Erythrocytosis complicated by multiple paraganglioma.

We report the case of a 22-year-old woman with onset of erythrocytosis at the age of 9 years. Endocrinological and radiological examinations revealed an elevated catecholamine level and the presence of multiple abdominal tumors. After the removal of the tumors, the catecholamine level normalized, whereas erythropoietin remained at the same level and erythrocytosis persisted. The tumor lysate contained considerable amounts of catecholamine but not erythropoietin. Moreover, no erythropoietin mRNA was detected in the tumor by in situ hybridization. These data suggest that this paraganglioma did not produce erythropoietin. A review of the literature showed the existence of patients with early-onset erythrocytosis complicated with paraganglioma, whose erythrocytosis was not relieved even after the resection of paraganglioma.

Laser beam in treating congenital choanal atresia in three patients.

We present the results of 3 patients with congenital unilateral choanal atresia who were treated surgically at our department. We report a method of surgical approach for repair of choanal atresia. A transpalatal procedure offers an approach which is straightforward. The use of carbon dioxide or Nd:YAG laser seemed better for recurrent cases. A tube stent should be placed for at least 6 weeks postoperatively.

Serum collagen type IV for the assessment of fibrosis and resistance to interferon therapy in chronic hepatitis C.

Sixty-nine patients with chronic hepatitis C (CH-C) were treated with interferon therapy, and serum collagen type IV (s-collagen IV) levels were measured by enzyme immunoassay to analyze the responsiveness to interferon therapy. Classified by the improved pattern of serum alanine aminotransferase levels after interferon administration, 23 patients were judged as sustained responders, 23 as transient responders, and 23 as non-responders. Fibrotic grades of the liver sample correlated statistically with the levels of s-collagen IV (P < 0.01). Pre-therapy s-collagen IV levels of sustained responders were significantly lower than those of the other responders, and only sustained responders showed a significant decrease of s-collagen IV levels after interferon therapy, in accordance with histologic improvement. Multivariate analysis showed that s-collagen IV and hepatitis C virus genotype were the most important factors affecting the response to interferon therapy of all variates. Thus, s-collagen IV is one of the most useful aids for the evaluation of liver fibrotic grade in CH-C and a potent predicting indicator for the responsiveness to interferon therapy.

Absorption mechanism of polaprezinc (zinc L-carnosine complex) by an everted sac method.

1. The absorption mechanism of polaprezinc (zinc-carnosine chelate compound) was studied in rat by an everted gut sac method. The rates of transport and accumulation of 14C-L-carnosine were proportional to the mucosal concentration of L-carnosine, whereas the rates of transport and accumulation of 65ZnCl2 had become saturated. The Michaelis-Menten constant (Km) = 4.41 mM and maximal rate (Vmax) = 71.83 nmol/min/g for zinc transport and, similarly, Km = 6.21 mM and Vmax = 92.51 mumol/30 min/g for accumulation. 2. The addition of ouabain, 2,4-dinitrophenol (2,4-DNP) and low temperatures reduced the rate of zinc uptake, indicating that zinc transport was considered to be a carrier-mediated process based on Na+,K(+)-ATPase-dependent mechanisms. 3. The concentration of zinc in the gut of the non-fasted rat was greater than that of the fasted rat, suggesting different rates of transport and accumulation. It is suggested that zinc intestinal uptake in rat is regulated by zinc content in the gut. 4. A pharmacokinetic model for transport and accumulation of zinc saturation from the lumen side to the gut was designed, and the calculated values obtained by simultaneous multiline fitting of transport and accumulation of zinc data were in good agreement with the observed values.

[The activity of superoxide dismutase in patients with taste and smell dysfunction].

It was revealed that Zinc deficiency might cause taste and smell dysfunction. Superoxide dismutase (SOD), which scavenges superoxide, is a type of zinc enzyme. It was also demonstrated that the overproduction of superoxide damages normal tissue. We therefore measured the activity of SOD in serum and saliva of patients with taste or smell dysfunction by the cytochrome C reduction method using a xanthine-xanthine oxidase system. As a result, in the patients with taste dysfunction, the activities of SOD in both serum and saliva were normal, but mean level of serum Zinc was near to the lower normal limit. On the other hand, in the patients with smell dysfunction caused by chronic sinusitis or common cold, the activity of SOD in serum was significantly lower than that of healthy volunteers, and that in saliva was normal. These results suggest that Zinc enzymes, except serum and salivary SOD, are involved in the sense of taste, and that further investigation is necessary to clarify the relation between serum SOD deficiency and olfactory dysfunction.

