[A case of juvenile Alzheimer's disease with various neurological features such as myoclonus, showing grumose degeneration in the dentate nucleus].

A case of juvenile Alzheimer's disease with various neurological features such as myoclonus, also showing grumose degeneration in the dentate nucleus was reported. In a 35 year old woman, at first myoclonus, and 5 years later, progressive dementia were found. She, then, fell into apallic syndrome 9 years later, and died of pneumonia at the age of 53. She had no particular family history. Neurological examination disclosed cerebellar ataxia, left hemiparesis, convulsion in addition to myoclonus. Neuropathologically, there were a lot of senile plaques and neurofibrillary tangles in the cerebral cortex. A marked myelin loss in the white matter and a neuronal loss in the basal ganglia were also found. And grumose degeneration in the cerebellar dentate nucleus distinctively characterize the present case. The myoclonus and cerebellar ataxia could be attributed to the grumose degeneration. Two similar cases had been previously reported. Juvenile Alzheimer's disease with grumose degeneration like the present case was considered to be one of the subgroups of Alzheimer's disease.

Quality of life of patients receiving antihypertensive treatment.

Ninety-four hypertensive patients, 52 females and 42 males, 38 to 79 years old, were investigated for quality of life under antihypertensive treatment, using a self-administered questionnaire. Their social situation, work and leisure activities and hypertension-related symptoms were evaluated when first diagnosed and after treatment with antihypertensive drugs. Of the 94 patients, 85 (90%) felt uneasiness due to hypertension, 21 (22%) felt their work was limited, and 27 (29%) reported restricted leisure activities due to hypertension. Eighteen patients (19%) had sexual problems. Females who were severely hypertensive at first visit (mean BP greater than or equal to 120 mmHg), those who were well-controlled by treatment (mean BP at last visit less than 105 mmHg), and those whose mean BP decreased markedly (decrease in mean BP greater than or equal to 20 mmHg) showed significant improvement in hypertension-related symptoms. Side effects varied depending on the antihypertensive drug used. It is recommended that the quality of life of patients be taken into consideration when choosing antihypertensive drugs.

[Pathophysiological analysis of cardiac function by computer processing of echocardiograms].

Automatic image processing system has been developed for analysis of cardiac function with echocardiograms. Echograms of apical long axis view were transferred to microcomputer system and processed to display three dimensional images of left ventricular myocardium. With this technology, analysis of left ventricular wall thickness in three dimensional manner revealed pathophysiological changes in ischemic heart disease.

[Clinical evaluation of serum S100ao protein in patients with urogenital diseases and healthy volunteers].

We investigated the clinical significance of the serum S100ao protein in patients with urogenital diseases. The serum levels of S100ao protein were measured in 179 patients with urogenital diseases and 180 healthy volunteers. The mean value of S100ao protein in serum from healthy volunteers was 203 +/- 107 pg/ml (Mean +/- SD). Therefore, the cut-off level was set to 524 pg/ml (Mean +/- 3SD). The levels of S100ao protein in serum were significantly higher in men than in women (P less than 0.05). The levels of S100ao protein in serum were significantly high in the patients in their fifties and sixties compared with the other patients (P less than 0.01). When serum levels exceeding the cut-off level were considered to be positive, the percentages of positivity in each disease were as follows: renal cell carcinoma; 38.7%, bladder tumor; 9.1%, prostatic carcinoma; 12.5%, testicular tumor; 0%, benign prostatic hypertrophy; 7.4%, urolithiasis; 7.1% and chronic renal failure; 100%. The levels of S100ao protein in serum were significantly correlated with those of BUN, serum creatinine and endogenous creatinine clearance, respectively. S100ao protein in serum was increased immediately after operation and returned to the normal range within one to two weeks after operation. As described above, the level of S100ao protein in serum was affected by renal function, operative procedures and age. However, the positive rate of S100ao protein was so high in patients with renal cell carcinoma that serum S100ao protein might be a valuable tumor marker in those patients.

[Hereditary progressive dystonia with marked diurnal fluctuation--a report of two siblings, one of them showing age-dependent changes of symptoms].

