Percutaneous puncture of pancreatic cysts guided by ultrasound.

Fifteen pancreatic cysts have been successfully punctured percutaneously under the guidance of ultrasonic B scanning. The procedure may be used for verification of ultrasonic diagnosis, for definitive therapy, for decompression in the case of threatening rupture and for allowance of time for maturation of the cyst membrane before surgical intervention.

Ultrasonically guided percutaneous fine needle biopsy of the pancreas.

By ultrasonic scanning, it is possible with reasonable accuracy to demonstrate space occupying lesions in the pancreas, and with a delicate ultrasonic technique, a puncture needle can be guided to a predetermined mass in the pancreas. In 25 patients with suspected pancreatic lesions, scanning demonstrated a solid mass lesion in the pancreas. Twenty-one patients had carcinoma; four had chronic pancreatitis. In 17 of the 21 patients with carcinoma, tumor cells were aspirated. The cells were numerous and easily recognizable, fulfilling the usual cytologic criteria of malignant disease. In three of the 16 patients with carcinoma, the aspirations were inadequate, and one showed normal pancreatic cells. In the four patients with chronic pancreatitis, normal pancreatic cells were aspirated. There were no immediate complications due to fine needle biopsy. The combination of ultrasonic scanning and ultrasonically guided percutaneous fine needle aspiration is an important adjunct to the management of pancreatic lesions.

Prenatal obstruction of the ileum diagnosed by ultrasound.

A case of low atresia of the ileum, diagnosed prenatally by real-time ultrasound scanning, is presented. The ultrasound examination showed progressive distension of intestinal loops, with strong peristaltic movements. The stomach was also distended, presenting as a large cystic area in the upper left abdomen. Real-time ultrasound technique is most advantageous in the diagnosis of fetal ileus. Prenatal diagnosis of fetal intestinal obstruction is of great importance, making early and safe treatment of the newborn possible.

Ultrasonically guided puncture of solid pancreatic mass lesions.

Ultrasound scanning in combination with ultrasonically guided fine needle aspiration biopsy is a well established method in the evaluation of patients with suspected pancreatic cancer. Fine needle puncture should be performed when a solid mass lesion is outlined by ultrasound. The dynamic scanning technique provides the best identification of the biopsy needle during its insertion. In a series 203 patients including 126 with malignant mass lesions of the pancreas or malignant periampullary tumors from a 10 yr period, false negative cytological diagnoses occurred in 24%. Two false positive cytological diagnoses occurred in patients with chronic pancreatitis. The technique is most valuable in the demonstration of cancer of the pancreas or of the periampullary region (pVpos = 98%) but less valuable in the exclusion of malignancy (pVneg = 69%).