[Magnetic resonance spectroscopy of naso-sinus tumors].

Magnetic Resonance Spectroscopy (MRS) has been developed as a new application of nuclear magnetic resonance. By means of MRS, internal chemical information can be non-invasively measured. In this study, the 31P MRS of naso-sinus tumors was measured and evaluated. The peaks of 31P MRS, such as Adenosine tri-phosphate (ATP), Creatine phosphate (PCr), Phosphodiester (PME), Inorganic phosphate (Pi), and Phosphomonoester (PME) can be identified. Phosphomonoesters (PME) are precursors of membrane phospholipids. Phosphodiesters (PDE) are metabolites of phospholipids. Compared with benign tumors, the 31P MRS of malignant tumors shows a higher PME peak and lower PDE and PCr peaks. These results indicate that malignant tumors are characterized by chemical changes in membrane metabolism.

Prevalence of hepatitis C virus with different genotypes determined by a group-specific antibody assay in Japanese patients with chronic liver diseases due to hepatitis C virus infection.

Hepatitis C virus (HCV) genotypes (groups I and II) were determined by a newly developed C14 assay in which antibodies against group-specific recombinant proteins of the NS4 region were measured by ELISA (enzyme-linked immunosorbent assay). The genotypes determined by the C14 assay were compared to those determined by a polymerase chain reaction (PCR) in 50 patients. The HCV genotypes determined by both methods were consistent in 78% of the patients. The results of the assays were not contradictory in any patient. The sensitivity of the C14 assay was as high as over 90%. Frequency of HCV genotypes was studied in 300 patients with chronic liver diseases type C by using the C14 assay. The prevalence of groups I and that of II in patients with chronic hepatitis were 73% and 21%, the correspondence figures in those with liver cirrhosis 80% and 13%, and in those with hepatocellular carcinoma 81% and 13%, respectively. The clinical characteristics were similar between the patients with group I and II HCV infections in each disease category. In conclusion, the C14 assay is useful for epidemiological studies of HCV genotypes, and group I is a major HCV genotype of chronic liver diseases type C in Japan.

[Clinical experiences of granulocyte colony stimulating factor for granulocytopenia after AC bypass].

We have successfully treated 3 cases of granulocytopenia, using Granulocyte Colony Stimulating Factor (G-CSF) after AC bypass surgery. All cases were male, aged 68, 75 & 54 years old, and underwent AC bypass surgery without transfusion except in case 1. Granulocyte cell counts decreased to 135/mm3 on 39th post-operative days (POD) in case 1, 624/mm3 on 13 rd POD in case 2, and 514/mm3 on 20th POD in case 3. In case 1, 300 micrograms of G-CSF was given intravenously for 3 days. In case 2, 100 micrograms of G-CSF for 1 day and 50 micrograms for 3 days was given subcutaneously, and in case 3, 100 micrograms of G-CSF was also given subcutaneously for 2 days. And their granulocyte cell counts all increased within few days. G-CSF is effective for patients with granulocytopenia after AC bypass surgery, but we need more experiences to know the best way of administration.

Localized aerosol hyperthermia in patients with nasal allergy.

A double-blind study was performed to evaluate the efficacy, the safety and the usefulness of localized aerosol hyperthermia (LAH) in 57 patients with nasal allergy. We used two localized aerosol hyperthermia devices, the equipment (AH) with an insufflated aerosol of 43 degrees C and a placebo device (PH) with aerosol of 32 degrees C. In the evaluation of clinical efficacy, efficacy rates were 46.4% in AH and 3.7% in PH during 2 weeks, respectively, and 53.3% in AH and 7.7% in PH during 4 weeks, respectively. There was a significant difference (p < 0.01) between the two treatment groups. The usefulness rates were 63.3% in AH and 7.4% in PH for patients with nasal allergy, showing a statistically significant difference (p < 0.01). No patients dropped out of the study due to adverse effects. These results suggest that the LAH is a very useful instrument for the treatment of nasal allergy.

[A case of coronary artery bypass grafting without cardiopulmonary bypass in a patient of both coronary ostial stenosis due to Takayasu's arteritis].

A 61-year-old female admitted with the diagnosis of both coronary ostial stenosis & the severe calcified aorta due to Takayasu's arteritis. She underwent coronary artery bypass grafting successfully, using the right internal thoracic artery to RCA & the left internal thoracic artery to LAD, without cardiopulmonary bypass. Coronary artery bypass grafting without cardiopulmonary bypass is one of the useful surgical technique for the patient with the severe calcified aorta due to Takayasu's arteritis.