Two siblings of hereditary progressive dystonia with marked diurnal fluctuation are reported in this communication. The case 1 is a 50-year-old woman who developed dystonia in the right lower extremity at the age of 8 which deteriorated in the afternoon. At the age of 13 she became unable to walk in the afternoon because of dystonia, but after the age of 23 her symptoms began to ameliorate and at the age of 29 her daily activities became no longer disturbed by dystonia even in the evening. However, at the age of 42 dystonia in the lower extremities began to worsen again toward the end of the day and after the age of 47 dystonia appeared in the all extremities. Since the age of 49 she had been able to walk only for three hours after awakening. Her 37-year-old half sister, case 2, also developed a slight dystonia in the left upper extremity at the age of 8. At the age of 33 she felt tight in the neck, waist and extremities after hard works in the evening, and the worst of it was in the left arm which would not move. Both cases were well responsive to L-dopa. Their family history revealed that their father had a history of dystonia the course of which was similar to that of the case 1. Dominant inheritance could be considered in this family. These findings would suggest that hereditary progressive dystonia with marked diurnal fluctuation could show not only a diurnal fluctuation but also age-dependent changes of symptoms. Its basic abnormalities might exist in the dopamine storage pool.

[High-dose intracavitary brachytherapy of a recurrent cervical cancer following surgery].

Forty-nine postoperative cancer patients with a recurrent cervical cancer at the vaginal stump were treated with high-dose intracavitary brachytherapy with or without external irradiation. The intervals between their previous surgery and the vaginal recurrence ranged widely, from 4 months to 36 years. The overall 10-year survival rate after radiotherapy was 48%. The patients were classified into two groups, according to the tumor size that was evaluated by rectovaginal examination at time of recurrence, that is, one group without a palpable mass at the vaginal stump and one with a palpable mass. The 10 year-survival rate with or without the palpable mass was 8% and 80%, respectively. External irradiation combined with brachytherapy could not modify the survival rate of those that had been treated by brachytherapy alone in the group without the palpable mass. On the other hand, the incidence of late intestinal complications was increased by the combination of external irradiation. This result suggests that the most important prognostic factor in cases of a recurrent cervical cancer at the stump is the tumor size.

[Experimental study on N-isopropyl-p-iodoamphetamine lung uptake].

Experimental study on N-isopropyl-p-iodoamphetamine lung uptake was performed by means of analyzing microautoradiography obtained by administering 125I-IMP to Wistar male rats. In the normal rat lung at one minute after the intravenous injection of 125I-IMP, there is increased grain density in the interstitial space including the capillary lumen and interalveolar septum, but not in the alveolar space. On the other hand, at 24 hours after the intravenous injection, increased grain density was noted in the alveolar space of the rat lungs which had received a dose of 10 Gy to the unilateral one. But, definite difference was not found in grain density between the irradiated and non-irradiated lung. The results indicate that part of IMP which was taken up by the endothelial cell is capable of passing through capillary-alveolar barrier.

[Tumor imaging using 99mTc-hexamethylpropyleneamine oxime].

Tumor imagings have been performed in 32 tumor sites in 28 patients with a variety of malignancies by using 99mTc-HM-PAO. They have been compared with radionuclide angiography and conventional tumor imagings such as 67Ga citrate or 201TlCl scanning, and then their possible values have been evaluated for the investigation of tumor blood flow. Tumors were clearly visualized in 19 out of 32 sites, with 59% of positive rate. Good correlation was observed between the radioactivity in the tumor site on static image of 99mTc-HM-PAO and the radioactivity at arterial phase on radionuclide angiography, which represents tumor blood flow. The radioactivities in the marginal regions tended to be stronger than those in the center of large tumors. These results indicated that tumor imaging by 99mTc-HM-PAO mainly reflected tumor blood flow. On the other hand, 99MTc-HM-PAO imagings were not well correlated with 67Ga-studies, while they were coincided with all 201Tl scannings performed. It suggested that the affinity of 99mTc-HM-PAO to capillary wall also involved in its accumulation in the tumor. In conclusion, tumor imaging by 99MTc-HM-PAO was able to estimate tumor blood flow, and this examination would be helpful for the management of malignancies.

[Development of a converging collimator for thyroid scintigraphy].

