[Results of the treatment of Hodgkin's disease: the importance of establishing and executing a protocol].

A retrospective analysis was performed on 32 patients seen at Keio University Hospital and its affiliated hospitals from 1976 to 1987. Twenty-two patients were treated with our protocol, and the other 10 were not for several reasons. In this study, we compared the results of the treatment of these two groups. There were 20 males and 12 females. Age ranged from 13 to 72 years with a median value of 36 years. Histologically, six had lymphocytic predominance, 12 nodular sclerosis, 14 mixed cellularity. In the protocol group, seven were clinical stage I, 12 were CS II, two were CS III, and one was CS IV. Nine patients in the protocol group underwent staging laparotomy, and four CS II patients were upstaged to PS III. In the non-protocol group, five were CS I, four were CS II, one was CS III. According to the protocol, staging laparotomy was done excepting CS III.IV patients and some of CS I patients. In case of CS I without laparotomy, pathological stage(PS) I.II, or PS III1 with minimal splenic involvement, they were treated with radiation only. The rest of the patients were treated with chemotherapy and booster irradiation. Ten-year survival and 10-year relapse-free survival were 100%, 75%, respectively in the protocol group. In the non-protocol group, they were 31% and 32%, respectively. When picking up 17 patients treated by the authors through the whole process, 10-year survival and 10-year relapse-free survival were 100%, 92%, respectively.(ABSTRACT TRUNCATED AT 250 WORDS)

[Effect of the time interval between irradiation and chemotherapeutic agents on normal tissue damage--comparison between in vivo in vitro experiments].

Experiments have been carried out to determine the effect on the cell survivals at different time intervals between irradiation and chemotherapeutic agents (BLM, cisDDP, ADM and ACNU) in either the in vivo or the in vitro system. The intestinal epithelial assay was applied on the in vivo system. The clonogenic cell survivals of V79 cells, both in the proliferative and the plateau phases, were determined in the in vitro system. The V79 cells in the plateau phase were more sensitive to BLM, cisDDP and ACNU than those in the proliferative phase, however, the result was reverse with ADM. When BLM, cisDDP or ACNU was combined with irradiation at different time intervals, the response of the plateau phase V79 cells to combination therapies were very similar to those of the intestinal epithelial cells. On the other hand, V79 cells in the proliferative phase, which were treated with ADM and irradiation, showed the similar response as the intestinal cells. These results suggest that studies of chemo-radiotherapy with cultured cells which are sensitive to chemotherapeutic agents might be suitable to expect the in vivo damage of the normal tissue.

An enzyme-linked immunosorbent assay for immune complex of HTLV-I.

A sandwich enzyme-linked immunosorbent assay (ELISA) for immune complexes of human T cell leukemia virus type I (HTLV-I) was developed using monoclonal antibody (MoAb) 3G1 which recognizes a different epitope on HTLV-I to that with which natural human anti-HTLV-I antibody binds. The assay was capable of titrating artificial immune complexes not only at antigen-antibody equivalence but also at antibody excess. Although the antigen-antibody ratios could not be determined in the individual sera from patients with overt ATL, the level of immune complexes in three out of four sera was estimated to be 250 +/- 36 ng/ml. Immune complexes of HTLV-I could not be identified in sera obtained from one patient with overt ATL, three healthy HTLV-I carriers and three normal human controls.

Resonance Raman characterization of hog thyroid peroxidase. An SERRS study.

Resonance Raman (RR) spectra of hog thyroid peroxidase (TPO) were observed for the first time and compared with those of lactoperoxidase (LPO) and horseradish peroxidase (HRP). Since TPO purified by monoclonal antibody-assisted immunoaffinity chromatography was strongly fluorescent, the surface enhancement technique using Ag colloid adsorption was used for the oxidized form, but ordinary RR spectra could be obtained for the reduced form. The RR spectra of TPO were distinct from those of HRP in both the oxidized and reduced states and indicated the presence of six-coordinated iron-protoporphyrin.

[Digital subtraction angiography in lower extremity phlebography].

