[Five cases of acute pulmonary failure associated with nitrogen dioxide inhalation].

Six men were accidentally exposed to NO2 when they were changing a propeller using a gas burner in the poorly ventilated hold of a ship. All of them were admitted to hospital with shortness of breath and cough. Chest X-ray on admission revealed diffuse, patchy and infiltrative shadows, leading to the diagnosis of pulmonary edema in all five patients. Steroid therapy was effective and followed by a quick recovery in all patients. None developed third phase manifestations characterized pathologically by bronchiolitis obliterans.

Immunohistochemical detection of ras 21 in oral squamous cell carcinomas.

The expression of the ras p21 in oral squamous cell carcinoma was examined immunohistochemically with the use of a monoclonal antibody NCC-RAS-001 with the avidin-biotin-peroxidase complex method. The expression of ras p21 product was detected in 65.7% (44 of 67 cases) of cancer patients. On the basis of the degree of histologic differentiation of the cancer cells, the incidence of ras p21 was found to be as follows: 63.4% (26 of 41 cases) were well differentiated; 86.7% (13 of 15 cases) were moderately differentiated; and 45.5% (5 of 11 cases) were poorly differentiated. The highest incidence was found in patients in their sixties--80.0% (16 of 20 cases). The incidence decreased to 40% in patients over 80 years of age. The incidence of ras p21 on the basis of location was as follows: 81.8% (9 of 11 cases) involved the buccal region, 70.6% (12 of 17 cases) were in the gingiva, and 55.0% (11 of 20 cases) were in the tongue.

Immunohistochemical localization of c-myc oncogene product in oral papilloma.

The expression of c-myc oncogene products in oral papillomas was studied by using an immunohistochemical method. The oncogene products were detected in 17(70.8%) of the 24 oral papillomas under study. The expression of the products was evaluated, and the histologic localization in proliferating epithelial cells of the oral papillomas was determined. In the basal cell layer, the products were detected in the nuclei of 16 oral papillomas, and in the cytoplasm of 12 oral papillomas. In the nuclei of cells in the spinous cell layer, the products were detected in 4 oral papillomas, and in the cytoplasm of 13 oral papillomas. In the keratinized cells, the products were not detected in the nuclei, but they were identified in the cytoplasm of three oral papillomas. The results suggested that the c-myc oncogene product might play an important role for proliferation and differentiation of the oral papilloma.

Electrical stimulation-evoked release of endogenous taurine from slices of the hippocampus, cerebral cortex and cerebellum of the rat.

Release of endogenous taurine by electrical stimulation of slices of the hippocampus, cerebral cortex, cerebellum and medulla oblongata of the rat was studied and compared with that of alanine and/or gamma-aminobutyric acid (GABA). Electrical stimulation caused a calcium-dependent release of taurine from slices of the hippocampus, cerebral cortex and cerebellum but not from slices of the medulla oblongata. The stimulus-evoked release of taurine in the hippocampus was rapid in onset and declined to baseline fast, which was essentially similar to the time course pattern of the stimulus-evoked release of GABA. In addition, there were distinct regional differences in the relative amounts of taurine released. Electrical stimulation did not release alanine from any regions examined. These results support the hypothesis that taurine plays a neurotransmitter role in the hippocampus, cerebral cortex and cerebellum of the rat.

6-Gingesulfonic acid, a new anti-ulcer principle, and gingerglycolipids A, B, and C, three new monoacyldigalactosylglycerols, from zingiberis rhizoma originating in Taiwan.

By monitoring the effects on HCl/ethanol-induced gastric lesions in rats, a new antiulcer principle named 6-gingesulfonic acid was isolated from Zingiberis Rhizoma, the dried rhizome of Zingiber officinale Roscoe (cultivated and processed in Taiwan) together with three new monoacyldigalactosylglycerols named gingerglycolipids A, B and C. Their chemical structures were elucidated on the basis of chemical and physicochemical evidence. 6-Gingesulfonic acid showed more potent anti-ulcer activity than 6-gingerol and 6-shogaol.

[Preparation of water-soluble hydronaphthoquinone derivatives and its application to the determination of hydrogen peroxide in clinical chemistry].

Several 1-alkoxy-2,3-dichloro-4-hydroxynaphthalene derivatives were prepared. These compounds were oxidized by hydrogen peroxide in the presence of a peroxidase to give 2,3-dichloro-1,4-naphthoquinone, which then reacted with various alkyl benzoylacetates to develop blue coloration with absorption maximum at greater than 600 nm. The color development reaction can be applied to the determination of appropriate serum constituents such as cholesterol.

[Stomachic principles in ginger. II. Pungent and anti-ulcer effects of low polar constituents isolated from ginger, the dried rhizoma of Zingiber officinale Roscoe cultivated in Taiwan. The absolute stereostructure of a new diarylheptanoid].