Thyroid scintigraphy is generally performed using a pin-hole or a parallel-hole collimator. The former yields a high resolution image, however, the acquisition time prolongs because the collimation system magnifies a thyroid image. The latter has somewhat low resolution, because the collimation system cannot magnify the thyroid image and it is difficult to close the collimator surface to a patient thyroid. The defects of the above two collimators are distortion or blurs at the thyroid edge of the acquired image. To improve the image quality of the thyroid scintigram, we made computer simulations concerning contrast resolution in comparison of a parallel-hole, pin-hole and converging collimator. The simulations results showed that the contrast resolution by the converging collimator is superior to the other collimators. According to the results, we developed a specially-designed converging collimator (thickness: 100 mm, focal length: 195 mm). We cut an edge of the collimator to decrease the distance between the neck and the collimator surface. Under almost the same conditions for spatial resolution and sensitivity, we compared the effectiveness of the converging collimator with that of the other two collimators; the pin-hole type [medium resolution] and the parallel-hole type [high resolution]. As a result of experiments using a phantom, the converging collimator showed less distortion compared with the other collimators at the deep area of the thyroid phantom.(ABSTRACT TRUNCATED AT 250 WORDS)

[The effect of OK-432 on modifying a spontaneous lung metastasis from murine fibrosarcomas].

The authors have investigated the effect of OK-432 on the spontaneous metastasis from a murine fibrosarcoma, either an NFSA or a SANH, that had been transplanted into the right thigh of C3H mice. When the NFSA or the SANH tumors grew to 7 mm in diameter, they were irradiated with a single dose of 30 Gy and 20 Gy, respectively. A local administration of OK-432 (2.5 KE) around each of the tumors was initiated immediately after irradiation and continued twice a week. It was found that regrowth in both tumors was not modified by the OK-432, though the mean number of metastatic lung nodules per mouse from either tumor was significantly decreased. The incidence of a lung metastasis also was reduced by the OK-432 but not significantly. Further, mice that had received a SANH tumor transplant developed a lymph node metastasis in the abdominal cavity that was depressed by the OK-432.

[An early clinical study of Z-100 in leukopenia caused by radiotherapy].

The clinical efficacy of Z-100 for the treatment of leukopenia caused by radiotherapy for malignant tumors was evaluated in an early clinical study using three doses, 2, 10 and 20 micrograms. According to the evaluation by investigators and time-course of WBC, the highest effective rate was achieved by 20 micrograms. Adverse reactions occurred in all groups, but there were no serious or clinically relevant reactions. These results suggested that Z-100 is clinically useful for the treatment of leukopenia caused by radiotherapy.

[Estimation of scatter component in SPECT planar image using a Monte Carlo method].

A Monte Carlo simulation was performed to estimate quantitatively the scattered photons in planar images of SPECT. As a phantom we used a water-filled cylinder with a line source and calculated the energy spectra of primary and multiple scattered photons separately at each pixel of the planar images. The energy spectra of primary and scattered photons were studied on following parameters: the size of the phantom; the location of the source; the width of the energy window centered at 72.3 (Tl-201), 141 (Tc-99m), and 159 keV (I-123); and the view angle of the planar images. Obtained results were; (1) the energy spectra of Compton scattered photon varied with the phantom size, the source location, and the photopeak energy, (2) the scattered photons within energy windows of 10-30% centered at the photopeak energy were mainly composed by Compton scatter of the first order, (3) the higher order scattering component of the Compton photons did not represent the location of the line source on the planar image, and (4) the scatter fraction defined by the ratio of the scattered photons to the primary photons increased with increasing the size of the phantom and the width of energy window at the low photopeak energy. From the results, we discussed on the scatter subtraction methods.

[Digital subtraction angiography for congenital heart disease].

From January 1988 to May 1989, ten infant patients (15 times) with congenital heart disease were studied by digital subtraction angiography (DSA) pre and/or postoperatively. The diagnoses of the patients include total anomalous pulmonary venous return (TAPVR), transposition of great arteries, patent ductus arteriosus with pulmonary hypertension, giant hemangioma of the left leg, asplenia (two cases), tetralogy of Fallot (two), and tricuspid atresia (two). Two newborn patients (TAPVR, giant hemangioma) were in critical condition preoperatively, but were diagnosed precisely and nonivasively by DSA. Ten cases were studied for estimation of operative repair and consequently five of them were diagnosed to require reoperation. It is concluded that DSA is of use for diagnosis of congenital heart diseases and for postoperative evaluation of infant patients under critical condition.

Direct evidence for local generation and release of angiotensin II in human vascular tissue.

