A thrombotic tendency in patients with infective endocarditis.

An attempt was made to evaluate a thrombotic tendency in cases of infective endocarditis (IE) from the viewpoint of the changes of platelets and coagulo-fibrinolysis. Qualitative changes of platelets by thrombi formation were detected by showing activated responses of platelets to adenosine diphosphate or collagen and by showing a high level of plasma beta-thromboglobulin which is released by platelets during aggregation. A mild hypercoagulable state without acceleration of fibrinolysis was also detected by showing low levels of plasma antithrombin III which is the most potent antithrombin in the blood and normal levels of plasma alpha 2-plasmin inhibitor in the blood. From above findings, it was concluded that a thrombotic tendency in cases of IE was clearly detected by qualitative changes of platelets by thrombi formation and by mild hypercoagulability without acceleration of fibrinolysis.

Acute effect of cigarette smoking on serum angiotensin-converting enzyme activity in normal man.

The acute effect of cigarette smoking on serum angiotensin-converting enzyme activity was evaluated in 6 healthy subjects consisting of 4 non-smokers and 2 habitual smokers. Cigarette smoking resulted in rapid increases in serum converting enzyme activity in 5 of 6 subjects within 5 min. and the converting enzyme activity remained above the control value at 30 min. The increase in the enzyme activity of non-smokers was higher than that of habitual smokers at any time when the enzyme activity was determined. It is therefore suggested that cigarette smoke (or smoking) can cause the secretion of angiotensin-converting enzyme from the pulmonary endothelial cell, in which the enzyme may be produced, to the systemic circulation. It is also speculated that the increase in the enzyme activity may contribute to the initiation of cardiovascular changes associated with cigarette smoking.

Radiation therapy of early glottic carcinoma. A Japanese experience.

From 1969 and through 1979, 105 patients with T1 N0M0 and T2 N0M0 glottic carcinoma were treated with irradiation alone. The minimum follow-up period was 2 years, and only one patient was lost to follow-up. There were 62 T1a lesions, 14 T1b, 19 T2 with normal mobility, and 10 T2 with impaired mobility; relapses occurred in 10, 4, 1 and 3 patients, respectively. All but one patient who relapsed had local recurrence with or without nodal or distant spread. One patient had only nodal metastasis. Six of the 18 relapses were successfully cured by surgery. None of the patients who had nodal or distant spread survived. The relapse rates had no clear relation to total dose or TDF value. T1 lesions treated through larger fields (greater than or equal to 25 cm2) had a lower relapse rate than lesions treated through smaller fields, but the difference was not statistically significant.

[Experiences with intravenous drip infusion therapy of amikacin for severe infections in patients of hematological disorder].

Amikacin was studied for clinical effect in 7 patients with acute leukemia, 1 patient with chronic myelogenous leukemia-blastic crisis, 1 patient with malignant lymphoma and 1 patient with aplastic anemia, who were suffered from severe infection such as sepsis, pneumonia or subcutaneous abscess. Most of these patients had bleeding tendency, so amikacin was administered by intravenous drip infusion in a dose of 200 mg--400 mg for 1 hour. Total doses of amikacin were between 3.2 g and 12.6 g. These doses of amikacin gave good response to 3 patients with sepsis, 1 patient with subcutaneous abscess and 1 patient with pneumonia. We didn't observe any side effect most likely associated with amikacin. Therefore, intravenous drip administration of amikacin might be useful drug for management of severe infections in patients of hematological disorder, and seemed to be as safe as intramuscular administration.
