Hibernoma: report of two cases.

Hibernomas are rare tumors of fat which are most often benign. Arteriographic fingings may be misleading with respect to the diagnosis of malignancy preoperatively. These uncommon tumors may be confused prior to surgery with lipomas, soft tissue sarcomas, and hemangiomas.

An evaluation of the local effects of triamcinolone acetonide on normal soft tissues in monkey digits.

The lack of quantitative information on the observed effects after subcutaneous injections of triamcinolone prompted a double blind, prospective study of the drug's effects. The results demonstrate that there are gross losses in the subcutaneous fat. No significant quantitative differences were observed in the collagen content (microscopic or biochemical) in these monkey digits.

Cocaine concentrations in the blood during rhinoplasty.

After applying 5% or 10% cocaine to the nasal mucosa, blood concentration were measured in 9 patients undergoing rhinoplasty and in 6 unoperated controls. The concentrations in the unoperated persons were lower than in those undergoing rhinoplasty, and lower concentrations were found after 5% solutions in both groups than in those exposed to 10% solutions. There was no correlation between the strength of the solution used and the degree of bleeding or the success of the anesthesia. One patient developed symptoms of toxicity within two minutes of a 5% cocaine application to his nasal mucosa. The cocaine concentrations in his blood were the highest seen in our studies, and we cannot explain this.

Dog bites.

Dog bites can be prevented by proper education of children, appropriate selection of family pets and the restraint of guard dogs in the community. An unusually high incidence of bites by German shepherds was observed in this study. Treatment of injuries inflicted by dogs should include: careful wound toilet, appropriate excision of damaged tissues, careful wound repair with adequate drainage and systematic therapy with selected antibiotics.

A technique for radical dissection of the neck.

Modifications in the technique for the performance of standard radical neck dissection have been used for the past 12 years at this institute. The use of transverse skin incisions, removal of the platysma muscle and proceedings from a posterior to anterior direction are stressed in this procedure. Primary healing, reconstruction and cosmesis have been quite good, and local recurrences have been minimal. The five year survival rates for each type of cancer of the head and neck are comparable with those obtained at other cancer referral centers.

Inflammatory pseudotumor of the orbit. Case report.

A case report and a review of the literature of inflammatory pseudotumors of the orbit is presented. The lateral approach, using microsurgical technique, is described. The incidence of this primary granuloma is comparable to other orbital tumors, and the diagnosis is confirmed by histological study of a biopsy specimen. Surgical excision offers the best mode of treatment. Although steroids may help postoperatively, the prognosis is poorer if the tumors are large. Radiotherapy offers no benefit.

This case represents another unusual presents another unusual presentation of a carcinoid tumor in which the cervical area has rarely been reported. In addition, it demonstrates the elusiveness of the tumor despite exhaustive diagnostic studies.

Metastatic hypernephroma to the head and neck.

Hypernephroma is one of the most common visceral adenocarcinomas which metastasize to the head and neck. The metastasis may precede discovery of the primary or follow it. The most common sites of metastatic hypernephroma to the head and neck are the sinonasal tract, skin, cervical lymphatics, and mandible. Three cases of metastatic hypernephroma are presented and one of these is the first documented report of a metastasis to the parotid gland. The other two were found in the sinonasal tract.
