Clavicular fracture of the neonate: can it be predicted before birth?

A retrospective study of 3873 cephalic vaginal births was undertaken to evaluate which factors are associated with clavicular fractures and thereby assess their clinical relevance in antepartum prediction. Fifty-two neonates with a fractured clavicle were identified, an incidence of 1.3%. Neonates with fractures were compared to a control group without fractures. The two groups were matched for age, parity, and mode of delivery. The neonates with fractures were found to be significantly heavier than controls, with mean birthweights of 3.7 kg and 3.2 kg, respectively (p = 0.000). Significantly more newborns in the fracture group weighed more than 4 kg (p = 0.02). A similar trend was noted when birthweights of previous pregnancies were compared (p = 0.03). Fracture cases had a greater incidence of low 1-minute Apgar scores (p = 0.02), and a higher proportion of births following abnormal progress of labor (19% of fracture group and 2.9% of controls; p = 0.001). Despite these statistically significant differences between the groups, it is not possible to predict most cases of fracture before birth, since the majority of fractures occur in cases not identified by any of the studied parameters.

[Gunshot trauma to the common carotid artery].

A case of gunshot trauma to the common carotid artery which presented with a juxtaclavicular pulsating hematoma is presented. After precise diagnosis was established by angiography, reconstruction by interposing and autologous saphenous vein graft was successful. Aspects of this highly lethal type of trauma include the need for close observation of the asymptomatic patient, indications for exploring penetrating neck trauma, the need for routine angiography and reconstructive arterial surgery in the presence of central neurological damage, relevant surgical techniques, and the availability of parasurgical facilities. The current consensus is to perform reconstructive arterial surgery when feasible, regardless of preoperative neurological status. Preoperative angiography is definitely recommended in any hemodynamically stable patient who is not suffocating. An aggressive approach, surgical expertise and perioperative support might reverse the poor prognosis in these otherwise fatal cases.

Sonographic demonstration of endometrial fluid collection following termination of pregnancy.

Intrauterine fluid collections were demonstrated by transvaginal ultrasound in 19 of 21 patients, 4-6 hours after first-trimester termination of pregnancy. Similar but lesser findings were seen 3 days later in some of the cases. Five days postoperatively, the endometrium appeared normal in all patients. Appreciation of the normal sonographic appearance of the uterine cavity following abortion is important for detecting pseudosac of ectopic pregnancy or the possibility of inappropriate curettage of an intrauterine pregnancy.

Serological evaluation of Ehrlichia canis infections in military dogs in Africa and Reunion Island.

Sixty-eight dogs from four African countries and Reunion Island were tested for antibodies against Ehrlichia canis. Twenty-six dogs (50%) in Tunisia, Senegal and Chad were found positive using the indirect fluorescence antibody test. Dogs from both the Central African Republic and Reunion Island were all negative. Thus, this preliminary report confirms the presence of E. canis in Africa. Larger studies will be necessary to evaluate the current epidemiologic situation of canine ehrlichiosis in these countries.

Effect of low-intensity pulsed ultrasound on prepubertal rat testis.

The testes of prepubertal male rats (N = 12) aged 21 days were stimulated with low-intensity pulsed ultrasound (1.5-MHz frequency, 1-KHz repetition pulse rate, 200-microseconds pulse width, 30-V peak-to-peak amplitude and 20-mW/cm2 intensity) applied to the skin for 20 min/day for 7 days. Control rats (N = 8) were manipulated in the same manner but not submitted to ultrasound. Ultrasound stimulation promoted a significant increase in plasma testosterone (62%) leading to a significant increase in seminal vesicle relative weight (35%) as well as an increase in the fructose (92%) and DNA (200%) contents of the gland. No differences were detected between ultrasound-treated and control animals in terms of body weight and the relative weights of testis, cauda epididymidis, testis DNA and mitosis.

[Critical study of 200 surgically treated lumbar disk hernias].

The authors reviewed 200 cases of operated lumbar disk hernias. Hernias of the L4-L5 disk were twice as frequent as those of the L5-S1 disk. When clinical examination revealed simultaneous involvement of the L5 and S1 nerve roots, the L4-L5 disk was more frequently responsible (75%). Clinical examination alone was not sufficient to localize the level of the disk hernia and CT scan used for this purpose had a sensitivity of 93.2%. In 20% of cases, the CT scan demonstrated a second asymptomatic lesion. The results of surgical treatment were assessed as 83% good and very good results and 50% complete cures. The authors propose the following recommendations: Look for involvement of the L5 and S1 nerve roots which is suggestive of an L4-L5 hernia; only operate after radiological demonstration of the level of the herniated disk; do not operate on asymptomatic lesions detected by CT scan.

Systemic immunosuppression induced by photodynamic therapy (PDT) is adoptively transferred by macrophages.

