[US or CT-guided percutaneous drainage of pancreatic pseudocyst].

Authors, after illustrating the pathogenesis of Pancreatic Pseudocysts (PPC) and classifying them into secondary to acute pancreatitis and arising in course of chronic pancreatitis, underline that the use of Ultrasound (US), CT-scan, Magnetic Resonance, Endoscopic Retrograde CholangioPancreatography and, above all, percutaneous drainage of the cysts has modified the therapeutic approach to this pathology. They describe indications and technique of US or CT-guided percutaneous drainage, and report their experience about 12 cases of PPC secondary to acute pancreatitis and 4 of PPC arising during chronic pancreatitis. They analyze the limits of this technique (risk of infections, recurrences, fistulas), and conclude that Percutaneous Drainage of Pancreatic Pseudocysts is a useful therapeutic approach in the treatment of PPC secondary to acute pancreatitis, while its use is complementary to surgery in the treatment of PPC due to chronic pancreatitis.

[Nuclear risk and thyroid neoplasms in infancy].

Thyroid carcinoma is a disease with low incidence in adult and rare in child. The authors give oncogenic risk factors of the disease and underline environment factors as iodine deficiency and radiation pollution that, as shown in certain world areas, has a strong effect in the epidemiology of this disease. Eventually radiation's are the most important oncogenic risk factors but and adequate iodine prophylactic program can be considered a valid shield to prevent this disease.

[Primary adenocarcinoma of the duodenum].

Adenocarcinoma of the duodenum represents a rare neoplasia characterized by an indefinite symptomatology, at least early, and deceitful, that, therefore, arrives at surgeon, almost always, in advanced stage. The Authors, taking from a clinical case, recently observed, as a starting point, review the literature, referring the etiopathogenetic hypothesis that explain the relative rarity of this neoplasia. They therefore report the diagnostic and therapeutic procedure to carry out in these patients.

[Ambulatory treatment of inguinal hernia].

The Authors report their experience about the inguinal hernioplasty operation by implantation of prostheses in prolene following the Licthenstein technique. Prolene mesh strengthens the inguinal canal wall, with no suture tension point. The technique is performed under local anesthesia, in day-hospital system, consenting the patient a rapid reinstatement in the social-working ambit.

[Hemangiopericytoma: diagnostic-therapeutic considerations].

The Authors report a clinical case of hemangiopericytoma recently observed. The biological behaviour of this tumour presents a character of unforeseeableness and, in every chance, needs a long period of follow-up. Based on literature data, the Authors report some diagnostic-therapeutic precise information and underline the scanty reliability of the prognostic factors.

[Gastric precancerous conditions].

The Authors examine the anatomopathological and clinical features of the main gastric precancerouses and explain the diagnostic therapeutic protocols used. The diagnostic and therapeutic strategy can not leave a strict interdisciplinary collaboration out of consideration between radiologist, endoscopist, anatomist, gastroenterologist and surgeon. They conclude, in fact, affirming that the mentioned collaboration represents the one possibility for improving the results at distance of this neoplasia.

[Compression suturing devices in surgery of lateral colostomies].

The Authors refer their experience about two cases of colo-colic anastomosis executed by employment of biofragmentable ring BAR (Valtrac). The Authors used, for the bowel fragments preparation, the ASP-50 rake, overcoming the frequent inconvenient of mucosa extroversion thanks to circumferential mucosectomy extensive for a few mm. of colic fragments. Based on BAR (Valtrac) behaviour and the obtained results, in accord with the most other Authors, they conclude affirming that the compression anastomosis represents a simple methodology, sure and reliable for closing of the lateral colostomies.

[ Tissue and blood CEA levels in stomach cancer].

The assays of serum CEA is a useful prognostic marker in patients with stomach cancer. In this study pre-operative serum CEA and tissue CEA in tumour or biopsy were assayed in a group of patients with gastric carcinoma in order to assess their prognostic roles. Based on an analysis of the results the authors affirm that high serum CEA levels in patients with advanced stages of stomach cancer indicate a fatal prognosis, and likewise the finding of tissue CEA in loco-regional lymph nodes at the same concentration as that in the primary tumour is also the sign of a severe prognosis.

[Idiopathic retroperitoneal fibrosis].

The authors, on the basis of a case recently observed dwell upon the natural history, the etiopathogenesis, and diagnosis of idiopathic retroperitoneal fibrosis. After confirming that medical therapy may be successful in selected cases only, different surgical procedures are analysed. It is concluded that outcome and prognosis of these subjects depend on early diagnosis.

[Tumor markers in colorectal cancer].

Over the past ten years numerous studies have been carried out to identify tumour markers able to diagnose cancer of the digestive tract. The paper reports the combined use of some markers (CEA, TPA, GICA, CA 125) in patients with carcinoma of the colon-rectum. The Authors conclude that although these markers are of little use, especially in association, in the early diagnosis of disease, it is very important to utilise these markers to monitor patients following surgical, chemotherapeutic or radiotherapeutic treatment.

[A frequent complication of enterostomy: prolapse].

Among the other complications of enterostomy, prolapse deserves particular mention on account of the therapeutic problems involved in its correction. Based on a report of the authors' personal experience, the paper focuses on the mechanisms of onset and, above all, on the most appropriate forms of treatment, before continuing to describe the different surgical reparative techniques. In conclusion, the authors affirm that an optimal tailoring of the enterostomy and its appropriate preparation represent the ideal method of preventing the onset of prolapse.

[Study of the APUD cells of the gastric mucosa].

The results of an histochemical and microscopic study of the APUD cells carried on gastric biopsies are reported. Endocrine cell hyperplasia, microscopically detected, was particularly evident in atrophic gastritis (type A chronic gastritis) and adenomas. The presence of the APUD cells confirmed by electronmicroscopy, which revealed granules of 200 NM with a central nucleus and a membrane-separating electron-lucent ring, gives a further contribution to the hypothesis of a possible association between endocrine cell hyperplasia and intestinal metaplasia.

[Tumors of the testicle].

The authors describe, in the light of the current literature, the clinical, diagnostic, and therapeutic characteristic of testicular cancer. They remark the good results of adjuvant therapy in this type of tumor.

[Unilateral polycystic renal dysplasia].

The Authors report two cases of unilateral multicystic kidney dysplasia comparing the etiopathogenetic and clinical characteristics with those of polycystic kidney disease. They conclude with some considerations on the current therapeutic trend in these cases.

[Clinico-statistical findings in Fournier's gangrene].

Pathophysiological mechanisms of Fournier's gangrene are reported. Variety and progression of clinical manifestations are stressed as well. The importance of an accurate therapeutic protocol in the treatment of this peculiar and serious disease is therefore underlined.