DSA was applied to the lower extremity phlebography on 21 cases (36 legs). The patient lay in the supine position and a tourniquet was placed around the ankle. Forty ml of contrast medium, diluted to 25% of the original concentration with normal saline, was injected into the dorsal vein of the foot. The anterior tibial vein, posterior tibial vein, peroneal vein and muscular vein of the calf were identified in 24 (63.2%), 36 (94.7%), 37 (97.4%) and 7 legs (18.4%), respectively. The poor opacification of the anterior tibial vein was attributed partly to the compression effect of the tourniquet. The abnormal findings were deep vein thrombosis (5 legs), reflux from the deep to the superficial vein (14 legs) and irregularity of the venous wall (16 legs). The superficial varicose veins were not demonstrated in DSA phlebography. The examination was comfortable because the patient position was supine and the dilute contrast medium caused no burning sensation. We believe that DSA phlebography is a safe and useful method for diagnosing the deep venous system disorders.

[Evaluation of the lung uptake of 123I-IMP by bronchoalveolar lavage].

A 67-year-old female with radiation pneumonitis in the right upper lobe underwent lung scanning and bronchoalveolar lavage 4 hours after the intravenous injection of 123I-IMP. The lung scanning showed increased accumulation of 123I-IMP, corresponding with the area of radiation pneumonitis. The ratios of radioactivities of the cellular and noncellular components in the bronchoalveolar lavage fluid to that of the serum, were 27.0 and 0.39, respectively. Our results suggest that 123I-IMP or its metabolites are transported through alveolo-capillary barrier and taken up by free cells in the alveolar space after 123I-IMP has been bound to nonspecific receptor sites in the endothelial cell in the capillary lumen.

[Feasibility of CHOP chemotherapy--with special reference to age, diabetes mellitus, liver cirrhosis and obesity].

Since 1982, we have performed 384 courses of CHOP chemotherapy for 89 patients with malignancy including 70 with non-Hodgkin's lymphoma, adhering to the original regimen as strictly as possible. As severe acute reactions, myelosuppression, fever, arrhythmia, hemorrhagic cystitis, and perforation of duodenal ulcer were seen. Rates of fever had no tendency to increase with advancing age. Three patients only with diabetes mellitus had no severe side effects. Three patients with liver cirrhosis showed severe myelosuppression and fever. One patient both with liver cirrhosis and diabetes mellitus died from the infection due to CHOP chemotherapy, however the other febrile patients did not have life threatening infection. Thirty three percent (11/33 courses) of the patients with obesity experienced severe myelosuppression (WBC less than 1,000), while 55% (33/60 courses) of the patients without obesity. However satisfactory treatment results were not obtained in the patients with obesity. We consider that CHOP chemotherapy is excellent in feasibility even for the aged patients or the patients with diabetes mellitus. However, we suggest that the dose of CHOP chemotherapy should be reduced for the patients with liver cirrhosis.

Role of culture supernatant of cytotoxic/cytostatic macrophages in activation of murine resident peritoneal macrophages.

Peritoneal macrophages elicited by Lactobacillus casei YIT9018 (LCEPM) were incubated in culture for 18 h with L. casei; the culture supernatant (LCM) was then harvested and tested for its ability to increase the cytostatic activity of resident peritoneal macrophages (RPM) and LCEPM. Treatment of RPM with LCM induced activation of macrophages to a cytostatic state against L929, Colon 26, P815, P388D1 and L1210 cells. A combination of recombinant human tumor necrosis factor (rhTNF), recombinant mouse TNF (rmTNF), recombinant human interleukin-1 (rhIL-1) or bacterial lipopolysaccharide with recombinant mouse interferon gamma (rmIFN-gamma) resulted in the synergistic induction of cytostatic activity in RPM. Recombinant mouse granulocyte-macrophage colony-stimulating factor (rmGM-CSF) plus rhTNF increased the cytostatic activity of RPM a little but rmGM-CSF or rhTNF combined with rhIL-1 or alone had no effect. The effect of LCM on RPM was not inhibited by polymyxin B, anti-mTNF antiserum or below 20 U/ml monoclonal anti-rmIFN-gamma antibody (anti-rmIFN-gamma) but was inhibited by more than 40 U/ml anti-rmIFN-gamma, and LCM did not have any interferon antiviral activity. These results suggest that the cytostatic activity of RPM was augmented by the LCM, and that the effect of the LCM may be not due to IFN-gamma, TNF, GM-CSF, IL-1 or a small amount of contaminating lipopolysaccharide.