By using the effects on HCl/ethanol-induced gastric lesions in rats, beta-sesquiphellandrene (2), beta-bisabolene (3), ar-curcumene (4) and 6-shogaol (5) were isolated as anti-ulcer active principles in ginger, the dried rhizoma of Zingiber officinale Roscoe (Shokyo in Japanese) which was cultivated in Taiwan, together with nine known compounds and a new diarylheptanoid. The absolute stereostructure of the diarylheptanoid was characterized as (3S,5S)-dihydroxy 1-(4'-hydroxy-3',5'-dimethoxyphenyl)-7-(4''-hydrox y-3''-methoxyphenyl)heptane (15) on the basis of chemical and spectroscopic evidence which included the application of the benzoate chirality method. The pungent effects of several constituents isolated from ginger were examined.

[Guanidino compounds in the elderly].

To examine the metabolism of guanidino compounds in the elderly, we measured the serum concentrations of urea nitrogen, creatinine (Cr), guanidinoacetic acid (GAA) and creatine (CR) in middle-aged and elderly subjects. We also measured muscle mass in the elderly. The elderly subjects tended to have lower serum GAA concentrations than middle-aged subjects. On the contrary, CR concentrations of elderly subjects were higher than those of middle-aged subjects. Bedridden elderly subjects tended to have lower serum GAA concentrations and lighter muscle mass than ambulatory elderly subjects. On the contrary, serum CR concentrations of bedridden subjects were higher than those of ambulatory subjects. CR is an essential substance for muscle energy metabolism. These results indicate that high serum CR concentrations due to low CR metabolism in skeletal muscle might suppress glycine amidinotransferase (GAT) activity, resulting in decreased GAA production in the elderly.

[A case of small cell lung cancer occurring in a patient with pulmonary sarcoidosis].

A 62-year-old man was diagnosed as having sarcoidosis on the basis of uveitis, and the findings of bronchial alveolar lavage and transbronchial lung biopsy, in April 1988. He was admitted to hospital in September 1990, because of left hemiplegia. The chest X-ray film on admission revealed a new mass shadow in the left S6 and some increase of nodular shadows in both lung fields. A bronchial biopsy from the left B6 bronchus revealed small cell lung cancer. Although he was treated with whole brain irradiation and combination chemotherapy, he died of respiratory failure after three months. Speculations about the association between sarcoidosis and lung cancer have been made, but the mechanism is not understood, and their co-existence in the same patient is rare.

[A case of nephrotic syndrome associated with bronchogenic carcinoma].

A 58-year-old fisherman was admitted because of an abnormal chest X-ray shadow on the right side. Bronchoscopic examination revealed tumor of right B7. Transbronchial biopsy showed squamous cell carcinoma. He was treated with four courses of CDDP and PEP. Two years later, he developed nephrotic syndrome and relapse of lung cancer. Proteinuria and pedal edema continued. Renal biopsy revealed the characteristic light and immunofluorescent microscopic features of membranous nephropathy. Oral administration of low dose etoposide resulted in reduction of the carcinomatous lung lesion and a decrease in proteinuria as well as pedal edema.

[A case of pulmonary alveolar proteinosis with invasion of macrophages in alveolar interstitial region].

A 24-year-old woman was admitted to our hospital on November 2, 1989 for investigation and treatment of abnormal shadows detected in her routine chest radiograph on July 19, 1989. The chest X-ray film on admission showed diffuse infiltrative shadows in the peripheral regions of the bilateral middle and lower lung fields. In addition centrilobular shadows in the subpleural regions with increases irregular densities were found on chest CT. We therefore suspected pulmonary alveolar proteinosis. Bronchoalveolar lavage from the right middle bronchus and transbronchial lung biopsy of the right upper and lower lobes were performed. Electron microscopic examination of a specimen of bronchoalveolar lavage fluid revealed many multilamellar bodies, characteristic of pulmonary alveolar proteinosis. In addition, proteinaceous material containing multilamellar bodies was observed within the alveolar lumina. We diagnosed this case as pulmonary alveolar proteinosis. Electron microscopy also revealed many macrophages in the alveolar walls, some of which contained a few or numerous multilamellar bodies within the secondary lysosomes. We treated this patient with oral and inhaled Ambroxol, with improvement of her clinical condition.

Is focal chronic autoimmune thyroiditis an age-related disease? Differences in incidence and severity between Japanese and British.

The incidence of chronic lymphocytic thyroiditis in autopsy material from Japanese and British subjects was evaluated. Lymphocytic infiltration in representative thyroid sections from 1826 Japanese cases collected from four different institutions was analysed. The overall incidence of lymphocytic infiltration was significantly higher in females (22.2 per cent) than in males (13.9 per cent). In females, the incidence reached 23.2 per cent in the fourth decade and showed no increase with age thereafter. The overall incidence of lymphocytic infiltration in thyroid sections from 810 British cases was 42.5 per cent in females and 19.4 per cent in males; an increase in the incidence of thyroiditis from the sixth decade onwards was noted in British females, the figure reaching 50.0 per cent in those aged over 70 years. These findings suggest possible racial differences in susceptibility to chronic thyroiditis. The disorder is not necessarily related to age, increasing severity of disease with age being found only in British females.

Malignant hemangiopericytoma of the liver: report of a case.

