[Creutzfeldt-Jakob like syndrome due to lithium intoxication--a case report].

A woman with mania who had been treated with lithium carbonate since 69 years of age presented mild tremulousness of both hands at the age of 76 years. She subsequently developed dysphagia, dysarthria, unsteady gait and progressive deterioration of the higher cortical function over 1.5 months. Her tremulousness deteriorated until it resembled myoclonus. EEG showed periodic sharp wave complexes appearing predominantly over the bilateral parieto-occipital areas. Although the EEG abnormality was not identical with that usually observed in the fully developed stage of Creutzfeldt-Jakob disease (CJD), it was reminiscent of that seen in the early stage of CJD. Thus, her clinical symptoms and signs were considered to resemble those of CJD. The plasma concentration of lithium, however, was found to be over the therapeutic range. Reduction of the dose of lithium carbonate almost completely resolved her symptoms within 3 weeks. Consequently, her clinical condition was considered lithium intoxication. Antidepressant and bismuth as well as lithium have been reported to induce a Creutzfeldt-Jakob like syndrome. Awareness of drug-induced Creutzfeldt-Jakob like syndrome is clinically important because of its excellent prognosis as opposed to the ominous prognosis of CJD.

[A case of transitional cell carcinoma of renal pelvis with extremely high serum levels of CA19-9 and CEA].

We report a case of transitional cell carcinoma of the renal pelvis with extremely high serum levels of carbohydrate antigen 19-9 (CA19-9) and carcinoembryonic antigen (CEA). A 69-year-old woman was admitted with gross hematuria. Imaging diagnosis revealed a tumor in the left renal pelvis and multiple liver and bone metastases. Serum levels of CA19-9 and CEA were 2,557 U/ml (normal < 36) and 523 ng/ml (normal < 3.8), respectively. She died of cancer progression 3 weeks after admission. An autopsy diagnosis was transitional cell carcinoma of the left renal pelvis. No abnormal findings were recognized in the gastrointestinal organs. Cancer cells showed a positive immunohistochemical staining for both CA19-9 and CEA.

[Eumycotic mycetoma of the lower leg].

Eumycotic mycetomas are extremely rare diseases in Japan. Only four cases have been reported. In the present report, eumycotic mycetoma of right lower leg of a 44-year-old male of Thai origin, resident in Japan for more than five years, was presented. An induration was noticed on the antero-medial side beneath the knee. Computed tomography showed slightly high dense subcutaneous mass-like lesion at the medial side of the right tibia. Though no draining sinus was formed, histologic examination disclosed grains composed of septated hyphae with Splendore-Hoeppli material. Since more people from foreign countries tend to reside in Japan, physicians and pathologists should be aware of various infections which are not commonly found in Japan.

Long-term survival in a case of high-risk type IgD myeloma; a possibility of effectiveness of interferon-alpha and erythropoietin.

The prognosis of patients with IgD myeloma is poorest among all types of multiple myeloma. We report a case of a patient with IgD myeloma who survived for an extraordinarily long period post-diagnosis (65 months). According to the new risk grouping of IgD myeloma (Shimamoto Y. et al., Eur. J. Haematol. 47, 262 (1991), he was classified as being in the high-risk group, with a zero percent expected 5-yr survival. Nevertheless, he survived for 65 months. We discuss the usefulness of interferon-alpha and erythropoietin for the treatment of IgD myeloma.

Cytokeratin-19 fragment (CYFRA 21-1) in bladder cancer.

The present study was designed to evaluate the role of the cytokeratin-19 fragment (CYFRA 21-1) as a tumor marker in bladder cancer. The bladder cancer cell line JMSU1 was used in the preclinical study. Subjects comprised 120 healthy volunteers, 20 patients with benign bladder diseases including acute cystitis and bladder stones, 12 patients with chronic renal failure, and 117 patients with histologically confirmed primary bladder cancer. CYFRA 21-1 concentrations were measured by a sandwich enzyme-linked immunosorbent assay. The preclinical study in vitro and in vivo showed that JMSU1 produced and released CYFRA 21-1 in the culture supernants and serum of JMSU1-bearing nude mice. The cutoff level was set at 3.5 ng/ml (mean + 3 SD) by the analysis in the healthy volunteers. Under this condition, sensitivity was 0% in benign bladder diseases, 83% in chronic renal failure, and 41.9% in bladder cancer. Serum CYFRA 21-1 levels increased significantly as tumor stage advanced or tumor grade increased. A serial follow-up study demonstrated that patients with progressive showed a gradual increase in serum CYFRA 21-1 levels while patients who responded to the treatments had a marked decrease in serum CYFRA 21-1 levels. Higher serum CYFRA 21-1 levels were related to poor survival. The present study suggest that serum CYFRA 21-1 is a useful marker to monitor the clinical course of bladder cancer and to provide prognostic information.