The behavior of 99mTc-hexamethylpropyleneamineoxime (99mTc-HMPAO) in blood and brain.

99mTc-hexamethylpropyleneamineoxime (99mTc-HMPAO) is a reagent for scanning cerebral blood flow. We investigated how 99mTc-HMPAO changed in the blood and brain. The 99mTc-HMPAO, which was prepared by adding of 99mTcO4- to HMPAO and Sn(II), consisted of primary and secondary complexes, reduced hydrolyzed 99mTc, and 99mTc-pertechnetate. The percentage of the primary complex in 99mTc-HMPAO decreased with time after preparation. The primary complex converted to the secondary one very rapidly in the presence of plasma. When 99mTc-HMPAO was injected into patients, 99mTc activity was immediately partitioned in the plasma fraction, with approximately 60% in whole blood. In plasma, 99mTc was found to be associated with proteins such as albumin and globulin. 99mTc trapped in red cells was not washed out with either plasma or saline. Biodistribution studies showed that the less lipophilic compounds of 99mTc-HMPAO could not pass through the blood brain barrier (BBB), and therefore did not accumulate in the brain. The results of gel chromatography and equilibrium dialysis indicated that no specific 99mTc binding protein was present in the brain. Considering the instability of 99mTc-HMPAO in vivo, we proposed that the speed at which the primary complex converted to the less lipophilic compounds was important in allowing 99mTc-HMPAO to pass through the BBB and to be fixed in the brain.

An autopsy case of disseminated trichosporonosis with candidiasis of the urinary bladder.

A case of disseminated trichosporonosis associated with Candida infection of the urinary bladder is presented. Coffee bean shaped or crescent shaped yeast-like elements are characteristic of Trichosporon and useful in differentiating Trichosporon from Candida but such histological features are less efficient than the immunohistochemistry in identifying mixed fungal infection. In the present case, an application of avidin-biotin-complex (ABC) method with anti-Trichosporon antiserum and anti-Candida antiserum enabled us to diagnose a mixed infection by both fungi.

Usefulness of pinhole collimator in differential diagnosis of metastatic disease and degenerative joint disease in the vertebrae; evaluation by receiver operating characteristics (ROC) analysis.

In order to evaluate the diagnostic efficacy of pinhole collimator (PHC) imaging combined with an X-ray for vertebral metastasis, our prospective study has employed receiver operating characteristics (ROC) analysis in 21 patients, 11 with osseous metastasis and 15 with degenerative joint disease in the lumbar vertebrae. PHC imaging provided better anatomic information on the extent of 99mTc-MDP accumulation. PHC vertebral scintigraphy had a considerable impact on the decision-making process, although with variations and not very satisfactory results among the physicians with little experience. Our study suggests that PHC imaging and X-ray film are useful in differentiating between osseous metastasis and degenerative joint disease in the vertebra.

Staurosporine-induced neurite outgrowth in PC12h cells.

Treatment of PC12h cells with staurosporine (100 nM), a potent inhibitor of protein kinases, promoted rapid outgrowth of neurites. The mechanism of neurite formation elicited by staurosporine is different from that elicited by nerve growth factor or by dibutyryl cyclic AMP, based on the independence from transcription or from activation of cyclic AMP-dependent protein kinase, respectively. Comparative experiments showed that of these three neurite-promoting agents, staurosporine was the most effective in eliciting neurite initiation.

Residual masses on follow-up computed tomography in patients with mediastinal non-Hodgkin's lymphoma.