A 30 year old female was admitted with right upper abdominal pain and fever. Ultrasonography and computed tomography revealed a large cystic mass in the right lobe of the liver, and aspiration bacteriology was negative. A right hepatic lobectomy was performed for a suspected cystadenocarcinoma, however, the tumor was histologically diagnosed as a hemangiopericytoma with prominent venous invasion. The patient died within a short time of multiple pulmonary metastases. Primary hepatic hemangiopericytoma is extremely rare, and according to our research, this is only the 4th case reported in the literature.

Effects of chlornitrofen, a herbicide, on reproduction of Brachionus urceolaris (Rotatoria) through water and food (Chlorella).

Chronic effects of chlornitrofen (CNP) on the reproduction of Brachionus urceolaris (Rotatoria) were investigated by exposure of individuals to CNP from the egg stage, which had been attached to the adult. The survivors of 12 neonates, which had been exposed to CNP100, 70 or 40 microg liter(-1) decreased to 50% at the age of c. 2, 4 and 6 days, respectively, compared to c. 6.5 days for those exposed to 0, 10 and 20 microg liter(-1) CNP. Release of offspring (mostly two individuals per day) started at two days old. At the peak, four days old, a control female produced 8.1+/-0.9 offspring per day compared with 4.5+/-1.3 (mean+/-SD, n=12) at 40 microg liter(-1). The cumulative numbers of offspring produced by a female were 25.8+/-1.2, 24.2+/-2.9, 22.3+/-3.6 and 13.6+/-3.1 (mean+/-SD, n=12) at control, 10, 20 and 40 microg liter(-1) CNP exposure, respectively. The 50% reproductive impairment concentration was calculated to be 37 microg liter(-1). Growth of neonates was barely detectable at 70 microg liter(-1), and the rapid increase in the effect of CNP from 40 to 70 microg liter(-1) was attributed to an increase in tolerance with growth of the neonates. The effects of CNP on reproduction were also tested by CNP exposure through food (CNP-accumulated Chlorella). The 50% reproductive impairment concentration of CNP in the alga was calculated to be c.60 microg g(-1) (wet weight) by the same method used to assess the dissolved CNP, although the effect of CNP which may have been released from the alga to the water could not be estimated precisely.

Glucocorticoid-induced G1 arrest and the release effect of epidermal growth factor on the human salivary gland adenocarcinoma cell.

Dexamethasone (1 microM) decreased the distribution of cells in S phase (about 75%) and increased that of G1 cells (1.1-fold) in the DNA histogram of human submandibular salivary gland adenocarcinoma cells (HSG) reversibly. In synchronized cells at G1 phase, glucocorticoid delayed the initiation of DNA synthesis by about 3-4 h. The conditioned medium (50%) or exogenous human epidermal growth factor (EGF, 10 ng/ml) significantly nullified these effects by glucocorticoids. These results suggested that glucocorticoids arrested the cells at G1 phase, which implied the inhibition of production of some progressive factor, probably EGF, in the cell cycle of HSG.

Morphological evidence for two types of idiopathic 'Sertoli-cell-only' syndrome.

Biopsies of testicular specimens taken from 41 patients that were diagnosed as having idiopathic Sertoli-cell-only syndrome were classified into two types, A and B, on the basis of histological and immunohistochemical findings. Thirty eight specimens that were classified as type A exhibited seminiferous tubules of small diameter and with tubular wall hyalinization, but containing normal adult type Sertoli cells. The other three specimens that were classified as type B showed no seminiferous tubular wall hyalinization, and their Sertoli cells had vimentin distribution localized in the subnuclear cytoplasm and had a pseudostratified lining, features resembling the appearance of fetal Sertoli cells. In one patient with a seminoma, a comparative study of the same testis prior to and post-irradiation was undertaken. Judging from this, postpubertal depletion of the germ cell population was considered to be responsible for the tubular atrophy observed in type A. Type B testes, though small in number, were characterized by a morphology distinct from the type A, but their pathogenesis remains unknown.

Human autoantibody to Sertoli cells detected in healthy individuals.

Human autoantibody to Sertoli cells was detected in normal human sera. This IgM-type autoantibody was undetectable during the neonatal period and was found in 11.5% of 365 serum samples taken from adult healthy persons of both sexes. This human autoantibody to Sertoli cells exhibited quite the same target-organ specificity and multi-organ reactivity (salivary gland ductules, pancreatic intercalated duct, renal lower nephron, pituitary acidophilic cells) as those of the murine monoclonal autoantibody (IgM class) to Sertoli cells (TM-1: WHO registered code T43). The TM-1 monoclonal antibody could recognize testicular antigens with molecular weights of 67,000 and 23,000 in Sertoli cells, and had already been demonstrated capable of inducing murine experimental spermatogenic disturbance when administrated together with murine monoclonal autoantibody to seminiferous tubular basement membrane (TM-2: WHO registered code T44). These observations may suggest that human spermatogenic disturbance could be easily induced by the multi-organ reactive autoantibody to Sertoli cells even in healthy individuals under particular conditions where this autoantibody can be allowed to reach the target Sertoli cells across the barrier of seminiferous tubular wall by either autoantibody to seminiferous tubular basement membrane or other toxic damage.