Treatment of ipsilateral delayed endolymphatic hydrops.

Nine patients with ipsilateral delayed endolymphatic hydrops were treated. All cases were initially treated by conservative therapy using isosorbide or other drugs and 4 patients have been doing well without surgical intervention, though they occasionally have slight dizziness. Five patients required surgery to control vertigo. Three patients first underwent cochleosacculotomy to maintain the vestibular function in the diseased ear, but vertigo recurred after 6 months in all 3 cases. These 3 patients and another patient who wanted definitive treatment right from the start underwent transmastoid labyrinthectomy which resulted in complete control of vertigo. One patient received chorda tympani nerve section because he did not want his vestibule destroyed. After the nerve section, he has had no vertigo though sometimes dizziness. Transmastoid labyrinthectomy seems to be the best treatment for ipsilateral delayed endolymphatic hydrops.

Effect of oral administration of Lactobacillus casei on alloxan-induced diabetes in mice.

The effect of Lactobacillus casei (LC) on the onset of alloxan (AXN)-induced diabetes in 7-week-old BALB/c mice were examined. It was observed that the mice given a diet containing 0.1% or 0.05% LC or orally administered LC showed significantly decreased incidence of diabetes induced by intravenous injection of AXN and that the plasma glucose level was slightly lower than that in the control group. The body weight in the LC-treated groups was higher than that in the control group, although the food intake weights were almost the same. Pathological analysis revealed that the AXN-induced disappearance of insulin-secreting beta-cells in the islets of Langerhans was strongly inhibited in the LC-treated groups. It was also shown that the serum nitric oxide level was maintained at a normal level in LC-treated mice, whereas the level in the control group was increased by AXN administration. Taken together, these findings suggest that oral administration of LC to AXN-treated BALB/c mice contributed to the reduction of diabetes and the increase in plasma glucose level.

[A case of sporotrichosis with numerous fungal elements].

We report a case of cutaneous sporotrichosis with numerous fungal elements in a 59-year-old Japanese male treated with topical corticosteroid ointment. Histologically, granulomas were found with a few of neutrophils, many lymphocytes and plasmocytes in the upper dermis. Numerous yeasts were found in the granulomas. They were stained with periodic acid Schiff, but not with hematoxylin and eosin. No asteroid body was detected. After treatment of the corticosteroid, histological pattern is different from usual pattern, such as the number of fungal elements is larger, extent of neutrophil infiltration is slighter and very rare appearance of asteroid body. These histological differences are likely to be induced by the local immunosuppression after corticosteroid treatment.

[Necrotizing sarcoid granulomatosis diagnosed by video thoracoscopic lung biopsy].

A 28-year-old woman was admitted to our hospital because of chest pain. A chest roentgenogram and a chest computed tomogram revealed many nodular shadows on both sides. Examinations of specimens obtained by and by transbronchial lung biopsy during fiberoptic bronchoscopy were not diagnostic, and therefore video thoracoscopic lung biopsy was done. The lung lesion was characterized by aggregates of epithelioid cell granulomas, along with granulomatous and necrotizing angitis. We therefore diagnosed necrotizing sarcoid granulomatosis, and began to administer prednisolone. The nodular shadows disappeared within four weeks. In this case video thoracoscopic lung biopsy was useful in the diagnosis of necrotizing sarcoid granulomatosis in the lung.

[A case of infiltration of scar of sarcoidosis after blepharoplasty].

A case of a 28-year-old woman with infiltration of sarcoidosis scar tissue after blepharoplasty is reported. Nodules developed two times in her right upper eyelid about 1 and 2 years after blepharoplasty of both eyes and they were resected each time, but eruption recurred. Ophthalmic examination revealed aqueous flare and cells, snowball vitreous opacities, and retinal periphlebitis. A chest X-ray disclosed bilateral hilar lymphadenopathy (BHL). Laboratory studies showed an elevation of the serum angiotensin converting enzyme (ACE). Light microscopy revealed epithelioid granuloma with no caseation necrosis in a biopsy specimen, Viewing through polarized light demonstrated crystalline-like foreign bodies with bi-refringence in the epithelioid granuloma. Electron microscopic X-ray microanalysis confirmed these foreign bodies to be composed of Si, Mg, and O. These findings indicate that this skin lesion was caused by an infiltration of sarcoidosis scar tissue.