A direct measurement of both angiotensins I and II immunoreactive substances was made in the perfusate from isolated human umbilical vein perfused with Krebs-Ringer solution which was free of any component of the renin-angiotensin system. The identity of the immunoreactive peptides was confirmed as angiotensin I and angiotensin II by high-performance liquid chromatography in reference to standard compounds. The rate of release of angiotensins was 41.9 +/- 7.4 and 63.4 +/- 12.0 pg for angiotensins I and II, respectively, during the first perfusion period of 30 min, and it remained stable at least for 3 hours. Angiotensin-converting enzyme inhibitor captopril, added to the perfusion medium (10(-9) to 5 x 10(-6) M), suppressed immunoreactive angiotensin II release in a dose-dependent fashion; the maximal percent inhibition of angiotensin II release evoked by captopril (5 x 10(-6) M) was approximately 56%. These results taken together with the previous observations of presence of essential components of the renin-angiotensin system in vascular tissue provide direct evidence for local generation and subsequent release of angiotensin II in vascular beds of human beings.

Phase II randomized clinical trial of LC9018 concurrently used with radiation in the treatment of carcinoma of the uterine cervix. Its effect on tumor reduction and histology.

The clinical efficacy of LC9018, a biological response modifier prepared from heat-killed Lactobacillus casei YTT9018, used in combination with radiation was studied in a randomized controlled trial on 61 patients with carcinoma of the uterine cervix of Stage IIB or III. The combination therapy with LC9018 demonstrated a significant effect on tumor reduction, compared with radiation therapy alone, at the cumulative doses of 15-Gy and 30-Gy external irradiation (P less than 0.05). Histologic study confirmed that LC9018 also enhanced the therapeutic effect of the irradiation. Moreover, LC9018 seemed to be useful in protecting the patients from leukopenia during radiotherapy. This study suggests that LC9018, when used in combination with radiotherapy, will be an effective adjuvant immunotherapeutic agent. More studies in a large series of patients will, however, be needed to establish its long-term efficacy, safety, and effects on both prognosis and enhancing radiotherapy.

Assessment of transesophageal Doppler echography in dissecting aortic aneurysm.

To assess the clinical value of transesophageal Doppler echography in the diagnosis of dissecting aortic aneurysm, both transesophageal and conventional echograms were performed in 22 cases of dissecting aortic aneurysm. Of the 22 patients, 17 underwent angiography; 8, X-ray computed tomography; 4, both; and 12, surgery. The performance of each method was assessed in the following four segments: A, ascending aorta; B, aortic arch; C, thoracic descending aorta; and D, upper abdominal aorta. The results by angiography were presumed to be correct. In the group of 17 patients who underwent angiography, the rate of correct detection of an intimal flap using the transesophageal approach was 100% in all four segments, significantly better than detection by the conventional approach (segment A, 65%; segment B, 47%; segment C, 35%; segment D, 53%) (p less than 0.01), and the rate of correct detection of the entry sites using the transesophageal approach was 100%, significantly better than that by conventional approach (42%) (p less than 0.05). X-ray computed tomography was not capable of detecting the site of entry in all cases. The presence of thrombus, aortic regurgitation and pericardial hemorrhage were all revealed clearly by the transesophageal approach, and the results were partly proved by other methods. In conclusion, transesophageal Doppler echography provides a rapid and accurate method of diagnosing and evaluating dissecting aortic aneurysm and permits prompt initiation of appropriate treatment.

Upstream half of adenovirus type 2 enhancer adopts a curved DNA conformation.

The putative enhancer domain of human adenovirus type 2 (Ad2) was revealed to contain a bent DNA structure in the upstream half. By using "deletion analysis", this unusual structure was identified experimentally to span from nucleotide 194 to 269 (figures; nucleotide numbers from the left terminal nucleotide of the viral DNA). This region has almost the same nucleotide sequence as the upstream half of the enhancer and packaging region of Ad5 and, therefore, is thought to contain multiple enhancer elements for transcription and the elements required for packaging of Ad2 DNA. The bent DNA structure of this region was further characterized by analyzing temperature-dependent changes in electrophoretic mobility of the DNA fragments used for bending analysis. Ad2 enhancer provides a good system to investigate the relationship between higher order structure of DNA and transcriptional activation.
