Monoclonal antibody-defined B lymphocyte subpopulations in primary immunodeficiency disorders.

Peripheral blood from 23 patients with common variable immunodeficiency (CVI) and 5 with selective IgA deficiency (SIAD) was examined for the proportions of B lymphocytes defined with monoclonal antibodies FMC 1 and FMC 7. In healthy controls, FMC 1 monoclonal antibody reacts with antigen present on almost all surface Ig+ B lymphocytes; FMC 7 monoclonal antibody reacts with antigen present only a subset of surface Ig+ and FMC 1+ B cells. In 13 of 23 patients with CVI and all patients with SIAD, a relative increase in FMC 7+ B cells, resulting in proportions of FMC 7+ B cell comparable to surface Ig+ or FMC 1+ B cells was observed. These data suggest abnormality of the proportion of B cell subsets in patients with CVI and SIAD that could suggest an intrinsic defect of B cells in these disorders.

Acute effects of dialysis on T lymphocytes in patients with end-stage renal disease.

The acute effect of dialysis on T-lymphocyte responses was studied in 11 patients with end-stage renal disease (ESRD). The in-vitro mitogenic response to concanavalin A and pokeweed mitogen was decreased (p less than 0.05) after single passage of blood through the dialyzer, accompanied by a reduction in the proportion of monoclonal antibody-defined total T lymphocytes (Leu 1+ cells) (p less than 0.01), an increase in the percentage of monoclonal antibody-defined monocytes (M 2+ cells), and a decrease in interleukin 2 (IL-2) production (p less than 0.05). Depletion of adherent cells from mononuclear cells isolated from blood after single passage through the dialyzer restored the mitogenic responses to normal levels. Post dialysis mitogenic responses were comparable to pre-dialysis mitogenic responses although IL-2 production (p less than 0.05), and the proportion of T lymphocyte (Leu 1+ cells) remained depressed (p less than 0.01). Cumulative effects of long-term intermittent hemodialysis may contribute to the impaired immunity and the increased frequency of infections and neoplasms in patients with end-stage renal disease.

Spectrum of renal abnormalities in acquired immune-deficiency syndrome.

Data from 27 patients with acquired immunedeficiency syndrome (AIDS) and AIDS-related complex (ARC) managed at the University of California, Irvine Medical Center since 1982 were extracted from medical records. Functional renal insufficiency occurred with considerable frequency among the AIDS patients. In contrast renal function was stable in the ARC patients studied. The majority of the AIDS patients exhibited persistent or transient proteinuria. Hematuria, leukocyturia, bacteruria, and nonvenereal urinary tract infections were seen with considerable frequency among AIDS patients and much less frequently among ARC patients. Similarly various disorders of fluids, electrolytes, acid base and abnormal phosphorus and calcium levels were common among AIDS patients and uncommon among ARC patients. These observations point to the prevalence and significance of the renal and associated abnormalities in acquired immune-deficiency syndrome.

Spontaneous intrahepatic cutaneous biliary fistula.

A rare case of spontaneous intrahepatic cutaneous biliary fistula due to obstruction of the biliary tree by ampullary fibrosis is reported. The diagnosis was made clinically by the presence of constant flow of the bile through the fistulous opening and confirmed by roentgenography and by surgery. The definitive treatment consisted of removal of the obstruction by choledochoduodenostomy. The fistula healed spontaneously within two weeks of surgery.

Inhibitory effects of sulfhydryl reagents on acetyl-CoA carboxylase from rat mammary gland.

Rat mammary gland acetyl-CoA carboxylase (acetyl-CoA:carbon dioxide ligase (ADP forming), EC 6.4.1.2) is rapidly and irreversibly inactivated by micromolar concentrations of S-(4-bromo-2,3-dioxobutyl)-CoA (BDB-CoA) or p-hydroxymercuribenzoate (PHMB). Inhibition of both half reactions (i.e., the biotin carboxylation and the carboxyltransferase) catalyzed by acetyl-CoA carboxylase closely parallels loss in overall activity (malonyl-CoA synthesis). The presence of a substrate or product (acetyl-CoA, ATP, ADP, Pi) or inhibitor (palmitoyl-CoA) does not protect the enzyme from inhibition caused by BDB-CoA or PHMB. On the other hand, citrate, an activator of acetyl-CoA carboxylase, affords substantial protection against inhibition by BDB-CoA and PHMB. Covalent modification by BDB-CoA or PHMB appears to lock acetyl-CoA carboxylase in an inactive conformation (15-30 S) that is unable to undergo citrate-induced self-association into the catalytically competent polymeric form.

Voltage-gated potassium channels are required for human T lymphocyte activation.