[Clinical experience and early results of aortic valve replacements with Medtronic Hall 20 A].

We experienced aortic valve replacement for 18 patients (4 male and 14 female, ranging from 40 to 81 years old, mean 64 years old) with small aortic annulus with new small prosthesis. Medtronic Hall 20 A (MH 20 A). The average body surface area was 1.47 m2 (range 1.28 to 1.81 m2). One patient was died of lung cancer at 3 months postoperatively. One nonstructural dysfunction and one low cardiac output syndrome was noted during operation. Sixteen survivors with MH 20 A, showed satisfactory clinical results, except for minor thromboembolism, in 2 patients. The MH 20 A had the same function as that of MH 21 A, already reported. We concluded that aortic valve replacement by Medtronic Hall 20 A prosthesis was usefull for the patient with small aortic annulus.

[Liver cirrhosis and hepatitis viruses].

The main etiological cause of liver cirrhosis that account for approximately 80% of liver cirrhosis in Japan in hepatitis virus, and HBV and HCV account for 38% and 59% of this hepatitis virus-induced liver cirrhosis, respectively. Liver cirrhosis develops in patients with persistent infection of HBV or HCV. A diminution in HBV replication can occur due to the immune response in cases with chronic hepatitis. In such cases, the liver disease may become almost normal or inactive after conversion to the nonreplicative phase. To the contrary, in some cases with HBV infection, such conversion does not occur leading finally to liver cirrhosis. Chronic infection takes place in 65% of the patients exposed to HCV, which results in the development of chronic hepatitis, and only quite rarely resolves. About 40% of these patients with chronic HCV infection will develop liver cirrhosis in 10 to 20 years.

[A case of Bacillus cereus prosthetic valve endocarditis].

A rare case of prosthetic valve endocarditis by Bacillus cereus was reported. The patient was 43-year-old Japanese man, who had mitral valve replacement 5 months prior to this admission. Remitral valve replacement was immediately done successfully. His postoperative course was uneventful for 8 months.

[A case report of thymolipoma with high titer of serum anti-acetylcholine receptor antibodies--thymolipoma, a review of 49 reports in Japan].

A 22-year-old man was diagnosed as thymolipoma by chest CT scan & MRI. In spite of high titer of serum anti-acetylcholine receptor antibodies, he showed no symptom of myasthenia gravis. An extended thymectomy was done successfully. Tumor was 21 x 15 cm in size and 620 g in weight. Histopathologically the tumor was diagnosed as thymolipoma. The titer of serum anti-acetylcholine receptor antibodies was decreased within normal limit and postoperative course was uneventful. To our knowledge, the 49 case reports in Japan were reviewed.

Effects of metal chelating agents on the oxidation of lipids induced by copper and iron.

The non-enzymatic oxidations of soybean phosphatidylcholine liposomes, methyl linoleate micelles and low-density lipoprotein in aqueous dispersions induced by copper and iron have been studied aiming specifically at elucidating the action of the metal chelating agents such as ethylenediaminetetraacetic acid disodium salt (EDTA), nitrilotriacetic acid (NTA), adenosine-5'-diphosphate disodium salt (ADP), desferrioxamine (DFO), penicillamine (PCM), and triethylene tetramine (TTM). The effects of chelators on chemiluminescence emitted from its probe luminol in the decomposition of tert-butyl hydroperoxide by metal ion were also studied. The effects of chelators on the oxidations depended both on the metal ion and the substrate. Namely, in the oxidations of both liposomes and micelles, EDTA and NTA suppressed the copper-induced oxidations, whereas they enhanced the oxidations induced by iron. ADP had little effect, while PCM and TTM had accelerating effect for both metal ions. On the other hand, in the oxidation of LDL, none of these chelators enhanced the oxidation. Especially, TTM and PCM suppressed the copper-induced oxidation of LDL, suggesting that the chelating agents blocked the access of the metal ion to the hydroperoxide within LDL. The effects of chelators on chemiluminescence emission were similar to those on the oxidations of liposomes and micelles. The cyclic voltammograms of metal complexes were also measured. The multiple effects of chelators on the rate of non-enzymatic, metal-catalyzed oxidations of lipids were interpreted by their influence on redox-potential and accessibility to hydroperoxide of the metal-chelator complex.