Some derivatives of hematoporphyrins are strongly retained by tumor tissue as compared to normal tissue, and exposure of these photosensitizers to radiation in the visible spectrum can cause serious biological damage. These properties have been exploited in the development of a new treatment for cancer termed photodynamic therapy (PDT). However, recent studies have also demonstrated that PDT can also induce a state of systemic immunosuppression. The purpose of this study was to determine whether PDT-induced suppression of contact hypersensitivity (CHS) responses was an active phenomenon that could be adoptively transferred by viable splenocytes from PDT-treated mice. Although induction of adoptively transferable suppressor cells in PDT-treated mice required exposure to antigen, the suppressor cells were found to be antigen nonspecific in their function. Furthermore, splenocytes from PDT-treated mice were capable of generating levels of allospecific cytotoxic T lymphocyte (CTL) activity which were comparable to those generated by normal control mice, but the ability of irradiated spleen cells from PDT-treated mice to stimulate a mixed lymphocyte response (MLR) was dramatically impaired. Finally, chromatographic separation of T cells, B cells and macrophages showed that the cell type which mediates adoptively transferable suppression of CHS responsiveness is in the macrophage lineage.

[Antibodies against extractable nuclear antigens. Clinico-biological correlations in 100 cases].

The clinical significance of antibodies directed against extractable nuclear antigens with currently well-known antigenic targets, is studied in 100 patients affected with various connective tissue inflammations. The anti-Sm is the most frequently encountered antibody in lupus disease and overlapping connective tissue inflammations with a lupus component while the anti-RNP is related to mixed inflammation of the connective tissue and the dry syndrome. These antibodies, without being specific markers for a given disease, represent an additional biological argument for a better nosological classification of connective tissue inflammations. The use of immuno-impression, a more sensitive technique than immunodiffusion, could provide a better understanding of the immune response during these diseases and specify the diagnostic and prognostic value of various auto-antibodies. Analysis by immuno-impression of 36 sera among the 100 with antibodies directed against extractable nuclear antigens has enabled us to compare the two techniques and study the different specificities of these antibodies.

Anal submucosal injection: a new route for drug administration in pelvic malignancies. II. Methotrexate anal injection in the treatment of advanced bladder cancer. Preliminary study.

The clinical efficacy of submucosal anal injections of methotrexate in advanced bladder cancer is investigated. An experimental study on 20 mice has shown that methotrexate injected into the anal submucosa has no clinicopathological effect on the anorectum. The clinical study comprised 18 patients with advanced bladder cancer (13 with stage T3 and 5 with stage T4 disease) as a test group in whom methotrexate was injected into the anal submucosa and 8 (6 with stage T3 and 2 with stage T4 cancer) treated concurrently with intravenous methotrexate. The dose in both groups was 50 mg. every 5 days for 5 consecutive doses. The course was repeated at 3-week intervals. Most patients received methotrexate as outpatients. Methotrexate blood levels were measured 4 and 24 hours after administration in both groups. In the test group 10 of the 18 patients showed complete tumor regression and were alive 21 to 50 months after the start of treatment. Partial regression was observed in 8 patients. Hematological reserve remained unchanged. Mild toxicity occurred in 3 patients. Of the 8 patients treated intravenously the tumor showed partial regression in 1, was stable in 3 and progressed in 4. Side effects were severe in 5 patients. Our results show that methotrexate injection is highly effective in the treatment of advanced bladder cancer. It is safe, well tolerated and can be used on an outpatient basis.

Hepatic macrophage function in schistosomal glomerulopathy.

Hepatic fibrosis in the pathogenesis of schistosomal glomerulopathy cannot be explained by any positive influence of hepatocellular injury. In order to examine the potential role of impairment of hepatic macrophage function, the t1/2 plasma clearance of 99mTc-sulphur colloid was studied in 30 patients with schistosomal glomerulopathy, ten normal volunteers, ten cases of uncomplicated intestinal schistosomiasis, ten non-schistosomal cirrhotic patients and ten non-schistosomal nephrotic patients. Liver and renal biopsies were obtained from appropriate groups and examined by light microscopy and glomerular immunofluorescence. There was a significant correlation between t1/2 of sulphur colloid clearance and proteinuria, mesangial hypercellularity, and predominance of IgA glomerular deposits. These data indicate that hepatic macrophage dysfunction is an important factor in the pathogenesis of schistosomal glomerulopathy, and that IgA plays a major role in advanced glomerular lesions. The degree of impairment of hepatic macrophage function may influence the pattern and severity of glomerular lesions depending upon the affection of IgA clearance mechanisms.

[Complete deficiency in the C2 fraction of complement associated with a glomerular nephropathy: apropos of 2 cases].

Two patients with non-systemic idiopathic glomerulonephritis were found to have a complete deficiency of C2, the second component of complement; the clinical course and serological abnormalities are reported in detail. An immunogenetic analysis of the patients of two families was carried out. It was demonstrated that the homozygous C2 deficiency was associated with homozygosity for HLA-A25-B18-DR2-BfS-C4A4-C4B2 haplotype.
