on 10 patients with myelomatosis from three different maxillo-facial surgery departments are compiled and case histories, clinical courses, radiographs and therapies are studied. The data are compared with 51 cases reported in the literature. The main emphasis of the studies was on: 1. Diagnosis on the basis of the first symptoms in the maxillary region. 2. Prophylaxis and therapy of oral complications.

Tattoo marking for registration of relapse after oral vestibuloplasty.

A simple method for registering extension and relapse after vestibuloplasty based upon tattoo marking has been developed. Tattoo marking with autoclaved India ink was tested on 10 rats and seems to be harmless. This method is considered an alternative to other numerical expressions of extension and relapse after vestibuloplasty, such as radiocephalometrics and analysis of standardized impressions and castings.