[Development of index of social activities for the elderly].

In order to develop indices of social activities for the elderly, two surveys with 2 year intervals were conducted on the same 5,201 elderly subjects in four areas in Japan using a self-administered questionnaire. Social activities were defined as "activities which required contact with society" and were measured by 4 major facets of social activities, which were based on 21 questions relating to job activity, socially-plated activities, learning activities, and personal activities. The results were as follows; 1. The Wilcoxon scores in indices for 4 facets were given in sex and age groups. 2. Means of scores of indices increased with the degree of social activities from a subjective judgment. 3. Rank correlation coefficients between indices in two surveys were 0.60-0.71 for the persons whose answers were "no" to the question "did degrees of your activities change over the two years?" 4. Differences between indices in two surveys were higher in the persons with answers of "increase" to the above question than those with answers of "no", and were lower in those with answers of "decrease". These findings suggest that indices are available for assessing social activities as indicated by the reproducibility, validity and responsiveness found in this study.

[Usefulness and early detection of acute exacerbation by peak expiratory flow rate in the management of asthma].

The monitoring of peak expiratory flow (PEF) and maintenance of a symptom diary have been proposed as means to monitor asthma severity. This study assessed via longitudinal analysis, the usefulness and limitation of daily PEF monitoring (amplitude percent mean) as an index for asthma severity, and the usefulness of recording daily intake of beta-agonists as a measure of symptom severity. Nine subjects with moderate to severe asthma were treated with inhaled beclomethasone dipropionate (BDP). Sustained-release theophylline and oral corticosteroids supplemented inhaled BDP according to both individual symptom prevalence and PEF measurements. The patients kept records four times daily of their PEF and asthmatic symptoms (cough, sputum and number of attacks), from the beginning to the end of treatment. They also recorded the daily frequency of beta-agonist inhalation (as needed for symptomatic relief). Minimum daily PEF (expressed as percent best) correlated significantly with symptom scores: cough (r = -0.69), sputum (r = -0.69) and attack (r = -0.69). Minimum PEF correlated strongly with the daily frequency of beta agonist inhalation (r = 0.76). Each symptom score and the daily frequency of beta-agonist inhalation was zero when the minimum PEF was approximately 80%. We evaluated the correlation between minimum PEF and daily PEF variability in all data (r = -0.72): for a minimum PEF > or = 20% (r = -0.72), a minimum PEF > or = 30% (r = -0.77), a minimum PEF > or = 40% (r = -0.82) and a minimum PEF > or = 50% (r = -0.80). We determined the correlation between minimum PEFs of > or = 35 to 45% in 1% increments and daily PEF variability in relation to the severity of asthma. With a minimum PEF > or = 40%, the strongest correlation was demonstrated between minimum PEF and daily PEF variability. Daily PEF variability was a reliable indicator for the management of asthma when the PEF > or = 40%. When the minimum PEF was 80%, daily PEF variability was approximately 10%. When the minimum PEF was 40%, daily PEF variability was 46%, and the daily frequency of beta-agonist inhalation was 2.6. We concluded that, 1) The target values of minimum PEF and daily PEF variability are 80% and 10% respectively in the treatment of asthmatic patients, 2) The daily frequency of beta-agonist inhalation is recommended as an indicator for the severity of asthmatic symptoms, and 3) Daily PEF variability in combination with the daily frequency of beta-agonist inhalation is very useful in the management and early detection of acute asthma.

[Expression of CD44 standard and CD44 variant 6 in human lung cancer].