During the last 4 years we have treated four patients with extensive mediastinal non-Hodgkin's lymphoma (NHL). None had extrathoracic disease and they were clinically stage I-II. All received intensive combination chemotherapy and radiotherapy resulting in good response. Nevertheless, when treatment was completed, computed tomography (CT) showed a residual mediastinal mass in all four patients, who have remained alive and well for more than 19 to 45 months after completion of therapy and without any supplemental treatment. The residual masses gradually regressed or resolved on follow-up CT, suggesting that they did not contain malignant components.

Normal sonographic findings of the infant temporal lobe in coronal sections.

Coronal sonograms of the temporal lobe of the infant brain were obtained using a real time sector scanner with a 5.0 MHz transducer probe. The precise normal anatomy in the coronal sections was determined by correlating with corresponding brain specimens. It was revealed that the spatial arrangement of structures in the medial part of the temporal lobe, i.e. the hippocampal formation and the inferior horn, varied in the three different planes. In the plane through the anterior part of the temporal lobe, these structures were seen as a horizontal lamellar configuration, whereas in the plane through the middle part these structures were depicted as a typical interfolded arrangement. However, the hippocampal formation was barely visible in the plane through the posterior part.

Dynamics of ischaemic brain oedema fluid: 'dense tracer distribution around the focus', evaluated by peroxidase injection in the rat cortex.

Experimental cerebral ischaemia was produced by microembolization in rats. HRP (horseradish peroxidase) was then injected into the cerebral cortex and the extracellular fluid dynamics were observed. In the control group, there was a basic pattern in the migration of the tracer. In the ischaemic group, a 'dense tracer distribution around the focus' was found in addition to the basic pattern. This phenomenon could be observed from the second day after the ischaemic insults. The perivascular DAB reaction was marked, particularly around the focus. There was a vesicular uptake of the tracer in the vascular endothelium. This phenomenon appeared almost simultaneously with the active resolution of the oedema fluid. In addition, frequent mitotic figures were seen around the focus. It was thus possible to hypothesize that the 'dense tracer distribution around the focus' appeared at the site of active tissue reaction and had a close relationship with oedema resolution.

N-isopropyl-p-[I-123]iodoamphetamine lung imaging in a patient with chronic pulmonary thromboembolism.

A patient with a brain infarct was found to have chronic pulmonary embolism associated with perfusion defects on blood flow imaging with N-isopropyl-p-[I-123]iodoamphetamine (I-123 IMP). However, gradually increasing uptake of I-123 IMP was seen in later images, in areas distal to the embolus. The increased uptake to the embolized lung seemed to be through the systemic collateral circulation.

Role of adenovirus type 2 early region 1B 19K protein stability in expression of the cyt and deg phenotypes.

The cell line KB18 constitutively expresses adenovirus type 2 (Ad2) early region 1B (E1B) genes in the absence of E1A expression and thus is useful for understanding the function and properties of E1B gene products. We report here that KB18 cells complement the cyt and deg phenotypes of Ad2 cytocidal (cyt) mutants and an Ad12 cyt mutant. Thus the E1B 19K polypeptide in KB18 is functional. Expression of E1B mRNA and synthesis of the 19K polypeptide were studied in wild-type Ad2- and Ad2cyt15-infected KB cells at 15 h post-infection, and in KB18 cells. Although E1B mRNA synthesized in KB18 cells at 5 to 7% of the level in Ad2-infected KB cells, the amount of E1B 19K polypeptide formed was similar. In contrast, the amount of 19K protein in Ad2cyt15-infected KB cells was about one-tenth of that in Ad2-infected cells, although expression of E1B mRNA was comparable to that during Ad2 infection. The half-lives of the 19K polypeptide in wild-type-infected KB cells, in Ad2cyt15-infected KB cells and in KB18 were approximately 90 min, 25 min and 22 h, respectively. The cyt phenotype was expressed at 30 to 35 h post-infection, thus showing that the instability of the 19K polypeptide in Ad2cyt15-infected cells is not due to alteration of cell morphology or cell destruction.