The calcium channel blockers, verapamil and diltiazem, inhibit phytohemagglutinin (PHA)-induced mitogenesis at concentrations that block the T lymphocyte K channel currents. K channel blockers also inhibit the allogeneic mixed lymphocyte response in a dose-dependent manner with the same potency sequence as for block of K currents. K channel blockers inhibit PHA-stimulated mitogenesis only if added during the first 20-30 h after PHA addition, but not later, indicating a requirement for functional K channels during this period. We investigated the effect of K channel blockers on various aspects of protein synthesis for two reasons: first, protein synthesis appears to be necessary for the events leading to DNA synthesis, and second, the increase in the protein synthetic rate commences during the first 24-48 h after PHA addition. PHA-induced total protein synthesis was reduced to the level in unstimulated T lymphocytes by K channel blockers in a dose-dependent manner with the same potency sequence as for the block of K currents and inhibition of [3H]thymidine incorporation. Two-dimensional gel electrophoresis demonstrated that although the synthesis of the majority of proteins was reduced by K channel blockers to the level in unstimulated T cells, some proteins continued to be synthesized at an enhanced rate compared with resting cells. Two proteins, S and T, detected by two-dimensional gel electrophoresis in unstimulated T lymphocytes, appeared to be reduced in intensity in gels of PHA-treated T lymphocytes, in contrast to the increased synthesis of the remaining proteins. 4-Aminopyridine (4-AP), at concentrations that inhibit protein synthesis, prevented the apparent PHA-induced reduction of proteins S and T. These proteins may play a role in maintaining the T lymphocyte in a resting state and may be related to the translation inhibitory factors reported to be present at a higher specific activity in quiescent T lymphocytes than in PHA-activated T cells. The expression of the IL-2 receptor (Tac) during T lymphocyte activation was not altered by K channel blockers, whereas the production of interleukin 2 (IL-2) was reduced to the level in unstimulated T lymphocytes. Exogenous IL-2 partially relieved the inhibition of mitogenesis by low, but not by high, concentrations of 4-AP. These experiments clarify the role of K channels in T lymphocyte activation and suggest that functional K channels are required either for protein synthesis or for events leading to protein synthesis.

Treatment of thrombotic thrombocytopenic purpura with plasma exchange: five cases.

Five patients with thrombotic thrombocytopenic purpura (TTP) were treated with corticosteroids, plasma exchange (PE), and antiplatelet agents. ABO- and Rh-compatible fresh-frozen plasma (FFP) was used as the replacement fluid. None of the patients received FFP infusion without PE. The patients were followed for 14-32 months after initial treatment. Four of the five patients are in complete clinical remission. One responded to plasma exchange but died of heart disease. This report shows an 80% survival rate which is markedly better than that without the use of PE and FFP infusion. We found dramatic clinical and hematologic improvement beginning shortly after initiating treatment. Two patients showed reversal of their initial clinical and hematologic improvement when PE was discontinued, despite continuing treatment with antiplatelet drugs and in one with corticosteroids. Both patients showed hematologic and clinical remission after reinstitution of PE. None of the survivors had any neurologic or renal deficit. They were maintained on aspirin and dipyridamole for 1 year after discharge.

Plasma histaminase activity in carcinoma of the cervix--its clinical significance.

Plasma histaminase activity has been estimated in 79 patients with squamous cell carcinoma of the cervix. The mean plasma histaminase value is significantly higher in patients than in controls (P less than 0.001). The plasma histaminase activity falls significantly following radiotherapy (P less than 0.001). Pretreatment plasma histaminase values do not show any correlation with clinical stages and the posttreatment values decline significantly but remain higher in clinical stages III and IV than in stages I and II. The increased plasma histaminase activity also reflects the degree of differentiation within the tumor. Further it is noted that the plasma histaminase value is inversely proportional to the radiation response. It is suggested that estimation of plasma histaminase activity can serve as an important marker in assessing the treatment response and monitoring the course of treatment.

Evaluation of leukocyte adherence inhibition (LAI) test in primary and secondary hepatic malignancies.

Leukocyte adherence inhibition (LAI) has been demonstrated to be a simple, rapid, and reliable technique in the diagnosis of various malignancies including hepatocellular carcinoma. The LAI test was carried out employing modified tube LAI technique in patients with primary hepatocellular carcinoma (HCC) and secondary carcinomas of liver. Positive LAI response to HCC antigen was obtained in all six (100%) cases of hepatocellular carcinoma tested. None of the control cases, which included 8 healthy subjects and 16 cases of benign liver diseases, gave positive LAI response. Two out of 19 cases of secondary carcinoma gave a positive LAI reaction to HCC antigen. In secondary carcinomas, 19 out of 21 cases (90.48%) gave positive LAI reaction to secondary adenocarcinoma antigen. There were two false positives in controls (1 each of cirrhosis and amebic liver abscess), and 1 out of 8 cases of HCC also gave positive response to secondary carcinoma antigen. Thus, LAI test was found to be useful in the diagnosis of hepatic malignancies.