We immunohistochemically examined the expression of CD44 standard (CD44 st) and CD44 variant 6 (CD44 v6) in 112 cases of primary lung cancer, and their relationship to the clinical milieu, including the clinical stage. In 46 cases of squamous cell carcinoma, expression of CD44 st was observed in 45.7% of the cases, and expression of CD44 v6 was observed in 60.9%. In 43 cases of adenocarcinoma, positive staining of CD44 st and CD44 v6 was seen in 2.3% and 4.7% of the cases, respectively. None of 21 small cell carcinomas was positive for CD44 st or CD44 v6. In squamous cell carcinomas, the expression of CD44 st and CD44 v6 was observed at a rate significantly higher than in other histologic type. Most specimens positive for CD44 st stained positively for CD44 v6. Therefore, it seems likely that the CD44 expression observed in squamous cell carcinoma of the lung was a variant CD44 containing the domain encoded by variant exon 6. The expression of CD44 v6 was not related to the clinical stage. Significant association between CD44 v6 and differentiation of squamous cell carcinoma was seen; 2/7 (28.6%) for poorly differentiated, 19/31 (61.3%) for moderately differentiated, and 7/8 (87.5%) for well differentiated squamous cell carcinomas (p = 0.02 by trend test). It was previously reported that CD44 st and CD44 v6 were expressed in both normal bronchial epithelium and squamous cell metaplasia. These results suggest that the expression of CD44 v6 in squamous cell carcinoma of the lung may reflect the immunohistochemical characteristics of the tissue from which such carcinoma emerge.

Novel animal model of lymph node metastasis by intrauterine inoculation of the actively metastatic subline PL3 separated from rat Walker 256 tumor cells.

To investigate the cellular mechanism of lymph node metastasis by tumor cells through the lymphatic vessels in the uterine corpus, we selected an active metastatic subline (PL3) from rat Walker 256 tumor cells and used it to develop a novel experimental model of lymph node metastasis induced by intrauterine inoculation of the tumor cells. Light- and electron-microscopic examinations revealed that the inoculated PL3 cells could actively infiltrate the endometrium from the uterine cavity and form a primary lesion in the uterine corpus. A few PL3 cells in the myometrium were found in the lumen of the peripheral lymphatic vessels on day 7 after inoculation. The regional lymph nodes around the uterus were then invaded by the migrated PL3 cells, and finally (after 3 weeks), most of the parenchyma of the nodes was replaced by metastasized tumor cells. By flow-cytometric analysis, the metastatic PL3 cells expressed CD44, like Walker 256 cells, but lacked integrin alphaL- and alpha4-chains. However, expression of ICAM-1 was considerably down-regulated in the PL3 cells compared to the parent cells. More aggressive invasion was shown by the PL3 cells compared to the parent cells in the in vitro invasion assay. These findings suggest that this experimental model and the separated PL3 cells are suitable for thorough investigations of the unidentified metastatic process and the related cellular behavior involved in the onset of lymphatic invasion by the primary lesion. Furthermore, our model more closely reproduces the clinical conditions related to lymph node metastasis of malignant carcinomas through the lymphatic vessels than does any previously reported animal model.

Vitamin B12 enhances the phase-response of circadian melatonin rhythm to a single bright light exposure in humans.

Eight young males were subjected to a single blind cross-over test to see the effects of vitamin B12 (methylcobalamin; VB12) on the phase-response of the circadian melatonin rhythm to a single bright light exposure. VB12 (0.5 mg/day) or vehicle was injected intravenously at 1230 h for 11 days, which was followed by oral administration (2 mg x 3/day) for 7 days. A serial blood sampling was performed under dim light condition (less than 200 lx) and plasma melatonin rhythm was determined before and after a single bright light exposure (2500 lx for 3 h) at 0700 h. The melatonin rhythm before the light exposure showed a smaller amplitude in the VB12 trial than in the placebo. The light exposure phase-advanced the melatonin rhythm significantly in the VB12 trail, but not in the placebo. These findings indicate that VB12 enhances the light-induced phase-shift in the human circadian rhythm.

Ultraviolet resonance Raman evidence for the absence of tyrosinate in octopus rhodopsin and the participation of Trp residues in the transition to acid metarhodopsin.

The ultraviolet (244 nm) resonance Raman spectra of octopus rhodopsin and its photoproduct, acid metarhodopsin, do not give any evidence for a tyrosinate. This finding excludes the possibility that Tyr-112 serves as the counter anion to the protonated Schiff base as does Glu-113 in bovine rhodopsin. Upon photoconversion from rhodopsin to acid metarhodopsin, Trp and Tyr Raman bands decrease in intensity and concomitantly a Trp band shifts in frequency. The changes of Trp Raman bands are ascribed to changes in hydrophobic interactions and conformation, suggesting a possible role of Trp in the photoconversion process of octopus rhodopsin.
